From owner reports, verbatim
This is a follow-up to my original complaint #XXX, filed XXX, regarding the failure of the 10R140 10-speed automatic transmission in my 2021 Ford F-250 Super Duty (VIN: [XXX]). The transmission began exhibiting symptoms of gear slipping, jerking, delayed and harsh shifting, and pinging from the torque converter around 100,000 miles. These symptoms progressively worsened until the transmission suffered a catastrophic internal failure. As documented in the attached Sid Dillon Ford repair order, an authorized Ford dealer found metal contamination in the transmission fluid and confirmed that a complete transmission replacement was required. The attached Friesen Ford repair order also shows that harsh engagements were present and that a transmission flush failed to resolve the issue. I have also attached a sworn statement from [XXX], an independent automotive technician who has serviced vehicles for my family for several years. In his sworn statement, he confirms that these 10R140 transmissions have known serious ongoing issues and that many mechanics avoid working on them due to these problems. Ford Motor Company was provided with the above repair records and the sworn statement but has denied any responsibility or reimbursement for the transmission failure. The failure created a serious safety risk due to the sudden loss of propulsion while driving. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
Transmission has shifting issues. Sometimes is don't down shift below 4th gear and when in an intersection truck barley moves then it shifts hard and can move . It also is constantly shifting between gears. It will shift form 10th to 5yh while driving.
Suspension
Reports naming suspension components — springs, struts, shocks, control arms and related hardware.
From owner reports, verbatim
I am the original owner of this 2021 F250, (by my notes, dates & miles may differ slightly by Ford/Dealer records) I first noticed Death-Wobble around [XXX] @ 3,040 miles & Ron Alpen Ford checked factory alignment & ordered an updated steering stabilizer. Updated steering stabilizer was on back order until around 8/8/23 @ 26,647 miles when it was installed on the F250. As you can see, over two thirds of this truck's three year bumper to bumper warranty was dissolved by back order. This is where I believe Ford Motor Company's engineering group got involved & recommended a custom alignment, I believe this was performed by Ron Alpen Ford around 4/8/24 @ 33,486 miles. As you can see, over half of this truck's remaining warranty was dissolved waiting on Ford Motor Company's assistance. Ford Motor Company fell silent after recommending I take the truck to a different Ford Dealer for a second opinion... I still do not believe a second opinion was the correct opinion of Ford... Ford should have supported Ron Alpen Ford further in this repair. I set up an appointment at Courtesy Ford, Courtesy had the truck for two full weeks straight... Ford claimed that the truck had conveniently made it outside of the warranty period... 12/3/24 @ 35,922 Courtesy replaced the drag link & the steering stabilizer a SECOND time as the continuing death-wobble had destroyed the first updated steering dampener at my expense because the truck was conveniently out of the 3 year warranty period... Courtesy also decided to return the truck to its original factory alignment even though they were asked not to. Death-wobble continued as of [XXX] @ 36,105 miles. I have contacted Ford Motor Company, Ron Alpen Ford & Courtesy Ford on 12/11/24 to inform them the truck still has Death-Wobble & as of 12/27/24 no reply from any one. I have called Courtesy Ford requesting my money back because they guaranteed the truck would be fixed & it clearly is not. [XXX] 275m rear axle tubes are crushed in and it has eaten the end off of the driveshaft. Truck has not towed more than 17000 lbs via gooseneck trailer. Reached out to Ford Motor Company and go no solution for repairs. Same rear-ended has s recall for the very same damage to the axle tubes. Upon further inspection, corrosion was found on the body control module from water leaking into the unit. The contact stated that the front passenger’s side windshield was loose, and pressure was applied, possibly causing the water to leak into the unit. The vehicle was not repaired because the VIN was not included in NHTSA Campaign Number: 21V090000 (Visibility). The manufacturer was made aware of the failure. The failure mileage was approximately 75,000.

The vehicle was not yet repaired. The manufacturer was notified of the failure, but no assistance was offered. The failure mileage was 49,000.
Steering
Reports naming the steering system — racks, columns, power-steering assist and steering linkage.
From owner reports, verbatim
While driving the vehicle, the steering column released all of the bearings, allowing for the steering column to move up and down and in and out freely, making it unknown if the steering would follow which way the steering wheel was turned. I reached out to our personal mechanic in regards to this, and had to pay out of pocket. Originally, I thought it was a failed part, but after research, it was brought to my attention of recall 25S94, which my truck did not qualify for. I replace the part at my expense, even though, at this time, the vehicle was only 3 years old. No failure signs appeared.
The contact owns a 2021 Ford F-250. The contact stated that while driving between 40 to 70 MPH and driving over speed bumps, uneven pavement, or potholes, the vehicle was pulling hard to the left, making it difficult to control the vehicle. The contact had experienced the Death Wobble while driving. The vehicle was not taken to be diagnosed or repaired. The contact related the failure to the Customer Satisfaction Program: 20N04 (Steering Linkage Damper); however, the VIN was not included. The manufacturer was notified of the failure and informed the contact that the vehicle was not included in the Customer Satisfaction Program. The failure mileage was 30,600.

I have up to 16 different codes when the check engine light comes on including numerous communication U-codes indicating faulty software download causing no run/no start problems along with severe safety concerns while driving. I returned the vehicle to Swant Graber today July 1, 2026 after 2 weeks being this was their earliest appointment availability. They then deleted the codes and told me to drive it until there is more issues which is a severe safety concern considering the condition of the PCM of this vehicle they returned to now which is unfunctional and not road worthy INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

Body & structure
Reports naming the vehicle structure, frame, body panels, doors, hood and trim.
From owner reports, verbatim
Cab trim is coming off vehicle, causing likelihood of vehicle getting water inside the body/ structure of the vehicle, which can lead to degradation and loss of structural integrity during a collision.
The drivers side door latch does not allow the driver to exist the vehicle as the door lock on the drivers door has only worked intermittently. When the lock doesn’t unlock on the drivers door, I have to use the passenger or rear doors to enter and/or exit the vehicle. The drivers door won’t even allow me to manually unlock the door using the latch—when I grip the latch tight enough to grab the door lock pin it cannot even be pulled up manually, it’s as if it is being pulled back down with heavy force. This is a significant hazard in the event of an emergency as I cannot exit the vehicle. The truck only has -7,000 miles on it and is in like new condition. This began back in August of 2024, but then stopped for a while and returned intermittently, but now seems to be a regular occurrence. This need to be addressed by Ford ASAP.
Brakes
Reports naming service brakes, the antilock braking system, brake hoses, master cylinders or the parking brake.
From owner reports, verbatim
I have been experiencing a brake problem with the trailer brake system in my 2021 Ford F250 Crew Cab. When my travel trailer is connected to the truck the trailer brakes do not always apply and sometimes the brakes apply, then release and then apply again. On my most recent trip, I traveled half way through an intersection before stopping because the trailer brakes did not apply. The trailer brakes were replaced two years ago and were just inspected on January 27th of this year. They were rated in excellent condition. I took the trip soon after the trailer brakes were inspected and I still am experiencing trailer brake controller issues. I tested the condition at low speed just using the brake controller and the brakes applied then released and then applied again. I spoke with Ford today and they stated that the current recall campaign 25V132000 does not apply to my truck. I requested that they consider expanding the recall as my truck is exhibiting the exact conditions described in the recall. I am attempting to get the truck into the local Ford Dealer, but they are backed up four weeks until they can inspect the truck. This is a very dangerous situation and needs to be addressed.

Exterior lighting
Reports naming headlights, tail lights, turn signals and other exterior lighting.
From owner reports, verbatim
All of my rear lights stopped working with no warning. My blind spot monitoring and cross traffic detection also stopped working. This is a serious issue as the light go out intermittenly without warning. Especially dangerous at night. Dealership thinks it might be a faulty harness. Dealership thought they fixed it by cleaning contacts but the very next day they went out again while driving in a snow storm. Luckily no one was behind me because they would not have been able to see me.
The contact owns a 2021 Ford F-250. The contact stated while driving at an undisclosed speed at night, the headlights failed to illuminate as intended. The vehicle was taken to a local dealer where it was diagnosed and determined that the headlights might have been damaged due to heat exposure or chemicals. In addition, upon inspection, the contact noticed an abnormal fuel consumption. The vehicle was not repaired. The manufacturer was made aware of the failure. The failure mileage was approximately 17,000.

Airbags
Reports naming the air-bag system — deployment behaviour, inflators, sensors and warning lights.
From owner reports, verbatim
The contact called to report a failure on a 2021 Ford F-250. The contact, who is a Crash Investigator, stated that the owner of the vehicle was involved in a crash. While the owner was driving at an undisclosed speed in the opposite lane, the driver ended up in a head-on collision. No other information was available. While inspecting the Air Bag Control Module after downloading the information, there was no crash or air bags deployment information retrieved. The contact stated that there was a pattern of the faulty part being distributed from a manufacturing plant for the manufacturer located in Kentucky, because this was not the first issue with a Ford F-250 vehicle experiencing the exact failure. The contact was concerned that if vehicle owners were involved in a crash, the air bags modules would not collect or store data, making it difficult to investigate the crash. The failure was due to the Air Bag Control Module, Event Data Recorder, and Restraint Control Module, which were unable to download the needed information. The vehicle was towed and taken to the insurance company's lot. The manufacturer was notified of the failure. The failure mileage was not available.

On multiple occasions now I have experienced the death wobble after hitting a bump in the road. Today the vehicle shook so violently that it crossed into the lane next to me on a curve. I do not feel safe in the vehicle now and pray that someone addresses this problem asap before an innocent person is killed. This has not been inspected by any dealerships or insurance reps. No warning lamps or other anomalies observed. The vehicle currently has 53k miles on it.
