TL*THE CONTACT OWNS A 1994 FORD CARGO VAN TRUCK. WHILE DRIVING APPROXIMATELY 30-35 MPH HE ENGAGED THE BRAKES AND THEY FAILED CAUSING HIM TO CRASH INTO THE REAR END OF A POLICE VEHICLE. A POLICE REPORT WAS FILED. THE PASSENGER SUSTAINED INJURIES TO THE NECK, BACK, SHOULDERS AND HEAD. THE VEHICLE WAS LATER CHECKED BY A MECHANIC WHO STATED THAT THE VEHICLE HAD A BROKEN BRAKE PAD. THE OTHER THREE WHEELS FAILED TO WORK BECAUSE OF THE BROKEN BRAKE PAD. THE VEHICLE WAS NOT REPAIRED. THE FAILURE MILEAGE AND CURRENT MILEAGES WERE 100,895.
