Driver gauge instrument screen does not illuminate upon starting vehicle and at times when driving. No MPH or other gauges display. This defect has occurred several times and is intermittent.

The backup camera and infotainment screen is black, the backup camera and all controls on the infotainment screen was not usable because the screen was black. This is dangerous because no rear view camera.

On 2026-07-05, my car started except the digital instrument panel was blank/black. This meant the digital gas and battery gauges weren't visible, in addition to the speedometer, etc. I tried restarting the car to see if it was a fluke but it did not come back. On 2026-07-06, the car worked fine / as expected, but after a google search, it seems like this happens to other owners as well in the past with no clear reason why. Wanted to report in case it becomes a larger thing.

I started my vehicle and thr complete infotainment screen was black, no display, no backup camera, nothing but a black screen. The volume button still worked and you could touch the screen and her things change but you could not see anything because the screen was black. It is like turning the brightness completely off a touchscreen, it works but you can't see anything. There are current recalls for this for other models but they say mine is not included. But I had the exact same problem with mine. It did not work until I turned my car off, returned and restarted my car, after that it worked normally. My can needs to be included in the recall.

While driving on the highway, something (likely a small rock) from oncoming traffic flew into the windshield. Upon contact, the windshield cracked and shattered in a manner I have not seen a windshield break before, immediately spraying glass shards across the interior of the vehicle (to include dashboard, seats, and floors) and the driver. From the outside of the windshield, the area of damage is soft to the touch such that it is likely applying any pressure would collapse that area and/or enable you to punch straight through to the other side, so for safety concerns it is undrivable until the glass is replaced. I am concerned the glass could have been defective in some way, as I don't believe this is a typical response to common road hazards like small rocks/other projectiles. The windshield was the original factory installed windshield. The vehicle has not yet been inspected by the manufacturer but the incident has been reported and owner is awaiting contact from the manufacturer regarding inspection. It can be available for inspection until the glass is replaced, but owner will have glass replaced as soon as possible. Additionally, because it is not drivable, it would require inspection at residence or towing. The incident was reported to insurance. There have never been any previous warning lamps/other indicators that the glass was defective.

3 unresolved safety defects on 2024 Lexus NX 450h+ plug-in hybrid leased May 2023: ELECTRICAL/BATTERY: Rear seat occupant sensor produced continuous false alerts since weeks after lease began, preventing vehicle from locking. High-voltage components including on-board charger located beneath/near the rear seat area per dealer representation at time of sale. Service manager acknowledged liability in writing July 2023. Lexus TSB L-SB-0035-24 addressing battery monitor module failure (DTC P162B87) never applied. Recall 25LD01 covering high-voltage floor wire corrosion unverified for this vehicle. Not charged since approximately April 2025 — over one year — due to fear of battery malfunction, fire, or explosion. Operating exclusively on combustion engine, eliminating primary plug-in feature. Reported in writing for 2 years 8 months. Never repaired. GM confirmed in writing February 2026 defect never submitted to manufacturer. DRIVETRAIN/TIRES: Severe premature tire wear from approximately 2,252 miles. At 7,800 miles dealer declared vehicle unsafe for freeway driving. Owner used alternative transportation for court appearance due to safety warning. Dealer's own alignment printout January 2026 shows multiple out-of-specification values. All 4 tires replaced after 72 consecutive days dealer possession, classified as goodwill, never submitted as warranty claim per GM written admission February 2026. HVAC/BIOHAZARD: Persistent mold/mildew odor from HVAC since early in lease. Service advisor initially attributed cabin odor to rat feces in engine bay entering cabin through air circulation — admitting biohazard exposure to occupants. Rodent nesting caused by manufacturer's soy-based wiring insulation. Rat feces constitute hantavirus and salmonella risk. Same defect across three consecutive vehicles from same dealer. Engine bay sanitized and evaporator cleaned during 72 days, classified as goodwill only, never submitted to manufacturer. Documentation available for inspection.

The contact owns a 2024 Lexus NX 350. The contact stated that while his wife was driving approximately 40 MPH, the instrument cluster went blank. No warning lights were illuminated. The vehicle was taken to the local dealer, but no fault was found. The failure was self-corrected. The vehicle was not diagnosed or repaired. The manufacturer was contacted, but no assistance was provided. The approximate failure mileage was 20,000.

Possible malfunction involves vehicle speed control and braking during a low-speed parking maneuver in my 2024 Lexus NX350h. While slowly pulling into a parking space, I was actively applying the brake and creeping forward. While my foot was on the brake, the vehicle suddenly surged/accelerated forward instead of continuing to decelerate. The vehicle ran over a sign and jumped a curb before I could stop it. The vehicle is available for inspection and is currently with the Lexus dealer. My safety and the safety of others were put at serious risk because this occurred unexpectedly while parking. If a pedestrian, fixed structure, or another vehicle had been in front of me, this could have caused severe injury or worse. Thankfully, I was not physically injured, but this was a traumatic event, and I was terrified. The problem has not been reproduced by me after the incident. At the time of this report, it has not yet been confirmed by the dealer or an independent repair facility. I have requested a full diagnostic inspection and review of all available stored data before any codes are cleared. The vehicle has been delivered to the Lexus dealer for inspection. The incident was also immediately reported to my insurance company. I do not know if Lexus manufacturer representatives or other parties will inspect it. I did not observe any warning lamps, messages, or unusual symptoms before the event, and this had never happened before.

While driving, which occurred 20 minutes after i started the car, the driver’s door gets unlocked automatically and i started hearing car warning with ⚠️ appearing on my dashboard. I also heard wind noise from the highway. With the electric, i could not simply open and close the door while driving, but had to pull the door with my left hand for the next 40 minutes while driving until i hit destination. When i pulled door towards close position, wind noise and warning sound went away, but comes back as soon as i release the door. Happened numerous times during summer, spring and winter.

The incident happened on August 27, 2025, at about 8:30 in the morning when I was trying to park the car in a parking lot. As I came to the parking space to park the car with my foot on the brake pedal, the car accelerated all of a sudden after it had stopped before I could shift the gear lever to “Park”. The car went over the curb. I reversed and tried to park the car again in the same spot. The car accelerated again and went over the curb, and this time ended up going all the way to the two-way traffic road located ahead of the curb. Fortunately, there was no traffic on the road at that time and no pedestrian on the sidewalk, and no one got hurt. The car stopped in the middle of the road. I drove back to the parking lot and tried to park the car again in the same spot. This time the car stopped without any incident. The dashboard said, “System Malfunction” and said to take the car to the dealership. I drove to the Lexus dealership without any incident. There was significant damage in the front and both sides of the car. I told Lexus what had happened. A case manager was assigned. An inspector was sent to inspect the car. After the inspection, he sent me a report. The report was technical in nature and had graphs and data points without any explanation. I could not understand the report. He told me that the report was sent to Toyota and Toyota would get back to me after they had analyzed the data downloaded by him. Toyota sent me the report, and the report said that my foot was on the accelerator at that time and the car functioned properly. I am pretty sure that my foot was on the brake pedal at that time and not on the accelerator. Toyota was not willing to answer any questions I had and told me to get a second opinion. Currently, my car is in the auto body shop being repaired via my insurance. Even though, no further incident has happened to date, I am afraid that this could happen again in the future, possibly badly hurting or even killing people.

Vehicle: 2024 Lexus NX 450h+ (plug-in hybrid) Odometer at latest event: ~14,600 miles Dealer: Motor City Lexus of Bakersfield (Bakersfield, CA) Lexus Corporate Case #: 250823001008 Complaint Description: While driving, the vehicle suddenly lost all electrical power without warning on two separate occasions, causing the instrument cluster to go dark and an immediate loss of power steering and brake assist. I had to coast to a stop—this is a serious crash risk. Timeline of events: • 08/20/2025 – In-motion total electrical blackout with loss of steering/brake assist at approximately [speed] mph. • 08/21/2025 – Dealer replaced the 12-volt battery. • 08/23/2025 – Repeat in-motion blackout with my children in the vehicle; dash went dark, loss of steering/brake assist, coasted to a stop. • After 08/23/2025 (at dealer) – During diagnostic testing, a dealer technician reproduced the same failure (total electrical power loss while moving, dash dark, loss of steering/brake assist). This confirms a repeatable safety defect. • There have also been three additional immobilizations that required Lexus Roadside Assistance on other dates. Current status: Vehicle remains at the dealer for the second repair attempt. I am within the 18-month window from delivery (Mar/Apr 2024). Safety concern / suspected area: Sudden total electrical loss while driving with loss of steering/brake assist. Please investigate potential defects in the 12-V/DC-DC charging system, hybrid/electrical system (inverter/DC-DC/grounding), main fusible link, or related wiring/connectors/grounds that could cause complete power loss while the vehicle is in motion.

The contact owns a 2024 Lexus NX350. The contact stated while driving at an undisclosed speed and making a left turn into a fast-food restaurant, the vehicle made an abnormal popping sound. The contact stated while in the drive through and shifting from park(P) to drive(D), the vehicle failed to shift into the intended gear; however, the instrument panel and the gear shifter indicated that the vehicle was in drive(D). The message "Park" was displayed. A bystander advised the contact to shift to neutral(N). The contact shifted to neutral(N); however, the message "Park" remained displayed while the vehicle was in neutral(N), and the vehicle failed to respond as needed. The contact stated that three bystanders were unable to push the vehicle. The contact's husband, who was on the phone, advised the contact to turn off the vehicle. The contact turned off and restarted the vehicle, and the vehicle responded as needed upon shifting to drive(D). The vehicle was taken to the residence. The contact stated that the failure had previously occurred; however, the dealer was unable to duplicate the failure. The dealer was contacted, and an appointment was scheduled for a diagnostic test. The vehicle was not diagnosed or repaired. The manufacturer was not notified of the failure. The failure mileage was unknown.

Purchased this 2024 NX in October 2024. Since purchase the CarPlay aspect of the car where you can talk to text and voice call has not been functioning normally. It cuts in and out of bluetooth so if I am on a hands free call with someone, sometimes it cuts over to the phone randomly and won't connect to the car. The talk to text feature and any talk to the car feature for CarPlay doesn't work as it does not recognize any voice it just hears what the car is playing and asks for repeating. This is a known issue in this vehicle. I have brought it up to Lexus on each service and they are aware but it apparently needs a software update that they have been waiting on for over 1 year. This feature is crucial for the driving that I do as I need to be able to respond to work calls/texts using the hands free feature as GA is a hands free state and it is illegal and unsafe for me to have my phone in my hands while driving.

The fire then spread to another vehicle that was parked in front of the vehicle. The fire department arrived and defused the fire. The contact stated that the fire department advised her that the fire originated from the engine compartment. The vehicle was towed to a salvage lot. A fire report was taken, and a police report was pending being filed. The contact mentioned that while driving the vehicle prior to the fire, there were no warning lights illuminated. The manufacturer was not notified of the failure. The failure mileage was unknown.

My 2024 Lexus NX 450h+ experienced a complete 12V battery failure after normal short-distance driving, leaving me stranded without warning. This is a known defect in Lexus plug-in hybrids that the manufacturer has not properly disclosed to buyers. Incident Details: After several short trips (all under normal driving conditions), my 2024 Lexus NX 450h+ failed to start. The vehicle was completely non-responsive. Because the doors and liftgate were locked and inoperable, I was forced to crawl through the rear cargo area with a pre-existing mobility issue (bad leg), causing significant physical pain and emotional distress. I later had to visit the Emergency Room due to pain and strain from the incident and was prescribed medication. When I reported the problem to Lexus Corporate, they initially attempted to shift blame onto my roadside assistance provider (AAA) instead of acknowledging the mechanical defect. No solution was offered to ensure the vehicle’s reliability for daily use. Ongoing Concerns: •Lexus has not issued a recall or fix for the widespread 12V battery failures affecting plug-in hybrid vehicles. •This issue poses a significant safety risk, particularly to older drivers, disabled individuals, and anyone who relies on their vehicle for essential transportation. •I purchased this vehicle at a premium price, expecting reliability, and it has failed in fundamental ways. •Being stranded without warning and physically harmed while trying to exit my own vehicle is completely unacceptable for any car, let alone a brand-new luxury model. Given the known nature of this defect (widely reported by other owners online), Lexus must be held accountable for addressing the safety hazards caused by faulty 12V battery systems in its 2024 NX 450h+ plug-in hybrid.

My son and I were almost killed the other day because the engine of my 1 year old car failed. We were in the middle of 6 lanes of rushing highway traffic when the car stopped accelerating and came to a dead stop. I had just had the car serviced 10 days before and didn't feel, hear or see anything hit my car. The message on my dashboard said "go to the dealer" when it should have said "pull over immediately, turn off your car and call for help". The dealership told me that a rock must have gone through the grill and punctured my radiator. They explained that radiators used to be made of copper and now are made of aluminum and are very fragile. They also said that the spaces in the grill should be smaller so it's not so easy for rocks or debris to get to the radiator. I asked if this was a common occurrence and they told me they've seen it 3 times in 2 years just at this one dealership. I feel the design of the grill is a safety hazard and warrants a recall. I'm afraid someone else won't be as lucky as me. We could have easily been killed or caused a massive car pile up on that highway that resulted in many people being injured. It was terrifying. I called manufacturer's customer service department to share this and asked that the error message be changed. They referred me to you regarding the design flaw of the grill.

There’s something wrong in the electrical system. In order to start the car, it requires boosting the battery only. Boosting under the hood doesn’t work with the lithium power packs charged 100%. After signing the financial documents the day I test drove it, I couldn’t drive it home if I wanted to. The car wouldn’t start. I don’t know how the dealer got it started but I have been intermittently boosting it to start. This happens whether the car sits a day or more. The last frequent boost was 3/17/2025. Another thing, the car can be in driver assistance with lane tracking and the lane tracking will shut off automatically. Also, when the lens of the cameras on the car gets wet, the cameras don’t work. They become foggy blank.

7 months after purchasing a Lexus 350h, the driver seat started having noise when it moves. Three steel balls and several pieces of white plastic detected in the rail. Look like a ball bearing, which is part of the power seat, was disintegrated.

My vehicle has a vibration in the steering wheel and floorboard between approximately 38-48 Mph but mostly around 43 mph. It is random in severity and sometimes feels like the vehicle has a flat tire.With this issue it is impossible to determine if the vehicle has a tire out balance, an alignment issue or some other drivtrain component problem. This could cause someone to pull the car over to check the tires and impede traffic, possibly causing an accident. The dealer couldn't detect the vibration. After 4 months an FTS finally detected this vibration on a test drive. He then said it was normal operation characteristic from the hybrid powertrain. I was told all of these vehicles perform this way. I've driven 2 other vehicles, identical to mine, that absolutely do not exhibit any of the vibration and harshness my vehicle does. This is an unacceptable "driving characteristic" of a $52,000 luxury vehicle. There have been no warning lamps or notifications on the instrument panel. This problem has been present since new (December 2023)but took me about 3 months to narrow down what was happening and at what speed. Rough roads can disguise this vibration but can be felt nearly every time the vehicle is driven.

Lexus [XXX] & [XXX] [XXX] [XXX] [XXX] Lexus P.O. Box 259001 Mail Drop E3-2D Plano, TX 75025-9001 To Whom It May Concern, I am writing to express my serious concerns regarding a safety issue we encountered with our 2024 Lexus NX350, purchased from your dealership at 1000 Interstate Parkway, Akron, Ohio, 44312, in August 2023. Our purchasing experience met the high expectations associated with the Lexus brand, and we were pleased with the vehicle's features. During the delivery process, we received a walkthrough of the car's functionalities, and the technician assisted us in connecting her phone to download the app. However, there were no tutorials or instructions provided regarding the safety features, particularly in the event of an emergency except for showing us the SOS button on the at the roof panel On [XXX], my wife, [XXX], was involved in an accident while driving the NX350. I have included a photo to illustrate the extent of the damage. When the police arrived, [XXX] found herself unable to open either the driver or passenger doors; all doors were unresponsive. The officer (Hetrick #426) from the Macedonia, Ohio Police Department was also unable to assist her from the outside because the doors were not functioning from his side, as he could not determine if she was injured. She used the SOS button, which the call center was responsive, however, the could not assist with helping her to make the doors function. Faced with the urgency of the situation, [XXX] had no choice but to climb out through the window, which was extremely difficult for her given her injuries. This incident raises significant safety concerns. When we contacted the dealership to inquire about any known defects, we were told there were none, and the response was defensive, focusing on how we were instructed to manually open the doors during our walkthrough. However, neither of us was shown how INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

On Sep 16, 2024 I purchased a 2024 EX450 Plug-in Hybrid. During the purchasing process that new car delivery person recommended using the Lexus app which I installed. From Sep 18 through Sep 23 I drove the car in the EV mode around my community. I put on approximately 15 miles. On the morning of Sep 24, 2024 I needed to go to an important meeting. On Sep 23 in order to prepare for a 40 mile drive I plugged the car in and obtained a full charge. At 6:15am I opened the hatch and packed my car. The rear hatch opened without any issues. At 6:30 am I tried to enter the car and the car was locked. Fortunately I left the hatch open and I was able to crawl through the back of the car and open the car doors. I then tried starting the car and the car would not function, no click, no lights, all electric systems were dead. I reviewed the app on my phone and what was 36 miles 100% charged the previous night now stated 0% charged and -1 miles. Something had discharged the car overnight. There were no lights on or doors left open. The car was transported to the dealer. After a day of testing the battery (which was within spec) and the functions of the car the tech recommended the app that I used to monitor the charge status and other status's of the car including the ability to remote start the car be removed from my iphone. It is thought that the car overly communicates with the app thereby draining the battery. Doing on-line research I learned that others have come to this same conclusion. I am requesting that Toyota Motor Company immediately investigate this issue and if the app is the culprit immediately issue a warning to all its consumers that use the app to immediately remove the Lexus app from their phone. If a person had placed their children in a car seat, activated the hatch and closed the doors the doors would automatically lock. A caregiver could inadvertently lock their child in the car.
