On 6/11/26, around 2:10 pm, when i was slowly pulling over to park on a road side, the car suddenly rushed forward unexpectedly. It was so powerful, at the time I could only hear the panic reactions from the two passengers and sense the imminent collision with the parked bmw in the front of several car length away. Right before the collision i steered the wheel to the right to avoid the bmw and my car came to violent halt less than a foot next to the bmw. I came out of my car to check the tire position before backing out to park a bit behind. This was on a country road, a Thursday afternoon with not much traffic. There was no distraction. If not were the enough spacing i would have no doubt crashed into the bmw in the front. When the dust settled, I did a "power off" reset to the model S and drove the rest of the day normally without any incidents. I went to Tesla service center near where i live the next morning, 6/12/26, to report it. They asked the precise time the incident occurred. But did not inspect the car. I was told an inner inquiry will be opened and it would take at least a few days. They said i could continue to drive the car normally which i have not because of not knowing what had caused it. I went to the service center today 6/29/26 after more than two weeks of no feedback. I was very disappointed in the answers given. The guy said basically based on the design of the car the incident could not have been caused by the car ,without providing any info as to what had been found from the promised inquiry. There was no written report on this. The person offered to check the brakes but also said he did not think its necessary. Just to made me feel better he said. At last, he provided the tesla web site to request "private data" during time window of the incident. I told him i will seek help from NHTSA now that tesla is not. I was told by AI, tesla has collected data on every car they sold to the details that can reveal how the paddles were pressed.

I own a 2016 Model S P100D equipped with the Large Drive Unit (LDU). The component that fails is the rotor coolant seal in the rear drive unit motor. Coolant leaks internally into the motor and inverter, causing bearing wear, corrosion, isolation faults, and eventual complete drive unit failure. The affected drive units are still installed and available for inspection upon request. This defect puts my safety and the safety of others at risk because it can cause sudden loss of propulsion (especially at highway speeds), power reduction, or total drivetrain shutdown, potentially leading to a crash. Coolant in high-voltage areas also raises electrical fault/fire risks. The problem is widely reproduced and confirmed: Tesla service centers are familiar with it and replace failed units with updated “-U” revisions that include a coolant bypass. Independent EV shops confirm the exact root cause (rotor seal failure) via encoder inspections showing coolant intrusion and through disassembly. This is a documented pattern failure in all 2012–2020 LDU vehicles. No police or insurance inspections have occurred yet, as no crash or total failure has happened. Tesla is aware of the recurring issue through service records on similar vehicles. Early symptoms before full failure typically include rear motor whining/grinding (especially under load), reduced power warnings, “Rear Drive Unit Fault” messages, and visible coolant on the speed sensor. These can appear gradually. My vehicles have not yet experienced total failure, but the defect is known to be inevitable. I am proactively monitoring encoder sensors. Because this is a systemic design defect affecting thousands of vehicles with clear safety implications, I request NHTSA open an investigation and require Tesla to issue a recall for free repairs/replacements on all affected 2012–2020 LDU vehicles.

multitude of electrical problems: Fuel/Propulsion System, Lighting, and body controls along with all ADAS: Adaptive Cruise Control, Automatic Emergency Braking, Blind Spot Warning, Forward Collision Warning, Lane Departure Warning, Lane Keeping Assistance systems are impacted by random intermittent glitching as computer screen bubbling progressed with goo leaking out of the bottom of the main computer. Most recent symptom was disabling of ALL ADAS systems along with automatic high beam headlight switching. Long ago as screen began to bubble and express fluid the low windshield washer alert came on and remains even when the tank is full. Then the charge port responsiveness degraded and stopped automatically functioning requiring user to bypass by clicking the "open charge port", then waiting 3 seconds, then tapping "unlock charge port". User must perform this as remote manual operation from a NACS charging handle button push will FAIL. Long ago the computer controlled 'home link' module would have excessive time delays on the order of hours or days from when the user would program and or operate the function. I even had a "phanthom" car appear by the computer calculations with E-Braking before the ADAS system failed fully, a 1.0 Tesla system should not have that... Windows have randomly rolled down and the driver side rear door will unlock and open on its own as the door handles present themselves. The safety risk class is unknown unknown; if the root problem is the screen filling leaking on electronics any number of unknown, unplanned, unstudied risk failure modes exist that can not be predicted. I was parking the car and gently tapped on the accelerator to inch forward. The car accelerated suddenly, crashed into a concrete sign holder, and knocked down a sign, until I was able to stop it using the breaks. The accident caused $5000 worth of damage that I paid out of pocket to fix, since I assumed I must have made a mistake. However when I got the car back a month later, unintended acceleration happened AGAIN - under the same circumstances. I was parking and inching forward. I tapped the accelerator and the car accelerated suddenly. This time I was able to stop it before causing any damage. At this point, I KNEW that the first accident was not my error, since it had now happened again, and I was 100% sure that I did nothing wrong. I looked up "unintended acceleration - Tesla" and found examples of the exact same thing happening while parking. I looked for a fix, and it suggested changing the mode of the car to "creep." This so far has worked well.

My 2016 Tesla Model S (45,500 miles) is experiencing a complete failure of the Media Control Unit (MCU1), resulting in a total loss of the rearview camera display and defrost/defogging controls. This is a critical safety hazard. The unit was previously replaced by Tesla in 2021 under NHTSA Recall 21V-035. However, this replacement part has failed again after only a few years of low-mileage use. Tesla Service is refusing to provide a free repair or a goodwill discount for an MCU2 upgrade, claiming the 'recall repair' only had a 1-year warranty. It is unacceptable that a safety-critical component replaced under a federal recall fails twice in 45,000 miles. Tesla has failed to provide a permanent fix for the eMMC defect identified by NHTSA. By refusing to restore the camera functionality without charging full price, Tesla is knowingly allowing a safety defect to persist. I request an investigation into these premature failures of recall-replaced components

My car is equipped with Full Self Driving. When I use that mode the car drives over , on, or dangerously close to the middle yellow line. Basically it drives you into oncoming traffic every single time I use it. It also has erratic fluctuations in throttle control that would confuse other drivers and possibly cause an accident. It’s constantly increasing and decreasing the throttle and feels jerky going down the road. I took the car to the Tesla Service center yesterday and they confirmed the issue and checked the hardware associated with the system. They indicated that the hardware was all working and adjusted properly and that the issue is a software issue. They told me to not use it and that eventually maybe Tesla will put out an update that may fix the problem. Tesla has sold thousands of cars with FSD and continues to call the system in BETA. They have since moved on to 2 more versions of the software and not offered an upgrade to the thousands of early adopters with the original hardware and software. The system is dangerous to drivers and everyone on or near the roadways when these are activated. Tesla said that can upgrade these “older” 2015 and newer models but haven’t because it will cost too much for them. They charged over $8000 for the system and it has yet to work. They need to be forced recall the system and fix these cars for everyone’s safety. The service manager told me he has a similar issue with car and he just opts to not use it. So even the employees at Tesla know of the issue.

The accelerating pedal is the one that was affect it is available upon request for inspection. It put me in danger because while I was driving, it snapped off. Has not been inspected by others. There was no warning lamps and it just happened out of nowhere as well. I was driving.

The contact stated that while turning into a parking spot, there was an unintended acceleration of the vehicle; it surged forward, jumped the parking barrier, and crashed into a wall. There were no warning lights illuminated. A police report was filed. There was no reported fire, injuries, or air bag deployment. The vehicle was towed to a body shop. The local dealer was not contacted. The vehicle was not diagnosed or repaired. The manufacturer was not contacted. The failure mileage was approximately 93,000.

On September 7th, 2024 around 7:35 PM, my tesla model S 2016 Suddenly accelerated, smashed in the siding of my house breaking the the siding of the wall and was totaled. All of this happened while my mom was trying to park the car in the garage the car suddenly took off without her pressing the accelerator. I have tesla to send an engeneer to collect the data of the car in which they said they did but have not yet gotten back to me on the issue. They informed me that they have collected the data on 09/20/2024

The infotainment unit in the car that controls functionality, I.e. navigation used to monitor battery availability is malfunctioning because of glue seeping from the unit and causing the screen to go black intermittently while driving creating a safety hazard for the driver and others due to a distraction. When this happens the driver loses visibility to navigation and other controls central to the vehicle functionality. Tesla is aware of this manufacturing defect that is a safety concern because it requires the driver to not focus on the road.

Incident Tesla Model S was up to date on recalls as far as we can tell, including the recent autopilot update via Tesla internet vehicle update releases. While driving in autopilot mode on the freeway in the lane next to the HOV lane, the autopilot got confused as to which lane it should keep and failed to maintain its lane. Incident vehicle drifted across the double lines into the HOV lane, where another Tesla Model S was driving. Both cars were damaged in the ensuing side bump impact, then the incident vehicle made a slight impact to another car rear bumper in front of the incident vehicle. No injuries were reported. A subsequent Traffic Crash Report was filed as required with CA Highway Patrol. We are awaiting this report in the mail. Our safety and others was put at risk due to the failure of the autopilot to maintain its lane. Insurance was contacted. The insurance company, USAA, has deemed the incident vehicle (our vehicle) damage too expensive to fix due to the very high cost of replacement/repair parts and has deemed our vehicle a "total loss." We have not been able to access the software of the vehicle. Since USAA has deemed the vehicle a total loss, they are selling the car for salvage and thus we have no further access to the vehicle for legal purposes. As far as I know, USAA has not pulled the vehicle software records nor has requested such records and has completely ignored the autopilot issue. That is part of the reason for our complaint here as well. Since there were no injuries, legal representation in this case has not been forthcoming. We ask the NHTSA to investigate since we can get no help from Tesla, USAA or any other legal advocate.

1.) Passenger Side Door is a potential hazard. Door latch and door mechanism does not open from the outside. 2.) Lane keep assist on Autopilot veers off to the right when it is first engaged and does not want to stay centered until it's been on for awhile.

You guys have ruined auto-pilot and made it 10x more dangerous and will cause more accidents in this format. It needs to be reversed back to what it was. Now when looking at the screen to change the temperature or change a song, or looking at the mirror, it asks you to touch the wheel immediately and causes unnecessary stress and potential mistakes, I've already oversteered the wheel several times swerving after it warns me to touch the wheel because I'm trying to look at a mirror quickly to not have the alert come on. In addition, it has 3 marks against me, and will be suspended from using it soon. never before have I even had a mark against me on the system. I believe your approach to fixing the situation is fundamentally flawed. People get into accidents with Autopilot because of their negligence, not because of autopilot. That is on the end user who has insurance and a driving record. Do not punish all of those who are good drivers, you are making us worse drivers now with the new autopilot to save a few accidents from bad drivers, but it will now cause many more in this format. This is a terrible decision on the regulatory part, and a sign of OVER regulation.

Driving 75mph on cruise control. Car sensed a vehicle in front of me (which there were no vehicles in sight) my Tesla started flashing the red car light and making the alert noise. My vehicle automatically slammed on the brakes dropping me to about 40mph instantly. My greatest fear continuing to drive this vehicle is if I use cruise control, and it senses a vehicle in front of me without their actually being a vehicle and slams on the brakes again myself or someone is going to get hurt. This has the potential to cause death or cause severe injury.

So essentially I stopped for a charge about 30 minutes before, I charged it, then hopped back on the turnpike as I was heading to Outer Banks NC, while I was on the turn pike where I believe at the time I started to go around a curve not a sharp one but a curve right after coming off this bridge. Every 1 minute and 30 seconds you have to put your hands on the wheel and give it a jerk to keep it enable so it had to have popped up within the 1 minute and 30 seconds. As I was changing the album of the artists I was listening too I heard the BEEP BEEP BEEP which means you are approaching something and are about to hit it, As I looked up all I saw was blinking lights and really smacked the rear end of this traffic control truck, anywhere from 65-75 miles per hour with no time to react. If I remember correctly I had my car on 7 lengths away (maximum amount you can have away). As I hit the control I hit my head pretty hard on the steering wheel so the whole memory is blurry following when it happened and my adrenaline was very high but I didn’t wanna go to the Emergency room even though they suggested highly because I have no health insurance and that would have had to come out of my own pocket and I was stuck in Pennsylvania by myself and didn’t wanna get stuck in the hospital too. Thank you so much for taking your time to look at this it means a lot.

Sudden unintended acceleration appeared to cause the Tesla while parking the car on the street in front of home accelerate go out of control and crash into the home totaling the car and damaging the house. Two days prior the vehicle was brought into a Tesla service location where Tesla replaced the 12 volt battery that they believed was causing the car not to power on properly at that time. After the crash and googling to see if others suffered similar issues it appears the NHTSA recently reopened a review of sudden unintended acceleration and this incident seems to align with concerns of the petitioner [XXX] . The police filed a report and it’s at the towing place where the insurer Allstate is going to evaluate the car for a total loss. Dear NHTSA, I am writing to file a complaint regarding a SUA incident that occurred on October 20, 2023, involving my 2016 Tesla Model S. On the day of the incident, I pulled into my garage and began to park my car. When I pressed the brake pedal, the car suddenly accelerated forward, crashing into the garage wall and totaling the vehicle. The garage wall was severely damaged and will need to be rebuilt. This is not the first time that my Tesla has experienced SUA. In 2017, I filed a complaint with NHTSA regarding a similar incident. In my previous complaint (issue # 11000097 dated June 19, 2017), I described a similar incident that occurred in December 2016. At the time, Tesla denied that the incident was caused by SUA and no further action was taken. I am convinced that the SUA incident on October 25, 2023, was caused by a defect in my Tesla Model S. I am also concerned that other Tesla owners may be at risk of similar incidents. I urge NHTSA to investigate this matter and take appropriate action to protect consumers. I believe that Tesla should be held liable for the damages to my vehicle and my garage wall, and that the company should be required to recall all Tesla Model S vehicles that are at risk of SUA. The car was on enhanced auto pilot and was moving at about 50 mph, the traffic in front of me slowed down. The car failed to give an alert, it failed to slow down and stop. I applied the breaks, but by then the car crashed into the pick-up truck in front of me. The car came to a complete stop after the crash and the computer system in the car completely shut down. The car had to be towed to the garage. My safety and the safety of the driver in front of me were at risk due to malfunction of the enhanced auto pilot system We immediately filed a service ticket with Tesla and upon repeated requests we were told that car could not be inspected and the breaks or auto pilot system could not be tested as the car was not drivable. Police and car insurance company were notified. Tesla was able to provide us with a EDR There were no warning signs or messages prior to the malfunction.

I’ve been complaining about my mcu1 glitching, freezing and my rear view camera not working since June 2022. I also filing a complaint because I have the fsd package on my 2016 mcu1 car and that has not been delivered after a 6 year wait. They released fsd to the mcu1 platform. But less than 50 cars got it and that very software got recalled. They have fixed every other platform but mcu1 since. I truly believe there is a issue with mcu1 hw3. I’ve complained several times about my mcu under warranty about these issues and they said the only fix is mcu2 . Which means they couldn’t fix it under warrranty. That’s a breach of warranty. I almost hit a elderly gentleman a few weeks ago because I was backing up and my screen went black. This has been happening and tesla knows all about it. But Tesla says the only fix is the paid mcu2 upgrade for 2500 dollars. Tesla has been really deceptive and is trying to force it customers to their upgrade. This has been a ongoing issue with many loyal customers. Pretty much anyone that has a 2016-17 tesla s or x with mcu1. Losing basic functionality shouldn’t be a issue but here we are. Please look into this I’m sure you find several others just like me. Thanks for your help.
