I am writing to formally report a critical safety defect regarding the driver-side footwell design in my 2023 Subaru Forester Sport, which poses an immediate and severe risk of a crash. A hard plastic lower extension for the HVAC air duct sits dangerously low directly above the brake and accelerator pedals. For drivers with larger or longer feet, this duct extension acts as a physical obstruction that interferes with safe pedal operation in two specific, highly dangerous ways: Delayed Braking: When attempting to transition from the accelerator to the brake pedal, the toe of the driver's shoe frequently catches on the lip of the air duct extension, delaying the ability to depress the brake pedal in time-sensitive or emergency situations. Pedal Entrapment: When the brake pedal is depressed, the driver's foot can become completely wedged between the brake pedal arm and the air duct extension. This traps the foot, making it impossible to smoothly or quickly release the brake. The physical proximity of this hard plastic duct to the primary driving controls is a severe design flaw. The air duct should not protrude into the clearance space required for standard footwear, as it directly compromises the driver's ability to control the vehicle's speed and braking. I have photo and video documentation clearly demonstrating the lack of clearance and exactly how a shoe becomes caught and wedged during normal driving maneuvers. I am requesting that the NHTSA Office of Defects Investigation (ODI) review this hazard immediately, as this spatial interference requires a recall and physical redesign of the lower ductwork to prevent potential collisions, injuries, or fatalities. The component causing the issue is the physical HVAC ductwork extension, and the vehicle is available for inspection upon request. Because this is a physical design flaw rather than an breakdown, there were no warning lamps or dashboard messages. I have photo and video documentation.

Eyesight collision braking system engaged as I was rolling up my driveway to go into garage. Garage door was rolling up and was about 5 feet off the ground when Anti Collision system engaged and slammed the brakes on. I was going under 1 mph and would not have hit the door. It beeped right before brakes engaged. This is the second time it has engaged when it shouldn't have. The first time was in a right lane, a car about 6 car lengths in front of me was and did turn right as I was driving down the road. It almost caused an accident with the car behind me. The brakes slammed on so hard my back and neck are now sore and it bent my fingernail backwards somehow on the steering wheel.

The contact stated that while driving at an undisclosed speed, the vehicle independently accelerated, and the Forward Collision Avoidance system was inoperative, causing the front center of the vehicle to crash into five other vehicles while trying to stop the vehicle. The air bags deployed as needed. No warning lights were illuminated. The vehicle was deemed a total loss at the scene. A Police report was filed. No injuries were sustained, and medical attention was not needed. The manufacturer was not notified of the failure. The failure mileage was approximately 15,000.

Eyesight camera randomly turns off and on while driving. Has been consistent over several months pierced to be an ongoing issue that was previously recorded from 20 14–20 21 super Forrester. Very similar issue now with my 2023.

In three years I've had the Automatic Emergency Braking system engage and almost cause accidents several times. The breaking system engages when it's not needed. Going down a road driving straight at 25 MPH, no obstacles in road and breaks engage. Verry scarry, what if I was going 65 MPH on the highway! Another incident was on an off ramp in a curve, and another making a U-turn. This is not ok!

Traveling at approximately 45 mph through a green light at a 4-way intersection a car making a right on red turn turned into the outside lane in front of me. I had no warning sounds or automatic braking, to avoid a collision I had to steer to the left after an on coming car passed. There were no chimes or auto braking.I have had several instances similar with no alerts. My eyesight system has been problematic for the life of the car and required many diagnosis and recalibration with many trips to the dealership and no permanent repair. Frequent software updates and days with no eyesight -see manual. Finally had a total eyesight replacement with the assurance this was a long awaited technology all under 60,000 miles. A few months later eyesight malfunctioned again and required another update. Eyesight still remains unresponsive and goes in and out a couple of times a day. The technology is defective and unreliable. Subaru has provided no lasting fix.

In driving our 2023 Subaru Forester at 45mph at 10:00 am in the morning, with NO cars in front of us, in back of us, or on either side of us- suddenly the brakes locked up! Had there been anyone behind us, there could have been a dangerous collision. We have owned this car for 2 years, done all maintenance on it, and have NEVER had this happen before. We feel unsafe driving it now. We pulled the car over, got out to look under the car, thinking maybe we hit something in the road...but there was nothing...no warning....we contacted Subaru corporate, was given a Case number, but are still waiting to hear from them.

The check engine light and a flashing 'S' and other lights stating the safety features such as Eyesight, lane departure, RAB, etc. are turned off. This started on 1/15/25, and I took the car to the dealership on 1/16/25. The dealership stated the EVap System Leak Detection Pump sensor indicated a leak, and the EvaporateLeak Check Module needs to be replaced but is back-ordered until mid-March 2025. They turned off all the lights and told me I could keep driving the vehicle. On 1/20/25, all of the lights were back on. The car lags to accelerate and is using gas at a rapid rate. I feel unsafe driving the car for another two months with the check engine light on and the acceleration capacity decreased. The dealership will not allow me a rental vehicle unless I leave it on their lot for two months until the part arrives. I cannot agree with that. The dealership informed me that multiple Forester and Crosstreks models have the same problem. This car was purchased brand new from the Olympia dealership and it currently has about 22,000 miles on it.

On January 3, 2024, my Subaru forester suddenly lost emergency safety features such as the blind spot warning system. Many warning messages flashed on the screen as I drove north on rt 1 and the blind spot warning system was totally disabled. I believe that other safety features were likewise disabled, as well. I was upset as I had traded in my 2015 Forester because it lacked those safety features that in my new car had abruptly just stopped working! I drove my car to the Concordville Subaru dealership for diagnosis and repair. I was told that the tech found error code P0455. The tech also found that the gas cap was secured and he reset the code and test drove the car. The error code did not re-occur upon test driving the car. I was told that that error code meant that gas fumes had escaped from the tank and that, when that occurs, all safety features are disabled. As my gas cap was secured that explanation seemed very weak to me and I asked how that explanation could be correct. I was told that Subaru could do a "smoke test" to ensure no leaks of gas fumes. I was given a loaner car and the test was done and no issues were found. As my car was running fine, I took my car home. I am writing this complaint over a year past the incident because I have now had a second incident, different in detail, but I am fearing some major problem with the car. I will write a complaint regarding the second incident which occurred on January 5, 2025.

I was driving on a two lane highway in the mountains and came around a small right-veering turn in the road. I straightened out the wheel to resume traveling straight, and my car’s auto-steer kicked in and pulled left into the center divider, and then right across two lanes and crashed into the side of the mountain. The auto-steer has pulled me out of lanes prior to this incident as well.

While driving on a highway, one lane in each direction, my lane was going to end. In front of me were painted yellow highway lines directing me to head right to the new lane on the right side of the steel railings. My adaptive cruise control with lane keeping assistance still had me heading straight instead of to the right. I had to strongly turn my steering wheel to the right to avoid hitting the railing. Also I was following a car into a rotary with my adaptive cruise control on. Thankfully I wasn't going fast and the car started to speed up a little as the car in front of me turned around the rotary. I was able to steer it in the right direction. My point is, some of the new technology is showing commercials with drivers with their hands off the steering wheel, relying on the technology. I was glad I had complete control of the car, but these features aren't perfect.

I see black/gray spots on the headlights and sometimes I cannot detect if it is something in the road. Especially if it is raining. I called today for my maintenance and I am going to address this. Another issue that scared me to death. I was backing out of my garage, I was clear to continue backing but it kept beeping. Again I checked and nothing there, I began to continue and all of a sudden the car braked on its on hard. It was so alarming. I’m glad I was not in traffic. I am addressing this as well at my dealership. Another issue, I am feeling a hesitation when I accelerate, like it is not getting gas, then it starts going without hesitation. I am addressing this as well at my dealership. Another issue all of a sudden the eye sight crossed through came on. No reason for it to come on. I will address with the dealership.

The accelerator pedal was depressed, and the vehicle jolted forward. The contact was able to drive to a parking lot. After shifting to drive, the vehicle unexpectedly reversed. While in reverse, the vehicle would shift to drive erroneously. The vehicle jolted forward. The vehicle was shaking, jerking, and skipping gear. The engine and oil warning lights were illuminated. The local dealer was not contacted. The vehicle was not diagnosed or repaired. The manufacturer was not contacted. The failure mileage was approximately 11,426.

I was driving the car, towards the end of about a 40-minute drive the car, I can best describe, lost its mind. Without any prior signs of symptoms, in about the span of 5-10 seconds, the check engine light came on, then eyesight warning came on, then traction system error appeared and I lost all power. It started braking on its own and completely acting on its own. I could still steer, but I had no other control over the vehicle! THANKFULLY I was in a 25 MPH zone, with no other vehicles or pedestrians anywhere near me. I was able to steer the piece of junk into a vacant loading dock and at least get it off the street. I shudder to think what would have happened to me if the car did this on the interstate that I had exited from not five minutes prior to this incident. The car was rendered undriveable. I had it towed to the dealership. I don't know what is wrong with it yet as the dealer is closed on Sunday. This was an EXTREMELY DANGEROUS SITUATION! If the car lost its mind a mere five minutes earlier than it did, I could have been killed! I am still shaking from this incident.

I was driving down the main street of a small city. There is a decent amount of traffic, so I’m not sure what speed I was going but likely no more than 25 mph. All of a sudden, for no apparent reason, the pre-collision braking went into effect and slammed me to a total stop. I wasn’t too close to the vehicle in front of me, there were no pedestrians in the road, etc. thankfully the vehicle behind me was able to stop and did not hit me. I was able to move forward and pulled into the closet parking lot. I looked and could see anything directly on my vehicle. Subaru is saying that one of my tire’s pressure was low and that may have caused it or sunlight may have caused it. There was no low tire pressure light on my vehicle. The light had been on not too long before this and I had filled the tires. But no warning light on the day this happened. I was able to successfully drive further with no issues. Subaru does not seem to be taking this seriously. But, no matter the cause, if either of these things will cause a vehicle to come to an immediate stop, that is definitely a safety issue. And it is a safety concern that should be looked into.

The contact depressed the brake pedal, but the vehicle failed to respond. There was no warning light illuminated. The contact shifted to park(P) and turned off the vehicle, and the vehicle decelerated. The contact was able to pull to the side of the road. The contact stated that upon restarting the vehicle, the vehicle operated as needed. Additionally, the contact stated while his wife was reversing, the vehicle accelerated unintendedly and hit a chained gate. The contact's wife depressed the brake pedal, but the vehicle failed to respond. There was no warning light illuminated. The vehicle went into the woods, hit several small trees, and then the vehicle crashed into a big tree and bounced 30 ft. away from the tree. The air bags failed to deploy. The contact's wife sustained a sore neck, but did not seek medical assistance. A police report was filed. The dealer was notified of the failure. The vehicle was towed to an auto collision repair shop, awaiting for a damage estimate. The vehicle was not diagnosed or repaired. The manufacturer was not notified of the failure. The failure mileage was approximately 9,000.

Subaru Infotainment system frozen I've been experiencing an issue where the screen suddenly freezes and emits a loud beeping sound. I've tried restarting and resetting to factory settings multiple times, but the issue persists. The situation is worsening; my infotainment system has been unusable for a couple of days now, which means my backup camera is also not functioning. My friend mentioned that I could report this to the NHTSA, as it directly relates to safety concerns.

We were traveling on a limited access highway at approximately 65 miles an hour. Both the adaptive cruise control and the driver assist were engaged. A car approximately four car lengths ahead of us exited the highway into the breakdown lane. My car slammed on its brakes and reduced speed to less than 40 miles an hour with no warning. Had there been a vehicle behind us we certainly would have been hit. This is not the first time we have had difficulty with the adaptive cruise control braking unpredictably , but it’s the first time it endangered our lives.

The contact stated that while in a parking lot, the contact depressed the brake pedal and attempted to shift into gear and the vehicle accelerated inadvertently, jumped a curb, and crashed into a tree. The contact was able to put the vehicle in reverse; however, while the contact’s foot was on the brake pedal, the vehicle accelerated backward independently. The contact was treated by a local urgent care for whiplash. No one else was involved in the crash. The Police were dispatched to the scene however, no Police report was filed. The contact had the vehicle towed to an independent mechanic, but the mechanic was unable to assist. The vehicle was not diagnosed or repaired. The manufacturer was not made aware of the failure. The failure mileage was approximately 4,500.

If you have the adaptive cruise control (or regular cruise if you disable adaptive) and accelerate to pass another vehicle, as soon as you take your foot off the gas to coast back down to the cruise setting the car slams on the brakes to bring the vehicle back to the set speed. I have almost been rear ended several times when this happens. This has occurred since I purchased the vehicle. The dealership says this is a design feature and not a defect. It doesn't matter what speed the cruise is set at. If you are exceeding the set speed, the brakes apply when the accelerator is released.

Forward collision braking activated for no reason. No traffic in front or behind when activated. Steady speed of 35-40 mph when activated - conditions were clear and dry. No warning prior to incident, message appeared on dash during braking. Seat belts activated and car came to a complete and very abrupt stop. Thankfully no vehicle behind us or most likely would have been rear ended. Problem has not been reproduced as we will not drive the vehicle without disabling the Forward Collision feature on the car. Vehicle had less than 2500 miles on it when incident occurred. Vehicle has not been to the dealer for inspection yet but will have them check the Forward collision system at first oil change service unless requested sooner. Emailed Subaru customer service to notify them of the incident.

The contact stated while parking the vehicle, the brake pedal was depressed, but the vehicle accelerated unintendedly, and drove over a concrete curb and uphill toward a building. The contact quickly made a right turn, however, the fender and driver's side of the vehicle crashed into the edge of the building and scraped it. The vehicle then drove downhill through the grass and stopped on a sidewalk. The air bags did not deploy. The contact did not recall whether a warning light was illuminated. There were no injuries sustained. A police report was filed. The vehicle was towed to the dealer. The vehicle was not diagnosed nor repaired. The dealer informed the contact that it could take approximately two months for a Subaru engineer to diagnose the vehicle. The manufacturer was notified of the failure and informed the contact that they would follow up with her in two days. The failure mileage was approximately 547.
