This happens about 30%to 40% of the time when using Full Self Driving Supervised on HW3, FSD version 12.6.4, software version 2026.14.6. When changing marked lanes to the right, either initiated by FSD or the driver asking FSD to move to the right by engaging the right turn signals, it will do one of 2 things: 1. Change lanes properly about 70% of the time 2. Start and abort the lane. Hange swerving back and forth between the 2 lanes, ot 3. Does nothing and drives straight without changing lanes, all the while with the turn signals on. #2 is particularly dangerous.
Using FSD. Stopped at a stop sign making a UPL into a 4-lane 35 MPH posted street. TWICE now there was a small opening which would require an aggressive accelerator - it started the turn stopping a full car length into the first lane and decides it's going to wait for closing traffic from the right. Obviously stopping in the middle of new oncoming traffic from the left is not very wise. This has happened 3 times with 2026.8.3 and I've never seen it before - twice i gunned it to force the short turn but missed it once. Definitely a pucker maneuver.
Electrical

2017 Tesla Model S P100D. Vehicle received a paid MCU2 infotainment upgrade (MCU1 to MCU2), which replaced both the center touchscreen computer and the instrument cluster. The original MCU1 architecture used an independent computer for the instrument cluster, providing redundancy. The MCU2 retrofit consolidated both displays onto a single processor with no fallback mode. The first MCU2 replacement unit experienced a touchscreen digitizer failure — touch input stopped responding but the displays remained powered and the instrument cluster continued to function. Tesla replaced it with a second MCU2 unit. The second MCU2 unit has experienced three complete blackout events where both the center display and the instrument cluster simultaneously lose all power — no backlight, no image, no data. One of these events occurred while driving at speed on the road. I lost all instrumentation: speedometer, gear indicator, turn signal feedback, warning lamps, backup camera, and climate controls. I had to pull over with no way to confirm my speed or signal to other drivers. The vehicle remained mechanically drivable — steering, brakes, and motor continued to function. During the recent blackout a few days back, the vehicle had been stored in a closed garage and was not heat-soaked. I have video evidence of the car with both screens completely dead while the vehicle is in gear and moving. A scroll-wheel reboot restored the screens each time. This is my fourth service visit to Tesla for this upgrade. The MCU2 architecture provides no independent fallback display for critical safety information such as speed, gear state, or warning indicators during a processor crash. The prior MCU1 system maintained instrument cluster functionality independently of the infotainment computer.
My Tesla wouldn’t charge at Level 3, 2, or 1 charger resulting in the need for the tow and further service hours. Following the service I was told it was a software issue and that a power cycle resolved the issue which locked out the charging. It’s deeply concerning and baffling that this could happen at anytime resulting in days or weeks or down time for what is ultimately a software issue. This is my fourth hardware related software issue following a Tesla update.
Suspension

I pulled into a parking lot, and noticed that my passenger side rear wheel was leaning into the car. Luckily I was next to a Firestone and the mechanic told me the rear control arm broke off. Had I have been driving even for 2 more minutes, the wheel would have came off completely. When I pulled in, there was no noise, vibration, or any other indication that something was wrong. I know for a fact I didnt hit any curbs or large potholes. It was a normal drive.
I am reporting multiple safety-related defects on my 2017 Tesla Model S (VIN [XXX] ) that were present at the time of purchase but not disclosed by the selling dealer (Starling Ford, Titusville, FL). A certified independent EV repair facility (The Electrified Garage) inspected the vehicle on 11/17/2025 and documented the following safety defects: • failing/worn suspension components (control arms, thrust arms, cracked bushings) • torn sway-bar link boots • inoperative right-rear window regulator • driver window contacting internal wiring harness • moisture in the rear inner tail lamp • previous A/C drain blockage causing interior water intrusion These issues pose risks related to steering stability, electrical safety, visibility, and structural integrity. All of these conditions existed prior to the sale and were not disclosed. I am requesting NHTSA to log this complaint for federal safety review as these defects involve suspension, electrical moisture intrusion, and visibility issues that may impact the safe operation of the vehicle. Steering

Exterior lighting

I have documented examples of 3 total incidents now where a Tesla OTA "Update" on my vehicle caused a safety issue. Event 1: Nov 27, 2025. Tesla OTA Update renders my left turn light non-functional for ~24 hours. Resolution per forums only was to not touch the card for 24 hours to allow it to shift into "sleep" mode which would resolve the issue. Instructions were that only a service center could resolve the issue. Event 2: Sept 3, 2025. Tesla OTA Update renders my car unable to charge. The issue is resolved following a ~24 hour "sleep" cycle which allows my car to somehow resume charging. Telsa advised the only resolution was to come in for service and to replace a $3000 part. Event 3: Sept 23, 2025. Tesla OTA Update once again renders my car unable to use it's left turn light. Power cycling the vehicle was able to resolve the issue but once again the instructions was only that service could resolve the issue. Each "update" risks sigificant "safety" related issues and Tesla's only resolution is to pay hundreds of dollars for someone to "troubleshoot" that which shouldn't be a problem.
Tesla originally confirmed issues with the MCU1 system in my vehicle and upgraded it to MCU2. I Paid $1,599 for the repair/upgrade. Since then, the center screen has continued shutting off while driving. When this happens, the instrument cluster and controls go dark, I cannot see my speed, use the blinkers to signal, operate the AC/defroster, or access other critical vehicle functions. This has occurred multiple times on highways, including US-22 in New Jersey, which created serious danger because I could not monitor speed or signal properly. Although Tesla addressed the MCU1 problem by upgrading me to MCU2, they told me afterward the vehicle was still “safe to drive,” despite the failures continuing. I believe this is unsafe because I lose visibility of all driving information and access to basic controls. Tesla now refuses to repair these ongoing problems. After I raised complaints about the severity of the issue, I was dismissed and mocked for my speech disability during phone calls, and Tesla placed me on a service restriction. As a result, I am unable to obtain further repairs for this safety defect. I am afraid to drive the car. No Adaptive Cruise Control, Automatic Emergency Braking, Blind Spot Warning, Forward Collision Warning, Lane Departure Warning, Lane Keeping Assistance, Parking Collision Warning, or Rear Cross Traffic Warning features were involved in this failure.
Brakes

Full Self driving did not show signs of slowing down at Everett traffic slow down from about 60 miles to a standstill. I realized the speed not slowing down (about 5 car distance from stopped distance) I tried to apply the brakes that were completely nonresponsive, instead the car sped up. I avoided backend collision with the first car be maneuvering to the shoulder, however the next car was parked halfway into the shoulder that didn't leave enough room to not hit it. The car hit two other cars before coming to a full stop. While checking for any injuries for myself and passenger, after a delay and hitting 3 cars the airbags uselessly deployed. Total failure of brakes, auto emergency braking and airbag deployment. I have requested Tesla to look into the logs but havent gotten any feedback.
While in autosteer, I pressed on the brakes which normally deactivates the autosteer while coming to a stop. In this particular case the car did not brake upon command and the car maintained acceleration thru a stop light and veered into a median causing a collision with a curb causing structural damage to the frame of the vehicle. I have put in a claim with both my insurance and Tesla and advise that the brakes did not activate in a timely fashion upon disengaging from Autosteer thus causing the collision and was advise by Tesla that I would be responsible to cover the damages when the accident was caused by the vehicle not disengaging. I was only advised after the fact of the recall for the very reason I complained to Tesla in the first place and have been waiting 5 months for a resolution.
Visibility

After the latest software update, the intermittent settings on the windshield wipers no longer work. At 10:15am on December 11, the Tesla Service group confirmed in a message that this is a known firmware characteristic. There is no ETA on a new firmware update to correct the issue. There notice states that it is an issue with the auto wiper feature, but the wiper does not work in manual mode either. Initially, they stated that they could inspect the car in a few days. Now they have pushed my appointment 2+ weeks.
Body & structure

Trunk latch fails, trunk latch safety mechanism also failed so manual release does not work. This is a safety issue
on 1 occasion the front trunk opened on its own, on 2 occasions the rear trunk opened on its own. The last occasion was on August 3. all these events occurred with in 3 weeks. The car was brought to a service center where the problem was misdiagnosed since the incident on August 3 was after the service call.

On December 26, 2022 I was driving my Tesla S 2017. Full self driving was engaged. I have a video of the car footage showing the incident in full detail. The car took a left onto a 2 lane road and then without warning ran off the road hitting the curb and a metal drain. The force was so great that I suffered a contusion to my right knee from hitting the under side of the dash.
The car was left parked for 12h in a parking lot were I work. Whilst driving there and upon exiting the car, nothing out of the ordinary was observed. Getting back in the vehicule, the car seemed ok. Upon backing up the car, a loud sound plus weird wheel camber on the rear driver's side wheel were observed. Upon verification, it looks like a tie rod broke. Given the condition (no excessive load on the car, car parked, not significant impact that we are aware of, the sudden break), it tends to point to a fatigue fracture.

This is a supplemental filing to my prior NHTSA complaint (#11697718) regarding a high-voltage isolation fault and shutdown risk. The vehicle displayed warnings including “vehicle may shut down during driving,” “may not charge,” and repeated BMS isolation fault messages. It became inoperable at a public charging station. The manufacturer confirmed isolation faults in the drive unit and the A/C compressor. Their own service notes stated that the vehicle “may shut down during regular driving” and that charging “may not be safe.” Despite this, the vehicle was left at 0% state of charge for an extended period and then charged by the manufacturer. They later stated to a regulator that the vehicle was “trickle charged at 1 amp for 6 hours,” but the vehicle’s range increased by approximately 100 miles, which is not physically possible at that charging current (1 amp at 240V for 6 hours = 1.44 kWh ≈ 4–5 miles). This discrepancy suggests the vehicle may have been charged at a higher rate than claimed and without proper cooling, despite the documented A/C failure that is necessary for battery thermal management. Battery condition is unknown because the manufacturer refused to perform or provide a battery health analysis, isolation resistance values, thermal logs, or charging session data. The safety defect could not be verified as repaired. While this safety issue remained unresolved, the manufacturer rejected a Tesla roadside tow arranged under regulator instruction to second service center and instead arranged an unauthorized tow to remove the vehicle from their facility without my consent. The vehicle was taken to a non-manufacturer storage lot while still in an unsafe state. I am concerned about: • high-voltage isolation faults, • shutdown risk during driving, • unsafe charging performed by the manufacturer, • possible battery damage from low SOC and charging without A/C cooling, and • mishandling of an active safety defect. The vehicle is available for inspection.

The defect affects the HV bus and can cause loss of propulsion, shutdown during driving, and charging failure. Tesla has not provided numeric test results (megohm isolation readings, insulation tests, or validation data) proving the vehicle is safe. This is an ongoing, manufacturer-confirmed HV isolation defect that can lead to sudden power loss. I request NHTSA review Tesla’s handling of this issue and link this filing to Case #XXX. coolant leaking, before tesla can even look at my car they say i need new valves. Only way they could know this is if there is a faulty part. This should be a recall! Believe this is the Battery coolant.
Interior & seats

The contact owns a 2017 Tesla Model S. The contact stated that while his father was driving at a slow speed, the rear driver’s side door independently opened. There was no warning light illuminated. The contact stated that his father pulled over and closed the door; however, the door opened independently. Due to the failure, his father was forced to drive the vehicle back home with the door ajar with the chime and the door ajar message displayed. The contact was able to shut the door; however, the door handle failed to operate as needed the next day. The contact called a Tesla service center and was given no assistance. The contact stated that prior to the failure, all three of his other door handles had failed while raining. The vehicle had been repaired. The failure mileage was approximately 83,000.
Tesla driver seat bolster hurts my bottom after 10/15 mins of driving. The seat cushion behind the bolster to the back sinks so low; and fails to keep me lifted. As a result, eventually after driving 10/15 mins bolster starts hitting my bottom and it starts to pain; and also numbs my legs. At this point, I have to take a break, get out stress and start driving. I have driven many cars upto 500+ miles in a single day no issues. Having a comfortable seat is a basic requirement in any car. I have reported this issue many times to various Tesla Service Centers. The seats installed in my cars seems to launched very pre-maturely without proper testing. There are many complaints online forums on this bolster pain and how badly it hurts. Tesla wants me to pay for the seat and no guarantee that the new seat will not such issues. If the installed seat itself is defective, why should I been paying for another defective one. Tesla is not willing to own the issue. I have done upgrades and paid alot of taxes so not willing to sell and buy another car and pay taxes again. I want to report this issue as I foresee safety issues with this car and similar many other cars; and how Tesla is not doing their basic customer service and offering bad products at very high price.

During a road accident, Driver airbags were deployed and caused injuries to driver in left hand, left leg and right chest by flying debris’s due to airbag deployment.

WHILE ATTEMPTING TO PARK THE CAR IN A PARKING SPACE THE CAR ACCELERATED ON IT'S OWN AND CRASHED OVER A SMALL CONCRETE RETAINING WALL AND CRASHED INTO ANOTHER VEHICLE. MY CAR AND THE CAR THAT WAS STRUCK WERE TOTALED.
