On June 3, 2026 at approximately 3:48 PM, I was operating my 2025 Ford F-150 Lightning Flash in front of my home. The vehicle was at a stop or near-stop. I shifted the vehicle into Reverse, and it rapidly accelerated unexpectedly while in one-pedal driving mode. I then attempted to shift the vehicle into Park to stop it. The vehicle then stopped abruptly and surged forward, ultimately crashing into the front of my house. The vehicle movement and acceleration were not consistent with my intended driver input. One-pedal drive was on, cruise control/BlueCruise/driver assist were off, road conditions were dry, no warning lights or messages appeared, and I noticed no floor mat obstruction. The exact speed reached is unknown, but the impact caused substantial exterior and interior structural damage to the house. I sustained injuries to my head, neck, right leg, and left elbow and went to the hospital for evaluation and treatment. A police report is pending. I am filing this complaint because the vehicle appeared to experience rapid unexpected acceleration and movement not consistent with intended driver input.
On September 19, 2025, my wife was at the wheel of our 2025 Ford F-150 Lightning (VIN: [XXX] ), a vehicle we had leased just months earlier on June 22, 2025, from Sam Galloway Ford at [XXX] . Traveling at a mere 15-20 mph when she was involved in a routine multi-vehicle collision (no citations were issued). No collision warning was sounded. The front-end impact triggered an instantaneous and ferocious eruption of flames from the truck's forward section. The vehicle was swiftly consumed in a raging inferno, leading to its complete and irreparable destruction. My wife narrowly escaped with her life, scrambling from the cab amid the explosive intensification of the fire while relying on the urgent assistance of bystanders to reach safety—mere seconds from what could have been a fatal entrapment. She endured profound terror, acute anxiety, and an overwhelming fear for her very survival in the face of this abrupt and merciless blaze. Responding emergency crews, including the fire department, pinpointed the origin at the vehicle's front end, aligning with hallmarks of a catastrophic battery or electrical malfunction. The contact owns a 2025 Ford F-150. The contact stated that while driving at an undisclosed speed, the message "High Voltage Battery Fault" was displayed and then the message "Take Vehicle to Dealer" was displayed. The vehicle was taken to the dealer, but no cause of failure was found. The contact stated that while driving at an undisclosed speed, the message "Stop Safely Now" was displayed and the vehicle was towed to a second dealer, Szott Ford (8800 E Holly Rd, Holly, MI 48442). The vehicle was not yet diagnosed or repaired. The manufacturer was made aware of the failure, and a case was filed. The failure mileage was approximately 10,900.

Description On your vehicle, the integrated park module ipm may not properly engage the park pawl, preventing the vehicle from locking into park when park p is selected. If this condition occurs, a wrench light and shift system fault message will be displayed in the instrument panel cluster. Additionally, the vehicles electronic parking brake epb will automatically apply. Safety Risk If the park pawl is not properly engaged and the electronic parking brake epb is not applied, your vehicle may experience a roll-in-park, increasing the risk of a crash. Remedy Software is available to repair your vehicle. Ford Motor Company has authorized your dealer to update your vehicles secondary on board diagnostic module c sobdmc software free of charge. I am writing to report a serious safety issue introduced by a recent firmware update regarding the Sport mode and 1-pedal driving behavior. After driving the vehicle in Sport mode, parking, and restarting, the dashboard prompts me to return to Sport mode. When I select "Yes," the dashboard indicates that the vehicle has switched to Sport mode. However, the vehicle silently reverts to Normal mode, which has a much more relaxed braking pattern for 1-pedal driving than the expected, shorter braking path of Sport mode. Compounding the danger, this issue occurs inconsistently. Sport mode occasionally does stick upon restart, which creates a false and dangerous level of confidence in the system. The unexpected drop in braking responsiveness has already caused multiple near-miss collisions.
