Electrical
Reports naming the electrical system — wiring, battery, charging, control modules, infotainment and electrical accessories.
From owner reports, verbatim
The car's lithium batteries caught fire to the point of smoldering. This happened at the end of the charge cycle. The car was parked in our garage and if the fire department did not come to help, the fire could have intensified and burned down our whole house. We are very thankful we caught it so quickly. No warnings or issues were apparent with the vehicle when it was parked and put on the charger.
What component or system failed or malfunctioned, and is it available for inspection upon request? - Internal charging system or vehicle battery pack malfunctioned & caught fire. Vehicle is available for inspection now, but not sure about when it is removed from our property. How was your safety or the safety of others put at risk? - It was a big fire! Close to structures, vehicles & dry foliage – super lucky it didn't spread. Smoke was very noxious and affected our entire neighborhood. Neighbors have reported respiratory irritation since the fire. I now have a cough, burning throat and headaches. Has the problem been reproduced or confirmed by a dealer or independent service center? - There's a burned out van at the end of my driveway... specific instance of this problem does not need confirmation Similar issues are reported on the internet. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? - Not yet. Chrysler says it'll take 2-3 days for issue to be "escalated". Insurance is sending someone. Fire department HazMat crew verified that the battery pack is now "cold" but toxic chemicals are a public safety hazard. Oakland Fire incident #[XXX]. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? - July 2022 the vehicle refused to charge & displayed a "turtle" icon. Took it to the dealer and told of the charging/fire recall. Stopped charging until all the recall parts were available which wasn't until August 2023. After repair, we were told the car was OK & it was safe to resume charging the car. Other details: - The vehicle caught fire while charging, engulfed in flames, had to be extinguished by Fire department, total loss. Car plugged in at ~4:30pm and we heard a "pop" at ~8:20pm. So the battery had reached 100%. Ran out and there was small fire dripping from middle underside of the car– quickly spread & consumed the entire car INFORMATION REDACTED PURSUANT TO THE FREEDOM OF I

I have been trying to take care of this urgent safety recall since February 2023. The local dealership (4520 Shelbyville Rd, Louisville, KY 40207) keeps telling me that they don't have a "loaner" care to provide to me while my vehicle is being repaired. I am a single father with 7 year old and primary care giver to 77 year old mother with Alzheimers, I must have a replacement car while mine is being repaired especially since this is the manufacturer responsibility. Please help.

I have Pacifica Hybrid 2017 model minivan with a lot of problems. More than 6 times the minivan was not starting and if starting it would have all the check engine (so i needed to leave the minivan with 2 small kids and take a car service ) some of the signs that would all of a sudden start are service Hybrid Vehicle system , service electric throttle control park sense unavailable ,Automatic High Beam Disabled ,press break to prevent rolling ,Blind spot alert temporarily unavailable in the middle of the road while I am driving . I towed the car to Chrysler and they won't pay for it as it was a hybrid problem, and it was under warranty. The dealer agreed that it was a hybrid problem under warranty, and they would fix it as it was a problem from the manufacturer. In a week the dealer said it was fixed. After I was taking the van home and while i was on the road all the signs came up on the control panel. It has all sorts of warning on the panel as mentioned above. I realized the car had the same issue. It won't let me change the gears and stopped completely when I came home. Then I called the dealer and complained, and they said to bring it back however they won't help me with the towing service. Again aftre 1-2 days the car started so i took to the dealer they said they have found the problem they don't know so Chrysler tech team is working on it. Then after 2 weeks they said they have fixed the car and also test drove . However i came home and after 3-4 more days the car still had same issues. It stopped on the highway in the. I managed to move to the rightmost lane and stopped over there on highway. This was a very scary incident. I gave the van back to the dealer and Chrysler reps again said they wil look at it . However, after 4 th attempt I gave them and brough it back . The problem still persists. No help from Chrysler or the dealer. each time they take more than 3-4 weeks and leave you on with no rental help as well.
REPEATED, VIDEO DOCUMENTED OCCURENCES OF VEHICLE STALLING, LOSING POWER STEERING, AND LOSING ALL DRIVER-ASSIST FEATURES, TO INCLUDE TRACTION CONTROL, BLIND SPOT MONITORING, POWER STEERING, FORWARD COLLISION AVOIDANCE, ADAPTIVE CRUISE CONTROL, AIRBAGS, AND OTHER SYSTEMS. THE PROBLEM HAS REPEATEDLY OCCURRED WHEN THE VEHICLE IS IN MOTION; HAS OCCURRED ON A HIGHWAY AS WELL AS IN PARKING LOTS. THE VEHICLE WILL SUDDENLY EXHIBIT 'HAYWIRE' BEHAVIOR, WHICH INCLUDES FUEL AND POWER GAUGES RANDOMLY FLUCTUATING UP AND DOWN, AND ALL ABOVE SAFETY SYSTEM WARNINGS INTERMITTEDLY INDICATING THEY ARE OFFLINE. IN EXTREME SCENARIOS, POWER STEERING WILL BE LOST. ADDITIONALLY, IN ONCE SCENARIO, THE 'HAYWIRE' BEHAVIOR PRECLUDED THE ABILITY TO TURN THE VEHICLE OFF WITHOUT RESORTING TO DISCONNECTING HARD-WIRE BATTERY CONNECTIONS. MULTIPLE VIDEOS HAVE BEEN MADE DOCUMENTING THE BEHAVIOR. PREVIOUS ATTEMPTS TO RESOLVE WITH MANUFACTURER RESORTED IN NO CORRECTIVE ACTION DUE TO 'FAILURE TO DUPLICATE' THE ISSUE, DESPITE HAVING VIDEO SHOWING THE OCCURRENCE. ORIGINALLY BROUGHT TO DEALER AND NHTSA ATTENTION IN 2020 (NHTSA COMPLAINT #11292415). DEALER HAS DONE NOTHING TO RESOLVE THE ISSUE AND IN THE PAST WEEK THE ISSUE HAS RECURRED, NOW MORE CONSISTENTLY THAN THE INCIDENT PREVIOUSLY REPORTED. WITHIN THE PAST WEEK, THE VEHICLE HAS STALLED TWICE, RESULTING IN COMPLETE LOSS OF POWER STEERING. THE "HAYWIRE" BEHAVIOR CONTINUES, WITH PERSISTENT WARNINGS AND LOSS OF ESSENTIAL SYSTEMS, AND THE "HAYWIRE" BEHAVIOR INCREASES DISTRACTIONS BY IMPOSING FALSE BLINDSPOT WARNING INDICATOR LIGHTS AND INTERMITTENTLY CHANGING BRIGHTNESS OF THE ENTIRE ELECTRONIC DISPLAYS AT RANDOM, AS WELL AS PERSISTENT AND INTERMITTENT LOSS OF FUNCTIONALITY OF ESSENTIAL SAFETY SYSTEMS, INCLUDING AIRBAGS AND OTHER DRIVER SAFETY FEATURES. VIDEOS OF THE BIZARRE, "HAYWIRE BEHAVIOR" ARE AVAILABLE UPON REQUEST. [XXX] INFORMATION Redacted PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6).

THE BRAKES ON MY VEHICLE RANDOMLY GO OUT. SOMETIMES THE BRAKES GO OUT LONG ENOUGH THE ENTIRE CAR STARTS SCREAMING AT YOU TO BREAK BECAUSE YOU GET TOO CLOSE TO THE VEHICLE IN FRONT OF YOU. IT'S UNPREDICTABLE. THE CAR HAS ALWAYS BEEN IN MOTION WHEN THIS HAPPENS.
WHEN BACKING UP IN A NEW POORLY LIGHTED PARKING DECK, THE SAFETY BRAKING SYSTEM AND BACK UP CAMERA WITH ASSOCIATED WARNING BEEPS DID NOT ENGAGE. THE RESULTING CRASH DAMAGED THE BUMPER, REAR LIFTGATE, REAR WINDOW, AND ME.*BF *TR

I BOUGHT A NEW PLUG IN HYBRID CAR WITH AN ADVERTISED "AUTOMATED EMERGENCY BREAKING" AND 32 MPG GAS ONLY MILAGE, AND MPGE OF 84. IT DOES NOT HAVE THE AUTOMATIC BREAKING, AND GETS 24 MPG ONLY. I HAVE HAD 25 DOCUMENTED COMMUNICATIONS WITH THE DEALER, CHRYSLER CUSTOMER SERVICE (CASE 33518003) WHICH WAS "ESCALATED" TWO LEVELS - BUT NEVER ADDRESSED. THE DEALER PROPOSED I SELL IT BACK TO THEM AS A "USED CAR" AT A HUGE DISCOUNT - AND I WANT A FULL REFUND. NORTH CAROLINA LEMON LAW OFFICIALS (MR. DAVID FOX - 1-877-5-NO-SCAM) SAID I SHOULD TAKE CASE TO YOUR AGENCY.

Visibility
Reports naming windshields, glass, wipers, defrosters and mirrors.
From owner reports, verbatim
THE REAR VIEW BACKUP CAMERA FAILS TO FUNCTION (GOES VIRTUALLY DARK) UNDER CERTAIN LIGHTING CONDITIONS, PRESENTING A CONDITION THAT IS FAVORABLE TO COLLISIONS WITH VEHICLES, OBJECT, OR PEDESTRIANS. THE WEB HAS PLENTY SUCH COMPLAINTS ON THIS VEHICLE, AND THE PROBLEM MAY NOT BE SPECIFIC TO THE HYBRID. TWO SPECIFIC SCENARIOS WHERE I EXPERIENCED THE PROBLEM: 1) IN A PARKING GARAGE, IN BROAD DAYLIGHT, WHEN BACKING OUT OF A SPACE, AND 2) WHEN BACKING OUT OF THE GARAGE IN MY HOUSE AT NIGHT. TO SPECULATE ON THE CAUSE: IN BOTH OF THESE SCENARIOS, LIGHT (EITHER FROM THE SUN OR A LAMP) WAS HITTING THE FRONT OF THE VEHICLE, WHILE THE AREA BEHIND THE VEHICLE WAS RELATIVELY DARK. IT IS POSSIBLE THAT LIGHT SENSORS ON THE VEHICLE MISCALCULATE THE AMOUNT OF AVAILABLE LIGHT AND SO FAIL TO ADJUST THE LIGHT LEVEL ON THE CAMERA. WE PURCHASED THIS VEHICLE NEW ABOUT A MONTH AGO AND THERE ARE NO OPEN RECALLS ON IT.

Reports naming the vehicle structure, frame, body panels, doors, hood and trim.

The driver-side rear sliding door is not closed securely and it reopened while closing without any other button access and obstacles on the door. But seems like it was fixed with an ECU software update. I have a problem with that and it was fixed with a repair visit last week. That means it was a known issue, but I must pay the repair cost. I have a short video clip to show the problem and my dealership was able to reproduce same symptom while they doing a diagnosis.
