My transmission started to have a noticeable whine at 53k miles. It became noticeably louder during acceleration. It was diagnosed at the Toyota dealership as a failing transmission. My vehicle was a few months out of warranty. I was told to contact Toyota corporate to ask for assistance. I was told since I did not have a service record at the dealership, they would deny my claim. I advised them the transmission was a sealed unit with no dipstick. My vehicle has been meticulously maintained with records. They said sorry, and the conversation ended.
Vehicle is experiencing a distinct, high-pitched mechanical whine or grind sound from the transmission during acceleration that increases in pitch with vehicle speed. The noise does not happen while cruising or coasting. This symptom aligns directly with Toyota Technical Service Bulletin T-SB-0008-21 ('Whine or Grind Noise From Transmission'). I am concerned about a premature, catastrophic internal bearing or gear assembly failure of the UA80 8-speed transmission while driving at highway speeds, which poses a severe sudden loss of propulsion safety risk.

I had the dealer perform the repair described in Toyota Technical Service Bulletin T-SB-0059-23 (Hybrid Regenerative Brake Transition Feeling Improvement), which reprograms the brake actuator control module. Following this repair, the vehicle continues to exhibit the same lurching behavior at the same frequency as before the update. The TSB did not resolve the underlying condition. This is a safety concern because the unpredictable brake transition affects the driver’s ability to judge stopping distance and vehicle behavior in real time, particularly in traffic or on downgrades. The vehicle now has over 36,000 miles, placing it outside Toyota’s Basic Warranty; however, this is a safety issue that should be addressed by a recall. I am aware of numerous other Highlander Hybrid and Sienna Hybrid owners reporting the same defect, including reports that the same TSB reflash failed to resolve the issue for them as well.
When applying the brakes when slowing down fro a traffic light or in traffic, the brkaes suddely stop workign for a few sseconds at a speed of 5-10 mph, causing car to lurch forward before the brakes reengage. This has existed for a couple of year but has gotten worse in thelast 6 month.s The risk is that the lurch occurs prior ot the full stop ,potentilly causing the car to run into the car directly in ftont of our car. Toyota is aware of the problem and issued a technical sevce bulletinT-SB -0059-23 whichidentified this and a software update helps in resolving the issue. The bulletin however states the cost needs ot be paid by th cusotmer if it is out of warrnaty. This does not makes snse as it is a potetnail safety issue that coudl casue a collisiion and wodul seem to rise to the level of a recall. There are many refermces to the issue on social media. It seems Toyota should be rqeuired to make this a recall item at no cost to the customer.

This is a formal report of Recall Non-Compliance regarding Safety Recall 23V-720 (Toyota Recall 23TA12) on my 2021 Highlander Hybrid. An authorized dealership performed the recall on May 6, 2024, utilizing Op Code 23TA12R1 (Inspection & Install Fasteners). The dealership only addressed the lower bumper and failed to properly remedy the upper bumper retention tabs, stating it isn’t part of recall. The upper bumper cover has completely separated from the fenders during normal daily driving. The upper plastic mounting slots are torn out and clips will not hold. The dealership is refusing further repairs, instructing me to file a car insurance claim or pay a body shop myself to correct an un-remedied federal safety defect. The vehicle currently remains an active structural road hazard because the official recall remedy completely failed to secure the bumper assembly from detaching. A TAS case has been opened at the dealer, but federal intervention is requested to ensure recall compliance.
The back hatch of the car has issues closing regularly. Nearly half of the time you push the button to shut the back hatch when it is open it will not close and has to be shut manually. Typically, it shuts an inch or two and then beeps at you and when you push the button again nothing happens but beeping. This is an issue because it is very difficult to close. Even as a 6' 200+ lb man who does strength training regularly, shutting the back hatch is difficult to do manually let alone for my much smaller wife. This has let to her having to ask for help from strangers in a parking lot to help close the door which is unsafe, particularly since we have small children. Toyota has acknowledged that this is a known issue and have stated that there is nothing that they can do to fix it. This acknowledgement came when the car was very new and my father was the owner and again just a few months ago when we brought up the issue again with Toyota.

I own a 2021 Toyota Highlander with approximately 127k miles. The vehicle has developed a noticeable whining/whirring noise during acceleration. The noise appears to originate from the drivetrain/transmission area and has become increasingly noticeable over time. The condition occurs every-time during acceleration and is present under normal driving conditions. I am concerned that this may indicate a transmission or drivetrain-related issue that could affect vehicle reliability and safety. I have found reports from other 2021 Toyota Highlander owners describing similar symptoms involving whining noises during acceleration. The vehicle has been inspected by a Toyota dealership. And is certified through Toyota. The vehicles was bought new from the Toyota dealership and has had all service done through Toyota. I am submitting this complaint so NHTSA can evaluate whether this condition is part of a broader pattern affecting similar vehicles as a defect in the original transmission built in the SUV.

Four days after I took delivery of this used vehicle (April 25, 2026), multiple warning lights appeared at once: BRAKE, ABS, PARK, Check Engine, Blind Spot Monitor Unavailable, and RCTA OFF. An independent Toyota dealership diagnosed the vehicle April 27-28, 2026 (approx. 109,000 miles) and found (1) water leaking into the rear hybrid battery area through a sunroof drain "installed incorrectly," causing a Brake System Control Module low-voltage code, starting issues, a headlight only partially functioning on one side, possible air in the brake system, and a musty HVAC odor; and (2) blind spot monitor fault code C1AC2 (Slave Module Horizontal Axis Misalignment) - the blind spot and rear cross-traffic monitoring systems do not function reliably. At a second visit on May 4, 2026, recalibration was unsuccessful; per Toyota procedure the only remaining option is replacing the right-rear blind spot sensor. The technician wrote the cause is a mystery, "whether it be body damage related or electrical short." Toyota service records show the same C1AC2 code was documented during the selling dealer's own pre-sale inspection, and that in December 2024 another Toyota dealership pulled the rear bumper and found the RR blind spot sensor bracket bent, causing incorrect readings - a recurring safety defect that prior calibrations and repairs have not resolved. Separately, federal safety recall 23V865 (occupant classification sensors; the passenger airbag may not deploy as designed) was open and incomplete when I purchased the vehicle and remains incomplete, and Toyota service campaign 25TD01 (rear traction motor cable corrosion) is also outstanding. The defect has been reproduced and confirmed by two Toyota dealerships, reported to the selling dealer in writing, and reported to Toyota's customer-care center. The vehicle is available for inspection upon request. No crash, fire, or injury.

Fuel system

On July 25, 2026, while driving my 2021 Toyota Highlander Hybrid at high speeds on the freeway, the vehicle experienced a sudden loss of drive power accompanied by a check engine light and a severe hybrid system error warning. The failure occurred without any prior warning or degradation in performance, forcing an immediate emergency maneuver through traffic to reach the shoulder. The vehicle was towed to Toyota of El Cajon for diagnostic evaluation. The dealership service invoice details the following findings: - Primary Failure: Primary fuel injector failure (direct injection system). - Diagnostic Behavior: The engine experienced severe cylinder misfires under load at freeway speeds when the direct injectors were commanded on. - System Impact: Engine misfires triggered a hard safety shutdown of the hybrid system, immediately cutting motive power to the wheels. - Repair Executed: Replacement of the failed fuel injector assembly Safety Concern: A primary fuel injector failure at highway speeds causes an instantaneous engine misfire under load, which immediately triggers a hard hybrid drive system shutdown. Cutting motive power suddenly in heavy traffic without advance warning creates an extreme hazard for rear-end collisions. I am requesting NHTSA document and investigate this failure mode and its automated system shutdown logic.
This appears to be a fuel pump or something with the fuel issue on a 2021 Toyota Highlander L series. While driving it would stop and switch off at stop lights and stop sign. The codes that come up have to do with fuel being lean. However this also affects the braking system and causes it to be very hard to steer with this issue. The problem I have is that it cuts off without even you knowing what's happening and I was almost rear-ended twice. My car is now sitting outside unable to start. It doesn't even have 85,000 miles on it with all these issues. I believe it also said something about my transmission but unsure because the codes were flashing off and on.

2021 Toyota Highlander UA80 transmission failed at 100,000 miles. Codes P2808, P0741. Matches Toyota TSB-0160-18 and T-SB-0008-21. Dealer Alexander Toyota and TMS Brand Engagement denied goodwill. TMS rep stated "TSBs don't matter" despite pending class action Pallaya v Toyota, Case 2:25-cv-10458, which cites these TSBs as proof Toyota knew of defect. Toyota refusing to honor documented defect. Transmission slip/loss of power is safety issue. Request investigation.

This is a supplemental complaint to add more detail to a prior submission. I am the original owner of a 2021 Toyota Highlander with 10,000 miles at the time of the issue worsening to the point that I finally broke down and was forced to pay the Toyota dealership's hefty diagnostic fee. The air conditioning system has failed due to a refrigerant leak in the rear A/C lines, specifically near the exhaust area under the vehicle. The dealership diagnosed the issue and quoted me over $3,000 to replace “all necessary components in the rear.” They offered no coverage or goodwill repair despite the vehicle’s age and mileage. Upon researching the issue for longer than a year at this point, I discovered far too many similar complaints from other Highlander owners (years 2019–2021) describing identical failures — typically rear A/C lines corroding, cracking, or leaking prematurely. In some cases, customers were told Toyota is aware of the issue but not issuing a TSB or recall. In other cases, dealers said no permanent fix exists. These repairs are prohibitively expensive and, in some reports, recurring due to poor line placement or dissimilar metal corrosion near heat sources. This significantly impacts safety in several ways: Impaired Visibility: The AC system is integral to effective defogging and defrosting of the windshield, especially in humid or cold conditions. Without it, visibility is severely compromised, increasing the risk of an accident. Driver Impairment: In extreme temperatures, lack of climate control can lead to driver fatigue, distraction, and reduced concentration, posing an indirect safety risk. I am reporting it to request NHTSA monitor the trend. This seems to reflect a possible engineering or design flaw, and the number of similar owner reports suggests it may warrant further review by Toyota and NHTSA. Customers should not be facing multi-thousand-dollar repairs on near-new vehicles with basic systems failing. Thank you for tracking this issue.
Premature AC refrigerant leak and failure in system in the 2021 Toyota Highlander due to corroded/defective rear AC lines located near the exhaust/spare tire, observed at very low mileage and upon official inspection at the nearest Toyota dealership. Noticed early in ownership and decided to have the vehicle scanned when air being released only continued to grow warmer in the sweltering summer months. Reporting due to advice from friends and owners in my exact situation with the very same issue as well as the pricey three-thousand dollars and upwards quote to have this work done from Curry Toyota, 3026 E Main St, Cortlandt, NY 10567. Vehicle purchased new to avoid these very issues.
Steering

The vehicle experiences intermittent power steering malfunction while driving. Warning messages including “Power Steering Malfunction” and “Low Voltage Abnormality” appear, and the steering wheel becomes stiff and difficult to turn. This has occurred multiple times during normal driving conditions, including at low speeds and while turning. When the issue occurs, it requires increased effort to steer, creating a risk of loss of control. Additional warning messages have included AWD malfunction, pre-collision system malfunction, and lane tracing assist malfunction. These alerts appear simultaneously with the steering issue. The problem began shortly after purchase and has continued intermittently. The vehicle has been inspected, but the issue has not been resolved. This condition creates a safety risk, especially when driving with passengers.
Vehicle had a very loud high pitch whining noise the rapid accelerated I was braking and brakes were not working vehicle screen was flashing Vehicle Malfunction. yes vehicle is available. I could not stop the vehicle I struck three signs and a lady fell down as i was approaching the the building very fast, hit a wrought iron black fence and it fell on a park vehicle and then i hit a park vehicle and it stop i threw it in park. i am not sure. Insurance company After the vehicle accelerated there was a message saying VEHICLE MALFUNCTION, not before the accident.

The contact owns a 2021 Toyota Highlander equipped with Bridgestone Tires, Tire Line: Alenza Sport, Tire Size: 255/50/R20, DOT Number: 276100C2R. The contact stated the vehicle was inspected by the dealer, where it was discovered that all four tires were prematurely worn quickly and showed defects. The tires were replaced under NHTSA Campaign Number: 24V452000 (Tires). The dealer informed the contact that the issue was related to the previous recall. The manufacturer was not notified of the failure. The vehicle failure mileage was 80,000. The tire failure mileage was 7,000.
P 235 / 55 R 20 This tire has developed a sidewall bulge. It has always been properly inflated. I currently have the tire but the manufacturer has requested that I send it to them.

Since I got his vehicle i satated to the Toyota that having the breaks problem and dash is not working properly and blind spot is not working on time
The contact owns a 2021 Toyota Highlander. The contact received notification of NHTSA Campaign Number: 23V865000 (Air Bags); however, the part to do the recall repair was not yet available. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The dealer was not made aware of the issue. The manufacturer was made aware of the issue and confirmed that parts were not yet available. The contact had not experienced a failure. VIN tool confirms parts not available. I am VERY concerned that Toyota hasn’t taken this more seriously. I notice in the NTSB website it has injuries and fatalities listed to the left of the complains. Does someone have to be injured or killed before Toyota gets their act together??? Thinking we may need to trade this for another SAFER, RELIABLE vehicle. I have also had many people flash me due to the headlights being too high and in their face-another SAFETY concern.

I am reporting a serious safety defect involving the rear brake system of my 2021 Toyota Highlander, purchased from Penn Toyota (Glen Cove, NY). The dealership represented the vehicle as “Toyota Certified” and indicated that it passed all safety inspections. On November 15, 2025, I discovered that the rear brake rotors and pads were in extremely unsafe condition, showing severe corrosion, deep pitting, uneven pad wear, and structural deterioration. This damage clearly developed over a long period of time and indicates that the vehicle could not have passed a legitimate safety inspection at the time of sale. The rear brakes had to be replaced immediately to prevent potential brake failure. After replacing the rear rotors and pads, the vehicle passed the New York State Inspection on the same day, proving that the previous brake condition was unsafe and improperly inspected. This raises concerns of: Brake safety failure Improper or falsified dealership inspection Risk of reduced stopping power Potential loss of vehicle control I am submitting photos, the repair invoice, and the inspection result that confirmed the brakes were unsafe prior to repair.
The EPS foam has come detached when removing my child from the seat and will not reattach even with the cover

On XXX, I was driving my 2021 Toyota Highlander at approximately 55 mph when a deer entered the roadway and I had to brake suddenly to avoid a collision. My [XXX] daughter was properly seated in the second-row seat and was properly restrained by her seatbelt. During the braking event, the second-row seat unexpectedly folded forward while she was seated in it. As a result, my daughter experienced rib pain and soreness that lasted several days. After the incident, the seat was returned to the upright position and appeared to latch normally. The vehicle is subject to a recall involving the second-row seat. I contacted Toyota Customer Care on XXX, and Toyota provided a rental vehicle on XXX, while the recall issue is being addressed. I am reporting this incident because the recalled seat failed during an emergency braking event and resulted in injury to a child passenger. The rear hatchback of my Toyota Highlander fails to open. I tried resetting it and that did not correct the problem. The only way I’m able to open the trunk is by manually crawling into the trunk, removing the back panel on the hatch door and pressing an internal latch release. While no injuries have happened because of this problem, if someone was stuck in this car or if there were an accident and a passenger needed to be accessed through the rear trunk, this would create a problem as it is not accessible from the outside of the car.

Early failure of front struts on a 2021 Toyota Highlander XLE, leaking fluid at 29,000 miles. Manufacturer does not seem to have any reported issues with these components. Issue discovered just one month after warranty expiration. Given the low daily use and time, this might be a manufacturer defect/concern. Could potentially be a safety concern affecting the vehicle handling and maneuverability.
•The transmission malfunctioned and developed an abnormal internal noise at approximately 67,000 miles, which is an unexpected failure at low mileage. The vehicle had been properly maintained and had no prior mechanical issues. Atlanta Toyota inspected the vehicle and confirmed the transmission had failed and required full replacement. The dealer also stated that as a result of the transmission failure, other related components showed excessive and abnormal wear. A second independent service center inspected the vehicle and confirmed the same diagnosis. The vehicle is available for inspection upon request. • This unexpected transmission failure created a serious safety risk due to the possibility of sudden loss of power, inability to accelerate, or complete transmission failure while driving. I rely on this vehicle daily to transport my child to school and to commute to work. A sudden failure while in traffic could result in becoming stranded or involved in a collision. • The vehicle was inspected by an authorized Toyota dealer and an independent service center. Both stated that the failure was unusual given the low mileage and normal use of the vehicle. • There were no warning lights, messages, or diagnostic trouble codes prior to the failure and afterward. The only symptom was an abnormal internal noise from the transmission, which began suddenly at approximately 66,000 miles. • The vehicle has received regular maintenance and is used for normal daily driving. There was no abuse, heavy towing, or unusual use that would explain a transmission failure at this mileage.

Seat belts

Rear passenger seatbelt has torn in multiple places. I believe something inside may be cutting it when it retracts. The buckle is now stuck in a tear and the seatbelt will not fully retract. Toyota refuses to fix it under warranty because it’s “not usual wear and tear” and there’s no recall. I believe this is a significant safety risk.
We had to cut a seatbelt because it was cutting into a child’s stomach. The child was screaming and complaining and it left a scar on his stomach. If we would have taken into the dealer which was 20 min away it would have done more damage to the child. We tried using the steps in the book to release it, didn’t work. As an emergency we had to cut the seat belt to release it.

My vehicle has 67,100 miles. A whining/whistling noise recently started and the dealer has diagnosed that a new transmission is needed at a cost of $6,000-$8,000 and it is out of warranty. Researching this shows that this is a known manufacturing defect referred to under T-SB-0008-21. I am concerned that my transmission will fail while I am operating the vehicle.
The contact owns a 2021 Toyota Highlander. The contact stated that while driving at 40 MPH, there was an abnormal whining sound coming from the front of the vehicle. There was no warning light illuminated. The dealer was contacted. The vehicle was not diagnosed or repaired. The manufacturer was not made aware of the failure. The approximate failure mileage was 49,729.
