My front windshield broke from a heat stress crack from the top seal above the ADAS camera/driver assist module and runs down alongside the camera housing and then cracked all the way in front of driver's side above my steering wheel. The car was not moving and was just sitting in my carport. I have had problems with the ADAD system not functioning properly since I bought the car in 2022 from heat related issues.

The entire ADAS system fails when the temperature is over 100°. It has applied the emergency braking system with no cars in front of me. All the warning lights go off and the entire system just fails. Also, my windshield just (2 days ago) randomly cracked while sitting in my carport. The crack begins at the top front windshield seal on goes down right alongside the ADAS camera housing and then goes to the left and down right in front of my driver's side view above the steering wheel. This is a heat stress crack and was not from an impact. There is definitely a current and ongoing issue with the ADAS since I bought the car with 14 miles on it. There is something seriously wrong with the system itself and how it was installed at the factory and is really dangerous.

While driving my 2022 Honda HR-V, multiple Honda Sensing warning messages appeared simultaneously, including front camera system errors. Features such as Collision Mitigation Braking System, Lane Keeping Assist, Road Departure Mitigation, and Adaptive Cruise Control became unavailable. The warnings affect important safety systems and also create a distraction while driving. This appears to be the same front camera defect that many other Honda owners have reported.

Since purchasing my 2022 Honda HR-V in 2024, I have experienced ongoing unintended or “phantom” braking while driving at highway speeds with adaptive cruise control, lane keeping assist, and the Collision Mitigation Braking System activated. When a vehicle is traveling in an adjacent lane—most frequently on the right side—the dashboard displays an orange brake warning, and the vehicle abruptly brakes without driver input despite no obstruction in my lane. This issue occurs approximately four times per week, and the vehicle currently has about 75,000 miles. During each incident, I disengage adaptive cruise control using the steering wheel controls to regain manual control. Although no accidents or injuries have occurred, these sudden braking events have resulted in multiple near-miss situations and present a significant safety concern.

My car was impacted by the exact issue impacting Honda HRV's manufactured in 2022, but my VIN does not show that I am eligible. This is a mistake and I would ask NHTSA to investigate other 2022 HRVs which may be impacted by this recall (information posted below). Summary American Honda Motor Co., Inc. (Honda) is recalling certain 2019-2022 model year HR-V vehicles. The power circuit in the display audio unit was improperly designed. When the engine is started with an ignition key, the cranking can cause the battery's state of charge to drop, which may prevent the display audio from booting up, resulting in the failure to display the rearview camera image. Additionally, vehicles previously repaired with the initial software update received a version that insufficiently corrected the display audio unit boot up procedure, resulting in the failure to display the rearview camera image. Safety Risk If the display audio unit does not boot up, the rearview camera image will not display, which may affect the driver's view and increase the risk of a crash or injury. Remedy Registered owners of all affected vehicles (including previously repaired vehicles) will be contacted by mail and asked to take their vehicle to an authorized Honda dealer. The dealer will reprogram the display audio unit with updated software containing the proper boot up procedure. Owners who have paid to have these repairs completed at their own expense may be eligible for reimbursement, in accord with the recall reimbursement plan on file with NHTSA. Subject: Safety Complaint – 2022 Honda HR‑V – Intermittent Unintended Acceleration While Braking I am submitting this complaint regarding a serious safety defect in my 2022 Honda HR‑V. On multiple occasions, the vehicle has unexpectedly accelerated while I was pressing the brake pedal. This behavior is intermittent, unpredictable, and dangerous. Description of the Issue: While slowing down or coming to a stop, the vehicle will occasionally surge forward despite my foot being firmly on the brake. The engine revs and the vehicle attempts to move forward against braking force. This is not driver error. It occurs without any warning lights, alerts, or diagnostic messages. Operating Conditions When the Problem Occurs: - Low‑speed braking (parking lots, stop signs, traffic - Foot fully on the brake pedal - No cruise control active - No pedals obstructed - Normal road and weather conditions Safety Concerns: This defect creates a clear risk of: - Rear‑end collisions - Pedestrian injury - Property damage Loss of vehicle control The behavior is consistent with an electronic brake/throttle control malfunction, including possible issues with the brake pedal position sensor, accelerator pedal sensor, brake booster pressure sensor, or driver‑assist modules (ABS, ESC, CMBS). I was driving to work when suddenly my car lost motor function and just stopped, despite me pushing the gas pedal. This has happened to me at least 4 times now since owning the car for less than two years. I have almost been rear-ended each time. This time, however, my car did not want to start back up again. It flashed the battery and oil light on my dash and proceeded to tell me to hit the brake and push button to start (even though it was running), so I turned the vehicle off and attempted to start it again like I have in the past when encountering this issue. When I went to re-start the car, it sputtered and wouldn't start until I waited and tried again. I am very concerned, as I have a dog that I have in the backseat and if I get rear-ended, I will sue if there are injuries. I want a call about this immediately, as it is often pushed aside and I am fed up.

Two times I have been driving in busy areas and when I brought my car to a stop at red lights and it’s turned green, my car will no longer move, but stays running so then I have to turn it off and turn it back on for it to gain motor function again. I have almost been rear-ended twice.

When driving on the highway at around 76 mph, the vehicle suddenly presents power loss and won't accelerate nor rev even if I step into the pedal all the way (No light in the dashboard). Going in the high speed lane, I had to stop in the shoulder lane and turn off the vehicle. After turning it on again, a couple of minutes later the problem was back. This would be the third time I have had the problem. Took it to Honda for a checkup and told them that no light came on in the dashboard, and also told them that the problem has been present when going over 70mph, and what they did is try to replicate the problem at 70mph.

Vehicle loses throttle response multiple times a day while traveling on the highway or roads where slowing/stopping isn't frequent. Vehicle will be traveling 50-80mph and will not accelerate even with the pedal on the floor. Vehicle will either maintain speed or start slowing down, occasionally it will rapidly slow down. I will have to fully release the accelerator pedal and lightly slam it all the way to the floor and then the vehicle will decide to respond. A few times while driving in moderate traffic (30-45mph) the throttle will lose all response and will not respond to any pedal input at all for about 5-8 seconds and then will all of a sudden start functioning as intended again. I assume the third issue the vehicle is having with acceleration is related to the first two as well, occasionally it will have very slow acceleration when trying to get up to speed to merge onto the highway. It will take about ¼-⅓ of a mile or about 45-60 seconds to reach 60-70mph. When this occurs I will have the pedal all the way to the floor from the beginning of a fairly long on ramp and by the end of the ramp I will be going no faster than 50mph maybe 55mph. I assume all of this is caused by the CVT transmission or possibly the ECU. I have contacted Honda regarding these issues and they stated that they are aware of these issues and that they are fairly common, and that they also believe that it is a transmission or computer issue, but there is nothing they can nor will do about it because Honda corperate requires them to diagnose every issue through the vehicles computer. So if the vehicle doesn't have a warning light or any DTCs (diagnostic trouble codes) set in the computer they can't (or wont) do anything about it eventhough there is clearly an issue that is entirely able to be diagnosed without a DTC. 90% of the time a bad CVT will never set a DTC. I don't speak on all this because I THINK I know, I speak on this because KNOW I know as I am an ASE certified master technician

My HRV with 1800 miles suddenly stopped in an intersection only after owning the vehicle for 2 weeks. The Honda dealership tested the computer system, drove the car, and found no abnormalities. The dealership then asked that i pick up the car and drive it as they were unable to replicate the problem. If it were to happen again I was to let them know. I made it clear that I was not going to take the car back as I believed that the driving the car was posed a safety issue.

The contact owns a 2022 Honda HR-V. The contact stated that while his mother-in-law was driving 65 MPH, the vehicle accelerated independently to over 100 MPH. Additionally, the contact's mother-in-law depressed the brake pedal; however, the vehicle failed to stop. The brake warning light was flashing. The contact's mother-in-law started pumping the brake pedal and was able to stop the vehicle. The vehicle was towed to the dealer however, the dealer was unable to duplicate the failure. The vehicle was not repaired. The manufacturer was made aware of the failure. The failure mileage was approximately 29,000.

The contact stated while driving at approximately 5 MPH and slowing for a traffic signal, the brake pedal was depressed, and the contact heard the engine rev before the vehicle lunged forward. **I have made the dealership aware of the ongoing situation as well as Honda Corporate but I am waiting on a call back.* A month ago, I had my car on cruise control going around 76mph on flat land. After a while, while driving on flat land, my RPM's shot up to almost 6000 and the speed got up to 88mph in less than 10 seconds. I pulled over, shut the car off so I could calm down before driving again. Later that day, I was driving around 77mph on the interstate again but this time I was NOT using cruise control and my car dropped to around 50mph almost immediately, even with my foot on the pedal all the way to the floor but the car would not accelerate. During this time the car jerked forward really hard (I was told the CVT is why) but also multiple dash lights came on. I had picture and video proof of this, took it to the dealership but they could not find any reason. Since this day, my car has lost speed multiple times while driving but the steering wheel has started locking up the last few times making it impossible to even steer and get off of the roadway while the car is dropping speed. This is a huge liability for myself with my children in the car as well as other drivers on the road, as I was almost rear-ended by an 18 wheeler yesterday when this happened. It does not do this consistently but over the past month, it has happened about 4-5 times a week, sometimes more.

This is a continuation of NHTSA ID Number: 11463644. Secondary issue is minimal acceleration output when entering the freeways. At times, it can take between 20-30 seconds to get 0-60 mph - creating a very dangerous environment with the vehicles behind me braking hard to avoid crashing into my 2022 Honda HR-V Sport. This accident took place on April 19, 2022 - - ten days after I took possession of my new 2022 Honda HR-V Sport from the dealership - - sudden unintended acceleration. The facts are I came home on April 19, 2022. I drove slowly up the driveway. I stopped. I inched forward. I stepped lightly on the gas pedal. Then the vehicle suddenly accelerated without warning at high speed. The hard-hitting impact resulted in damage to the front of my vehicle. The building supplies on the driveway took the full impact of the accident. Otherwise, the vehicle could have crashed through the garage door and hit another vehicle - -likely resulting in serious injuries or death to myself and extensive property damage. American Honda denied my claim to replace ECU and repair body damage to the vehicle after examination.

The SUV decelerated rapidly while driving uphill on the grapevine in Los Angeles. I was driving 70 mph and then the SUV's speed just started to drop and I was pressing down hard on the accelerator. This may haven been an issue with the engine or the transmission. My daughter and I could have been seriously injured or killed since we were surrounded by cars going 70+ mph and no one could have expected our SUV to decelerate so quickly on this stretch of highway. We really could not figure out what to do. The dealer said they could not replicate it, but I have had this happen more than once. I told them how serious this was, but they said they could not help me. I have read online of others complaining about the same issue for both the CRV and the HRV (www.thedrive.com) and I read that this issue has been reported to the NHTSA before so I wonder how the Honda dealer has never heard about this before. The SUV has not been inspected by anyone but the dealer. The SUV also intermittently shakes on the highway as it picks up speed. This also appears to be an issue others have experienced. No warning lamps, messages or other symptoms appeared for either issue. Both issues present safety concerns.
