TL* THE CONTACT OWNS A LAMAZE CHILD SAT SEAT, MODEL NUMBER VIA 1470, MANUFACTURED AUGUST 16, 2011. THE CONTACT STATED AFTER SEVERAL ATTEMPTS, THE BASE FAILED TO DETACH. IN ORDER TO REMOVE THE CHILD SEAT AND BASE THE SEAT BELT WAS CUT. THE MANUFACTURER WAS NOT NOTIFIED OF THE PROBLEM. THE CHILD SEAT HAD NOT BEEN REPLACED WITH A NEW ONE.
TL* THE CONTACT OWNS A LAMAZE REAR FACING CHILD RESTRAINT, MODEL NUMBER: VIA I470, AND MANUFACTURED APRIL 20, 2011. THE CONTACT STATED THAT WHILE ATTEMPTING TO REMOVE THE CHILD SEAT FROM THE BASE, IT FAILED AND THE SEAT COULD NOT BE UNLOCKED FROM THE BASE. THE MANUFACTURER WAS NOT MADE AWARE OF THE FAILURE. THE CHILD SAFETY SEAT WAS NO LONGER BEING USED.
