Traveling distance of 0.2 miles in a residential complex with 15 mph speed limit, on an uneven road that loops around. The road goes on a slight incline as it bends towards the left reaching my parking destination, a 2-car stall. At slow speed I made a right turn into the stall (a break-release-break maneuver) when the car lurched and suddenly picked up speed. Visualizing a wide pedal in the middle I hard-pressed the brakes with my right foot, same time thinking I should not pass the white post (about halfway of the stall) instinctively put my left foot over the right to press harder on the brakes. Next thing I know the car stopped and noticed white particles floating around. The airbags deployed - front, knee and side airbags activating roadside assistance (SOS). The engine would not start, and I can’t open the driver’s side door and window. The car crashed into the guardrail and right front wheel went over the concrete bump on the ground. I sustained chest and leg injuries. There is no other passenger, or third party involved. I did not hear or see any indication of system alerts or warnings before and during the incident. Car was totaled. While waiting for assistance and thinking about what happened it never occurred to me to wonder if I mistakenly stepped on the gas until the police at the scene brought it up. I had at least 3 lurching incidents on this vehicle, but the brakes worked. I could not replicate these incidents as it happened randomly.

WHILE DRIVING ON HIGHWAY 101 NORTHBOUND AT ABOUT 65 MPH BETWEEN 3RD AND DORE EXIT, I HEARD AN EXPLOSION OVERHEAD. I LOOKED AROUND AND DIDN'T SEE ANYTHING. BUT THEN I HEARD A WIND NOISE ABOVE ME. MY SUNROOF COVER WAS CLOSED AT THE TIME. IT WAS ABOUT 60 DEGREES OUTSIDE, AND I HAD BEEN DRIVING FOR ABOUT 35 MINUTES FROM SUNNYVALE. MY WINDOWS WERE NOT OPEN (SO NO PRESSURE CHANGE), AND NOTHING OUT OF THE ORDINARY WITHIN THE CAR. I WAS ON MY WAY TO SAN FRANCISCO FOR A COVID TESTING APPOINTMENT. I MADE IT TO THE APPOINTMENT, AND AFTERWARDS, ABOUT 20 MINUTES AFTER I HEARD THE INITIAL EXPLOSION, I PULLED OVER TO THE SIDE OF THE ROAD AND ASSESSED THE DAMAGE. MY SUNROOF GLASS WAS GONE. NOT MUCH GLASS INSIDE THE SUNROOF COVER, AND THE GLASS THAT WAS STILL ON THE SUNROOF FRAME WAS BENT UPWARDS, INDICATING THE SUNROOF EXPLODED UP AND OUTWARDS, SO I DIDN'T THINK SOMETHING HAD HIT IT. TSB: Some 2013 ? 2020 model year Lexus vehicles may exhibit a hard start and/or MIL ON condition with one or more of the following Diagnostic Trouble Codes (DTCs): ?DTC P0171 and/or P0174 (System Too Lean Bank 1/Bank 2) ?DTC P0087 Fuel Rail / System Pressure ? Too Low Techstream data list analysis may reveal positive fuel trims including a no pressure condition verified by a manual fuel pressure gauge. Follow the Repair Procedure in this bulletin to address these conditions.

DL: On December 20, 2022, a settlement of claims for certain low pressure fuel pumps was approved by the federal court. The settlement includes Lexus?s agreement to provide a Customer Support Program for certain 2013-2020 model year (MY) vehicles originally equipped with low pressure fuel pumps repaired under Safety Recall 20LA01 or Special Service Campaign 21LC01. The purpose of this Customer Support Program letter is to help clarify how to administer this coverage in accordance with the settlement.

EQUIPMENT:ELECTRICAL:NAVIGATIONAL SYSTEM(GLOBAL POSITIONING SYSTEM)
TSB: Some 2018 ? 2022 model year Lexus vehicles equipped with the Enform? 2.0 Navigation System, require a Special Service Tool (SST) USB flash drive to update the map, points of interest, and system software. The SST is used with applicable vehicles to deliver the update, however, the process for generating license keys differs from previous generations. Before attempting to perform a map update, the expiration date of the dynamic subscription (stored in the head unit), must be checked. If the expiration date has not passed, a license key may be generated without the purchase of an activation card number (ACN). Updating the map without the use of an ACN is considered an Embedded Navigation Update (refer to pg. 10) and performing this update does not extend the subscription to dynamic map services. If the subscription period has expired, or is expiring within the next 90 days, it should be recommended to the customer to purchase an ACN. Utilizing an ACN, will initiate the Dynamic Navigation Update (refer to pg. 4). This Dynamic Navigation Update will perform the embedded navigation update and provide dynamic map services for a period of two years from the date that the Dynamic Navigation Update is performed. ACNs are provided in a pamphlet available through the normal parts ordering system
POWER TRAIN
TSB: Some 2013 ? 2022 model year Lexus vehicles equipped with a 2WD drivetrain and the F Sport package may exhibit a clicking noise from the front wheel area during slow (parking lot) speed turns. The clicking noise that can be heard during sharp turns is a normal condition for certain vehicles originally equipped with a lightweight two-piece aluminum rotor assembly. For customers who find this objectionable, an alternative steel rotor is available. Before performing the repair in this bulletin, it is recommended to review the visual differences of the steel rotor with the customer.
