The contact's son owns a 2021 Ford F-150. The contact stated that her son reported that while accelerating from a complete stop, the vehicle hesitated significantly and shifted hard while the accelerator pedal was depressed. The vehicle was towed to the dealer, where it was diagnosed with transmission failure. The dealer determined that the transmission needed to be replaced, and the vehicle was repaired. The contact received notification of NHTSA Campaign Number: 26V402000 (Power Train), which the contact associated with the failure. The manufacturer was notified of the failure and informed the contact that reimbursement could only be provided when the recall remedy became available in April 2027. The failure mileage was approximately 113,468.
The contact owns a 2021 Ford F-150. The contact stated that while attempting to shift into park (P), the gear shifter became stuck, requiring the contact to remove the key from the ignition or depress the brake pedal until a whining sound ceased in order to shift into park (P). The contact stated that while operating the vehicle in reverse (R), the vehicle suddenly shifted into park (P) and stopped immediately. The failure recurred several times while in reverse (R). The vehicle was taken to the dealer, where it was confirmed that the VIN was not included in a recall. The failure mileage was approximately 53,000.

The contact owns a 2021 Ford F-150. The contact stated that the windshield wipers failed to activate as needed. No warning lights were illuminated. The local dealer was contacted; however, the vehicle was not diagnosed. The contact was informed that the VIN was not under recall. The vehicle was not repaired. The manufacturer was not notified of the failure. The contact associated the failure with NHTSA Campaign Number: 22V842000 (VISIBILITY). The failure mileage was 97,000.
When using the window wipers, they become inoperable. Stopping in the middle of the window, pausing when moving, or completely stopping and unable to get them to start moving until they begin to move on their own. This has occurred multiple times and at multiple different speeds of operation for the wipers. This was a previous recall issued by Ford that has been completed on the vehicle yet the same result is occurring.

Due to a dangerous design flaw in the 2021 Ford F150, I was put in a potentially dangerous situation by completely losing all power in the vehicle on a backroad. Design flaw is the DCN module, which floods into the vehicles electrical system/body of vehicle through the cowel drain that can supposedly be blocked by a singular piece of pine straw. Due to the design flaw, the vehicle completely lost all power and all electronics failed while driving down the road at 45 mph, putting both me and my father (riding passenger seat) in immediate danger. Ford knows and is aware of the design flaw and has refused to issue a recall, not only affecting my safety, but now I am going back and forth with ford over the fix to the tune of $2800 that they refuse to fix (vehicle is a gold certified cpo, has extended warranty). When asked how to prevent in the future, ford was and is not aware of how to prevent in the future, and I can not get to the cowel drain clog without professional equipment and ford performing. There is no way to tell the cowel drain is clogged or not, and there is not anything in the ford owners manual related to maintenance of the cowel drain. Again, due to the inappropriately designed drain, this caused water leakage into my truck, posing an immediate threat to my life and others around me. I have all of the videos and pictures needed to show the failure of the system to provide to move this forward to get resolved. I had complete loss of power and electronics on the road. I tried to attach video but it would not let me. I can attach via email when requested.
When backing up, the rear camera display shows a gray filter over the rear image on the display cluster and the right side warning sensors indicate a blue trapezoid on the cluster for the right side of the vehicle. The driver dashboard also has an alert that the rear brake assist feature is inactive
Exterior lighting

Daytime Running Lights stopped working. Unable to fix unless the whole light assembly is replaced.
Daytime Running Light in the headlight assembly overheated and stopped working. Thousands of 2021-2023 F150 owners have had the same issue. This is clearly a mass scale manufacturer known defect and Ford already tried sending an update to lower the LED brightness but the issue still happens.

On [XXX], I was driving westbound from Missouri into Oklahoma in my 2021 Ford F-150 XL. While traveling approximately 75 mph on the highway, I moved into the passing lane to overtake another vehicle. As I accelerated and applied additional throttle to pass, the transmission downshifted into passing gear. After completing the pass, the vehicle continued to accelerate on its own even after I released the accelerator pedal. The engine remained at a high throttle setting, and the truck increased speed to approximately 85 mph. At that time, I was not using cruise control, nor had I pressed the cruise control switch, which was involved in a previous acceleration-related incident I experienced with this vehicle. To regain control, I carefully created distance from surrounding traffic, applied the brakes, and maneuvered the vehicle onto the shoulder. After coming to a stop well off the roadway, I shifted the vehicle into Park and turned off the engine. I then restarted the truck, and the unintended acceleration condition was no longer present. Because of the incident, I did not attempt to use the cruise control system again during the trip, and I avoided using heavy throttle or passing gear out of concern that the condition could reoccur. The vehicle has since been inspected by a Ford dealership. As of this report, their evaluation has not identified the cause of the problem, and no fault has been found despite the incident occurring on the highway. This incident created a significant safety concern because the vehicle continued to accelerate after the accelerator pedal was released, resulting in an unexpected increase in speed and requiring me to pull off the highway and shut off the vehicle to regain normal operation. Vehicle Information 2021 Ford F-150 XL Supercrew approximately 35,000 miles. While traveling southbound on [XXX] near [XXX], Ok, I was operating my 2021 Ford F-150 XL at approximately 75 mph with the cruise control engaged. As I approached slower traffic ahead, I attempted to disengage the cruise control by pressing the cruise control cancel/off button on the steering wheel. Despite pressing the button multiple times, the cruise control would not disengage. Same thing while tapping the brakes it would not disengage. The vehicle continued to maintain speed, and the cruise control system remained unresponsive to my commands. In response, I applied the brakes to safely reduce speed, I did not perform a panic stop or abrupt braking maneuver. Once I had the vehicle slowed, I moved safely onto the shoulder of the highway, well away from traffic. I then shifted the vehicle into park, shut off the engine, and restarted the truck. After restarting the vehicle, I elected not to use the cruise control system for the remainder of my trip, which lasted approximately eight additional hours to Houston, Texas, I have not used the cruise control since the incident because I am concerned that the condition could recur and create a safety hazard. No collision, property damage, or injuries occurred. However, the apparent failure of the cruise control system to disengage when commanded raises significant safety concerns and warrants investigation. I am reporting a severe safety defect where Ford Motor Company's sequential safety recall software updates are actively corrupting powertrain modules and inducing physical mechanical failures while the vehicle is in operation.Following the completion of the 24S55 Engine Intake Valve Recall (which reflashed the PCM and wiped the transmission's adaptive learning tables), my 2021 Ford F-150 developed an immediate operational shudder. Shortly after, a forced Over-the-Air (OTA) update for the 26C10 Trailer Brake Module triggered a secondary network reset via the CAN-bus. The intersection of these two software-driven actions caused an operational fault that physically broke the torque converter clutch.A compromised torque converter clutch can cause sudden vehicle stalling, unpredictable acceleration lags, and catastrophic powertrain locking while driving at highway speeds, creating an imminent crash hazard. The local Ford dealership service department confirmed the hardware failure and noted that pushing automated software updates over unstable tables leaves the entire transmission highly vulnerable to total systemic failure.Ford is currently refusing to open an escalation file or address this issue. Because Ford's official remedy for safety recalls 24S55 and 26C10 directly introduces a secondary, severe safety risk to the vehicle's powertrain integrity, I request that NHTSA investigate the technical compatibility and safety of these sequential software updates.

Fuel pump failed while driving causing vehicle to be disabled in a roadway blocking traffic. The vehicle was then incapable of shifting to neutral so that it could be moved out of the roadway. A Ford dealer confirmed the faulty fuel pump and replaced the part. Ford appears to know this is a safety issue as per recall campaign 25S75/25V455 but they have refused to pay for the replacement fuel pump.
My husband was driving on the highway with my two daughters in the car when the vehicle stopped driving and he was barely able to pull off the road. It appears to be a defective low-pressure fuel pump. This is an active recall for this model and year, however Ford says that our specific VIN was not involved in the recall. My family could have been killed in this incident!
Brakes

ABS Control module and brake booster assembly failed. Vehicle would not crank or start. Disconnected battery negative pole for one minute to reset PCM. Vehicle started. Drove immediately to independent service center without difficulty. Service center confirmed vehicle no start the next morning. Found several DTC codes including U3001 (Control Module Improper Shutdown) U300A (Ignition switch) U0121 (Lost communication with ABS) U0554 (Invalid Data recieved from Accessory Protocol Interface) and U0415 (Invalid Data Received from ABS) Flowchart determined ABS module is bad. Anti Lock Brake System control module failed which rendered the system inoperative. Warning on dash that Anti Lock Brake System failed was prominent among other warnings. (Traction Control, Service Needed, Door Ajar (was closed), Hood ajar (was closed) Assembly was replaced and ABS module reprogrammed by independent service center. They retained the bad assembly.
THIS TRUCK THAT I HAVE PURCHASED FROM PALM COAST FORD HAS A RECALL EVERY MONTH THAT KEEPS ME FROM UTILIZING THE VEHICLE. THE LATEST RECALL THAT THERE IS NOT A FIX FOR YET, I WAS TOLD IS A COMPLETE RISK AND I SHOULD NOT EVEN DRIVE THE TRUCK. I REQUEST THAT YOU LOOK INTO ALL OF THE RECALLS THAT HAVE BEEN LISTED ON THIS PARTICULAR VEHICLE AND KNOW THAT I AM TAKING THIS UP WITH FORD, BUT ALSO WANTED TO INFORM YOUR DATABASE OF THE ISSUES AS WELL. I HAVE ONLY HAD THE TRUCK FOR ABOUT A YEAR AND IT HAS BEEN RECALL AFTER RECALL AND THIS ONE THERE IS NOT EVEN A FIX FOR IT TILL NEXT YEAR, AND I AM NOT SUPPOSED TO EVEN DRIVE IT CAUSE IT IS HAZARDOUS.
Steering

Subject: Steering Rack Failure Reimbursement Request, VIN [XXX] To Whom It May Concern, I am requesting review and reimbursement for an electronic steering rack replacement on my 2021 Ford truck, VIN [XXX]. The electronic steering fault first appeared at start-up and when turning the wheel to full lock. Initially, I could pull over, shut the truck off, restart it, and the warning would clear. After a battery replacement, battery monitoring system reset, and multiple steering relearns, the steering failed completely. When the failure occurred, steering became extremely difficult. After hitting a bump, the steering wheel jerked suddenly and caused pain to my hand and thumb. This also created a serious safety risk for me and others on the road due to the reduced ability to steer. The dealership replaced the electronic steering rack at a cost of over $2,000. The truck had only about 40,000 miles at the time. I later found Ford Recall 21S28, which appears to cover certain trucks built between XXX. Based on my VIN and build date, my truck appears to fall within that range, but the dealership advised my VIN was not included. Given the low mileage, safety risk, and apparent build-date match with Recall 21S28, I request that Ford and NHTSA review my vehicle’s eligibility under this recall or any related program and reimburse me for the repair. My 2021 F-150 has been diagnosed with a failed electric steering gear Code PSCM C1B00 49 2F. I understand there was a recall on some 2021 trucks with similar steering issues, although my VIN is not included. The part is on national backorder and my dealer says there are approximately 150 vehicles ahead of mine. This is a safety-related steering component, part number "ML3Z3504G"

Took my 2021 Ford F 150 in for recall 23s65 and oil change and was told vehicle was unsafe to drive and even though parts according to ford where available the dealer said they were on back order with no ETA of when they would be available and they would put me in a rental for the time being. I’ve been told 3 times now by Ford parts are available but yet the dealership keeps saying they’re still on back order.
My Ford F-150 is subject to Manufacturer Recall Number23S65/NHTSA Recall Number23V896. I asked for the repair multiple times, but the dealership repeatedly told me that the parts were on backorder. The hub bolt on my right rear tire cracked and the truck was making a high pitched sound. I took it to the dealer and they said the bolt was cracked and driving it in that condition is dangerous. The truck is not safe to drive, but the parts remain on backorder indefinitely.

After a day of heavy rain, the vehicle stalled at a stop light and would not restart. The steering wheel and transmission locked, preventing the vehicle from being placed in neutral, creating a safety hazard as the vehicle was immobile in traffic. No warning lamps appeared prior to the failure. The vehicle required a tow. The dealer confirmed water had intruded through the passenger side cowl/firewall seam due to inadequate factory sealing, tracking down and pooling directly onto the Body Control Module (BCM) mounted on the passenger side firewall. The BCM failed due to water damage, causing PATS desynchronization and the no-start condition. The BCM failure subsequently shorted out the starter motor. Dealer replaced the BCM, repaired the leak, and replaced the starter. Total repair cost approximately $3,200. Failed components are available for inspection at the dealership. This identical failure; passenger side cowl water intrusion leading directly to BCM damage — is documented across numerous 2021+ F-150 owner forums, indicating a widespread factory sealing defect at the cowl/firewall/inner fender seam junction.

A certified independent inspection company discovered welding on the main chassis of my Ford F-150. The welding is located on the front left frame area. I purchased the vehicle brand new, and it has never been in any accident or collision. This welding on a critical structural part creates a serious safety risk. Frame integrity may be compromised in normal driving or in the event of an impact. I transport my children daily in this vehicle, and I am afraid to drive it due to the possibility of structural failure. The official Ford dealer inspected the vehicle and kept it for approximately three weeks but provided no explanation or repair. The regional Ford office has delayed their technical report for more than five months with no response. The defect has been confirmed and is permanent. No warnings or lights appeared prior to discovering the issue. This is a manufacturing-related structural defect affecting vehicle safety, stability, and integrity. A proper safety resolution is required.

While driving my 2021 Ford F-150 PowerBoost, the vehicle issued a pre-collision warning followed immediately by a restraint system warning. The airbag then deployed without any collision or impact. There was no crash, obstacle, or reason for deployment. The airbag deployment shattered the windshield and caused interior damage. This appears to be a false activation of the pre-collision system and unintended airbag deployment, creating a serious safety hazard. This could easily result in injury or loss of vehicle control. The vehicle needs to be investigated for a potential defect in the airbag system, sensors, or pre-collision assist system.

1. Electrical System Failure: While traveling on highway, alternator failed suddenly within 2 minutes of “service soon” and “check electrical system” warnings. Caused complete loss of motive power, electronic parking brake locked up while driving, loss of power steering, all gauges failed, transmission shifted into park while in motion. Battery was damaged (“fried”). Engine continued running for several minutes after stop without ability to shut off ignition. Incident documented by Ohio State Highway Patrol. 2. Seat Belt Issue: Driver-side seat belt interlock sticks in unlocked position, creating risk of failure to lock during a crash. 3. Electronic Parking Brake: Intermittently engages while vehicle is moving downhill to a stop (mostly at or below 5 mph). Parking brake light flashes three times then turns solid while in motion. Ford TSB indicates vehicle not eligible for related recall. 4. Infotainment System: Intermittently freezes, requiring forced reboot. Preceded by audio glitches, laggy dash computerized systems. 5. Transmission: Severe clunk and physical jolt when decelerating from ~30 mph to 8 mph. Also occurs during acceleration, particularly upshifts from 2nd to 3rd and 3rd to 4th gears, more pronounced when turning. 6. 4WD System: Intermittently fails to disengage from 4WD HI to 2WD HI. “Failure to shift” message appears several minutes after command to return to 2WD HI. 7. Front Differential/Axle: Previously, faulty front differential housing fractured due to system malfunction, causing intermittent loss of front driveline engagement in 4WD LO (250-500ms delay then re-engagement under throttle). Ford replaced front differential and axle under warranty after vehicle out of service for 2.5 months. The contact owns a 2021 Ford F-150. The contact stated that while driving at an undisclosed speed, the vehicle inadvertently lost motive power, leaving the contact stuck in the middle of the roadway. No warning lights were illuminated. The vehicle was towed to the dealer, who diagnosed a failure with the transmission. The contact was informed that the transmission needed to be replaced. The vehicle was not repaired. The manufacturer was notified of the failure, a case was opened, and the contact was referred to the NHTSA Hotline for assistance. The failure mileage was 127,000. The contact stated that while driving on a neighborhood street at approximately 25 MPH, a faint whistling sound could be heard from the rear passenger side, and when accelerating the sound progressed. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was made aware of the failure. Started after McGee ford did a service on my truck Apparently they told ford they repaired electrical recalls which I wasn’t having issues with nor did I know They did anything! They did however erase the recalls so that was 8-2024 shortly after my truck experienced major electrical issues, another dealership told me it was gonna be 6k and 4 weeks to fix I was in awe I left with my truck that had no blinkers no air bags and so on 15 safety codes! I battled this for a year and then my parking break locked up while driving 70 on the highway! That’s the recall megee claimed they fixed I’m not rich single dad 100% of rhe time and I sure my kids everywhere daily, I do research call ford find intact i had a recalll so im thinking it will get fixed! 6 weeks later they not fixed i was told 2 days Now heritage and ford won’t fix it they want megee ford the certified service and dealer to pay for it. It’s winter im a contractor in Vermont they let me borrow a Sentra with summer tires on it. Couldn’t make it off my road. I’m still driving my kids around! I almost reck once a day as the whole system is messed up now! I feel like I’m being bullied and picked on because of my nationality

Tires making noise and not in balance /out of round. I had a authorized tire company check tires on their machines and a report was written. They said the tires were defective.BIGBRANDTIRE COMPANY PASO ROBLES,Calif. 93446 Phone 805-238-5097
The contact owns a 2021 Ford F-150. The contact stated that while driving approximately 50 MPH, the transmission suddenly shifted to neutral and the vehicle would no longer accelerate. The vehicle began to roll forward and rolled off the road. The vehicle was towed to the local dealer who diagnosed that the transmission and transfer case had failed and needed to be replaced. The vehicle was not yet repaired. The manufacturer was notified of the failure. The failure mileage was 2,000.
