Within the 1st year I purchased it new, I would get a warning that the RSA is malfunctioning. It seems to happen when I park in the sun and the windshield is very hot. The warning in my dash prevents me from seeing my other readings; mph, fuel, and I can't use my other safety features; LDA, PCS or cruise control. I've taken it in but then it does a reset and the technicians say there's nothing wrong. The warnings happen every month, weekly. I just got tired of taking it in to hear they don't see anything wrong!

The contact owns a 2022 Toyota RAV4. The contact stated that while driving through a trench at 5 MPH, the brake pedal was depressed to avoid scraping the bottom of the vehicle however, the vehicle unintendedly accelerated and failed to stop. Additionally, the steering wheel failed to turn as needed, and the vehicle crashed into a picnic table and ran over the chairs. The vehicle came to a stop after crashing into a rock. The contact stated that the bumper and doors were damaged and had scratches from the crash. There was no injury sustained. A Police report was not filed. The dealer was contacted and performed an investigation and determined that the accelerator pedal was depressed during the crash and the vehicle was driving 25 MPH; however, the contact stated that the brake pedal was depressed during the crash. The manufacturer was made aware of the failure. The failure mileage was approximately 62,000.

The contact owns a 2022 Toyota Rav4. The contact stated that the economy feature disengaged while driving. While the lane assist was activated, the vehicle drifted over into another lane. The engine was revving and backfired. The vehicle lost motive power while driving at undisclosed speeds. The driver's side windshield had a glaze or wax on the outside. While idling, the vehicle surged forward. The driver hovered a foot over the brake pedal. The vehicle was taken to the local dealer but was not diagnosed or repaired. The vehicle was taken to several dealers, AutoNation Toyota Winter Park Service Center (225 N Semoran Blvd, Winter Park, FL 32792); Claremont Toyota (601 Auto Center Dr, Claremont, CA 91711); and Miracle Toyota (37048 US-27, Haines City, FL 33844); but the vehicle was not diagnosed or repaired. Our 3 yrs old + Toyota RAV4 Hybrid SUV with barely over 30,000 miles on it had 2 safety issues within 2 months of each other! First was the driver's side mirror- it was rattling, feeling like it might fall off! Seems common issue, perhaps due to cheap plastic parts inside. We had it fixed- Toyota Brand Engagement Center gave a 'Goodwill' gesture to cover us replacing it at Dealership, though it should never have happened on such a newer vehicle in the first place! Then not even 2 months later, a Warning flashed- "Pre Collision System Malfunction!" Also "Headlights" with same warning- "SEE DEALER NOW!" Toyota Dealer. They said Forward Recognition Camera had 'INTERNAL MALFUNCTION' and needed to be replaced. It was going to be quite expensive, but the camera part is not even available in foreseeable future as there's back order for hundreds and they're still in production! It seems this too is a common safety problem~ We can't understand why there's not a RECALL on this. Toyota took no responsibility for this issue, which is APPALLING as, like the first 'side driver's side mirror failure' was also a safety issue! The whole reason we bought this vehicle was for its safety features! Without the front facing camera that comes with the car in same compartment as rear view mirror resides, we now have NO Pre-Collision Warnings, NO Headlight safety feature, NO RSA (Road Sign Assist System). It's infuriating that Toyota, who obviously is making inferior safety products, does not offer to help out in any way. We're both [XXX] and entitled to these safety features as we bought the car both new and paid top dollar! Our service advisor at the Toyota Dealership agrees this is 'not normal', the car is too young to have this happen! He will attest to this & we have paperwork. It stated 'No immediate remedy'. (replacement camera not available) I believe this is happening to hundreds of other RAV4 owners. It's outrageous! Please help! These electronics for safety should not fail! Driving up a slope of about 20-30 degrees gradient of a straight lane going into a private parking lot. Braked and decided to make a 3-point turn to get back out. There was no traffic in the lane. Put gear on reverse while on the slope with the vehicle pointing in the up direction of the slope. Vehicle suddenly surge backwards, accelerating. Vehicle sped down the slope and crossed a street. Could not stop. Vehicle was stopped only after placing gear into park from reverse, and after jumping curb across the street. Fortunately, there were no cross traffic in either direction on the street, otherwise vehicle could have been T-boned. Tire wall on the rear passenger side busted. AAA was called to replace tire with spare. There were no warnings on the dashboard or any symptoms anywhere else, although the vehicle had all the safety features.. The next morning, vehicle was taken to Toyota of Carlsbad. Left there for diagnosis and tires change. Vehicle is left with Toyota of Carlsbad until this issue is answered/solved. Told by service manager of Toyota of Carlsbad to call Toyota USA at [XXX] to report incident by customer, and request Toyota USA to send expert from Toyota to diagnose issue. During a heavy rain period , the vehicle's forward collision warming sensor came on momentarily and after a few seconds it went off. This happen while driving on local roads and road was wet from the rain. And there were no cars ahead or behind me. Spoke about this with the service department at toyota dealer and they mentioned this was likely from from the rain blocking the sensor.

I am filing this urgent complaint regarding a life-threatening defect in my 2022 Toyota RAV4 SUV that gravely endangers my family and the public. This is a desperate plea for intervention. For over a year, my vehicle has repeatedly displayed a terrifying, unreadable red alert and piercing buzzer, occurring 3-4 times without warning and not real collision threat. These incidents are terrifying precursors to catastrophic failure. The Toyota dealership dismissed my concerns, stating they "cannot do anything" without replicating the intermittent problem. This dangerous negligence compounds my frustration, especially after prior unaddressed issues like radio malfunctions and door water infiltration. Crucially, the dealer's own CAR DTC logs confirmed C14FE14: "steering angle sensor supply voltage circuit short to ground or open." This documented electrical failure directly impacts the steering system. Toyota's own webpage [XXX] ) also details similar warnings, confirming my fear of brake system activation. My paramount fear is a false Pre-Collision System activation. Imagine driving at high speed when the brakes violently engage or steering falters due to this electrical short. The unthinkable consequences could be a devastating collision, causing severe injury or death to my family and others. This is a documented defect, met with manufacturer and dealer refusal to act. I cannot feel secure driving this car, nor can I responsibly sell this ticking time bomb to another family. I implore the NHTSA to launch an immediate, comprehensive investigation into this critical safety defect. This is a public safety matter demanding urgent attention. The contact owns a 2022 Toyota Rav4. The contact stated that on several occasions while driving at undisclosed speeds and accelerating, the transmission momentarily shifted into 2nd gear and then into neutral(N), causing the vehicle to start coasting. The contact released and then depressed the accelerator pedal and instantaneously and the vehicle accelerated normally. The vehicle was taken to an independent mechanic where the transmission fluid was checked but the vehicle was not diagnosed or repaired. The manufacturer was not notified of the failure. The failure mileage was approximately 10,000.

While driving in stop-and-go traffic on a major highway, the auto braking system engaged and the brake would not release. The car came to an abrupt halt and the brake would not release, the gas pedal would not depress. I had to turn the car off, place the car in park and restart the car, the car then continued driving. There was a message on the dash about EPB.

The Blind Spot Warning system fails to alert the driver when a vehicle in the adjacent lane is partially behind the current vehicle. It only activates when the nearby vehicle is completely behind the current vehicle. Due to this limitation, the system does not provide adequate warnings for nearby vehicles, which has nearly caused several accidents when the driver attempted to change lanes.

My wife and I were traveling down SR74 mp 12 and rear ended a cow at 68 miles per hour. The airbags did not deploy from impact and yet the car is totaled the hood started through the safety glass of the windshield.

This RAV4 is having acceleration issues. While driving it randomly has issues at the time of acceleration. It has happened from a stop to a go, and gaining speed. This issue has put me into an incredibly vulnerable situation as I was very close to being hit by a semi while trying to merge onto the high way twice. When I pressed down on the accelerator, the engine hesitated to start gaining speed slowing down the speed needed to safely merge on to the highway. I have taken it in and all I am told is that there are no codes found and the issue can't be duplicated.

Above checked ADAS items don't work appropriately, and can't relay on them all the time. Apart from that My Toyota RAV4 Moon Roof shield couldn't be able to withstand for the wind during raining when the vehicle speed was at 75mph on Highway i-35 in Oklahoma. The glass busted completely, and those glass particles causes inconvenience to the other vehicle on the high way. I reported this incident to the car dealer but they were so rude to me, and said that car is not designed for the 75mph speed. Ended up convincing myself that RAV4 is not safe to drive anymore.

The vehicle without warning will lose all power; "Accelerator System Malfunction Output Power Reduced Visit Your Dealer" and "Pre-Collision System Malfunction - Visit Your Dealer" are the warning messages that appeared when power was lost. The "Check Engine" light also illuminated as well as a collision error light appeared. The vehicle has done this twice; the first time, the dealer found a recall which was a software update. However, today the vehicle became unresponsive again with the same error messages as before; we were lucky no vehicle hit us from behind since we were turning towards the frontage road to access the highway. The first time this happened, my wife was entering the ramp towards the expressway when the vehicle lost all power and she had to go to the side. Vehicle is now again at the dealer but cannot be serviced until Monday; however, this needs to be addressed since it is a safety concern. On the Toyota App, it is also listing a hybrid malfunction; this needs immediate attention to avoid a catastrophic event. Losing all power with no warning is completely unacceptable.

The Cruise Control has issues. On 3 long rides so far, I have experienced that it suddenly loses speed and gives the message radar ready now. Furtunately i was maintaining very safe driving distance with other cars on the highway, which has helped me accelerate in time to continue driving and avoid collision. I dont know of any pattern which triggers this. This could lead to a serious problem if it fails in a tricky spot on highway. I am afraid to use it as its not 100 percent reliable.

My Car is Toyota Rav4 2022 Premium XLE have many safety issues -During i'm braking/ hold button and standing on the light i find the car is jumping to front many times it was going to hit some one of pedsterian and car front of me ( it was like Starting engine with jumping) - Precollsion system doesn't work while doen't see any cars front of me while going to traffice light - Keep lane assistant doesn't work and is not senstive sometime my car get out of the lane during i'm driving - Sometime keep lane assistant system pushing the car to get out of the lane
