The contact owns a 2010 Lexus LS460. The contact stated while driving approximately 65 MPH, the brake pedal was depressed and extended to the floorboard. The contact stated that the ABS, brake, and Traction Control warning lights were illuminated. The vehicle was taken to the local mechanic who diagnosed that the ABS module was faulty and needed to be replaced. The vehicle was taken to the local dealer to be diagnosed. The vehicle was not yet repaired. The manufacturer was not contacted. The failure mileage was approximately 170,000.

TL* THE CONTACT OWNS A 2010 LEXUS LS460. THE CONTACT STATED WHILE DRIVING AT VARIOUS SPEEDS, A BRAKE FAILURE WARNING MESSAGE APPEARED ON THE INSTRUMENT PANEL. THE CONTACT STATED THAT SEVERAL UNKNOWN WARNING LIGHTS WERE ILLUMINATED. THE VEHICLE WAS TAKEN TO WILKIE LEXUS (568 LANCASTER AVE, HAVERFORD, PA 19041) WHERE IT WAS DIAGNOSED WITH THE BRAKE ACTUATOR NEEDING TO BE REPLACED. THE VEHICLE WAS NOT YET REPAIRED. THE MANUFACTURER WAS INFORMED OF THE FAILURE AND OFFERED TO COVER 20% OF THE PARTS COST. THE FAILURE MILEAGE WAS APPROXIMATELY 121,000.

The contact owns a 2010 Lexus LS460. The contact stated that while driving at 20-25 MPH, the warning message 'Have Your Dealer Check the VGRS System" was illuminated. The contact stated that he also noticed that the steering wheel was no longer centered. The contact was able to continue to drive the vehicle. The contact stated that the failure reoccurred while driving and the warning message remained illuminated. The dealer was notified of the failure and advised the contact that the vehicle could not be repaired under NHTSA Campaign Number: 10V218000 (Steering) because the recall had expired. The manufacturer was not notified of the failure. The vehicle was not diagnosed or repaired. The failure mileage was approximately 120,000.

TL*THE CONTACT OWNS A 2010 LEXUS LS460. WHILE TURNING APPROXIMATELY 5 MPH, THE CONTACT HAD TO USE FORCE TO MANUALLY RETURN THE STEERING WHEEL TO THE FORWARD POSITION. THE FAILURE OCCURRED INTERMITTENTLY. THE VEHICLE WAS TAKEN TO AN AUTHORIZED DEALER WHO STATED THAT THE VEHICLE WAS FINE. THE VEHICLE HAD NOT BEEN REPAIRED. THE FAILURE MILEAGE WAS 100 AND THE CURRENT MILEAGE WAS APPROXIMATELY 6,000.

TL* THE CONTACT OWNS A 2010 LEXUS LS460. WHILE DRIVING AT ANY SPEED, THE VEHICLE HESITATED WHENEVER THE CONTACT ATTEMPTED TO ACCELERATE. THE RPMS INCREASED AND THE VEHICLE MADE A GROWLING NOISE, BUT DID NOT ACCELERATE. THERE WAS NO WARNING INDICATOR. THE FAILURE RECURRED SEVERAL TIMES AND THE VEHICLE WAS TAKEN TO A LOCAL DEALER WHERE IT WAS DIAGNOSED THAT THE ISSUE WAS NORMAL FOR A NEW VEHICLE. THE FAILURE RECURRED AND THE VEHICLE WAS TAKEN BACK TO THE LOCAL DEALER WHERE IT WAS DIAGNOSED THAT THE FAILURE WAS DUE TO THE TYPE OF FUEL BEING USED. THE MANUFACTURER WAS MADE AWARE OF THE FAILURE AND ORDERED THE LOCAL DEALER TO PERFORM EXTENDED DIAGNOSTIC TESTING ON THE VEHICLE. THE DEALER WAS UNABLE TO REMEDY THE FAILURE. THE FAILURE MILEAGE WAS APPROXIMATELY 28,000.

IN ADDITION, THE CONTACT NOTICED A FUEL LEAK NEAR THE FILLER NECK. THE VEHICLE WAS PREVIOUSLY REPAIRED FOR THE FAILURE WHERE THE CHARCOAL CANISTER, FUEL FILLER NECK AND A PIPE SUB WERE REPLACED. THE VEHICLE WAS TAKEN BACK TO THE DEALER BECAUSE THE FUEL FILLER NECK BEGAN LEAKING AGAIN. THE MANUFACTURER WAS SENT A LETTER IN REGARDS TO THE FAILURE BUT HAD NOT RESPONDED. THE VEHICLE WAS NOT FURTHER REPAIRED. THE FAILURE MILEAGE WAS 4,486.

TL* THE CONTACT OWNED A 2010 LEXUS LS460. WHILE DRIVING 55 MPH, THE VEHICLE BEGAN TO SKID ON THE FREEWAY. WHEN THE CONTACT DEPRESSED THE BRAKE PEDAL, THE VEHICLE FAILED TO STOP. AS A RESULT, THE VEHICLE CRASHED INTO A SIDE EMBANKMENT, BOUNCED BACK ONTO THE FREEWAY, TURNED 360 DEGREES, AND CRASHED INTO THE EMBANKMENT AGAIN. THE VEHICLE THEN BOUNCED BACK A SECOND TIME ONTO THE FREEWAY BEFORE IT CAME TO A FULL STOP. BYSTANDERS HELPED TO COAST THE VEHICLE OVER TO THE SIDE OF THE ROAD. THE AIR BAGS FAILED TO DEPLOY. THE DRIVER SUSTAINED NECK AND BACK INJURIES THAT DID NOT REQUIRE MEDICAL ATTENTION. THE FRONT PASSENGER SUSTAINED A SPINAL FRACTURE, EYE SOCKET FRACTURE, AND BACK INJURIES THAT REQUIRED MEDICAL ATTENTION. THE REAR PASSENGER SUSTAINED HEAD INJURIES THAT DID NOT REQUIRE MEDICAL ATTENTION. A POLICE REPORT WAS FILED. THE VEHICLE WAS TOWED TO THE CONTACT'S RESIDENCE. THE VEHICLE WAS NOT TAKEN TO A DEALER OR AN INDEPENDENT MECHANIC FOR DIAGNOSTIC TESTING. THE VEHICLE WAS DESTROYED. THE MANUFACTURER WAS NOT NOTIFIED OF THE FAILURE.
