Car is less than a year old and forward camera is reporting visibility problem. Safety features, forward collision and autonomous driving is severely degraded. Tesla will not repair under warranty which requires disassembly of the camera housing. Front camera housing has been redesigned by Tesla but they are not retrofitting this fix for existing Model Y and have not issues a safety recall. This camera housing defect presents a risk to my safely and the safety of others.

I am submitting this complaint to report what I believe is a potential safety defect involving the Tesla Autopark feature on my 2026 Tesla Model Y Premium All-Wheel Drive. On 7/21/26 at approximately 7:40 a.m., I activated Autopark to park my vehicle in my driveway. A transit van was already parked nearby and remained stationary throughout the parking maneuver. While Autopark was controlling the vehicle, my Tesla continued reversing and failed to stop before striking the parked transit van. The collision caused damage to the right rear passenger side of my vehicle. Based on what I observed, the vehicle did not recognize or respond appropriately to the presence of the stationary van. This raises serious concerns regarding the ability of the Autopark system to detect nearby stationary vehicles and avoid collisions during low-speed automated parking. Although I understand that Tesla states the driver must supervise Autopark, I am concerned that the system continued reversing into an obvious stationary obstacle rather than warning the driver or stopping before impact. I respectfully request that Tesla: Preserve all vehicle diagnostic logs, camera data, and sensor data from approximately 7:40 a.m. on the date of the incident. Conduct a full engineering review of the Autopark event. Determine why the vehicle failed to detect or avoid the parked transit van. Advise whether this incident resulted from a software issue, hardware issue, sensor limitation, or another system malfunction. Provide me with a written response and a case number confirming that this has been recorded as a formal vehicle safety complaint. I am prepared to provide photographs, dashcam footage (if available), the exact incident location, repair estimates, and any additional information required to assist with your investigation. The contact owned a 2026 Tesla Model Y. The contact stated that while her daughter was driving 16 MPH with the self-driving feature activated the vehicle failed to stop at a stop sign. Seconds later the contact's daughter attempted to disengage the self-driving feature and then depressed the brake pedal to stop from entering a right-turn-only minor road stop-controlled intersection. However, the front passenger’s side of the vehicle was struck by another vehicle, before striking another vehicle. The contact stated that the air bags on the passenger’s side of the vehicle deployed. The contact's daughter sustained minor leg injuries. Medical attention was not needed, and the contact was not sure whether occupants of the other vehicle required medical attention. A Police report was filed. The contact stated that the vehicle had not detected the oncoming traffic and that the crash could have been prevented. The vehicle was towed to a tow yard where it was deemed a total loss. The vehicle was not diagnosed or repaired. The manufacturer was notified of the failure. The failure mileage was 13,986.

My tesla model Y was driving on FSD (full self driving) and just did hard full stop brake at a traffic light and brought the car to a complete stop even when the light was green. Both of toddler kids in the back got pulled jolted forward where the seat belt hurt them. This stopping was random was unnecessary as no threat of other cars or the light was present. Luckily no one was behind us or they would have rear ended our car causing a serious accident. This really left my children in shock.

I was driving the car inside the parking garage of the UCLA Medical Plaza in Westwood at around 3:10pm on XXX, looking for an appropriate parking spot. When entering the second basement floor and turning the steering wheel right, to follow the road, the steering wheel was unresponsive and the car felt as if in neutral gear. As the car was on a downward slope, it went straight into the car parked in front. It happened so fast, but it seemed as if the brakes were ineffective. The air bags on the sides were deployed. The Full Self-Driving (Supervised) software on my 2026 Tesla Model Y repeatedly performs unsafe driving maneuvers requiring manual takeover to avoid collisions. Documented incidents with timestamps since June 3, 2026 include: selecting the incorrect lane and then attempting a route correction against a red light; activating the left turn signal while executing a right turn; attempting to enter a freeway from an incorrect lane; repeatedly missing exits, on-ramps, and turns; and making unnecessary lane changes. In one drive on June 9, the system missed two exits, a freeway on-ramp, a left turn, a right turn, and a U-turn twice within 18 minutes. On July 8, 2026, hours after the vehicle was returned from service with the manufacturer's advised mitigation active, the system repeated the same errors and drove the wheel into a curb, causing wheel damage. My safety and that of other road users was put at risk by incorrect lane positioning, incorrect signaling, and a red light conflict. The manufacturer's service center has inspected the vehicle twice (completed July 8 and July 21, 2026). Both visits were closed with no repair performed. The only action was advising me to disable the Online Routing setting as a temporary workaround, which did not prevent recurrence. The manufacturer stated in writing on July 8 that this is a known software-related matter its engineering teams are actively working to resolve, and a service advisor described it verbally as a firmware issue. However, the manufacturer's written response on July 31 states diagnostics identified no faults. A service center supervisor verbally acknowledged experiencing similar behavior on his own vehicle, indicating the defect is not unique to my vehicle. Service records, complete timestamped incident logs, and dashcam footage of the incidents are preserved and available for inspection upon request. No warning lamps or fault messages appeared before or during any incident.

Tesla was in FSD and had the latest software update installed on 8/6/28 which included the new FSD version released. On 8/7/28 on the way to work that my wife takes every time she goes to work the FSD ran over a large pot hole which it had never done before on the previous FSD version. The car started having rattling of the steering wheel and a rattling noise from over the rear driver wheel area. The next available Tesla service appointment was on 8/27 but we walked in and Tesla offered to do an alignment and found that the driver front and rear rims were bent from the pothole and we had to be pay close to 1800$. Thereafter, the rattling over the rear wheel well is louder but Tesla has not accommodated us with an earlier service appointment and refused to make any accommodations for the FSD update resulting in this damage.

My car was parked in a residential neighborhood and when I was driving home on the evening of July 4th I noticed the windshield had a small line in it. The next day during daylight hours I went to inspect it further and concluded that the hood made contact with the windshield on both sides and there were two distinct impact areas that matched perfectly with small paint damage on the hood. I made an appointment with Tesla and waited about two weeks to get in, and they summized that this is not a defective part issue and that a wind so powerful came along, lifted the hood up, overpowered the gas struts, broke the windshield and closed itself perfectly. They denied any responsibility and refused to cover it under their manufacturers warranty. This is Tesla of Wilmington, DE

Tesla Full Self-Driving (FSD) — control handover protocol. Vehicle is available for inspection upon request. WHAT HAPPENED: On June 25, 2026 at approximately 4:17 PM, the vehicle was operating under Full Self-Driving (FSD) navigating into a residential driveway. The driver pressed the gas pedal to take manual control. At the exact moment FSD transferred control, vehicle telemetry confirms the steering wheel was at −299° (nearly 3 full turns of hard left lock). No warning was given. With a ~13:1 steering ratio, approximately 300° of wheel rotation was required before driver input would affect the front wheels — taking 0.4–0.5 seconds during which the front wheels remained pointed left despite the driver turning right. The vehicle struck a tree at approximately 13–15 kph. Secondary failure: FSD re-engaged once during the first override attempt, requiring a second override before full control was obtained, further delaying driver response. Telemetry confirmed: factory crash algorithm (RCM_CRASH_ALGO_WAKEUP_EVENT_ACTIVE), Electronic Stability Control, and Traction Control all activated at impact. Post-impact speed registered −1.67 kph (backward bounce), confirming direct collision. SAFETY RISK: FSD handed off control mid-maneuver at extreme steering lock with no warning, creating an unrecoverable situation in a confined residential driveway. PRIOR WARNINGS: None. No warning lamps, messages, or alerts before or during handover. INSPECTION STATUS: Vehicle photographed. Insurance inspection pending. Not yet inspected by Tesla or dealer. 2026 Tesla Model Y (VIN [XXX]) running FSD Supervised v14.2.2.5 / software 2026.14.200 experienced two unintended phantom braking events while FSD was active. Incident 1: XXX, approximately 9:16 AM, XXX southbound off-ramp at [XXX] exit heading [XXX]. Vehicle braked abruptly and without warning at highway approach speed with no obstacle present. Incident 2: XXX, approximately 2:08 PM, [XXX] westbound atXXX northbound on-ramp [XXX]. Same abrupt, unwarned braking with no obstacle present. Tesla service performed diagnostics XXX, at the Delray Beach service center. No hardware fault was found. Vehicle has been unable to receive OTA software updates. A nearly identical vehicle, a 2026 Tesla Model Y in the same household (VIN [XXX]), is currently running software 2026.20.6.11 and FSD v14.3.6, several versions ahead of the affected vehicle. Tesla confirmed the software disparity is due to VIN batch rollout scheduling and cannot be manually accelerated. Tesla regional management and engineering review team have been notified. While children were not in the vehicle at the time of these specific incidents, they are frequent passengers in this vehicle and this braking behavior represents a serious safety concern for our family. My 2026 Tesla Model Y front trunk ("frunk"), an Austin, Mar 2026 build, lacks internal lighting & emergency release (unlike 2025 models; the change started Oct 2025). Frunk size/capacity remained unchanged, allowing child up to 4yo to fit & get trapped w/ no escape or signal, esp. in low light. Violates FMVSS 401 (https://www.ecfr.gov/current/title-49/subtitle-B/chapter-V/part-571/subpart-B/section-571.401) which states: "Each passenger car with a trunk compartment must have an automatic or manual release mechanism inside the trunk compartment that unlatches the trunk lid, if the trunk is large enough so that the three-year-old child dummy described in Subpart C of Part 572 can be placed inside the trunk compartment, and the trunk lid can be closed and latched". I am reporting a recurring and unresolved electrical/safety issue with my new Tesla Model Y. The issue has occurred 5 times within the past 4 weeks and affects multiple systems simultaneously. When the issue occurs: * The air conditioning stops working completely. * The driver-side front and rear windows cannot be rolled down or up. * The driver seat adjustment may not function properly. The problem appears intermittently. It may disappear temporarily and then return hours or days later. Tesla service has inspected the vehicle multiple times, but the issue continues to recur. This creates a significant safety concern because: * The driver window may not function during rain or emergencies. * The AC may fail during extremely hot weather. * The driver seat may not adjust properly, potentially affecting safe vehicle operation and driver visibility. * The intermittent nature of the failure makes the vehicle unpredictable and unreliable to operate safely. The issue has not been permanently resolved despite repeated service visits. I am concerned there may be a broader electrical or control system defect affecting vehicle safety.

Steering wheel vibration occurs at highway speeds between 70 and 75 mph on a 2026 Model Y with under 1,000 miles at time of complaint. The vibration was reported to the dealer, who performed a tire balance and alignment check and confirmed both were within specification. The vehicle was tested by the dealer during a service ride-along and no vibration was detected by the technician. However, vibration was independently confirmed by the owner in a same-model loaner vehicle on the same road, suggesting the issue may not be isolated to a single vehicle or tire set. Similar vibration issues have been reported by other owners of the same model year in online owner forums, with no documented fix currently available. Safety concern: reduced steering feedback at highway speeds. The vehicle remains available for inspection upon request.

June 30, I was driving an highway, PA 376 during a rain storm, FSD 60 MPH 4-5 PM. The rain was heavy, vehicles in front of me flashing their emergency lights, I attempted several times to slow down, Acceleration Modes, towards me. Vehicle would not slow down, heavy rain and puddles blurred my vision, thinking I would rear end the vehicle in front of me. Traffic was light due to the rain, FSD needs better software to slow down during weather conditions.

This is a safety concern for visibility. How can a brand new, barely used windshield crack like this? There are no rock chips and it has barely been driven to fall as wear and tear on the vehicle. This is not okay and incredibly unexceptional for vehicle standards. This vehicle spends 90% of its time in a garage. I have contacted my insurance company.

I am reporting multiple safety-related failures with my Tesla Model Y, purchased December 18, 2025. Within four months of ownership, the vehicle has experienced escalating issues: On April 19, 2026, while driving approximately 70 mph on a freeway in heavy rain, the vehicle suddenly lost power and decelerated to approximately 20 mph. Multiple safety systems became unavailable simultaneously, including automatic emergency braking, traction control, stability control, and vehicle hold. I was able to pull to the side of the road, but the vehicle would not restart and required towing. This created a dangerous situation and significantly compromised my ability to safely control the vehicle. On April 9, 2026, a rear seatbelt unlatched under braking while in use. This is a serious safety concern, as the passenger was not properly restrained and had to brace themselves. On February 10, 2026, the vehicle experienced repeated software failures causing lockouts. In one incident, I was locked out while my [XXX] child was inside the vehicle. On January 2, 2026, the vehicle required service for a cracked window shortly after delivery. The vehicle has been taken in for service multiple times, and although repairs were performed, new and increasingly serious failures have continued to occur. I am concerned about the safety of operating this vehicle, particularly due to the loss of power at highway speed and failure of safety systems. On [XXX] I pick up my New Tesla Y. On [XXX], while traveling at highway speeds on [XXX] with Autopilot "FSD" engaged, my 2026 Tesla Model Y experienced a sudden vehicle shutdown and start giving multiple alerts without warning. The vehicle displayed multiple critical alerts: "Vehicle Shutdown", "Pull Over Safely", "Stability Control Disabled." Upon the failure, Autopilot FSD disengaged, and the power steering immediately became extremely heavy ("hard"), making the vehicle difficult to steer. The car lost all propulsion. I had to manually maneuver the heavy, unpowered vehicle through active highway traffic to reach the shoulder, which created an immediate and severe safety risk. The vehicle was towed to a Tesla Service Center in Fort Lauderdale with only 130 miles on the odometer. Driver’s seatbelt: This issue occurred twice. The first time, the car was parked; when I attempted to pull the seatbelt, it would not extend and instead locked in place. The second time occurred after I summoned the car. Again, I was unable to pull the seatbelt out. Because the vehicle was stopped in the middle of the street, I had to drive without wearing a seatbelt, which felt very unsafe. Middle rear seatbelt: This occurred once. I was initially able to fasten the seatbelt, but when I tried to pull the strap down to loosen it because it was too tight, it retracted and locked. I was unable to extend it again, leaving the belt tight against my neck. This also felt very unsafe. I brought the car to Tesla twice, but they were unable to identify the issue. I showed them a video demonstrating the seatbelt remaining locked while I attempted to pull it, as well as footage showing that I was driving without a seatbelt. No warning lights appeared during any of these incidents.

On [XXX] 1512PM. Drive on [XXX] with auto pilot driving. Seen on construction and hit some object driver side by front bumper , fender, Side mirror,Door and Wheel. While driving my 2026 Tesla Model Y in June 2026, the vehicle suddenly applied steering input on its own and began swerving left and right even though there were no obstacles in the road. I was driving on a straight section of roadway, centered in my lane, when the steering wheel started moving by itself as if the lane keeping system and steering were confused. I had to fight the wheel in both directions to keep the car from veering toward the curb and toward the adjacent lane. It felt like struggling against another person at the wheel. Fortunately there were no vehicles directly beside me at that moment, so I was able to avoid a collision, but this behavior could easily cause an accident for a typical driver. This incident is part of an ongoing pattern of safety issues with this vehicle. Since late 2025, I have experienced multiple events where the car fails to respond to the accelerator after shifting into Drive and behaves as if it is in neutral until I shift back to Park and then into Drive again including a complete loss of propulsion while driving in moderate traffic. Tesla has already performed recall repairs on the battery/battery contactors and multiple diagnostic visits for intermittent loss of propulsion and steering concerns, but the problems continue. I am filing this complaint because I believe there is a serious defect involving the steering, propulsion system, and lane keeping driver assist systems that could cause loss of control and a crash.

On XXX at approximately 9:30am, my 2026 Tesla Model Y suddenly accelerated without driver input while parking at The Center at [XXX], [XXX]. Vehicle surged forward unexpectedly. I immediately applied brakes. No warning messages appeared before or during the incident. Vehicle struck parking lot curb. Police responded and vehicle was towed. A day or two prior I had received an alert that my coolant was low and to get service on my vehicle. Through the Tesla app I was unable to orginally schedule until July but I kept looking and an opening opened up on June 20th. I was on my way to that appointment from New Bern (the nearest service center is 120 miles from my house). I was avoiding highways, driving early AM, and driving at or below the speed limit to try to avoid the car from overheating. The car is pretty much brand new with just over 10k miles. I was only an hour in and had just left Kinston when I got a warning to pull over and then another alert a few moments later as I was trying to get off the parkway that the rear motor was shutting down and I began to lose speed rapidly. I stopped the car which proceeded to make a ton of noise and called roadside assistance. I had never experienced this type of noise before and was worried about combustion. Tesla was excellent at responding, towing, and the car is currently being repaired under warranty but I wanted to report the incident as it is a 2026 model with only 10k miles on it and has not been in an accident. This should not be happening to a brand new car. The latest repair report shows the rear drive inverter inlet hose and the supermanifold coolant pumps required replacing.

I was driving in full self driving mode on my Tesla model Y. The car stopped at the left turn stoplight as it does normally. And then while the light was still red and the cross traffic was green (traffic flowing), the Tesla released the brake, accelerating into the intersection. I stomped on the brake and kept the car from fully entering the intersection, then continue home without using Full Self Driving mode any further. If I hadn’t intervened, the Tesla would have caused an accident, injury, or even death.

Owned car for only a week with 470 miles on odometer. At appromately 10am CST on June 26 while on an exit ramp the tire sidewall failed catastrophically causing extraneous damage to fender well liner and molding. Car was stopped immediately as emergency services police and towbtruck were called. Weather was clear and there was np obstructions on the road. There was also no indication of low tire pressure until the failure happened. All tires were at the recommended 42 PSI per Tesla vehicle data. The failure was a complete total circumferential separation of the inner bead /sidewall from the rest of the tire. All tires were replaced by owner with Michelins due to lack of trust in the manufacturing quality of the pirelli tires. Tesla and Pirelli are not acknowledging any responsibility for this failure. This is a major safety related incident which could have caused a lot more damage if the car was traveling at a higher rate of speed. On an exit ramp the speed was not more than 45 mph.
