DRIVER INADVERTENTLY APPLIED ACCELERATOR WHILE APPLYING THE BRAKES. WANTS TO KNOW IF THIS COULD HAVE BEEN RESULT OF FAULTY BRAKE/ACCELERATOR PEDAL DESIGN.
