The contact stated that while driving approximately 10 MPH, the steering wheel suddenly malfunctioned and erroneously self-turned to the left, causing the vehicle to crash into a pole, where the vehicle came to a stop. The damage was to the front of the vehicle. During the incident, no injuries were reported, and no police report was taken. The cause of the failure was not determined. The manufacturer was notified of the failure. The local dealer was not yet contacted. The failure mileage was 5,700.

The contact owns a 2024 Toyota Corolla Cross Hybrid. The contact stated that while driving 20 MPH, the Forward Collision Avoidance, Automatic Emergency Braking was unintentionally activated. There was no warning light illuminated. The contact turned off and restart the vehicle and continued driving; however, while driving at 20 MPH, the failure recurred, causing the vehicle to come to a complete stop. There was no warning light illuminated. The vehicle was taken to a local dealer, where the vehicle was diagnosed, and the contact was informed there no failure was found with the vehicle. The vehicle was not repaired. The contact stated that later, while driving on the highway at 60 MPH, the Automatic Emergency Braking warning light remained illuminated. The vehicle was taken back to the same dealer, where the vehicle was diagnosed; however, the contact was informed that no failure was found with the vehicle. The vehicle was not repaired. The manufacturer was notified of the failure. The approximate failure mileage was 14,000.

The contact stated that while driving at night, the low beam headlights failed to properly illuminate and the headlights were extremely dim, creating a visibility hazard for the driver. The contact stated that upon inspection, the contact discovered that the inside of the plastic headlight covers had several abrasions and cracks. The cause of the failure was not yet determined. The local dealer and manufacturer were informed of the failure, but no assistance was provided. The failure mileage was 100.

The contact stated while driving at night at approximately 35 MPH with the low beam headlights activated, there was a rectangular shadow cast ahead by both headlights. The shadow was cast directly to the front and the road was not fully illuminated. The contact stated that while using the high beam setting the shadow was not present. The contact stated that while driving at an undisclosed speed, the electronic braking system malfunctioned, and a message to contact the dealer was displayed. While at an intersection, the Pre-Collision System engaged, and the brakes seized with an oncoming vehicle approaching. While entering the parking lot, the brake pedal was depressed, but the vehicle failed to respond. The brake pedal was depressed to the floorboard.
