My car had a software update last night. This morning, there is now a forced "feedback" prompt that comes on the information screen whenever I disengage the autopilot. This prompt grays out the rest of the screen and does not disappear until I manually select a feedback option. This forced prompt is very distracting, requiring manual user input to get rid of, which makes driving distracting and dangerous.

Auto emergency braking is occurring when vehicle recognizes leaves as people. This has happened on three occasions. It is startling to say the least and a matter of time that I am rear ended due to this. Service center acknowledged these events but said they can't do anything about it.

Tesla just sold me a car with defective front half shafts axles. The car makes a reproducible and very concerning accelerator pedal shudder that makes the entire vehicle front vibrate, including the steering wheel and brake pedals while the car is accelerating between 30-60mph. This interferes with safety features like, lane departure assistance and blind spot steering wheel feedback, Confusing the driver and creating a distracting scenario where an accident can happen. Upon bringing this to the attention of the service center in Fort Myers Florida, the service center manager took a drive test with me, and was able to replicate the issue, but immediately started dismissing it as "normal" and within Tesla specs. This is becoming a regular practice at that service center, where they deem warrantable issues as normal and within (their tolerance). I insisted that this issue is not to be taken likely by Tesla, because it is something that involves a safety hazard. Upon further research, I was able to find that this issue is happening to many Teslas model S and X, where the front axles bend due to the weight and high torque of the vehicles. The issue falls completely under warranty and the half shafts should be replaced, but apparently Tesla has instructed their service centers to dismiss and disregard the symptoms, and just tell the customers that no warranty replacement will occur, because it is not necessary, we just have to drive the cars like this, until the car completely fails, or an accident happens because one of the bent axle shafts breaks on a highway during normal drive. I recording this report as evidence that the issue has been reported to the highest vehicle safety authorities, and I expect a corrective action so Tesla can fix my car under the existing unexpired warranty.

The contact owns a 2023 Tesla Model 3. The contact stated that while exiting a supercharging station with the Full Self-Driving (FSD) feature engaged, the message "Take Over Immediately" was displayed. The contact stated that the vehicle abruptly accelerated into an intersection and crashed head-on with another vehicle. The air bags deployed as needed. The contact stated that a Police report was filed; however, the report number was not provided. There were no injuries sustained. The vehicle was towed to an independent mechanic, where it remained undiagnosed and unrepaired. The manufacturer was notified of the failure but provided no assistance. The failure mileage was 150,332.

The contact owns a 2023 Tesla Model S. The contact stated that the horn button on the right side of the steering wheel was not easily accessible. The contact was concerned that the issue could prevent the driver from beeping the horn during emergencies, increasing the risk of a crash. The manufacturer was notified of the failure and advised the contact to purchase the improved steering wheel designed with a center horn. The vehicle was not repaired. The failure mileage was unavailable.

Experienced severe phantom breaking on [XXX] in route to Las Vegas at 70mph September 2024. In heavy rain, while windshield wipers set to auto...........wipers have stopped momentarily blinding the driver, a problem Tesla hasn't fixed on my neighbors 2019 Model 3. The stalkless design prevents safe use of turn signals, horn and gear shifts. Wife who only drives an ICE car can not safely operate turn signals, horn and gear shifts in my 2023 Tesla Model S. Camera system constantly renders 'Blinded' error messages halting Autopilot features. Tesla's resolution is to clean camera's before initiating Autopilot. There is a design flaw in the functionality of the windshield wipers. Instead of the functionality being on a stalk like every other car, Tesla decided to make it a 2 step process. To operate the windshield wipers, you first have to press a capacitive button on the steering wheel and then you have to scroll a wheel on the steering wheel. The first problem is that the button to turn on the windshield wipers is a capacitive button that is not raised on the steering wheel - so the button moves as you turn the steering wheel and thus it is not in the same place all the time. This makes it very difficult and practically impossible to find the button without taking your eyes off the road to find the button on the steering wheel. The second problem is that once you find that button, you then have to find the scroll wheel on the other side of the steering wheel to then adjust the speed. So picture you are driving into a downpour. You instantly can't see because of the rain. Because you are turning slightly on the highway, you have to take your eyes off the road to find the button on the steering wheel. That will activate the wipers but only in intermittent mode. You then have to use the scroll wheel on the other side of the steering wheel to adjust the speed so it is continuous. And while the car does have an "auto" mode, I've owned 3 different Teslas over 9 years and it has never worked properly - often turning on the wipers on a day that is 100% sunny. The auto mode also doesn't work well when it is raining because it often doesn't sense the correct speed (so staying in auto mode is not a great solution). The correct solution would be to return the wiper functionality to a stalk on the steering column and most definitely not require 2 separate buttons/functions to use it. It is very dangerous having to take your eyes off the road when you enter a downpour or when you are in a drizzle situation and then it starts pouring because you need to do 2 things.

The left turn signal is intermittent, it works most of the time, but many times the button on the yoke will not turn on the turn signal. Tesla wants $265 to fix it even though I still am under the factory warranty. After a quick internet search this appears to be a known problem. Because there is no turn yoke (which is a very poor design and that should be recalled) I have to rest my finger on the detent for the left and right switch. If I do not I am turning on the incorrect signal. This is either a button problem or software issue, I had to have my first yoke replaced on 4/22/24 as it was defective.

I was driving in dense fog about 9 am on December 2, 2024. It was about an hour after sunrise. I passed an approaching car without lights on and thought to myself how dangerous it was to be driving in dense fog without lights. I then realized that my automatic headlights were off! That means my fog lights were also off. They were on when I first left the house. The fog was dense enough that cars without lights appeared out of the fog with 10-15 feet warning. Very dangerous when turning left onto roads or at stop signs. The fog was dense enough that the driver assist cameras were complaining about being blocked or blinded, but the car had decided to turn off the headlights and fog lights without notification (except removing the lights on indicators on the dash). I have tried to contact Tesla support on their web support and haven't been able to figure out a way to provide comments. I only find a "help" section that provides answers to a few questions and seems to go in circles for anything else. I was able to move the headlights from Auto to On and solve the problem, but that required messing with the touchscreen while driving.

Driving on straight section of highway at dusk. No autopilot or other features on. Road slightly damp from earlier rain. Driving approximately 60mph. Without me moving my hands or feet then car suddenly slammed on the brakes and speed decreased to approximately 20mph. I pulled over and reset the car computer by pressing the two buttons on the wheel and no further problems but had someone been behind me there would have been a terrible crash and my daughter was in the backseat. Looking online it seems Tesla "phantom braking" is not uncommon. I am frightened to drive my car ever again but have a 7 hour drive home tomorrow.

​The driver's seat assembly on the vehicle suffers from a critical mechanical and software tracking defect that causes the seat to permanently jam into a mechanical lock, creating an immediate and severe safety hazard. ​The electronic calibration soft-stops for the driver's seat allow the mechanism to travel past the safe limits of the track when indexed or operated. When the seat reaches the absolute mechanical limit of the rear rail, the lead screw/worm gear mechanism binds tightly, resulting in a total mechanical lock where the motor lacks the breakaway torque to reverse direction. This causes the driver's seat to become permanently stuck in the fully rearward position. ​This defect directly compromises vehicle safety by potentially preventing a driver from safely reaching and operating the brake and accelerator pedals. ​The vehicle has been taken to the manufacturer's service center on three separate occasions for this exact issue. During these three visits, technicians physically replaced the seat motor, checked track alignment, and attempted recalibration. Despite these repeated hardware interventions, the defect persists, and the seat continues to travel past safe limits and bind into a mechanical lock. The manufacturer has now refused to correct the underlying track/assembly defect under warranty, incorrectly dismissing the recurring, hazardous mechanical failure as "customer education." The vehicle lacks a safe physical fail-safe to prevent the seat from self-destructing into a permanently jammed state.

The vehicle is over reporting it's speed by 5% this is a large discrepancy which effects safety by causing people drivers around you to change lanes because you are traveling under the speed limit. Tesla is unable to calibrate the speedometers are the service center. They have documented the failure. It also adds miles to the odometer prematurely. This problem started after Tesla upgraded the car with a Track Package which is a $24,000 upgrade

Driving on freeway when the rear tire tread peeled off the inside tire tread wheel/blow out almost causing an accident. The vehicle pulled hard while driving and almost lost control. The problem has been inspected and confirmed by the Tesla dealer. The tire warning did alert after the tire blew out. This is a very frequently reported incident with the Model S plaid. A defect in the factory camber causing the blowout. Extensive wear is seen on the very inside wall of the tire. You couldn’t see or measure the tire wear from the outside of the vehicle. This is a scary and dangerous problem that needs to be fixed. I bought this vehicle brand new from Tesla Factory and the vehicle had less than 13,000 miles. The alignment was also measured when getting repaired and had bad results.

Frunk latch does not open with the first tap due to misdesigned frunk liner trim piece. The only way to open the frunk is after first tap to simultaneously tapping open and pulling the hood upward. It is only feasible using the phone app as the vehicle comes with key-card. This creates a safety concern due to fire crew would have an issue reaching a first responder loop to disconnect a battery located under the hood in case of an accident. The issue was reproduced to Tesla service team but they claimed that it is yet impossible to repair.

This is the second time in the last two weeks that my backup camera has stopped working. I have talk to the service center about this I was told that it is not a safety issue but a convenience to the driver and that I should use my mirrors and turn my head around to see what is behind me. I have taken my car in the service center to see if they can fix the problem (8/23/2023). Backup camera worked for a while than stopped again . I have made another appointment with the service center to see if they can fix the problem (8/29/2023.

As I approached a red light with a car in front of me, I took my foot off the accelerator to use Tesla's regenerative braking, but instead the car rapidly accelerated and crashed into the car in front of me. It seemed like unintended acceleration.

While driving at midnight, I pulled away from a stop sign in a residential neighborhood, and hear a loud bang. The dash display turned red immediately, and the car lost all propulsion. The dash display said "Pull over safely, Vehicle shutting down." My phone app also showed a message of "Your car suffered a failure and will no longer drive. Contact Test Roadside..." I coasted to the side of the road. While calling Tesla Roadside assistance, within 15 minutes of the breakdown, the center display said "Low voltage energy remaining = 30%" and a few minutes later the entire car went dark. The doors, windows, trunks, glovebox, center screen, tow mode, and the hazard flashers no longer worked. The tow truck driver said he has seen this problem a lot and based on my description, he correctly diagnosed a failed inverter, which was Tesla's diagnosis. I searched online user forums and found many other people having the same catastrophic failure in new Tesla's. It's incredibly dangerous to lose all forward power. This is a new car with only 5K miles. And to lose all accessory power within 15 mins so not even the safety hazard flashers work is terribly dangerous.

The tire has an excessive and extremely disproportionate amount of wear on the inner part of the tire and sidewall. It is still on the car at this time. The location of the failure is hard to visually notice. My tire losing pressure was the cue something was wrong. It was on the verge of catastrophic failure and only luck prevented this from turning into an accident. This same failure has been reproduced on a large number of Tesla Plaid vehicles equipped with 21” rims and these Michelin tires. It has not been inspected by the manufacturer, police, insurance representatives or others. I will be taking it tomorrow for warranty review from Michelin. There were no symptoms of an impending failure at all. The only warning light that ever showed was a communication error with the sensor that read tire pressure. However, I always checked the pressure manually since that particular sensor was intermittent with its reporting. These tires are rated for 30k and I’m barely over 12k.

Front safety system fault with complaints on Nov 21, 2023, Nov 27, 2033 and then on March 6, 2023 then Tesla service finally repaired and advised as follows: “Technician Notes Verified customer concern alert was not present on drop off but when moving drivers seat through range of motion found faults appeared. Inspected pin fit of harness found no pin issues. Replaced the drivers seat harness and re tested during seat movement verified faults not currently present.”
