The contact owns a 2011 Mazda Mazda3. The contact stated that while operating the vehicle, the front driver’s side seat wobbled and was unsecured. The vehicle was taken to the local dealer, who determined that the front driver’s side seat lifter link was inoperable and needed to be replaced. The vehicle was not yet repaired. The manufacturer was notified of the failure, but no assistance was provided. The contact was informed that the VIN was not included in NHTSA Campaign Number: 17V082000 (SEATS). The contact stated that the vehicle had experienced the failure listed in the recall. Additionally, the contact stated that the dashboard was melted and warped. The contact was concerned that the front air bags would not properly deploy due to the warped dashboard housing in the front driver and passenger side air bags. The failure mileage was 167,850.
The airbag system failed to deploy during a major accident I was in, where my car rolled multiple times, and the collision that totaled my car. This includes the front airbags, impact sensors, and/or the airbag control module not activating. The vehicle is currently in the possession of the insurance company and can be made available for inspection upon request. During the crash, no airbags deployed, which significantly increased the risk of serious injury or death. The lack of airbag deployment left my partner and me completely unprotected at the moment of impact, including the number of times we rolled, seriously putting our safety at severe risk The vehicle is no longer drivable. The police report notes that the airbags did not deploy. The vehicle is in the process of inspection by my insurance adjusters as part of the total‑loss evaluation. The manufacturer has not yet inspected the airbag system, but the vehicle can be made available if Mazda requests an inspection. There were no warning lights, messages, or symptoms indicating an airbag or safety system issue before the crash. The airbag system gave no indication of malfunction before the failure occurred.

Defective Driver's seat construction made by Mazda Mfg Corp. I was the first person to report this cheaply made driver's seat failure to Mazda Corp in Irvine, CA in early 2015 or 2016. Recall was finally issued a year afterwards because Mazda dragged their feet, and only after I threatened to sue them if I was ever in an accident, and the seat was to totally fall apart, and maim me for life did they finally take action. The Recall fix (cheap bandaid clamp that again failed because seat frame welds had failed as well) was never done right, and as a result the seat totally fell apart, and Ed Howard Lincoln Mazda of Sarasota, FL didn't want to do anything further at that point (cost out of pocket wise). I was told by a different Mazda Dealership in Ft Myers, FL that that defective seat should have been totally replaced if they would've handled it correctly. So Ed Howard Lincoln Mazda really screwed me on that one! As related to the 2.5L Mazda engine, I also had/have a engine failure part there as well. It's called the ENGINE COOLANT (WATER) OUTLET FLANGE, and the bottom left hand bolt (1 of 4 bolts total) that's suppose to hold this cheap plastic part in place was stripped (over-torqued) at the production assembly line time that the engine was made (put together). Now the cheap plastic replacement part is leaking again from the same lower left hand corner, and this is a well Known Ford & Mazda 2.5L engine failure design as stated in many a Mazda online forum! This also should have been recalled, as now my 2011 Mazda 3 is down again. Engine overheated again on Oct 13, 2023. Prior repair to replace this defective Flange was done in Feb or Mar of 2022. Cheap Plastic parts put on a Metal engine just don't work well at all. Common Physics would tell you that! I have documents in my storage unit if you need them.
Electrical

The Dashboard is peeling above the display screen that shows information for Radio,CD, AUX, Time, Volume, Temp Outside, etc (black display screen with orange text/font). Above this display screen the dashboard is lifting up. Which has created about a 1 inch+ gap/spacing between the display and dashboard This gap has resulted in the following: 1). Inhaling of windshield fluid through AC vents (4 total on dashboard) -- when using windshield wipers to dispense windshield fluid to wipe windshield clean of dirt/debris. You can smell the funes from the windshield fluid. And windows are not open when using the windshield wipers. 2). The material used for the dashboard has peeled off and gone in between the gap. It has resulted in the AC vents dispensing the material when adjusting the open and close feature of the AC vent or making any direction adjustment of where I want the air conditioning to blow in the car. 3) The gap/spacing between dashboard and display screen has also resulted in the interior side of the windshield to have a buildup of smog on the windshield. It has resulted in issues seeing at night time when cars are driving towards me with their headlights on. When i wipe down, it will be covered again with smog/film in a few days.

I don’t know when it becomes a regular issue. 4-5 days ago, when I was turning right on a small street, the wheel locked again. I was about to crash. It’s a deadly malfunction, so I don’t want to wait until then.

UNKNOWN. Electric power steering just suddenly stopped working for no apparent reason. Very hard to steer. DANGEROUS!
When I’m driving the car, electricity of the car goes off, including headlights. The steering wheel gets locked. It doesn’t do that regularly. Sometimes, it takes weeks to do that, but sometimes it happens once in every five minutes. The red airbag light on the odometer turns on or off randomly. The contact owns a 2011 Mazda Mazda3. The contact stated while driving at an undisclosed speed, the clutch became inoperable. There were no warning lights illuminated. The dealer was not contacted. The vehicle was not diagnosed or repaired. The manufacturer was notified but no additional assistance was provided. The failure mileage was approximately 80,000.
I GOT THIS CAR BACK IN AUGUST NOT KNOWING ABOUT MAZDA'S CLUTCH ISSUES. I GOT THE CAR WITH ABOUT 101,000 MILES, AND WHEN I GOT TO ABOUT 112,000, MY CLUTCH FAILED OUT IN THE MIDDLE OF NOWHERE WHILE I WAS DRIVING ON A HIGHWAY IN KANSAS. I WAS SHIFTING UP FROM 4TH GEAR TO 5TH GEAR AND THE CAR FELT LIKE IT WENT INTO NEUTRAL, BUT NONE OF MY GEARS WERE WORKING. FORTUNATELY, I WAS ABLE TO PULL OVER TO THE SHOULDER AND WAIT FOR A TOW TRUCK, BUT MY CAR WOULD NOT DRIVE. I DO NOT KNOW WHEN THE CLUTCH WAS REPLACED BY THE PREVIOUS OWNER, BUT AFTER RESEARCHING THIS ISSUE, I BECAME WORRIED THAT THIS IS GOING TO CONTINUALLY BE AN ISSUE FOR MY CAR FOR THE REST OF THE TIME THAT I HAVE IT. IT IS AN EXPENSIVE FIX, AND I CANNOT AFFORD TO REPLACE MY CLUTCH APPROXIMATELY EVERY 35,000 MILES AS MANY OTHER MAZDA 3 DRIVERS HAVE. MAZDA SHOULD REALLY RECALL THE PART AND WARRANTY ITS REPLACEMENT.

my Mazda 3 2011 passenger headlight always had water coming into the headlight itself. It has condensation and it is hard to see when using the headlight itself at night. have to wait a few days or so to clear up and then my headlights will flicker because of the water seeping inside of it.
PASSENGER AIRBAG LIGHT WILL GO ON AND OFF WITH NOBODY IN THE SEAT. THIS HAPPENS RANDOMLY AND NOT CONSISTENTLY. SOMETIMES WHILE STATIONARY, SOMETIMES WHILE DRIVING. THE DASHBOARD HAS COME APART FROM THE CENTER PIECE ON THE DASH WITH THE DIGITAL DISPLAY. CAR WAS PURCHASED IN TEXAS IN 2013, DROVE TO MAINE IN 2015 AND DRIVEN TO VIRGINIA SINCE 2018. PROGRESSIVELY GOTTEN WORSE REAR VIEW MIRROR HAS STRANGE BROWN MARKS INSIDE THE MIRROR MAKING IT DIFFICULT TO SEE. OWNER DOES NOT SMOKE. STRANGE CLUNKY NOISE FROM REAR SUSPENSION, NOT CONSISTENT VERY RANDOM. SMOOTH ROAD, BUMPY ROAD, FAST OR SLOW, THE SOUND JUST RANDOMLY HAPPENS FROM TIME TO TIME. HEADLIGHTS FORM MOISTURE, OWNER BOUGHT NEW HEADLIGHTS WITH MECHANIC INSTALL AND THEY STARTED TO COLLECT MOISTURE ABOUT A YEAR AFTER INSTALLATION. OWNER OF VEHICLE SINCE 2013 TO PRESENT. VEHICLE PURCHASED AT APPROX 60K MILES, CURRENTLY APPROX 130K MILES.

MAZDA HAS TO DO SOMETHING ABOUT THE TCM. MINE GOES IN AND OUT AND I CAN'T PULL IN FRONT OF TRAFFIC WHEN THE ACCELERATION GOES DOWN. IT'S AN ACCIDENT WAITING TO HAPPEN AND IT KNOWN THE THIS PARTICULAR PART IS KNOWN TO GO OUT. IT'S A SAFETY ISSUE MAZDA NEEDS TO ADDRESS IMMEDIATELY BEFORE SOMEONE DIES. THE FIRST PICTURE IS WHEN THE CAR IS IN MANUEL MODE AS YOU CAN SEE THAT'S SUPPOSED TO BE A 5 FOR FIFTH GEAR WHICH U CAN SEE THAT IT'S HARD TO TELL AND THAT IS WHEN IT'S WORKING WHEN IT GOES OUT THE CARS ACCELERATION SLOWS DOWN AND THERE'S NO DIGITAL DISPLAY AT ALL. THE SECOND PICTURE IS WHEN IT'S IN AUTOMATIC THAT IS SUPPOSED TO BE A D LETTER LETTING YOU KNOW AS THE DRIVER THE GEAR IS IN DRIVE. THIS A FREQUENT PROBLEM AND I'M NOT WILLING TO RISK MY LIFE AND POSAR HE LIVES OF OTHERS FOR A MISTAKE THAT SHOULD BE CORRECTED BY MAZDA.
AIRBAGS: DID NOT DEPLOY UPON IMPACT OF A STREETLAMP POLE ON THE FRONT DRIVERSIDE QUARTER. FRONT FARRING DAMAGED AND REPLACED. I WAS TURNING LEFT, IT WAS RAINING, THE GLARE OF THE LIGHT BLINDED ME ENOUGH TO GO UP ON THE CURB AND IMPACT THE STREETLAMP POLE. I REPLACED THE FARRING AND HAD MY MAZDA3 FULLY ALIGNED. ELECTRICAL: PURCHASED VEHICLE IN OCTOBER 2016. REPLACED BATTERY 4 TIMES, ALTERNATOR 3 TIMES SINCE PURCHASE. SEAT HEATERS HAVEN'T WORKED EVER. DRIVERSIDE SEAT STARTED WIGGLING AFTER CONTACT WITH STREETLAMP POLE.

The contact owns a 2011 Mazda Mazda3. The contact stated that while driving at an undisclosed speed, the steering wheel started to shake. The contact stated that the tire pressure warning light and check engine warning lights intermittently illuminated. The vehicle was taken to the dealer where it was diagnosed that the suspension, seat connector, and an unknown software needed to be updated. The contact then stated that due to the suspension failure, the tires were worn prematurely. The vehicle was repaired; however, the failure recurred. The vehicle was not diagnosed or repaired. The manufacturer was notified of the failure and the contact was advised to call the NHTSA Hotline for assistance. The failure mileage was approximately 170,000.

DECEMBER 6, 2018 DEAR NHTSA, I HAVE A 2011 MAZDA 3 WITH FOUR (4) WESTLAKE RP18 205/55R16 91V RADIAL TOURING TIRES. I AM WRITING THIS LETTER TO NOTIFY THE COMPANY THAT AT LEAST THIS LOT OF TIRE OR THIS TIRE LINE IS STRUCTURALLY DEFECTIVE. I HAVE ATTACHED PICTURES ON PAGE 2. THERE ARE SEVERAL IMPLICATION TO THIS DEFECT INCLUDING BENDING OF TIRE SHIELD, EASY PUNCTUATION AND EASY BLOW-OUT. ALL OF THESE INSTANCES CAN LEAD TO TRAFFIC ACCIDENTS AND EVEN DEATH. FOR INSTANCE, RECENTLY ROAD OBSTRUCTIONS PUNCTURED BOTH OF MY 2 BACK TIRES. THESE TIRES WERE FLAT WITHIN 30 MINUTES WHILE I WAS OUT SHOPPING AND A 20 MINUTES DRIVING DISTANCE FROM MY HOME. I PURCHASED A FOOT AIR PUMP TO PUT ENOUGH AIR IN MY TIRES TO TRAVEL ONE BLOCK AWAY TO THE AUTO SHOP TO REPLACE BOTH TIRES. FURTHERMORE, I DROVE AT SPEEDS ON THE HIGHWAY AT LESS THAN 20 MPH (MILES PER HOUR). AT THE STORE THEY REMOVED THE TIRE AND IDENTIFIED THE OBSTRUCTIONS, HOWEVER, THE TIRES EXPLODED VERY EASILY. THESE TIRES CAN BE PURCHASED FROM WAL-MART AND AMAZON FOR $44.24 EACH OR $176.96 FOR 4 TIRES ONLINE. I THINK THESE TIRES SHOULD BE RECALLED AND THIS MATTER PURSUED VIGOROUSLY PARTICULAR BECAUSE OF THE UPCOMING 2018 HOLIDAY TRAVEL SEASON. ON OCTOBER 16, 2018, I MAILED A CORRESPONDENCE LETTER TO TIRECO INC., SUPER-REPRESENTATIVE OF WESTLAKE TIRE. THE LETTER IS LISTED ABOVE. THIS MAIL WAS CERTIFIED WITH THE UNITED STATES POSTAL SERVICE UNDER CODE: 7018 0680 0000 5488 3046. I HAVE NOT RECEIVED A RESPONSE FROM THE COMPANY. AS STATED ABOVE, I BELIEVE THESE TIRES ARE STRUCTURALLY DEFECTIVE AND NEED TO BE RECALLED. Transmission slips out and shift shock in first gear to second year. Over revving and bucking on the freeway. These are BAD transmissions

ON APRIL 8 WHILE PICKING UP MY DAUGHTER FROM SCHOOL ON NAUTILUS STREET IN LA JOLLA, POINTED DOWNHILL ON A STEEP GRADE, MY IDLING VEHICLE LOST POWER WITHOUT WARNING, I LOST CONTROL OF STEERING AND BRAKES HAD TO STOP THE VEHICLE WITH THE HANDBRAKE, THE PASSENGER CAR DOOR WAS OPEN AT THE TIME MY DAUGHTER WAS TRYING TO STEP INTO THE VEHICLE AS THE CAR TOOK OFF, THE DOOR HIT THE CAR IN FRONT OF ME BREAKING HER TAIL LIGHT, IT TOOK ME THREE TRIES TO GET THE ENGINE TO TURN BACK OVER.
TL* THE CONTACT OWNS A 2011 MAZDA MAZDA3. WHILE DRIVING 15 MPH, THE REAR BRAKE STOP LAMPS ILLUMINATED INDEPENDENTLY WITHOUT WARNING. THE CONTACT STATED THAT THE FAILURE WAS INTERMITTENT. THE VEHICLE WAS TAKEN TO AN INDEPENDENT MECHANIC TO BE DIAGNOSED. THE CONTACT WAS INFORMED THAT THERE WAS A POSSIBLE FAILURE DUE TO THE ABS MALFUNCTION. The Baffle in my tank has broken loose for uncertain reasons. This seems to be a common problem with Mazda and has had a recall in the 2015 models even.
CAR WAS HARD TO START IN 2014..REPLACED THE SPARK PLUG .CLEAN THE MAF SENSOR AND AIR INTAKE .USED 89 GRADE GAS .NOTHING WAS CHANGE .CAR WAS NOISE FROM THE BACK IN 2016.DIDNT FIND OUT THE PROBLEM .THEN 2WEEKS AGO SOMETIMES CAR STARTED AND FAILED RIGHT AWAY .DO MATTER THE ENGINE WAS COLD OR WARM .TOOK IT TO DEALERSHIP .REPLACED FUEL PUMP .COSTED $988.CAR JUST GO BACK THE NORMAL FINALLY .SO THE FUEL PUMP HAD PROBLEM IN 2014.BUT JUST DIDNT DIE.FUEL PUMP SHOULD LAST LIFETIME WITH THE CAR .WE HAD SOME OLDER CAR LIKE 11YEARS .THERE IS NO PROBLEM WITH FUEL PUMP .THEY SHOULD RECALL IT .I SEEN A LOT PEOPLE HAVE SAME PROBLEM ONLINE .AND THEY DIDNT FIND OUT THE PROBLEM

TIRE WEAR TO THE POINT OF REPLACEMENT @ 21K MILES. *TR
DRIVING SOUTH ON IH-35 IN SAN MARCOS, TEXAS, A BLOWOUT OCCURRED IN RIGHT REAR TIRE. DRIVING CONDITIONS WERE DRY AND SUNNY. THE TIRE IS A TOYO A20, SIZE P205/55R16 89H. THE INNER TIRE WALL SEPARATED COMPLETELY FROM THE REST OF THE TIRE. DRIVER DID NOT LOSE CONTROL AND WAS ABLE TO PULL OVER. THESE TIRES WERE ORIGINAL EQUIPMENT ON VEHICLE WHEN PURCHASED FROM DEALER IN 2011. *TR

ON FRIDAY MY WIFE WAS DRIVING MY CAR (SHE LEARNED HOW TO DRIVE ON A MANUAL AND HAS NO PROBLEMS DRIVING IT) ON A BACK COUNTRY 2 LANE ROAD WITH OUR SON IN THE BACK AND OUT OF CELL PHONE COVERAGE. SHE WAS DRIVING ALONG AT 60 MPH, IN 6TH GEAR WHEN THE RPS STARTED TO BOUNCE OFF THE REV LIMITER. SHE CHECKED TO MAKE SURE SHE WAS STILL IN GEAR. SHE TOOK IT OUT OF 6TH AND PUT IT INTO NEUTRAL THEN TRIED TO GO BACK INTO GEAR. NO DICE, IT WOULDN'T SHIFT. SO SHE PULLED OVER AND SHUT IT DOWN. SHE TIRED TO START AGAIN, TRIED TO GET IT INTO FIRST, NOPE, TRIED TO GET IT INTO SECOND, AND NOTHING. AMAZINGLY SHE GOT SOME SIGNAL ON THE PHONE AFTER A PANICKED 20 MINUTES AND GOT ROADSIDE ASSISTANCE OUT TO TOW HER IN. NOW MAZDA IS SAYING THAT THE CLUTCH IS BURNED UP AND THAT IT IS NOT A DEFECT SO IT IS OUR FAULT AND IT WON'T BE COVERED UNDER WARRANTY. *TR
