Recalls, owner-reported complaints, investigations and safety data

Owner complaintsComplaints are reports submitted to NHTSA by owners and drivers. NHTSA does not verify most reports before publishing them, and a complaint does not establish that a defect exists. Volume is influenced by how many of a vehicle were sold, how long it has been on the road, and how much attention an issue has received.
0
Safety recallsA safety recall is issued when a manufacturer or NHTSA determines that a vehicle has a safety-related defect or does not comply with a federal motor vehicle safety standard. Recall repairs are free. Whether a specific vehicle is covered depends on its VIN — a recall listed for a model year does not necessarily apply to every vehicle of that year.

InvestigationsNHTSA opens an investigation to examine whether a safety defect may exist. An open investigation is not a determination that a defect exists, and many investigations close without a recall. Investigations can, however, lead to one.
0
Crash or fire reportedComplaints whose submitter recorded that a crash or a fire occurred. These are the reporter's own answers on the NHTSA form, not verified findings.

FUEL SYSTEM, GASOLINE:FUEL INJECTION SYSTEM

Feb 16, 2005
46 units potentially affected
The defect: ON CERTAIN FIRE TRUCKS AND TRANSIT BUSES, THE FUEL LIFT PUMP COULD FAIL IN A MANNER WHICH RESULTS IN ENGINE FUEL STARVATION, WHICH COULD RESULT IN A STALL CONDITION. POTENTIAL LIFT PUMP FAILURE MODES INCLUDE: BRUSH WEAR OUT, BUSHING FAILURE, ROTOR TAB INSERT FAILURE, OR ANY OTHER FAILURE MODE THAT WOULD CAUSE THE FUEL PUMPING ELEMENT TO STOP SPINNING.
The risk: IF THE FUEL LIFT PUMP PUMPING ELEMENT STOPS SPINNING AND THE FUEL PUMPING VANES STOP IN A LOCATION WHICH BLOCKS FUEL FLOW THROUGH THE LIFT PUMP, THE ENGINE WILL BE STARVED OF FUEL. FUEL STARVATION COULD CAUSE LOW ENGINE POWER, ENGINE MISFIRE, OR ENGINE STALL. PRIOR TO THE ENGINE STALLING, THE OPERATOR COULD EXPERIENCE LOW ENGINE POWER, ENGINE MISFIRE, AND/OR DASH-MOUNTED FAULT LAMP ILLUMINATION.
The remedy: CUMMINS IS CONDUCTING THE OWNER NOTIFICATION AND REMEDY FOR THIS CAMPAIGN. OWNERS MAY CONTACT CUMMINS AT 1-800-343-7357 OR HME INCORPORATED AT 616-534-1463.

SUSPENSION:FRONT:WHEEL BEARING

Sep 29, 1999
17 units potentially affected
The defect: VEHICLE DESCRIPTION: FIRE TRUCKS EQUIPPED WITH 16,000 LB., 18,000 LB., OR 20,000 LB. HEAVY DUTY STEERING AXLES. THE INNER WHEEL BEARING CONE FOR THE STEERING AXLE WHEEL HUB ASSEMBLY DOES NOT PROPERLY FIT ONTO THE STEERING AXLE SPINDLE ASSEMBLY. IMPROPER FIT OF THE BEARING TO THE SPINDLE INCREASES THE STRESS LEVELS TO THE STEERING AXLE SPINDLE WHICH COULD CAUSE SPINDLE FAILURE.
The risk: IF THE SPINDLE FAILS, A LOSS OF DRIVER CONTROL COULD OCCUR, INCREASING THE RISK OF A VEHICLE CRASH.
The remedy: DEALERS WILL REMOVE THE LEFT AND RIGHT HUB/DRUM ASSEMBLIES FROM THE STEERING AXLES AND REPLACE THE INNER WHEEL BEARING CONES AND CUPS.

Higher-selling and older vehicles accumulate more reports. Counts are not failure rates and are not directly comparable between vehicles that sold in very different numbers. Older model years have had longer for reports to accumulate.
