I am reporting a safety-related transmission problem I experienced with my 2022 Ford F-150 equipped with the 10R80 automatic transmission. While driving normally, the transmission FAILS TO ENGAGE AT SLOW SPEEDS, THROTTLE WILL NOT RESPOND FOR UP TO 8 SECONDS This creates a potentially dangerous situation because the truck did not respond as expected when I needed predictable acceleration or propulsion. I believe this incident may be related to the known 10R80 CDF clutch-cylinder/sleeve problem. Ford technical information identifies axial movement of the CDF sleeve that can cause hydraulic-circuit leaks and harsh or delayed engagement and shifting. This condition can result in slipping, RPM flares, delayed engagement, unexpected gear changes, harsh downshifts, lurching, or loss of propulsion. This is a safety concern, not merely a transmission quality or reliability issue. Unexpected loss of propulsion or unpredictable shifting while merging, accelerating into traffic, crossing an intersection, turning, or traveling at highway speed can create a collision hazard. Because I personally experienced this unexpected transmission event, I request that NHTSA investigate the 10R80 CDF clutch-cylinder/sleeve condition in Ford F-150s. Please review Ford warranty claims, dealer repair records, transmission replacements, consumer complaints, and field-failure data to determine whether this is a safety defect requiring a recall. I believe owners should not have to wait for a potentially dangerous transmission failure before this known condition is addressed.

Highway Incident: While traveling at highway speeds, a sudden evasive braking maneuver caused the truck to drop power. Upon attempting to re-accelerate, the throttle pedal went to the floor with zero response, and the vehicle continued to decelerate on an active highway. Multiple trailing vehicles had to make emergency maneuvers to swerve around the decelerating truck to avoid a high-speed, rear-end collision. Subsequent Incidents: The vehicle repeatedly forces an uncommanded speed limit of 11 mph while attempting to accelerate from a complete stop or merge from stop signs. This severe lack of predictable acceleration creates an immediate risk of being T-boned or rear-ended by cross-traffic, making the vehicle entirely unsafe to operate in normal traffic conditions. An authorized Ford dealership has confirmed the underlying failure of the hybrid system. They kept the vehicle for approximately one month to replace the entire hybrid drive unit. Following that repair, the vehicle was returned to the dealer a second time, where technicians confirmed a software communication failure between the electric and gas systems, attempting to resolve it by flashing/reprogramming the ECU. The vehicle’s hybrid system and software have been extensively inspected and worked on by manufacturer-certified technicians at an authorized Ford dealership. It has not been inspected by police or insurance, as no physical collision has occurred yet. No warning lamps or messages.

In the second year of ownership I noticed that occasionally I would get no throttle response from the gas pedal when trying to accelerate. After several occurrences I began to pay close attention as to when this was occurring. I found that this occurs mostly, not exclusively, when I am slowing the vehicle to a near stop, not a full stop and then accelerating to continue, like slowing to round a corner on a green light and getting into traffic. For example, I was exiting a Home Depot parking lot, the driveway exits to the roadway. As I approached the driveway to enter the roadway I slowed, preparing to stop. Seeing no traffic on the roadway I then pushed the accelerator to enter the roadway. The pedal was dead, I got absolutely no acceleration. I pushed the pedal down slowly, nothing. I pushed the pedal to the floor, nothing. The truck rolled out onto the road, I coasted to the side of the road. Had there been traffic I'd have been hit. This has happened 20 times in two years and 80K miles. The most recent event was this week with my wife present, very scary, dead pedal, not throttle response. If you research "Dead Pedal" on GOOGLE, you will get the following response: The "dead pedal" in the Ford F-150 PowerBoost—a dangerous lag or total loss of throttle response—is a widely reported issue, particularly when accelerating from low speeds, turning, or right after starting. [1, 2, 3] The cause can range from a software glitch to transmission issues or failing sensors. Ford said there was a TSB, however the work performed had no effect on the dead pedal on my truck. Invoice attached from March 2026, it just occurred again this week. I am going to trade the vehicle in but I doubt that Ford will disclose this flaw to the next buyer, it will be their's to discover, hopefully not by "accident". I am going to trade this vehicle in and I'm sure the new buyer will have no idea of this potentially deadly flaw.

I just started the truck, put it in gear, and began driving slowly through the parking lot. The gas pedal was unresponsive—even when pressed all the way to the floor, the truck would not accelerate or move. This has happened multiple times, both while rolling slowly and sometimes from a complete stop. It occurs intermittently.

I was driving down the road and heard a loud clunking noise. Then I had lost power and the rear wheels were spinning out of control. There was a grinding/spinning/whirring noise coming from beneath the vehicle, sounded like from behind. I was fortunately able to regain control of the vehicle by shifting it into 4WD and avoided an accident. I brought the Vehicle to Gaudin Ford for repair and was informed it was a recalled part failure recall number 23S65. I had previously requested the recall repairs to be performed during routine maintenance, but was informed parts were not available. At this time, I've been 6 weeks without my truck and there is no end in site. It was insinuated that they might not even be making this part anymore, but the Ford recall center was unable to confirm or refute this. In the interim, I do not have a suitable replacement. I have been provided a small vehicle as a loaner, but I am unable to use this to drive >50 mile radius, I can't leave the state, can't loan it to my children, can't tow or haul. All the things I need my truck for, I'm unable to do. Either, I need access to a rental with similar capability to my truck without all the restrictions, or fix the truck in a timely manner, or provide me a suitable loaner without all the restrictions, or buy back my truck, or something. Instead I feel like I'm being held hostage by a company who is refusing to fulfill the recall responsibility in a timely and suitable manner. I have contacted the dealer, Ford customer service, and the Ford recall center on numerous occasions and have been told that other than getting an update when the part is available, there's nothing else they can do. On Sunday, [XXX], my truck spontaneously burst into flames while parked in my front yard. I had just returned from doing a number of errands. The truck had been running fine with no indications of a problem. I was outside tending to my smoker when I heard a sort of popping/crackling noise. I looked up at my truck approximately 40 feet away and saw flames shooting out from the engine bay through the hood gaps. I called 911 and attempted to douse the flames with a powder fire extinguisher with no luck. By the time the fire department arrived 15 minutes later or so, the truck was fully involved and burnt to the frame. I didn't smell gasoline during the initial startup, but a strong smell of burning wires. The concerns are of course for my safety and that of my family. It was just luck that the truck was parked in the front yard rather than next to my house where it's normally parked. We were have a sidewalk put in and thus my normal parking space wasn't available. I could've been driving at the time and with the speed of this fire, I'm not confident I would've been able to get out. The truck has been seen by the insurance company and is being towed out of my yard sometime today May 29th, 2025. I don't know where it will towed to. I have pictures and videos detailing the fire and it's progression. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

The manufacturer has failed to remedy this safety recall for my F-150 vehicle in a timely manner. Ford initially identified the axle bolt fault as Customer Satisfaction Program 22B34 on Feb. 14, 2023 to a limited range of vehicles but then expanded the action as a recall notification 23S65 on Dec. 22, 2023 covering all 2021-2023 trucks with their "Trailer Tow Max Duty package". Based on these dates, Ford has had over 2 years to engineer a solution to this safety recall but has not done so.

For no apparent reason, the vehicle jumped forward without any input from myself. Thought I was rear ended by someone behind me. Luckily, I always have my left foot on the brake pedal when stopped and was able to apply additional pressure to keep the vehicle from moving forward. This is a new to me, used vehicle. After doing research I see that this is an ongoing problem for the 2021 to present Ford F 150 PowerBoost, hybrid, trucks. Hopefully a safety recall will be issued.

When traveling at low speeds, 10-15 mph, and coasting (no accelerator input) then pressing the accelerator there is no response. The engine will not rev up and the truck will not accelerate for a good 6-8 seconds. It happens usually in parking lots or when making low speed turns onto street intersections or pulling out of driveways. This is dangerous, as pulling in front of traffic then having no acceleration for several seconds could cause a rear-ending accident. It also happens often when going over steep small speed bumps in the parking garage at my office. It's very annoying and in some situations very dangerous. This has been happening since I bought the truck in January 2023. It is now Sept 2024.
