The contact owns a 2022 Toyota RAV4. The contact stated while the contact's wife was driving at an undisclosed speed, the error message indicated that a malfunction in the pre-collision system. The vehicle was taken to the dealer where it was diagnosed with a failed forward recognition camera. The contact was informed that the forward recognition camera needed to be replaced. The vehicle was not repaired. The contact stated that the failure was a known failure of the vehicle. The manufacturer was made aware of the failure and a case was filed. In addition, the contact requested a good will repair, however the request was denied. The contact was advised to contact the NHTSA Hotline and report the failure. The failure mileage was approximately 60,000.

The moon roof glass seems somehow dangerously tight or prone to exploding. This could endanger passengers and other vehicles. Out of seemingly nowhere the moon roof exploded while driving off the high way. I saw no flying object. We were not behind other vehicles that could have kicked something up. We hadn’t gone under a bridge. Glass began to crack and tinkle and cascade off the vehicle, possibly endangering other cars behind us. Also, the broken glass might have fallen in onto passengers. We slowed and exited the highway, and inspected the glass. There did seem to be one location where some object might have hit first, causing the shatter. But the evidence of a single impact point is not strong. We will never know exactly why the Moon roof exploded. I tapped the glass moon roof extensively and we drove home. There was no warning. The vehicle is available to inspect, but is getting repaired tomorrow. The repair shop has inspected it. The insurance people have seen the report from the body shop and will pay ask cost above my deductible. Total repair cost estimated at $5600 minimum at this time

The contact stated that on several occasions while driving at various speeds, the Pre-Collision braking system activated when there was no vehicle or object nearby, and the vehicle slowed down unintendedly. The vehicle was taken to a dealer however, failure was not able to be replicated. The manufacturer was not notified of the failure. The failure mileage was 30,511.

The contact owns a 2022 Toyota Rav4 Hybrid. The contact stated while driving 30 MPH in rainy and cold weather, a message indicating that the Pre-Collision sensors of the vehicle were disabled was displayed. Additionally, the instrument cluster turned off. The contact stated that as her husband veered to the side of the road and wiped the front sensors on the front bumper, the instrument cluster turned on temporarily. The contact's husband wiped the front sensors on the front bumper several times; however, the failure persisted. The contact stated upon investigating the failure online, she became aware that the failure mainly occurred while driving in cold, rainy and snowy conditions. The dealer and the manufacturer were not notified of the failure. The vehicle was not diagnosed or repaired. The failure mileage was approximately 2,000.

The contact owns a 2022 Toyota RAV4 Hybrid. The contact stated that while driving approximately 70 MPH, the steering wheel suddenly began to turn to the left and the right causing the vehicle to move from lane to lane. Additionally, depressing the brake pedal did not immediately slow or stop the vehicle. The driver was eventually able to regain control of the steering wheel and stopped the vehicle. After turning off and restarting the vehicle, the failure was no longer present and the vehicle operated as normal. The vehicle was later towed to the local dealer and an engineer from the manufacturer was sent to evaluate the vehicle. The contact was informed that during the incident the lane assist system had activated and attempted to compensate when the driver attempted to steer the vehicle back into the proper lane. No failure was determined. The contact notified the manufacturer of the failure. The VIN was not available. The failure mileage was 800.
