Driver gauge instrument screen does not illuminate upon starting vehicle and at times when driving. No MPH or other gauges display. This defect has occurred several times and is intermittent.

The backup camera and infotainment screen is black, the backup camera and all controls on the infotainment screen was not usable because the screen was black. This is dangerous because no rear view camera.

On 2026-07-05, my car started except the digital instrument panel was blank/black. This meant the digital gas and battery gauges weren't visible, in addition to the speedometer, etc. I tried restarting the car to see if it was a fluke but it did not come back. On 2026-07-06, the car worked fine / as expected, but after a google search, it seems like this happens to other owners as well in the past with no clear reason why. Wanted to report in case it becomes a larger thing.

I started my vehicle and thr complete infotainment screen was black, no display, no backup camera, nothing but a black screen. The volume button still worked and you could touch the screen and her things change but you could not see anything because the screen was black. It is like turning the brightness completely off a touchscreen, it works but you can't see anything. There are current recalls for this for other models but they say mine is not included. But I had the exact same problem with mine. It did not work until I turned my car off, returned and restarted my car, after that it worked normally. My can needs to be included in the recall.

While driving on the highway, something (likely a small rock) from oncoming traffic flew into the windshield. Upon contact, the windshield cracked and shattered in a manner I have not seen a windshield break before, immediately spraying glass shards across the interior of the vehicle (to include dashboard, seats, and floors) and the driver. From the outside of the windshield, the area of damage is soft to the touch such that it is likely applying any pressure would collapse that area and/or enable you to punch straight through to the other side, so for safety concerns it is undrivable until the glass is replaced. I am concerned the glass could have been defective in some way, as I don't believe this is a typical response to common road hazards like small rocks/other projectiles. The windshield was the original factory installed windshield. The vehicle has not yet been inspected by the manufacturer but the incident has been reported and owner is awaiting contact from the manufacturer regarding inspection. It can be available for inspection until the glass is replaced, but owner will have glass replaced as soon as possible. Additionally, because it is not drivable, it would require inspection at residence or towing. The incident was reported to insurance. There have never been any previous warning lamps/other indicators that the glass was defective.

3 unresolved safety defects on 2024 Lexus NX 450h+ plug-in hybrid leased May 2023: ELECTRICAL/BATTERY: Rear seat occupant sensor produced continuous false alerts since weeks after lease began, preventing vehicle from locking. High-voltage components including on-board charger located beneath/near the rear seat area per dealer representation at time of sale. Service manager acknowledged liability in writing July 2023. Lexus TSB L-SB-0035-24 addressing battery monitor module failure (DTC P162B87) never applied. Recall 25LD01 covering high-voltage floor wire corrosion unverified for this vehicle. Not charged since approximately April 2025 — over one year — due to fear of battery malfunction, fire, or explosion. Operating exclusively on combustion engine, eliminating primary plug-in feature. Reported in writing for 2 years 8 months. Never repaired. GM confirmed in writing February 2026 defect never submitted to manufacturer. DRIVETRAIN/TIRES: Severe premature tire wear from approximately 2,252 miles. At 7,800 miles dealer declared vehicle unsafe for freeway driving. Owner used alternative transportation for court appearance due to safety warning. Dealer's own alignment printout January 2026 shows multiple out-of-specification values. All 4 tires replaced after 72 consecutive days dealer possession, classified as goodwill, never submitted as warranty claim per GM written admission February 2026. HVAC/BIOHAZARD: Persistent mold/mildew odor from HVAC since early in lease. Service advisor initially attributed cabin odor to rat feces in engine bay entering cabin through air circulation — admitting biohazard exposure to occupants. Rodent nesting caused by manufacturer's soy-based wiring insulation. Rat feces constitute hantavirus and salmonella risk. Same defect across three consecutive vehicles from same dealer. Engine bay sanitized and evaporator cleaned during 72 days, classified as goodwill only, never submitted to manufacturer. Documentation available for inspection.

The contact owns a 2024 Lexus NX 350. The contact stated that while his wife was driving approximately 40 MPH, the instrument cluster went blank. No warning lights were illuminated. The vehicle was taken to the local dealer, but no fault was found. The failure was self-corrected. The vehicle was not diagnosed or repaired. The manufacturer was contacted, but no assistance was provided. The approximate failure mileage was 20,000.

Possible malfunction involves vehicle speed control and braking during a low-speed parking maneuver in my 2024 Lexus NX350h. While slowly pulling into a parking space, I was actively applying the brake and creeping forward. While my foot was on the brake, the vehicle suddenly surged/accelerated forward instead of continuing to decelerate. The vehicle ran over a sign and jumped a curb before I could stop it. The vehicle is available for inspection and is currently with the Lexus dealer. My safety and the safety of others were put at serious risk because this occurred unexpectedly while parking. If a pedestrian, fixed structure, or another vehicle had been in front of me, this could have caused severe injury or worse. Thankfully, I was not physically injured, but this was a traumatic event, and I was terrified. The problem has not been reproduced by me after the incident. At the time of this report, it has not yet been confirmed by the dealer or an independent repair facility. I have requested a full diagnostic inspection and review of all available stored data before any codes are cleared. The vehicle has been delivered to the Lexus dealer for inspection. The incident was also immediately reported to my insurance company. I do not know if Lexus manufacturer representatives or other parties will inspect it. I did not observe any warning lamps, messages, or unusual symptoms before the event, and this had never happened before.

ELECTRICAL SYSTEM
The subject vehicles are equipped with a GPS antenna wire harness that will not allow the GPS to operate when the vehicle is off as intended. As a result, the vehicle location cannot be identified through stolen vehicle location services when the vehicle is off.

Some 2022 – 2024 model year NX 250 and NX 350, and 2023 – 2024 model year RX 350 vehiclesmay exhibit the following condition at vehicle startup when the vehicle is in Park: • A “Shift System Malfunction” message on the Multi-information Display (MID), as shown below. This condition is more likely to occur when the outside temperatures are near freezing. The Shift Actuator ECU logic has been modified to reduce the possibility of these conditions.
STRUCTURE:BODY
Some 2024 – 2025 model year NX 250, NX 350, and NX 350h North American Produced vehicles may exhibit a consistent/repeatable tick/abnormal noise from the top of the B-pillar while driving. The abnormal noise can be heard from either passenger or driver side. The spot welds between the top of the B pillar and the seatbelt anchor may contain weld slag which can create an abnormal noise.
