A recent over-the-air (OTA) software update deployed by Ford Motor Company to my 2025 Ford Explorer ST introduced a primary safety hazard and severe system latency. PASSENGER MIRROR FAILURE: Approximately 10% of the time upon vehicle startup, the passenger-side power-folding exterior mirror fails to unfold. This leaves the driver with no right-side mirror visibility, creating an immediate blind-spot hazard and violating FMVSS 111 visibility standards. APIM / NAVIGATION LATENCY: The software update severely degraded processing in the Accessory Protocol Interface Module (APIM). Navigation display systems lag behind the actual vehicle location by approximately 0.1 miles, causing delayed routing information and significant driver distraction. System resets (soft reboots and full master resets) fail to fix the issues. Upon contacting Ford In-Vehicle Technology support, representatives acknowledged the issue but refused to log or escalate the side-mirror failure as a safety defect.
the rear passenger door flew open while I was driving on a public road, with absolutely no warning or dashboard alert to indicate it was unlatched

I was making a left hand turn with incoming traffic. I had pleanty of time to make the turn. Next thing I know my forward pre collision alarm goes off, locks up breaks, alarms started going off. It only lasted a couple seconds, but that’s all it takes to killed.
When going around a curve the steering wheel is sticking on the right. I have to tighten my grip on the wheel and manually straighten the wheel. This happened to me several times today on my 50 min trip. This was very frightening to me making me scared the wheel would stick and I have no control of the car while driving. I purchased this car on sat June 27. It just started today July 3. I am planning to take it back to the dealership on Monday.
Brakes

The contact owns a 2025 Ford Explorer. The contact stated that while driving 50 MPH, the brake pedal was depressed; however, the vehicle failed to stop as intended. The vehicle crashed into the driver's side of another vehicle. The air bags failed to deploy. The vehicle was towed to a local tow yard. The contact was transported to the local medical center for medical attention. The contact sustained bruises and contusions during the crash. The contact sustained a continuous headache after the crash. The contact was receiving continuous medical treatment. A Police report was filed. The vehicle was scheduled for an inspection by the insurance company within days. The manufacturer was made aware of the failure, and a case was filed. The failure mileage was approximately 20,000.
On repeated occasions the vehicle’s braking system has malfunctioned without driver input, including uncommanded automatic brake engagement at highway speed and, most recently, a failure of the braking system to respond as commanded. These malfunctions occur without warning and directly impede my ability to control and safely operate the vehicle. On at least one occasion the malfunction occurred while I was driving on the highway with my three minor children in the vehicle and nearly resulted in a major collision.

Hybrid Fault - Service charging system fault displayed. Verified concern. Hooked up IDS and checked for codes. Multiple codes in multiple modules present for engine restart, battery, and low voltage conditions. Hooked up Ford battery tester and performed battery test on both the 12-volt main and auxiliary batteries - main 12-volt battery failed at 70cca. Received warranty code: 11VBE9140L3K and ACES code: HG1HR. Replaced 12-volt main battery and reassembled vehicle. Performed BMS reset, cleared codes, and retest, OK.
2025 Hybrid PIU/Explorer) for a CEL with PCM codes P0A1E:00:A8 for a starter/generator control module fault.

For the second time now, When changing out wiper blades, the wiper arm has sharp points which easily allow the arm to shatter the windshield. To change the wiper you have to push a button and pull quite hard which can easily cause the wiper arm to slip out of your grasp . This can cause the arm to shatter the windshield even at an inch away.
Steering

Steering fails to self-center after slight bear-left such as lane change manuver or steering to track a gentle left bend in road, typically holding ~10 degree left position of the steering wheel. Center feel is "notchy." Behavior is almost completely a bear-left problem and only a slight similar behavior in bear-right condition. Self-centering from hard left seems to be correct in fully returning wheel to center. This behavior has been confirmed by many owners' complaints posted online, and is a problem unique to the Explorer model and 2020+ generation. Video demonstrating problem while driving: [XXX] Explorer ST Owner Forum post confirms similar behavior by others: [XXX] My dealer is treating this as "not a problem, they all do this." No other vehicle in my life (dozens) has ever exhibited this behaviour of steering not returning to center while moving. After delivery I returned the car almost immediately to the dealer on noticing the steering just felt "weird." They found nothing. I narrowed the behavior down to the specific circumstance of bear-left and notchy on-center feel, returned it again, and again they found nothing. With ~3000mi, no change in behavior --"break-in" is long past. I know how to correct for the issue, but if I loan out the car? While it can be called a fit and finish flaw, on the outside it could be a safety issue for unfamiliar drivers. Engine

The contact owns a 2025 Ford Explorer. The contact received notification of NHTSA Campaign Number: 26V122000 (Engine and Engine Cooling). The contact scheduled an appointment at the local dealer after being notified of the recall. The dealer initially stated that the part would be available within a week, but the contact was subsequently informed that the part was on backorder and there was no estimated date when the part would be available. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The contact had not experienced a failure.
I am reporting a severe safety hazard regarding an immediate, drastic increase in fuel consumption and erratic fuel range calculations following a manufacturer recall repair. My 2025 Ford Explorer ST-Line was operating perfectly until a Ford Mobile Service technician performed safety recall 26S21 (IPMA Reset). According to official Ford manufacturer specifications, this SUV has an expected driving range of up to 501 miles on a full 17.9-gallon tank of fuel. However, immediately following this software update, the vehicle's total driving range drastically dropped. On a 100% full tank of gasoline, the dashboard's "Distance to Empty" (DTE) display now only reads 316 miles left to empty—nearly 200 miles below the manufacturer's expected capability. Furthermore, during a subsequent continuous, short 1.8-mile drive near my residence with no stop-and-go traffic, the DTE display rapidly plummeted from 15 miles down to 5 miles remaining, dropping visibly every two seconds. The vehicle is physically burning through gasoline at an accelerated, unsafe rate. This creates an immediate safety risk. If the vehicle is suffering from an uncalibrated software glitch in the powertrain/fuel modules due to the recall update, or if the procedure triggered a mechanical fault, the vehicle is at high risk of a sudden highway stall or leaving the driver stranded in live traffic due to inaccurate fuel signaling. I request that NHTSA look into whether the software installation process for recall 26S21 can corrupt engine/fuel management maps or trigger fuel delivery faults on the 2.3L EcoBoost platform.

This vehicle was a rental car. The rear door opens and closes at random times. On multiple occasions even after locking the car, we would come out to the door being open. The door came down on its own and smacked my wife as well as hitting my [XXX] daughter in the head. While driving down the highway that rear door decided to open on its own putting my family in immediate grave danger. The tailgate on the vehicle has experienced repeated misalignment issues. The first occurrence happened at 811 miles. The vehicle was taken to the dealer, and the body shop performed adjustments to correct the alignment. Approximately nine months later, at around 7,500 miles, the tailgate became misaligned again. The vehicle is currently undergoing another adjustment under warranty to address the same issue. This repeated condition raises concern about the integrity and durability of the tailgate alignment and related components. Improper alignment could potentially affect the secure closure of the tailgate and may pose a safety risk if it were to open unexpectedly or fail to latch properly while driving.

Just dropped off my [XXX] and [XXX] to elementary school drove to get gas and the passenger backseat behind the driver side fell forward and slid forward. I put it back in place drove again. It did it again and it is not staying in place. I called FORD. They told me they were supposed to be a remedy for it mid September. It is mid September but I can’t have my [XXX] getting hurt with a default seat and when I called today XXX at approximately 1:20 PM mountain time the lady on the phone told me pretty much. There’s nothing that they can do about it until there’s parts to replace it even though it opposes a danger to my child. Front and side curtain air bags failed. Was side swiped and hit a guard rail head on at 60-70 mph. I was going on the far left lane on the interstate 85. When a lady in the center lane hit the side of my car not paying attention that I was in my lane.

Exterior lighting

First, we took this vehicle to a dealer because the rear window buttons were not working. They wouldn't roll down. Second, the dome lamps inside only work part of the time, when we take the vehicle to the dealer or another Ford dealer that we decided to try, they say they can't do anything because they see no issue as the lights are working AT THAT TIME. Third, while connected to a trailer, an error message appears several times stating that the trailer has been disconnected. It just flashes and pings, and then disappears. This is a safety issue, as it has caused me to make sudden stops as I am made to believe that the trailer has became disconnected. All these things are stated in the recall we received on this vehicle, and we just want the body control module replaced as it says they should do, but both dealers(Bruce Walters Ford - Pikeville KY) and the dealer we switched to to try and remedy the issue (Freedom Ford of Wise VA) say that there is no issue, and refuse to do the control module replacement. This vehicle has been inspected at the least 6 different times for these issues, and still nothing has been fixed.
My vehicle originally showed more than 10 open safety recalls in the FordPass app and at the servicing dealer. I saved screenshots of each recall. After Ford performed a remote over-the-air software update, all but one recall suddenly disappeared from the app even though no physical repairs were done and no documentation was provided. NHTSA still shows Recall 25V543 as incomplete, involving BCM soldering defects that can cause trailer taillamp failure, electrical malfunctions, HVAC issues, mirror/window failures, power outlet issues, and other safety-related electrical problems. The dealer has not scheduled the repair despite parts being listed as available. Ford Motor Company and the dealer have not provided any recall history, OASIS report, repair documentation, or explanation for why multiple recalls disappeared. This creates a safety concern because the vehicle may have unresolved electrical or lighting defects that were not properly repaired or were prematurely marked as closed after a software update. The issue has not been resolved, and Ford has not responded to multiple requests.

Suspension

Alignment went bad 23489 miles and took to dealer. It was inspected and found out the front cam bushings and bolts were already worn out and had to be replaced both sides. Plus realignment after. Cost was 544.00 dollars had to pay because alignment from factory only goes to 12000 miles , how convenient on fords part , nothing would wear out that soon or shouldn't under normal wear. So , I had to pay. asked my service advisor and he said he has gone round and round with ford and to no end , basically ford said to bad. My advisor said ford knows and knew of this for the last few years on explorers , broncos and f150 trucks and doesn't care to want to cover the cost of repairs and or fix the problem. here's the main point , my advisor said ford knows they put less quality , inferior parts made from less strength material made parts and doesn't care. My advisor told me the after market parts are made of better material and better quality and I will never have this problem again. That said , YES , this is a safety problem if , the parts were to break or come apart , could cause an accident , crash and worse case could kill someone if this were to fail on the highway at high speeds. So , I feel Ford needs to correct this , recall all the vehicles involved and , I and many others so far should be repaid for customers having to fix this problem. As my advisor said , many others so far and ford corp doesn't care. WHERES THE RECALL. Before someone gets killed! No option but to fix as to replace the parts and get realignment to save the tires from wearing out and having to be replaced , of course at my cost! Per my work order of inspection and required repairs.
Vehicle has 5700 miles on it. At roughly 3000 miles I noticed a clunk or tension thud at speeds under 12 mph, both moving forward and in reverse. Brought the vehicle to the dealer and was told it was unsafe to drive. Thus far they have replaced strut mount bushings and have told me the transmission mount was loose and that they have not been able to determine why. My impression is the transmission mount is a significant safety issue in that the vehicle could loose power and subsequent control at any time if the connection worsened or just happened again.

Car was parked on a driveway, decent grade, with 50 miles left to empty (no low fuel light on). Facing downward. Car did not turn over. Had to use manual override to put vehicle into neutral and coast to a more level area and start it up. Seen this issue posted on the Explorer ST Facebook group multiple times, so I was lucky I knew what the issue is. Most other people would be stranded. There is a flaw in the Explorer's fuel tank system that will not allow the car to start if fuel level gets below a certain threshold when parked facing down a steep street or driveway.

The carpet on the wheel wells is not functional. We live out on a ranch on a dirt road and when it rains the mud gets stuck on the carpet and weighs is down. This can cause more damage to the vehicle. We took it in to a dealership and they said there was nothing they could do until the carpet gets ripped off. When we bought an expensive 4x4 vehicle we expected it to be sturdy enough to not have to worry about mud (especially when the car has a built in mud feature).

Seat belts

On a brand new 2025 Ford Explorer the 2nd row seatbelt was never bolted in. The seatbelt was hooked into the bottom so at a glance it looked correctly installed. But after having to remove a car seat the plastic covering looked like it was detaching. After gently pulling the plastic covering came up and the metal bottom of the seatbelt came out. Brought it to a Ford dealer and they confirmed that there is no bolt holding the seatbelt in and there were no threads, meaning there was never a bolt.
