Airbags

The contact owned a 2011 Toyota Corolla. The contact stated that while he was driving at 40 MPH, another vehicle that was speeding caused a multiple vehicle collision. During the collision, the contact was in the middle lane when the speeding vehicle crashed into a vehicle in the left lane, and that vehicle merged violently into the middle lane, causing the contact to rear-end the vehicle in front due to the driver abruptly stopping. During the attempt to stop, the vehicle failed to decelerate sufficiently. Upon impact, the frontal air bags failed to fully deploy, as they only partially emerged from the housing. The failure experienced was a loss of braking effectiveness at 40 MPH that resulted in a rear-end collision, during which the frontal air bags failed to deploy. The vehicle was towed to a family member's home. The tire pressure warning was the only warning light on at the time of the crash. The driver was injured. Injuries sustained were lower back, right side ribs, and neck pain that required medical attention. The vehicle was deemed totaled by insurance. The vehicle was not repaired. The manufacturer was not notified of the failure. The failure mileage was 124,000.
I am submitting a safety concern regarding airbag non-deployment involving my former vehicle, a 2011 Toyota Corolla LE (VIN: [XXX] ). This vehicle was subject to multiple Takata airbag recalls, including a 2018 front airbag recall and a May 2020 Takata non-deployment and seatbelt pretensioner interim recall. Beginning in Spring 2021, the airbag warning light illuminated intermittently. On May 30, 2024, diagnostic trouble codes B1813 and B1818 were recorded, indicating faults involving the driver and passenger airbag squib circuits and potential center airbag sensor assembly involvement. A recall-related repair was completed on June 6, 2024 with installation of a Harness Kit (04009-98112) under extended warranty associated with a class action settlement. Following subsequent service, the airbag warning light was no longer illuminated. After these repairs, the vehicle was later involved in a rear-end collision in which the airbags did not deploy. The vehicle was declared a total loss and is no longer in my possession. Given the vehicle’s documented history of airbag system recalls and prior SRS fault codes, I am submitting this incident for safety documentation and review. I was notified of the air bag recall a year ago but the car dealerships in my area does not have the parts and I need to use my vehicle daily. So I am not sure what to do from here for that but I believe that the appropriate time to remedy this recall has past already and the fact that we have our children riding with us and we are unaware that their life is in danger or because we can not get it fixed is not acceptable. Then on top of that I am having issues like when my 2011 Corolla is sitting idle it will rev up the engine without touching the gas pedal and does this randomly and without warning and also while I am driving around 55 MPH on the interstate or highway sometimes it will dramatically drop and lose about 10- 15 MPH speed for no reason and this happens randomly as well. And this fluctuation happens during cruise control the most so I no longer even use that function. Also both headlights have moisture in them and are so fogged up now that visibility is almost impossible at night now. Also the weather striping around all my windows have dry rotted and broke off and there is practically nothing left of stripping anymore. Then the center dash above the radio just broke and gave out under my phone holder while driving, it broke into about 15 different pieces. I know that my phone or the holder is even close to heavy enough to cause the dash panel to cave in like that. I called to get a price on the dash and it was like $250+ and who can afford that for a piece of plastic. Lastly my driver and passenger seat will get stuck if you try adjusting them and sometimes it will let you move the seat but to do so you have to hop up and down for several minutes and throughout the adjustment to get the seat to move in any direction.

While driving on a straight road steering wheel suddenly jerk hard left and right several times for just a few seconds and then went normal. This caused the car to cross lane into oncoming traffic. Fortunately steering went to normal before causing head on collision. I took the car into local Toyota dealer to be checked. They could not duplicate trouble. No warning lamps lit up and no fault codes per dealership technician.
WHILE DRIVING ON THE HIGHWAY, THE CAR BEGAN TO DRIFT SIDE TO SIDE, FEELING LIKE IT IS SLIDING ON ICE. I FEEL VERY UNSAFE AND HOPE TO BE ABLE TO GET SOME ANSWERS TO RESOLVE THE ISSUE.

The contact owns a 2011 Toyota Corolla. The contact stated that while driving at an undisclosed speed, the contact heard a loud knocking sound coming from the engine compartment. There was no warning light illuminated. The vehicle was taken to the dealer, where it was diagnosed and determined that the oil pressure sending unit had failed and needed to be replaced. The vehicle was repaired. The manufacturer was not made aware of the failure. The approximate failure mileage was 150,000.

THE VEHICLE WAS TAKEN TO BRYAN EASLER TOYOTA (1409 SPARTANBURG HWY, HENDERSONVILLE, NC 28793, (808) 693-7261) TO BE DIAGNOSED HOWEVER, THE MECHANIC WAS UNABLE TO RETRIEVE A FAULT CODE. THE MECHANIC REMOVED AND CLEANED THE SPARK PLUG TIPS AFTER EACH FAILURE. THE CONTACT STATED THAT CLEANING THE SPARK PLUGS FAILED TO FIX THE FAILURE. REPEATS 54,321 AND 72,889. THE FAILURE PERSISTED. I am writing to formally request a recall or resolution for a significant defect in my [2011, Toyota, Corolla] vehicle, which I purchased in 2011. Over time, the paint on my vehicle has begun peeling extensively, affecting not just the aesthetics but also potentially the resale value and durability of the car. This issue appears to be a widespread problem among vehicles of similar make and year, suggesting a potential manufacturing defect. Given the extent of the paint deterioration, I kindly request that Toyota Company acknowledge and address this issue through a recall or appropriate corrective measures. TL* THE CONTACT OWNS A 2011 TOYOTA COROLLA. THE CONTACT STATED THAT THE MOLDING ON THE FRONT DRIVER'S SIDE WINDOW DETACHED, MAKING IT DIFFICULT TO ROLL THE WINDOW UP OR DOWN. A DEALER WAS NOT CONTACTED. THE CONTACT CALLED AN INDEPENDENT MECHANIC AND WAS INFORMED TO CONTACT THE MANUFACTURER FOR ASSISTANCE. THE MANUFACTURER WAS NOT MADE AWARE OF THE FAILURE. THE ACCIDENT HAPPENED IN THE HOUSE-ATTACHED GARAGE. I ALREADY ENTERED THE GARAGE, AND THE CAR IS ALMOST STOPPED. THEN I NEED TO HIT THE BRAKE THOROUGHLY TO FULLY STOP THE CAR. I HIT THE BRAKE PEDAL HOWEVER THE CAR ACCELERATED SUDDENLY, JUMPED AHEAD AND CRASH INTO THE HOUSE WALL. EVEN THOUGH EVERYTHING HAPPENED IN A SECOND, I'M SURE THAT IT IT THE LARGER PEDAL(WHICH IS THE BRAKE) I ENGAGED WITH IN THAT SECOND. I HAVE DRIVEN THIS CAR FOR 3 YEARS, AND PARKED IN THIS GARAGE FOR MONTHS. SO I'M PRETTY FAMILIAR WITH THE CAR AND THE PLACE. LUCKILY, THERE IS NO PEOPLE GOT INJURED, AND THE AIR BAG DIDN'T EXPLODE. THE WALL WAS HIT SIGNIFICANTLY, AND THE FRONT OF MY CAR WAS DAMAGED.
BRAKES WARPED AFTER LESS THAN 60,000 MILES. CAUSE SHAKING OF THE CAR WHEN BRAKES ARE APPLIED

The contact owns a 2011 Toyota Corolla. The contact stated that the headlights and fog lights were blinking then became inoperable. The air bags, oil, and fuel warning lights were illuminated. Additionally, the tires were losing air. The contact had to plug an item into the aux for the glitch to stop. The local dealer was not contacted. The vehicle was not diagnosed or repaired. The manufacturer was notified but no further assistance was provided. The failure mileage was approximately 72,000.
TO WHOM IT MAY CONCERN, WE HAVE EXPERIENCED ANOMALOUS BEHAVIOR IN OUR VEHICLE THAT WE BELIEVE IS DUE TO MALWARE. WE WERE REFERRED TO REPORT THIS FROM THE PUBLIC SERVICE ANNOUNCEMENT: HTTPS://WWW.IC3.GOV/MEDIA/2016/160317.ASPX#FN1 1. OUR ENGINE WILL SUDDENLY ACCELERATE AND DECELERATE WITHOUT OUR FOOT PRESSING ON THE ACCELERATOR TO CAUSE THIS CHANGE. IT WILL ALSO SLOWLY CHANGE WHEN CRUISING AT A CONSTANT SPEED, THE CAR WILL INCREASE SPEED GRADUALLY WITH VERY LIGHT PRESSURE ON IT. 2. OUR CRUISE CONTROL WILL RESET THE SPEED AT VARIOUS TIMES, WE WILL BE GOING 70 MPH ON THE INTERSTATE AND IT WILL GRADUALLY SLOW DOWN WITHOUT US DISABLING IT WITH A BRAKE PRESS OR SWITCHING IT OFF. (MY WIFE DOES NOT USE THE CRUISE CONTROL AND CANNOT VERIFY THIS.) 3. OUR RADIO WILL TURN ON SUDDENLY WITHOUT PRESSING THE POWER BUTTON. 4. OUR RADIO WILL EJECT A CD WITHOUT US HAVING THE RADIO ON. 5. OUR AIR CONDITIONER LIGHT WILL TURN ON AND OFF INTERMITTENTLY WITHOUT US TURNING IT OFF. (I CANNOT VERIFY THIS ALTHOUGH MY WIFE HAS OBSERVED THIS BEHAVIOUR.) 6. OUR AIR CONDITIONER WILL FUNCTION INTERMITTENTLY. SOMETIMES IT WILL OPERATE NORMALLY AND OTHER TIMES IT IS COMPLETELY NONFUNCTIONAL. The contact owns a 2011 Toyota Corolla. The contact stated while driving 20 MPH, the accelerator pedal was released, and the vehicle made an abnormal sound. The check engine and traction control warning lights were illuminated. Additionally, the vehicle hesitated to accelerate while driving. The vehicle was taken to AutoZone, where it was diagnosed that the transmission valve body needed to be replaced. The vehicle was then taken to the dealer, where it was diagnosed that the transmission needed to be replaced. The vehicle was not repaired. The manufacturer was not made aware of the failure. The failure mileage was approximately 86,000.
My Transmission On My 2011 Toyota Corolla Has Been Experiencing So Troubles .when being put into gear to drive it acts as if it's in Neutral and put on reverse the same thing . At first it was driving fine and Then a week later when you put it on driver I notice my rpms when getting ramped up but my it was slowly going up to 20 miles pre hour and you can fill it slipping. Now my car is undriveable

"TAKATA RECALL" CASE # 1607202446 I HAVE BEEN GIVEN A LOANER VEHICLE SINCE SEPTEMBER OR OCTOBER OF LAST YEAR THROUGH WOLFCHASE TOYOTA IN GERMANTOWN TN. EVEN THOUGH I LIVE IN SOUTHAVEN, MS. IT WILL SOON BE A YEAR THAT MY PERSONAL VEHICLE WILL BE SITTING WITH NO USAGE. THE DAMAGES TO MY VEHICLE'S GAS TANK, FUEL PUMP, BATTERY, PAINT, TIRES, FLUIDS, RUST, OIL, BRAKES, ROTORS, PLUGS, AND MAYBE MORE COULD OCCUR WHILE SITTING SO LONG. THERE ARE NO OTHER SOLUTION BEING GIVEN OTHER THAN A LOANER VEHICLE THAT HAS TO BE RENEWED EVERY 30 DAYS, ABOUT 45 MILES AWAY FROM MY HOME. HOWEVER, MY OTHER CONCERN IS THAT ONCE MY PERSONAL VEHICLE IS FIXED THE REPERCUSSIONS FROM IT SITTING FOR SO LONG WILL CAUSE FOR ME TO HAVE EVEN MORE SAFETY PROBLEMS THAT WILL NOT BE WARRANTIED. ONE SOLUTION THAT COULD BE OFFERED WOULD BE TO MAYBE FIRST PROVIDE AN EXTENDED WARRANTY ON THE PARTS THAT ARE BEING AFFECTED, BECAUSE OF THE DAMAGES CAUSED WHILE THE VEHICLES ARE SITTING. ANOTHER SOLUTION COULD BE TO PROVIDE A CERTIFIED INSPECTION ON EACH VEHICLE BEFORE BEING RELEASED BACK TO THE CUSTOMER. OR THE LAST SUGGESTION IS TO MAYBE PROVIDE THE CUSTOMER WITH A SET AMOUNT TO GO TOWARDS A NEW VEHICLE SO THEY WON'T HAVE TO DEAL WITH THE STRESS OF THE LOANER. I FEAR THAT ONE SAFETY ISSUE WILL BE TAKEN CARE OF ONLY TO BE REPLACED BY MANY MORE. I DRIVE QUITE A BIT AND NORMALLY KNOW MY VEHICLES PRETTY GOOD. I HAVE RECENTLY OBTAINED THIS 2011 TOYOTA COROLLA S AND IT IS IN GREAT CONDITION OVERALL. I MADE A TRIP 2 DAYS AFTER GETTING THE VEHICLE AND IMMEDIATELY NOTICED A HESITATION BETWEEN 55-65 MPH. THE FIRST TIME IT WAS APPROXIMATELY 11AM IN THE MORNING AT 78F. DRIVING ON A FLAT WELL MAINTAINED BLACK TOP ROAD THE VEHICLE DOES IN FACT HAVE A CONSTANT HESITATION THAT DROPS APPROXIMATELY 1000-2000 RPMS WITH A NOTICEABLE FEEL IN THE VEHICLE. IMMEDIATELY (3-4 SEC) AFTER THE HESITATION THE VEHICLE RECOVERS BACK TO THE ORIGINAL RPM. AS LONG AS STAYING IN THIS SPEED RANGE (55-65 MPH) THE VEHICLE HAS THE HESITATION REPEATEDLY. THE SECOND ISSUE THAT I HAVE NOTICED ON THE SAME VEHICLE IS THE ACTUAL DECELERATION WHEN REMOVING MY FOOT FROM ACCELERATOR. WHEN TRAVELING ON THE SAME ROAD AND TIME AS STATED ABOVE THE VEHICLE SEEMS TO REALLY COAST RIGHT ALONG OR SO IT SEEMS. WHEN SAYING THIS, I REMOVED MY FOOT FROM ACCELERATOR AND THE VEHICLE ROLLS RIGHT ALONG AS IF I AM STILL PUSHING THE ACCELERATOR AT WHICH TIME I MUST APPLY THE BRAKE MORE SIGNIFICANTLY THAN LETTING IT JUST COAST. I WOULD LIKE TO KNOW HOW THIS IS EFFECTING MY GAS MILEAGE, IS IT GOING TO CAUSE UNDUE STRAIN ON THE DRIVETRAIN, IS THIS GOING TO BE A PROBLEMATIC ISSUE THAT TOYOTA IS GOING TO CONTINUE TO IGNORE?

While driving my car seat seat (driver side) starts to over heat. I do not have heated car seats.but within the past few weeks I have noticed that my car seat started to heat up. At first I thought it was because it has been sitting in the sun all day, but as the driving progressed in the fact this is happened three times I feel that there is a flaw In the interior.
TAKATA RECALL. NO ONE HAS ATTEMPTED TO GET MY VEHICLE REPAIRED. I DEMAND ACTION.

I WAS TOLD TO FILE A COMPLAINT REGARDING THE TREAD SEPARATION THAT OCCURRED ON MY FRONT DRIVERS SIDE TIRE. THE BRAND NAME IS KUMHO SENSE. A FRIEND INFORMED ME IT MAY BE A DEFECT WITH THE BRAND OF TIRE. I'M JUST THANKFUL IT DIDN'T BURST WHILE I WAS DRIVING 8 HOURS AND KILLED ME. *TR
THE RIGHT FRONT OF THE VEHICLE HIT A STATIONARY VEHICLE ON ITS LEFT REAR, AT 25 PLUS MPH, WITH NO ADVANCE BRAKING. DRIVER EXPERIENCED FULL SUN BLINDNESS. NATURALLY THE QUARTER PANELS WERE DESTROYED, BUT THE DAMAGE INCLUDED THE SUSPENSION, WHEEL UNIVERSAL, CAR DOORS WERE PUSHED TOGETHER, AND THERE WAS EVEN A RUMBLE ON THE ROOF. CAR TOTALED. NO AIR BAGS EVER DEPLOYED. THIS SAID, THE DRIVER WAS UNSCATHED AND THE ALIGNMENT WAS STILL GOOD. THE ISSUE GIVEN THAT THE DOORS WERE PUSHED TOGETHER, THERE WAS A RUMPLE IN THE ROOF, BOTH TIRES WERE DEFLATED, AND THE UNIVERSAL AND AXELS WOULD HAVE TO BE REPLACED (THIS WAS THE MINIMUM IDENTIFIED DAMAGE), THIS SEEMS LIKE A SIGNIFICANT IMPACT.

1.- Bolsa de aire. Si me lo solicita, por supuesto puedo llevarlo a inspección. 2.- Se puso en riesgo ya que pudo haber sido otro tipo de accidente y las bolsas de aire no se activaron. Yo usaba el cinturón de seguridad. 3.- No ha sido confirmado por un distribuidor local. 4.- No no ha sido inspeccionado por el fabricante. 5.- No hubo ningún tipo de luces o indicador de la falla de las bolsas de aire. I had an accident because a dog ran away on a road and hit my car in front and I was going at a speed of approximately 20 mph, breaking my front bumper and the airbags did not activate.

THIS CAR WAS INVOLVED IN A THREE CAR COLLISION. A VAN TURNED LEFT SUDDENLY AND WITHOUT SIGNALING. THE CAR BEHIND THE VAN SLAMMED ON BRAKES. THE NEXT CAR HIT THE FIRST CAR IN THE BACK. THE THIRD CAR (MY CAR) HIT THE SECOND CAR IN THE BACK AND PUSHED HIM INTO THE FIRST CAR. MY DAUGHTER WAS DRIVING AND I WAS IN THE FRONT PASSENGER SEAT. THE FRONT AIR BAGS AND ALL FOUR OF THE SIDE CURTAIN AIR BAGS DEPLOYED. WE BOTH HAD ON OUR SEAT BELTS BUT THEY DID NOT LOCK. I, SEATING IN THE FRONT PASSENGER SEAT WITH MY SEAT BELT ON, WAS PROPELLED INTO THE FRONT WINDSHIELD. THE WINDSHIELD HAS A CIRCULAR PATTERN OF CRACKS WHERE I HIT IT WITH MY FACE. LUCKILY, MY FACE WAS ONLY CUT AND SEVERELY BRUISED WITH NO BROKEN BONES. I AM CONCERNED THAT THE SEAT BELTS DID NOT LOCK. NEITHER THE SEAT BELT NOR THE AIR BAGS PREVENTED ME FROM HITTING THE WINDSHIELD. MY DAUGHTER'S SEAT BELT ALSO DID NOT LOCK BUT THE AIR BAG PREVENTED HER FROM BEING SEVERELY INJURED.
TL* THE CONTACT OWNS A 2011 TOYOTA COROLLA. THE CONTACT STATED THAT THE REAR SEAT BELT BUCKLES WERE REVERSED AND THE REAR PASSENGER BUCKLE HAD TO BE SECURED ON THE OPPOSITE SIDE. THE DEALER STATED THAT THERE WAS AN ERROR AT THE TIME OF ASSEMBLY BUT THEY WOULD NOT REPAIR THE PROBLEM. THE MANUFACTURER WAS ALSO CONTACTED. THE VIN WAS NOT AVAILABLE. p195 /65r/15 91 h. yokohama avid ascend. i bought 4 tires on 04/11/22 at kinderhook toyota my car had 61008 miles on it. on 08/04/25 brought my car to same dealer was told i needed new tires. multipoint inspection said front tires had 3 32nd of tread and back tires had 4 32nd of tread wear left. my car had 80678 miles on it. the tires lasted less than 20000 miles. they had an 85000 mile warranty on them. i can understand tires lasting maybe 60000 miles or so, but only 20000 miles? was told i can contact your office about this. could not find a different page for a non- accident problem. dealer said to contact yokohama customer service and kinderhook toyota would send the tires back.
I WAS TOLD TO FILE A COMPLAINT REGARDING THE TREAD SEPARATION THAT OCCURRED ON MY FRONT DRIVERS SIDE TIRE. THE BRAND NAME IS KUMHO SENSE. A FRIEND INFORMED ME IT MAY BE A DEFECT WITH THE BRAND OF TIRE. I'M JUST THANKFUL IT DIDN'T BURST WHILE I WAS DRIVING 8 HOURS AND KILLED ME. *TR

TRANSMISSION SHIFT 1ST TO 2ND GEAR IS HARD, ALL OTHER SHIFTS ARE FINE. SOMETIMES THE TRANSMISSION "CHOKES" AND YOU GET A HARD BANG IN THE DRIVETRAIN. ALSO, THE DRIVER'S SEAT DOESN'T LOCK PROPERLY, SOMETIMES I HAVE TO SLAM ON THE BRAKES REALLY HARD IN ORDER TO GET THE SEAT FRONT-TO-REAR ADJUSTER TO LOCK OR "CLICK" INTO PLACE. THE SEAT ALSO SQUEAKS WHILE THE CAR IS IN MOTION, AND I WEIGH ONLY 175 LBS. CAR HAS ABOUT 3000 MILES TODAY, AND THE SALES DEALER WILL NOT SCHEDULE ME FOR AN APPOINTMENT ON MY ONLY DAY OFF FROM WORK (SATURDAY) SO I GUESS I WILL HAVE TO TAKE A PERSONAL DAY OFF DURING THE WEEK BECAUSE APPARENTLY AN OIL CHANGE IS MORE IMPORTANT. *KB
IMMEDIATELY AFTER PURCHASE WE NOTICED A TRANSMITTION PROBLEM WHERE THERE WAS A FLUXUATION OF RPM WHILE DRIVING AT 40 - 65 MPH. THE COMPUTER IS TELLING THE TRANSMITTION TO SHIFT FOR SOME UNKNOWN REASON. IT CAN NOT FIND A CRUSE GEAR TO REMAIN AT SPEED. *TT

The contact owns a 2011 Toyota Corolla. The contact stated that the headlights and fog lights were blinking then became inoperable. The air bags, oil, and fuel warning lights were illuminated. Additionally, the tires were losing air. The contact had to plug an item into the aux for the glitch to stop. The local dealer was not contacted. The vehicle was not diagnosed or repaired. The manufacturer was notified but no further assistance was provided. The failure mileage was approximately 72,000.
