Suspension

In March of 2025, my steering of the model X became really stiff. As time went on, the problem became worse and no longer able to turn left or right without major force. I made an appointment with Tesla and took it in for servicing. At this time they changed the steering bolts and steering shaft April 23, 2025. This cost $489.70 (I will attach invoice). As soon as I picked up the vehicle my steering wheel was about 30% stuck to the right. I immediately sent a picture to tesla for them to conduct an alignment and fix the issue. They fixed the issue but around May 15th, I had a nail in my tire so I took my car in to fix it. At that time I find out that the tires on my car were exposed to the wire (I will add the picture of the tires). I replaced these tires on March 1st 2025 which is only 2 1/2 months since I replaced it. It was an indication that something is wrong so I make another appointment with Tesla to check it and get another alignment I dropped off car on June 3rd. Tesla tried to perform another alignment to find that they are not able to because the steering gear assembly needs to be replaced. I asked for clarification because I just fixed the steering components in April and they are now wanting $4900 for the steering gear assembly. As I was researching, I find that my model x was produced on May 2016 and there is a recall for Model x produced before Oct 2016 for the steering bolt and steering components if they are damaged. When I tell tesla that there is a recall they say that my vin is not associated with the recall. I am filing a complaint because my car is with in the period of vehicles produces in the recall. There is a huge safety issue if the steering bolt and steering gear assembly is malfunctioning. There were no warning lamps or messages to indicate this. This has put my family and other vehicles at risk and Tesla is aware of this problem but won't honor the recall or admit that the steering issue are all related.
Sudden and catastrophic shearing of the driver side rear suspension arm (control arm). My understanding is all these cars from Tesla have aluminum suspension arms which causes identical and catastrophic failure of the vehicle during driving due to choice of Aluminum instead of stainless steel(SS). Aluminum has an ultimate tensile strength of only 90 MPa while SS can exceed 500 MPa. Since my car got disabled completely (motor gets disconnected from the wheel) I almost lost my life with vehicles coming at high speed from behind. I saw the same control arm failures with Tesla cars including S and 3 and X (like mine) all over the world. Here is an example of a very similar failure to mine on a model 3 in Norway : [XXX] The failure is so similar that even the wheel tilted inwards to the wheel bay just in my case due to the clean shearing of the aluminum bushing on the control arm. I would like to make sure these failures do not continue with Tesla and people's lives are not at risk and thus this report. Electrical

I own a 2016 Tesla Model X P100D equipped with the Large Drive Unit (LDU). The component that fails is the rotor coolant seal in the rear drive unit motor. Coolant leaks internally into the motor and inverter, causing bearing wear, corrosion, isolation faults, and eventual complete drive unit failure. The affected drive units are still installed and available for inspection upon request. This defect puts my safety and the safety of others at risk because it can cause sudden loss of propulsion (especially at highway speeds), power reduction, or total drivetrain shutdown, potentially leading to a crash. Coolant in high-voltage areas also raises electrical fault/fire risks. The problem is widely reproduced and confirmed: Tesla service centers are familiar with it and replace failed units with updated “-U” revisions that include a coolant bypass. Independent EV shops confirm the exact root cause (rotor seal failure) via encoder inspections showing coolant intrusion and through disassembly. This is a documented pattern failure in all 2012–2020 LDU vehicles. No police or insurance inspections have occurred yet, as no crash or total failure has happened. Tesla is aware of the recurring issue through service records on similar vehicles. Early symptoms before full failure typically include rear motor whining/grinding (especially under load), reduced power warnings, “Rear Drive Unit Fault” messages, and visible coolant on the speed sensor. These can appear gradually. My vehicles have not yet experienced total failure, but the defect is known to be inevitable. I am proactively monitoring encoder sensors. Because this is a systemic design defect affecting thousands of vehicles with clear safety implications, I request NHTSA open an investigation and require Tesla to issue a recall for free repairs/replacements on all affected 2012–2020 LDU vehicles.

The dashboard also showed "Park Brake fault" and a red icon with a seatbelt and another with an airbag. Alarmed, I pulled to the side of the road. I then discovered that I could not use the parking brake or turn on the flashers. There is no manual parking brake, and the car thought it was still driving. I had to call 911 and could only safely exit the vehicle, after they had ensured it would not roll. Again, I could not exit the vehicle, or even put on flashers to warn traffic. The rack-and-pinion system failed, causing steering to become difficult at low speeds and worsening rapidly over weeks. It was inspected by a mechanic at EV Coastal in Santa Ana, who said it was not the U-Joint or bolt corrosion issue addressed by an earlier NHTSA recall. There were no notifications either visible to me or to the mechanic in service mode. The accident was at low speeds and a partial cause was my daughter's difficulty making a smooth adjustment with the steering wheel. She was pulling out of a parking spot and collided with a car going a little faster than it should have in a parking lot. Fortunately, the damage was limited to a scratch to the passenger side door of the other car (see photo) and scratches to the right side of the front fender of the Tesla.
Body & structure

The following is copied from: NHTSA ID Number: 11487520 Incident Date [XXX] Consumer Location SAN JOSE, CA On nine separate documented occasions, the passenger front wheel molding trim piece, in which one of the proximity sensors is mounted, completely detached, causing the proximity sensor to alarm continuously. On two of these nine occasions, the trim piece detached while the vehicle was traveling on the road, and on one of these two occasions, the vehicle was traveling on the highway at highway speeds. On this occasion, after detaching completely, the trim piece was observed through the rear view and side mirrors, shattering on the road and scattering debris across the highway, potentially causing a traffic hazard for the cars following. On this last occasion, the vehicle was also traveling with the assistance of the Adaptive Cruse Control and Forward Collision Warning features, which both caused the vehicle's emergency avoidance systems to activate when the proximity sensor was falsely triggered. If there were other vehicles nearby, this could have caused a high speed accident as my vehicle executed both an emergency braking and collision avoidance maneuver for no real reason. Please note that I have documented reports of the possible safety issues due to the wheel well trim detaching to the Tesla service department for several of the times I've had the vehicle in for repair. In all nine instances, I've been told that my vehicle is the only one that exhibits this problem. A statement I find difficult to believe as I have observed other vehicles of the same make and model driving around without the trim pieces, and just recently captured a photograph of a Model X driving on the highway with the trim piece detaching as the wind was catching it. See the red arrow in the photograph attached below. This report adds two additional photographs of the same issue that have been taken since the original report was filed.
The contact owns a 2016 Tesla Model X. The contact stated while the vehicle was parked, the contact’s wife attempted to open the driver’s side rear door however, the door failed to open as intended. The contact’s wife noticed a crushing sound. The driver’s side rear top door glass engaged with the fractured center spine trim and the driver’s rear door top glass simultaneously fractured. The vehicle was taken to the dealer where it was diagnosed with a fractured center roof trim. The contact was informed that the center spine roof trim and the driver’s side rear door glass need to be replaced. The vehicle was repaired. The contact was informed that the vehicle had been previously repaired under NHTSA Campaign Number: 24V615000 (Structure); however, additional repairs were not covered under the recall repair. The manufacturer was made aware of the failure but provided no assistance. The failure mileage was approximately 106,900.

The car door propped open as I was driving the vehicle. There have been multiple repeated concerns that I have had with this vehicle. It is unsafe.
The contact owns a 2016 Tesla Model-X. The contact received notification of NHTSA Campaign Number: 21V035000 (Back Over Prevention, Visibility, Exterior Lighting) however, the part to do the recall repair was not yet available. The contact stated that the center display intermittently turned blank while driving. Additionally, the contact stated that upon engaging the turn signals, there was no indication that the turn signals were engaged. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was made aware of the failure. The vehicle was not diagnosed nor repaired. The failure mileage was 30,000. Parts distribution disconnect.

2016 Tesla Model X experienced Electronic Parking Brake failure with alerts stating ‘parking brake did not release.’ Tesla is charging for replacement despite known EPB defect and recall affecting 2016 Model X vehicles. This is a safety-related braking issue.
Interior & seats

Problem: Driver door fails to open The door actuator and latch will intermittently fail and the door will refuse to open. In the event of an accident the driver can fail to exit the vehicle possibly leading to death. An independent party/dealer has not produced the problem. No representative has looked at the problem. No warnings of failure appear.
My 2016 Tesla Model X has a persistent and dangerous issue with the electronic parking brake. The brake frequently fails to disengage properly, and the car becomes jerky when attempting to move. When the vehicle is parked, a warning appears stating: “Parking brake not engaged. Vehicle may roll,” even when the brake is set. This poses a significant safety risk, especially on inclines. Additionally, the driver-side door latch mechanism has failed again. Tesla previously repaired this exact issue and charged me, but the failure has returned. The door sometimes fails to latch securely or does not open and close properly, creating a safety hazard. These issues have made me concerned about the safety of driving or parking the vehicle. I believe these components should be investigated as potential manufacturing or design defects.

I initially brought my vehicle to the Tesla Service Center in Richmond, Texas, to address issues involving the motor. During this visit, I was informed that mismatched tire sizes and worn parts in the front end were contributing to the problem. The service center proceeded with repairs, charging me for the work. However, the original issue persisted after I picked up the vehicle. Despite multiple follow-up appointments and additional repairs, the problem was never resolved.
Exterior lighting

MCU1 onboard computer system becomes overloaded with data that eventually and periodically causes the computer system to "crash" and requires a reboot, even with 8 GB eMMC replacement having already been installed. This means that vehicles with MCU1 (generally model S and X manufactured pre-2019) are increasingly becoming safety risks on the road because the MCU1 system crash can occur at any time, even while driving (not just when the car is going through it's start-up cycle). Whenever this occurs, it means the driver is unable to change the headlights which is especially harmful during dawn and dusk hours since these models do not have daytime running headlights. Drivers may be unable to use turn signals, unlock power doors (the Model X has power-driven doors on all sides) and advanced systems may be inoperable during the MCU1 rebooting process. This issue can be resolved by replacing the MCU1 operating system with the MCU2 operating system, but Tesla requires MCU1 owners with Auto Pilot Computer version 1.0 to pay $1,750 for what they are calling an "infotainment upgrade." This is disingenuous terminology used by Tesla because the MCU1 operating system is not just for "infotainment" - it is beyond that and includes significant safety features as mentioned above (headlights, doors, locks, windows, ADAS, and more). Tesla needs to cover the cost of replacing MCU1 with MCU2 or a new system that is stable and does not experience frequent data crashes. In other words, there must be a fix beyond the 8GB eMMC "band aid" that has not resolved the chronic failure of MCU1 systems. This is a continuing safety issue and driving hazard for all Tesla owners with MCU1 until a recall is issued.

While pulling forward to straighten out car into parking/charging spot at less than 1mph at EVGo charger in front of Meijer store the front passenger wheel made loud clunk and would not move. Another motorist charging said he heard it and offered to help. I called Tesla roadside from car’s service screen and waited over half hour before being disconnected. The Tesla App provides NO applicable options for this scenario to report. I then called my extended warranty (X-care) roadside and after almost 4 hours obtained a tow to Tesla Service in Clarkston. Tesla now diagnosed that the FORE LINK ASSY, 1.00 RH(1041575-00-B) WASHER, NORDLOCK, 1.00 15.2X30.7X3.4(1033093- 00-A) NUT HF M14X1.50 [10] 1.00 ZNNI NL INSERT(1004358-00-C) need to be replaced. $400 parts and labor. Invoice attached. This luckily occurred with only me in the car, on 40 degree day and at the charger…I was nearly out of charge. I would have been stranded. And begged a lot of what if’s… The car had just been serviced by Tesla on 11/14/22-11/18/22. Invoice attached. Tesla replaced bolts in steering rack housing when I brought it in for a windshield replacement? Bottom line though this is too critical a failure during such a basic, low speed operation to be safe. I cannot trust that the fore link assembly won’t fail in future in same or different wheel. It felt like the car broke its own leg by merely taking a slow step forward. This should not be permissible. Thank you for your work.
Car has 52000 miles. Car was parked OK. Upon returning to drive I noticed cracking sound and noticeable wobbling motion on right front wheel. Car was found to have broken right front ball joint and Tesla replaced all front parts left and right. Tesla said this was wear and tear and I disagreed and think it is a safety issue!
Airbags

Was stopped at a red light behind the car in front of me. I was stopped at a appropriate distance from the car in front of me. I had my right foot on the brake pedal, when the car accelerated on its own. It launched its self into the car ahead of me. I looked down and my foot was still on the brake pedal. None of the air bags inside the car deployed.
The drivers airbag in my 2016 Tesla Model X deployed without incident while in a drive thru so maybe 2 mph. I didn’t hit anything nor is there damage to the vehicle or previous repairs that would cause it to explode. Thankfully I wasn’t seriously injured just the airbag slapped my arm off the steering wheel leaving a red mark for a couple hours and my families ears running for 15 minutes. Embarrassed going through a drive thru with an airbag in my face but thankful it didn’t happen at high speed driving. I drive it home s as everything was functional, even steering wheel audio controls. About three weeks prior there was a safety restraint light on the dash but I thought it was a seat belt related issues as I replace the third row seat belt a couple months ago. Also about two weeks ago I heard what sounded like some loose beads in the steering wheel when turning a really slow speed, obviously something was coming apart but I had no clue it would be this. It was dropped off the next day at the Tesla service center in League City, Tx, [XXX]. At this time they have not had a chance to look into it. Engine

The contact owns a 2016 Tesla Model X. The contact stated that while stopped at a red light and activating the air conditioning, the computer system was stuck or frozen for a moment. The contact stated that the vehicle restarted independently, and the computer was buffering when the light turned green, and the vehicle hesitated before responding and driving as needed. There was no warning light illuminated. The local dealer was not contacted. The vehicle was not repaired. The manufacturer was notified of the failure. The approximate failure mileage was 100,000.
The car started giving error messages of systems failing one after another on the highway. It slowed down instantly as if someone pressed the brakes and I could not accelerate. Obviously I did not press the brakes. There were a lot of vehicles behind me and this could have easily caused an accident putting a lot of people at risk. I had to get off the highway asap and get the car towed. The error messages included charge port disabled, automatic emergency braking is disabled, power reduced, traction control disabled, stability control disabled, cruise control disabled, power reduced. Tesla diagnosed and confirmed this being an issue with the onboard charger being faulty and replaced that with charge port itself for a total of $3727.60. They refused to cover any part of this which I think should have been taken care of by the company.

IN JUNE OF 2020, THE ENTIRE POWER/ELECTRONIC SYSTEM IN MY 2016 MODEL X FAILED (AT 73K MILES), LEAVING THE CAR UNDRIVABLE (AND EVEN UNABLE TO BE PUT INTO NEUTRAL FOR TOWING). I HAD IT DRAGGED ONTO A TRUCK AND TAKEN TO A TESLA SERVICE CENTER, WHERE THEY DIAGNOSED A FAILED MCU. THE FAILURE, I WAS TOLD, IN RARE CASES CAUSED A FAILURE ACROSS THE ENTIRE SYSTEM. THEY REPLACED THE MCU AND CHARGED ME $1400. TESLA HAS SUPPOSEDLY OFFERED A REFUND ON THE REPAIR TO CUSTOMERS WHO HAD THE PART FAIL (WHILE REFUSING TO ACKNOWLEDGE THE SAFETY ISSUES ASSOCIATED WITH FAILURE), BUT HAS NOT INCLUDED MY VIN IN THEIR DATABASE OF IMPACTED VEHICLES. THE COMPANY IS NOT ACKNOWLEDGING MY CAR WAS ONE OF THE VEHICLES IN THE RECALL, BUT THE FAILURE CLEARLY HAPPENED. HOW CAN I HOLD THEM ACCOUNTABLE FOR THAT? IS THERE ANY WAY THE NHTSA CAN HELP?
THE MCU IS DEAD. CAR CANNOT BE OPERATED SAFELY. NO SIDE BLINKERS, NO HORN, NO AC. AT THIS POINT MY CAR CANNOT EVEN TURN ON AND CANNOT BE DRIVEN. CONTACTED TESLA, BUT IT IS NOT A SAFETY ISSUE AND THEY SENT ME HOME AS PARTS WERE OUT OF STOCK. IT IS NOT SAFE TO DRIVE AS BLINKERS DON'T WORK.

The seatbelt has stopped retracting fully and became twisted. Although Tesla repair personnel have repeatedly untwisted the belt, it immediately becomes twisted again and stops retracting.
