This is about a 2020 Tesla Model Y, with HardWare3 (HW3) Full-Self-Driving (FSD) feature. The older HW3 FSD-enabled Model Y cars started getting the new "v14 Lite" FSD updates recently - I got mine a few days ago. I immediately noticed a safety concern with the v14 Lite. Previous versions of FSD allowed me to set the maximum speed that the car would drive in the FSD mode - I can adjust it using the "wheel button" on my right thumb on the steering wheel, and the car would not exceed the maximum speed I choose. However, after the update to v14 Lite, the wheel button no longer controlled the maximum speed. Instead, it controlled the "speed profile" ("Sloth", "Chill", "Standard", "Hurry" are the options in my car). Regularly in highway driving, even in the Standard mode, the car would speed up to go well past the speed limit, more than 10mph over the speed limit, seemingly to match the speed of the rest of the traffic (69mph on a 55mph highway). This felt very uncomfortable. I tried to adjust the max speed down like I was used to, but the car instead switched to a different profile, "Chill", and rapidly slowed down to 4mph below the speed limit (51mph in this case=. The speed reduction was so sudden that it also felt dangerous (car behind me would have to break quickly to not hit me), and I didn't want to be driving well below the speed limit (also dangerous). Under "Self Driving" settings menu, there is also a feature called "Speed Limit", that used to allow you to set the "default" speed limit for FSD driving (for instance, +3mph above the speed limit). You can adjust it up and down with the right thumb wheel beyond that, but it set the default basic max speed limit for FSD. After the v14 Lite update, the FSD completely ignores this speed limit, and happily accelerates well beyond that. Not it seems like the "speed limit" feature is simply for warning the driver to not exceed the pre-determined speed limit when controlling the car. FSD simply ignores it.
My Tesla Model Y 2020 (VIN: [XXX] ) is stuck on software version 2023.44.1 for more than 2 years and has four incomplete NHTSA safety recalls: • 23V838 – Autosteer driver monitoring improvements • 24V051 – Brake/Park/ABS warning font size too small • 24V153 – Muted Pedestrian Warning System • 24V554 – Hood not detected as unlatched Tesla Service in the Baltic region and Tesla Service in Warsaw (Poland) both refused to deliver the required OTA safety recall updates. They classified the vehicle as “unsupported” because it is a US-market vehicle located in Latvia. I am unable to bring the vehicle to any Tesla Service Center. Tesla is not fulfilling its legal obligation to provide the free safety recall remedies. Please require Tesla to deliver the updates or provide a proper solution. There was a grinding sound coming from the LF wheel. This was progressively getting worse. Car was shaking while driving so dropped it off with my mechanic due to safety concerns. Per the mechanic that fixed the issue: Inspection found LF wheel bearing is loose. A bolt backed out on inside rubbing through axel shaft. Allowed wheel bearing to damage to damaged from moving in place. Axle shaft damaged from rubbing on backed out bolt. Replace LF axle shaft and wheel bearing. Informed by mechanic that wheel could have fallen off while driving. No prior work. Mechanic believes most likely issue was not having the bolts appropriately installed at time of manufactue
When I went to my local garage to have a wheel rotation and wheel alignment done, they discovered that my rear driver side break hose was leaking. They also discovered that the rear passenger side break hose was showing similar wear/damage. I had the car towed to Tesla repair center because the replacement parts were backordered for the local garage. At the Tesla repair center, I asked if this was a potential build quality issue or something else, which they couldn't answer. Besides tire related items, like new tires and wheel alignment, all my other servicing was done thru Tesla. They noticed that they had done some half-shaft lubrication work that would have required them to disassemble the breaks. Of course they are not going to speculate or admit wrong doing. Therefore, in my mind, the issue was either original build quality from the factory or maybe the Tesla service mechanic not properly reassembling the breaks during one of the service visits. Just incase this was a factory build issue, I wanted to put in this incident so it can be counted towards any tally that NHTSA maybe keeping.
Suspension

The contact's daughter owns a 2020 Tesla Model Y. The contact stated that while driving at various speeds, an abnormal, squeaking sound would emit from the vehicle without warning. The contact called the service center where they gave the contact an estimate for a diagnostic. The contact was also informed that the vehicle may be covered under a goodwill warranty. The vehicle several parts of the front control arm were defective and had caused several other defects related to the control arm. The contact was given an estimate for the repair. The manufacturer had yet to be notified of the failure. The vehicle remained in the service center's possession but had yet to be repaired. The failure mileage was approximately 48,000.

A 2018 Ford Explorer, marked police vehicles, was operating with lights and siren when it proceeded through a red traffic light at an intersection. The subject vehicle, 2020 Tesla Model 3, was proceeding through a green traffic light at the intersection and struck the Ford.
I would hear squeaking/tearing/rattling noises, this was found to be due to malfunctioning/broken nuts and compliance arms and control arms. This happened three times so far, attached are two repairs that was required previously. Per report with Tesla staff, this has been a recurring issue. Per additional Tesla owners, this has been a frequent issue amongst many different Tesla forums. Now I have to get it repaired for the third time. I bought a used Tesla that was certified by Tesla, problems occured at 37876 miles, but car was bought at 34529 miles.
Electrical

Odometer Fraud. The contact purchased a 2020 Tesla Model Y. It was discovered that there was a mileage discrepancy. The vehicle was a dealer sale. At the time of purchase, the vehicle mileage was 78,823, but after driving the vehicle from the dealer's lot, the contact discovered through the vehicle settings that the actual mileage was 80,810. The dealer was contacted, but no resolution was provided. The dealer then updated the advertised mileage on the website.
Observed intermittent center display/infotainment computer (MCU) crashes/reboots where the center display becomes blank and then restarts. In recorded events, when reverse was selected during an MCU reboot interval, the rearview camera image did not display until the reboot completed. Recorded rear camera unavailability during reverse was approximately 10–17 seconds. MCU crash/reboot behavior was observed to be repeatable, including predictable occurrence while the vehicle was in the built-in Service Mode. After a factory reset, repeated/double boot behavior was also observed. Additional observations: - With the turn signal activated, light leakage was observed in the side repeater camera image (glare/bloom present in the camera view). - With each windshield wiper sweep, the forward camera image was observed to briefly flicker to a purple/green distorted state and then return to normal. - During driving, the on-screen message “Auto Lane Change unavailable” was observed. A separate rear camera symptom has also been observed: intermittent freezing/stale frames (image appears not to update). This rear camera freeze behavior has not yet been captured on video. These conditions have been documented by video/screenshots and reported to the manufacturer/service center for inspection/diagnosis. No crash or injury occurred. The vehicle is currently located at the manufacturer’s service center and is available for inspection upon request.

Was driving in rain, had windshield wipers on, suddenly windshield wipers stopped working; could not see through windshield because of the rain; almost had collision before being able to move to the side of the road to safety. Cost $255 to have windshield wipers repaired.

I am reporting a recurring Safety Restraint System Fault error in my 2020 Tesla Model Y. The issue manifests as an alert on the vehicle's dashboard, warning of a problem with the safety restraint system. This error began appearing at approximately 75000 miles. What component or system failed or malfunctioned, and is it available for inspection upon request? The Safety Restraint System in my Tesla Model Y is giving errors. Yes, the component is still installed in the vehicle and is available for inspection upon request. How was your safety or the safety of others put at risk? This malfunction affects critical safety features, including the airbag system and seatbelt functionality, potentially preventing proper deployment of airbags in an accident. Additionally, when the fault occurs, it disables key driving safety features like Autopilot and cruise control, which increases the risk of accidents, particularly in scenarios where these features are expected to function. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes, the problem has been reproduced and confirmed by Tesla. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives, or others? Inspection by tesla is scheduled Were there any warning lamps, messages, or other symptoms of the problem prior to the failure, and when did they first appear? Yes, the "Front Passenger Safety Restraint System Fault" warning appeared on the vehicle's display prior.
The contact owned a 2020 Tesla Model Y. The contact stated that while driving at 60 MPH in rainy weather, the vehicle inadvertently spun in circles, and he lost control of the vehicle. The contact took his foot off the accelerator pedal. No warning light was illuminated. The vehicle crashed into an embankment and then spun twice before inadvertently driving into the forest and coming to a complete stop. The air bags did not deploy. A police report was filed. The contact and his father, who was a passenger, were uninjured. There was no medical attention received. The vehicle was first towed to the tow yard, and then the insurance company had it towed to the salvage yard, where it was deemed totaled. The manufacturer was made aware of the failure and advised the contact to contact the local dealer for the computer data. The failure mileage was approximately 68,000.

The mechanical door release cable for the rear doors is not accessible without a screwdriver. This safety door release mechanism should be accessible without tools in case of emergency.

I am submitting this service request and formal incident report concerning a serious malfunction of the Full Self-Driving (FSD) system on my 2020 Tesla Model Y. While operating the vehicle on [insert road name or location] with FSD fully engaged, the system initiated a left-hand turn but failed to complete it properly. As a result, the vehicle mounted the sidewalk, causing a blowout of the rear tire and visible damage to the passenger-side rims. Following this incident, I had the tires and wheels replaced. However, the vehicle now exhibits the following ongoing issues: - Rubbing noise from the front passenger wheel well when turning - Multiple system alerts, including: - Autonomous Emergency Braking Disabled - Steering Alignment Issue - Stability Control and Traction Control Warnings

Vehicle had "front passenger safety restraint system fault" multiple times intermittently. Serviced by Tesla service center in Dublin, CA who confirmed the problem. Documentation on the invoice stated that there was an open service bulletin related to this issue and was related to "the occupancy sensory in the seat was not functioning as expected." Upon inspection by the technician they found "an open connection in the seat harness and seat cover that can cause intermittent alerts." Additional repair notes indicate that this was to "address a service bulletin related to the front passenger seat's occupant classification system. We identified the need for an update to ensure proper functionality. To resolve this, we retrofitted the system in the front passenger seat as per the bulletin requirements." The fact that there is an open service bulletin for this issue indicates that it is not isolated to just this vehicle. An issue with the occupant sensors of a front vehicle seat directly impacts whether the air bags will deploy in a crash, therefore this is a serious safety risk.
Subject: Complaint Regarding Faulty Seat Belt on Tesla Vehicle (VIN: [XXX] ) Dear NHTSA Vehicle Safety Hotline, I am writing to formally report a safety concern regarding my Tesla vehicle, VIN [XXX] . I have experienced issues with the seat belt system, which I believe are related to a known defect. Despite this, my vehicle was not included in the recent recall for faulty seat belts. The problem I am facing is consistent with the conditions described in the recall notice, and I am concerned about the safety implications of continuing to use this defective component. Seat occupancy sensor failed for the second time. The car will randomly shut off if I am inside because it does not detect a passenger. When I exit the vehicle, the car will stay on for an extended period of time due to failed seat occupancy sensor. This is a widespread issue affecting all makes and models of Tesla. They are aware of the high rate of failure and have not made any attempt to issue a recall. This is dangerous and inconvenient as the infotainment system will not pair with your phone because it does not detect a driver. When moving, the car will also randomly throw itself into PARK at speed if you don’t put on your seatbelt before moving. The first time this happened, I smashed my head on the steering wheel.

While driving with Auto Pilot the car turns off the high beams whenever it sees too many reflectors. Such as on power poles of guard rails. The problem is twice this week I seen deer crossing the road ahead of me and the car turned off the high beaks. Then for a couple seconds I couldn't turn on the high beams manually. When I asked Tesla to look at this problem I was told they are in development. I own this car for three years and I think nothing is being done.
This occurs driving on Highway 50 between Placerville, CA and Meyers, CA a predominantly 2 lane winding mountain highway bounded by many cliffs and rocks. The only way to properly engage the high beam lights is to activate the automatic high beam system. The lights inappropriately switch to low beams leaving me on a curvy road with insufficient light to see the curves. In some cases I need to hold the high beam flash control which does not allow me to keep both hands on the steering while while navigating that road. The vehicle needs a typical manual high beam control as I think the automatic system does not work safely when the cameras receive light bouncing off the rocks and snow. Virtually all cars have a manual control for this important light. This makes for a very dangerous driving situation.

While driving at highway speeds, the car unintentionally went into neutral. The setup: The driver was on the highway with cruise control on. Upon turning cruise control off to prepare for entering an exit ramp, the car also went neutral. We lost power and the regenerative braking that you expect in this car. The center console stated something like “Please shift into drive or reverse”. The cause is that Tesla uses an up movement on the right hand gear stalk to cancel auto pilot, cancel cruise control, and to put the car into neutral. If that up movement is too long, the car will inadvertently go into neutral regardless of car speed. We reported this to Tesla and they suggested all model 3s and Ys do this. Some light searching online showed this happening to others too. This feels dangerous at highway speeds. And the point at which you are turning cruise control off could be a critical phase of driving and feels like a unsafe time for the car to loose power.
Touchscreen repeatedly freezes. Won’t show gear selection, speed, or any other items. The touchscreen freezes then takes some time to crash to black screen before reboot. Has happened daily since Tesla update. Tesla refuses to provide a fix and continues to tell us to wait for an update. This is unsafe.

Fuel system

Vehicle has 51,126 mi as of this writing. When new (June 2020) 250 miles of freeway range was typical in typical California weather for an efficiency rating of 280 Wh/mi for 100% to 1% state of charge (SoC). This was tested repeatedly, numerous times, on multiple road trips. At ~12K miles into ownership, OEM tires were swapped as they were dangerously useless in rain and snow. Repeated calculations confirmed ~10% range loss. This meant that average Wh/mi consumption went from about ~280 Wh/mi to about ~308 Wh/mi, for a real world freeway range reduction to about 227 from say 250 (again 100% to 1% SoC). This was expected and accepted. Last 2 years were spent in New England during which freeway range started to drop precipitously, reaching a low of 166 miles (100% to 1% state of charge; about 420 Wh/mi) at freezing temperatures; verified across two identical road trips. Cold weather range was expected, but in retrospect, this was far too much of a drop. Now back in California, range has dropped even further and settled at about ~150 miles -- or an abysmal 450 Wh/mi -- i.e., a 40% drop from the OEM tires, and a 34% drop from the new tires. All charging stats (and Tesla's remote tests) show the battery is at full capacity (69 kWh is a 1% to 100% recharge and the battery is rated at 70 kWh). Tesla claims no faults, and assign blame to driver (high freeway speeds; Sentry Mode; climate control; etc). Tesla now threatening -- as indicated in recent class action lawsuit -- to charge $275/hr if I continue to press the issue, even though I have explained rental Model Y vehicles have vastly better range. I have insisted on several options: (A) Pay for a loaner to test. - NO (B) I will rent another Model Y from Hertz and report back with data. - NO (C) Acknowledge the problem, I want to pay to fix it. - NO (D) Acknowledge the problem, but you don't know how to fix it. - NO It is clear I am now being strong-armed by their Diversion Team.
Without any warning, the display on my car froze up when my car came to a stop, and I could not accelerate. It seemed to say I was going 15 mph, when in fact I was stopped, and the display was completely unresponsive. Stepping on the accelerator did nothing. I could not use my turn signals, activate my 4-way flashers. No one was injured, since this happened on a quiet residential street, and I was able to wave to other motorists to go around me. After about 3 minutes, my car seemed to have reset, and I was able to drive it normally. I would gladly make my vehicle available for inspection upon request.

My 2020 Tesla Model Y Long Range/AWD vehicle has 46,000 miles and can no longer recharge its battery (engine) with an approved charger (L2) due to a commonly failed part inside the battery. This part is called PCS (Power Conversion System). This device attached to the Tesla battery is used to convert power from a charger which is AC (household alternating current) into DC (Direct Current) the battery can understand. This error appeared for the first time on [XXX] at [XXX]. This critical part has failed in thousands of Tesla vehicles, some multiple times easily verified by thousands of customer complaints online. The PCS is NOT a consumable part - such as a tire, wiper blades, brake pads, etc. There is nothing the vehicle Owner can do to prevent the PCS failure. The Cost to repair/replace the PCS part(s) is ~ $3,000 and may fail again after replacement. As required by Tesla, I scheduled a service appointment through their phone app, and received an estimate of ~$280.00 to repair my vehicle on June 26, 2025 at the Irvine, CA Tesla Service Center 2801 Barranca Pkwy. After diagnosis, I was informed the cost to fix/repair the PCS part in my vehicle so that I could charge and drive it, would be ~$3,000. The Manager Ryan Sutherland (949) 404-2989, further stated the repair would not be covered under my Vehicle Tesla Warranty (Tesla battery or drive unit warranty that does not expire until [XXX]). I tried to contact Tesla corporate for assistance at [email protected] and did not receive a response but instead was contacted by service manager Ryan Sutherland via email confirming Tesla's warranty position. Tesla is essentially the only service/repair resource available for these vehicles. This repair is obviously due to a faulty/defective PCS or faulty design by Tesla affecting thousands of vehicles and should covered under my vehicle warranty so that it can be driven.
