I am writing to bring to your attention a serious safety issue that occurred with my vehicle. While driving on the expressway, the engine oil burned and the oil warning light illuminated on the dashboard, posing a significant risk to my safety and the safety of others on the road. Specifically, the engine oil system malfunctioned, causing a sudden and complete loss of engine lubrication. This incident can be verified through an inspection of the vehicle, which is available upon request. My safety and the safety of other motorists were severely compromised due to this malfunction. The sudden failure of the engine oil system could have resulted in a catastrophic engine failure, potentially leading to a loss of control of the vehicle at high speeds. Fortunately, I was able to safely pull over to the side of the road, but the situation could have been much worse. Upon experiencing the issue, I had the vehicle towed to Service Center where the problem was reproduced and confirmed by a certified mechanic. They conducted a thorough inspection and confirmed that the engine oil had indeed burned, leading to the illumination of the oil warning light. The inspection report from the service center is available for review. Additionally, I reported the incident to the vehicle manufacturer and my insurance company. Both entities have documented the issue, and I am awaiting further instructions on how to proceed. There has been no involvement from law enforcement as there was no accident or injury resulting from the incident. Prior to the failure, I did not notice any warning lamps, messages, or other symptoms indicating a problem with the engine oil system. The warning light appeared suddenly while I was driving on the expressway, giving me no advance notice of the impending malfunction. Given the severity of this issue, I urge you to investigate this matter thoroughly to prevent any future occurrences. The safety of driver and passengers is of utmost importance.

Engine has oil use and knocking sound that neither Lexus know what is wrong. A lot of Lexus owners are experiencing the same issues with no resolve. Lexus/Toyota needs to fix the issue with these years models.

The brake booster assembly has developed a leak in the pressure accumulator resulting in constant actuation to keep the pressure sufficient. Complete failure is impending with a C1391 code and loss of power brakes. Toyota has offered a CSP to address this issue but only for a very limited time and at the time of creation of said TSP only covered model years up to 10 years old. 2011-2013 CT200h models are not covered by this CSP. Furthermore due to the limited time period in which the CSP was active, many cars were not covered due to not developing fault codes at that time. Part prices alone to resolve this issue from the dealership are above $2000.

At ~101,000 miles, my 2012 Lexus CT200h triggered error code C1307, indicating a failure of the brake pressure accumulator / brake booster system. Prior warnings appeared via dashboard alerts. This caused reduced braking performance, creating a serious safety risk. The issue was confirmed by a Lexus dealer and independently by a certified mechanic, who repaired it. I contacted Lexus Customer Support for coverage under CSP 21LE01, but they denied it due to the 10-year time limit, even though my vehicle was within the 150,000-mile limit. Dealer estimated ~$4,600; I had it repaired independently for $2,000, sourcing parts online at ~50% of dealer pricing. This is a safety-critical defect acknowledged by Lexus under CSP 21LE01. I respectfully request that NHTSA review this defect to ensure Lexus extends proper coverage and prevents future safety risks for other owners.

OUR HORN STOPPED WORKING. WE WERE ADVISED THAT THIS WAS A SPIRAL CABLE ASSEMBLY FAILURE, AND THE AIRBAGS ARE LIKELY AFFECTED. TOYOTA INITIATED A RECALL IN 2014 FOR THIS PART, HOWEVER WE ARE TOLD OUR VEHICLE DOES NOT QUALIFY FOR RECALL.

BEFORE THE BACKUP CAMERA IS DISPLAYED, THE SCREEN RISES FROM THE DASH BOARD AND SAYS "LEXUS' FOR 5 SECONDS AND "ENFORM" FOR 5 SECONDS. DRIVING BETWEEN 45-55 MPH ON US-52, THERE WAS A SUDDEN LOUD NOISE AND INSTANT CHANGE IN WIND NOISE. LOOKING IN THE REAR VIEW MIRROR, DARK DEBRIS WAS SEEN FALLING TO THE GROUND. WHILE DRIVING INTO WORK ON HIGHWAY 98 BETWEEN NEW AUGUSTA, MS, AND HATTIESBURG, MS, THIS MORNING, MY BACK SIDE WINDOW EXPLODED. I THOUGHT IT WAS A GUNSHOT BECAUSE OF THE SOUND, BUT NO BULLET WAS FOUND. THERE WAS NO TRAFFIC. THERE WERE TWO MEN WAITING FOR ME TO PASS SO THEY COULD PULL ACROSS THE INTERSTATE AND THEY STOPPED TO HELP ME. THEY WERE A WITNESS THAT THERE WAS NO TRAFFIC, NO WAY FOR A ROCK TO HAVE BEEN THROWN FROM A PASSING VEHICLE. THE WINDOW SIMPLY EXPLODED. IT MUST HAVE BEEN A WINDOW DEFECT. THE PASSENGER SIDE GLASS ON MY 2012 CT EXPLODED FOR NO PARTICULAR REASON A FEW DAYS AGO. I WAS ON THE HIGHWAY DOING ABOUT 65 MPH AND I SUDDENLY HEARD A LOUD 'BOOM' AND THE SIDE WINDOW WAS GONE... COMPLETELY GONE. NOTHING HIT MY CAR, NO ROCKS, NO BULLETS... NO BRICKS. THERE WAS NO ONE BESIDE ME AND I WAS ON I-81. HAD I OVER-REACTED, I COULD HAVE CRASHED THE CAR. HAD THERE BEEN A PASSENGER IN THE SEAT, SHE WOULD HAVE BEEN SEVERELY CUT FROM THE FLYING GLASS SHARDS AT 65 MPH. I EXPERIENCED CUTS ON MY RIGHT HAND AND ARM. LEXUS, IN A COMPLETELY DISAPPOINTING SHOW OF CUSTOMER "SERVICE" IS REFUSING TO PAY FOR IT UNDER WARRANTY. I AM NOT THE ONLY ONE WITH A CT THAT HAS HAD THIS HAPPEN. First noticed excessive oil usage. Then, engine rumble. Temperature warning light. Took to repair shop who was unable to repair stating ONLY Lexus has tools required to repair. Lexus will not divulge how to repair. Lexus states a letter was mailed to me (never received) in 2016 for a service issue - warranty enhancement. Thus, they are aware of the issue. Now that I’m having issues with the car (2021), the car is too old and has too many miles, thus, Lexus will not repair free of charge under service issue. I am Scared to drive because of hundreds of on line reports that state car can and make completely stop while driving. Toyota is fully aware of the issue and should be help liable to repair under a recall. Note: I’ve had my car repaired for other issues by Certified Lexus since 2016 and the service centers never mentioned the issue/warranty enhancement - a known issue apparently since 2016.

DRIVING UPHILL ON A DIRT ROAD @ ~ 20MPH. GAS ENGINE RUNNING AT THE TIME. FAIRLY STEEP GRADE. ~6% GRADE UPHILL. CAR STARTED TO ACCELERATE AT A RATE I WOULD COMPARE TO MERGING ONTO A HIGHWAY. WAS UNSURE OF WHAT WAS GOING ON AT FIRST SO I DIDN'T ACTUALLY HIT THE BRAKES FOR ABOUT 5 SECONDS AT WHICH POINT I WAS ALREADY AT 45MPH. THE FOUR PASSENGERS IN MY CAR WERE VERY CONFUSED. WE CHECKED THE FLOOR MAT FOR ANY OBSTRUCTIONS AND THAT IT WAS OPERATING CORRECTLY AND IT WAS. I SHUT THE CAR OFF, WAITED A MONTH NITE THEN TURNED IT BACK ON. IMMEDIATELY AS I LET OFF THE BRAKE THE CAR ACCELERATED UP THE HILL. THIS TIME I ONLY ALLOWED IT TO GET UP TO 15MPH BEFORE STOPPING THE VEHICLE. THE BRAKE WAS ACTING AGAINST THE ENGINE PROPELLING THE CAR. BRAKE APPLICATION DID NOT DISENGAGE THE ENGINE FROM TRYING TO MOVE THE CAR FORWARD. I EVEN PUT MY TOES UNDER THE PEDAL TO TRY AND LIFT THE ACCELERATOR MAKING SURE IT WASN'T MECHANICAL ERROR. ZERO MOVEMENT, THE PEDAL WAS IN PLACE. AFTER HAVING LEXUS "ANALYZE" MY VEHICLE THEY CALL ME BACK AND TELL ME "WE CAN'T FIND ANYTHING WRONG WITH IT" AND THAT ALSO THEY DID NOT UPDATE ANY COMPUTERS OR RESET ANYTHING OR SWAP OUT MY BATTERY. THEY EXPECTED ME TO COME PICK UP MY CAR WITHIN THE WEEK OR THEY SAID THEY WOULD REPORT THEIR LOANER CAR STOLEN AND I WOULD BE CHARGED FOR IT. MY ONLY OPTION GIVEN WAS TO GET BACK INTO MY VEHICLE. MY SUSPICION IS MY BATTER ALLOWED ITSELF TO OVERCHARGE WHILE DRIVING DOWN LONG STRETCHES OF MOUNTAIN ROADS AND THEN WAS NOT BEING DRAINED ON THE STRENUOUS UPHILL SECTIONS. THIS OVERCHARGE APPEARED IN THE FORM OF ULTRA ENGINE ASSIST WHICH I EXPERIENCED WITHIN 1 MILE OF STARTING MY CAR AFTER A 4 HOUR HIKE.

THE VEHICLE WAS TAKEN TO A DEALER FOR DIAGNOSIS AND HE WAS STILL AWAITING A FINAL DECISION ON WHETHER IT WAS A DEFECT OR NOT. THE MANUFACTURER WAS NOTIFIED AND STATED THAT THEY WOULD FURTHER INVESTIGATE. THE FAILURE AND CURRENT MILEAGE WAS 3,942.
