I took the van to get the safety recall done in 2026, Feb 25. For the rear view back up camera. I took it to the same dealership that I bought the car from, Jones Junction. However, upon further complications with the back up camera, this issue has never been fixed or resolved. If anything, the issue has become worse. I am unable to use my camera 98% of the time. It freezes. It goes black or is unavailable to see anything out of. It's a safety recall for a reason. I want this issue fixed. As I have been driving like this with the issue for quite some time. What are my next steps? I prefer not to have to dealer hop if I can help it. But I might have to. Is there a patch they may have to do to re-install the software for the camera? Or would they have to go and flash it again and what ever else? My true gut tells me that I dont think this specific dealership ever did the recall, correctly.

While driving at normal road speed, the vehicle suddenly lost propulsion and entered limp mode. A turtle warning icon appeared along with multiple system fault messages. The vehicle began slowing down on it's own and was unable to accelerate. I pulled to the side of the road. The vehicle displayed an error "Stop Safely Vehicle Will Shut Off Soon". After sitting for a period of time, the vehicle restarted and was able to be driven again, but the failure occurred without warning and created a dangerous situation in traffic. Diagnostic trouble codes were present related to the hybrid system and high-voltage battery, including P1D43, P1A21, U029A, and U0412. These codes indicate faults in the hybrid control system and battery communication. A Chrysler service department inspected the vehicle, confirmed the trouble codes, and returned it to me without repair because the issue was not immediately reproducible at the dealership. The vehicle has a history of similar intermittent issues, including prior loss-of-power events and repeated service visits for hybrid system faults. The condition is not consistently reproducible, but when it occurs it results in a sudden loss of motive power while driving. This creates a significant safety risk due to unexpected loss of propulsion in active traffic.

When adaptive cruise control is enabled and I’m at cruising speed, new vehicles entering the lane in front of my vehicle are not being acknowledged leading to my vehicle getting too close and collision becoming likely. This leads to me having to takeover control frequently to prevent accidents. This happens often, not always, but it is becoming more and more frequent. This happens at all speeds from 30 mph to 70mph

1. This vehicle was recalled for a software update due to the rear camera failing to operate at times. The software was updated by the dealer. However, this did not completely fix the problem. While the camera fails less frequently, it still does not energize at times. I took the vehicle back to the dealer. I was then informed that there is water intrusion into the rear lift gate, which causes the electrical to short out. I was told that I would have to replace the wiring harness which energizes the camera as well as the rear vehicle lights. I have not noticed any problem with the lighting as of yet. However, I have been told that this is a problem with the Pacifica in general, as well as the Durango. I was told that replacing the harness would not guarantee that the problem would be solved as there would still be water intrusion, which could cause the problem again, even with replacing the harness. This is obviously a safety issue, as the camera fails to operate at times, and there is a potential of losing lighting on the back of the vehicle. This is also a known defect by Chrysler. 2. When using the Adaptive Cruise Control on streets and highways with multiple lanes traveling the same direction, the vehicle will brake suddenly and hard when the vehicle in front of me changes lanes. This occurs after the vehicle in front of me has completely changed lanes and is not impeding my forward progress whatsoever. My vehicle speeds up to the set speed and then violently brakes as if it is about to crash into the vehicle ahead of me, even thought the lane is completely clear. The vehicle has been inspected for this scenario. I was told that it didn't perform as described above when the technician drove it. This is a safety issue as the vehicle suddenly and violently brakes at highway speeds, with traffic behind me.

My vehicle has no back up camera or blind spot warning. Took to dealer for recall and they said they were only responsible for a software update and not solving the recall problem. My vehicle still does not have either of these functions. I was told I would have to pay for the repairs for this recall.

I was backing out of the driveway. I had all the safety features on, and I was checking my surroundings. Typically, the blind spot and front sensors would alert me to dangers. However, on this and many other occasions the warnings didn't alert me to danger on my passenger side that I couldn't see. this resulted in damage to the front quarter panel of my van as well as damage to the bumper. The body shop said it would cost at least $5000 to repair the damage. I can't afford that; in turn I am driving around with the damage.

My Pacifica completely shut down. All the warning lights came on such as crash detected, ABS light, and the windshield wipers came on. The vehicle was towed to the dealership that I bought the vehicle from. They completed a hard reset of the vehicle, and I took it home. Two days later it did the same thing all the lights came on, but it allowed me to drive it to the dealership. Still waiting to see what they think the problem is this time.

For the most recent recall, the rear view camera has not been working since August 2024. I’ve been to the dealership 3 times, once in August and twice in November and they cannot resolve the issue. They told me that they’ve sent information to Chrysler and they would contact me in a few days and I have not heard back. This is a huge safety issue as my rear view display is not visible, it’s either a blue screen or grey screen. I need help please

The backup camera and rear collision warning randomly stops working. I put the car in reverse and there are just blank lines on the screen. Somethimes the screen resumes a normal display when I shift out of reverse, and sometimes the screen just stops working.

This is a follow up to an earlier submission (NHTSA Tracking Number 11591550). After the initial incident, the dealership replaced the hybrid battery on the vehicle. A month later, yesterday, while driving the vehicle on the interstate, I again received the dreaded, "Stop Safely, Vehicle Will Shut Off Soon" warning indicator and the car lost all power about 30 seconds later. Thankfully, I was on an offramp and was able to stop safely. Had I been traveling at full speed in the carpool lane, it could have been very dangerous. The van is now at the dealership for repair, and I hope they fix it for real this time. This is an extremely dangerous situation, with vehicles dying while going full speed.
