Description of the Safety Crash/Informed Incident: Vehicle Information: 2004 Toyota Sequoia (VIN: [XXX]) Incident Date: [XXX] Location: [XXX] Police Agency: California Highway Patrol (Report #:[XXX]) Speed at Incident: Highway speeds (~65 MPH) Description of Defect/Failure: Structural mechanical failure leading to sudden, catastrophic wheel separation at highway speeds. Nine days prior to the incident, on [XXX], the vehicle underwent a comprehensive brake, rotor, and tire rotation service at a certified Toyota dealership (Toyota of Chula Vista, Work Order #[XXX]). While traveling on the interstate, the right rear passenger wheel assembly completely detached from the moving vehicle. As a result, the wheel flew off crossing several traffic lanes, traveling over a jersey wall on the freeway shoulder, landing into a nearby drainage ditch. The vehicle dropped onto the remaining brake/rotor assembly, generating extreme sparks, causing an immediate loss of braking pressure, and creating an imminent threat of a rollover or multi-vehicle fatal collision. Subsequent post-incident inspection on a maintenance lift revealed that all wheel studs on the right rear hub were completely sheared or stripped off. This indicates a severe technician failure to properly torque the wheel lug nuts to factory specifications during reassembly, or an un-diagnosed structural failure of the wheel mounting components following certified dealer service. The contact stated that the check brakes and a traction control warning lights illuminated. The contact stated that the failure was intermittent at first but had become a recurring failure. When Toyota issued a recall for the braking issue, the contact did not receive notices, so the contact called them and was told it was a few selected 2004 Toyota Sequoia recalled for those who had complained but, it was not broadcasted like that. The contact had taken the vehicle to a local dealer, but the vehicle was not yet diagnosed. The vehicle was not repaired. The contact researched online and related the failure to NHTSA Campaign Number: 07E023000 (Service Brakes, Hydraulic). The manufacturer was informed of the failure. The failure mileage was approximately 143,000.

The contact owns a 2004 Toyota Sequoia. There was an abnormal rattling sound detected in the front area. The vehicle was shaking. No warning lights were illuminated. The vehicle was taken to an independent mechanic who determined that the frame was completely rusty and unsafe to drive. The vehicle was taken to the local dealer to have the frame checked, and it was determined there was minor rust detected. The vehicle was not repaired. The contact vehicle was taken to another local dealer, Crestmont Toyota Parts · Crestmont Toyota Service, 730 NJ-23 N, Pompton Plains, NJ 07444, who determined there was rust detected but was not repaired. The contact wrote a letter to the manufacturer. The failure mileage was approximately 198,000.

TOOK 2004 SEQUOIA TO KINGS TOYOTA FOR THE AIRBAG RECALL AS WELL AS DIAGNOSTIC EVAL OF VSC ABS LIGHTS ISSUE. RECEIVED A PHONE CALL THAT THE TECH COULD NOT DETERMINE THE ELECTRICAL PROBLEM UNLESS WE PAID AN ADDITIONAL $360 FOR MORE TIME TO DIAGNOSE AND THAT THE FRAME WAS RUSTED THROUGH WITH LARGE HOLES. WONDERED IF WE WANTED TO PURSUE THE ELECTRICAL ISSUE WITH THE LARGER SAFETY ISSUE WITH THE FRAME. ORIGINAL ISSUE OCCURRED WITH LIGHTS COMING ON (HAD HAPPENED SEVERAL TIMES PREVIOUSLY) FOLLOWED BY THE POWER WINDOWS CEASING TO WORK, SEVERAL HOURS LATER THE WIPERS AND WINDOW SEEMED SLUGGISH AND THE CAR LOST ALL POWER. HAD TO JUMP THE CAR SEVERAL TIMES TO GET OFF THE ROAD AND THEN WHEN JUMPING NO LONGER WORKED, WE HAD TO PUSH THE VEHICLE INTO A CHURCH PARKING LOT. AFTER BEING TOLD BY MR. HOWARD THAT THEY DON'T SEE VEHICLES COME INTO SERVICE WITH LARGE HOLES IN THE FRAME, I RESEARCHED ONLINE AND FOUND THAT 10S OF THOUSANDS OF TOYOTA TRUCKS, INCLUDING 05-08 SEQUOIAS, HAVE BEEN RECALLED - CRC APPLIED AND FRAMES REPLACED AS WELL AS TOYOTA BUYING BACK 95-00 TRUCKS WITH RUSTED FRAMES FOR 1.5 TIMES THE BLUE BOOK VALUE. 04 SEQUOIAS MENTIONED MORE THAN 300 TIMES FOR THE VSC, TAILGATE AND RUSTED FRAME ISSUES THAT ARE PREVALENT IN THIS MODEL - MY VEHICLE HAS ALL 3. SPOKE WITH COLIN, THE SERVICE MANAGER, WHO REITERATED WHAT I FOUND ONLINE. TOYOTA WILL DO NOTHING SINCE THE RECALL IS CLOSED AND DID NOT INCLUDE THE 04 MODELS.

AFTER CAR WARMS UP AND IF THE CAR IS TURNED OFF AND BACK ON SRS LIGHT WILL GO BACK OFF. FOLLOWING AIRBAG CODES ARE STORED IN SRS AIRBAG MODULE (ECU): B1141 B0116 THIS IS A KNOWN PROBLEM BY TOYOTA SINCE 2004 BUT THEY ARE NOT RECALLING THIS PART OR OFFERING REPLACEMENT. FRAME HAS RUST HOLES AND THE STEERING STABILIZER MOUNT RUSTED COMPLETELY OFF. VERY UNSAFE ! THE REST OF THE CAR IS IN EXCELLENT CONDITION SO THE SEVERITY OF RUST ON THE FRAME SEEMS VERY OUT OF PLACE. THIS ISSUE IS EXTREME AND NOT NORMAL. NOT ACCEPTABLE FROM A MANUFACTURER KNOWN FOR ITS QUALITY SYSTEMS. TPS SIX SIGMA EMBARRASMENT IN MY OPINION

FUEL GAUGE NOT MEASURING FUEL PROPERLY. AFTER A FULL TANK, GAUGE INDICATES EMPTY TANK AFTER FEW DAYS. SPEEDOMETER NOT WORKING AND ENGINE LOSES POWER FOR A WHILE WHILE THE GAUGE GOES TO 100 MILES ON THE HIGHWAY AND GOES BACK TO ZERO AND RESTORES POWER TO THE ENGINE. THE VEHICLE HAS ONLY 145,000 MILES ON IT. ALSO RUST ON THE BODY FRAME .

(706) 336-8450) AND EVERYTHING SEEMED TO BE FINE WITH THE VEHICLE UNTIL I TRIED TO SWITCH IT OUT IN CHARLOTTE, NORTH CAROLINA'S AIRPORT LOCATION ENTERPRISE RENTAL CAR COMPANY (I ALSO NOTICED A SCRATCH ON THE WINDSHIELD THAT I HAD NOT SEEN NOR EXPERIENCED ANYTHING HITTING ONCE I RECEIVED THE CAR BACK) . THEY HAD SWITCHED ME INTO A JEEP WHICH WAS EMMITING SOME TYPE OF CHEMICAL THAT MAKES YOU CHOKE AND BLEED IF YOU'RE PREGNANT (I'M PREGNANT). SO I SWITCHED BACK TO THE VAN THAT I HAD BEFORE BUT THIS VEHICLE STARTED DOING THE SAME. MY VEHICLE HAS NOW SINCE BEEN WRECKED (NOV. 2, 2012) AND SITS AT LANCE TOWING COMPANY IN LAWRENCEVILLE, GA. HOWEVER, THIS IS NOT AN ISOLATED INCIDENT AS I HAVE NOTICED THAT WHEN TRANSFER TRUCKS PASS BY MY THEY BLOW THEIR PIPES WHICH GIVE OFF THE SAME EFFECT BUT STRONGER IF YOU'RE TRYING TO OPEN THE WINDOWS TO GET FRESH AIR. I HAVE NOW HAD TO RESORT TO WEARING A MASK TO PROTECT MY HEALTH AND THE HEALTH OF MY UNBORN CHILD. I DO BELIEVE THAT THIS IS NOT AN ACCIDENT AND THAT IT IS EXTREMELY INTENTIONAL AS I HAVE HAD TO REPORT THE GOVERNOR OF GEORGIA, NATHAN DEAL, OF SUSPECTED CORRUPTION. IT IS ALSO MY BELIEF THAT THIS ACTION IS ALSO EXTENUATED BY MY EX, KIRK CHRISTIAN OF KINGSTON JAMAICA, AND THAT HE MAY BE PROVIDING SUCH CHEMICALS TO INCREASE THE CHANCES OF MY HAVING A MISCARRIAGE WITH MY SOON TO BE EX HUSBAND, WILLIAM EDWARD ROYSTON. CHECK ENGINE LIGHT COME ON. ALONG WITH 2 OTHER LIGHTS. THIS HAS OCCURRED OVER 10 TO 12 TIMES SINCE OWNING THIS SUV. EACH TIME IT IS NOTED ON MY PAPER WORK NO LEAKS FOUND AND GAS CAP REPLACED. I HAVE SPENT EACH TIME ON A AVERAGE $578 AT LEAST FOR A GAS CAP AFTER THEY HAVE CHECKED OUT THE CAR. I EVEN HAD THIS PROBLEM WHEN THE CAR WAS UNDER WARRANTY BUT OF COURSE NO CHARGE THEN. I OWN A 2004 TOYOTA SEQUOIA. I TOOK IT IN FOR NEW TIRES,WITH BALANCING AND AN ALIGNMENT. THEY ALSO DID BRAKE WORK ON THE FRONT. IT NEEDED CALIPER PINS AND THE ROTORS WERE TURNED. IT HAD 47,481 MILES ON IT. WHEN MY WIFE LEFT THE DEALERSHIP THERE WAS A FAILURE OF THE VSC (VEHICLE STABILITY CONTROL) SYSTEM. THE SYSTEM NEVER ONCE BEFORE MALFUNCTIONED UNTIL AFTER THE SERVICE AT THE TOYOTA DEALERSHIP. IT IS CLEAR THAT THE DEALER MESSED SOMETHING UP. THEY CLAIM THEY DID NOTHING TO CAUSE IT. WE TALKED THE HEAD MANAGER OF THE DEALERSHIP. THE MANAGER DENIED THAT IT COULD BE IN ANY WAY THEIR FAULT. IS IT POSSIBLE THAT THE SERVICE CAUSED THE FAILURE? I DON'T KNOW WHAT TOYOTA HAS THEIR MECHANICS DO BUT SOMEONE NEEDS TO DO A FULL INVESTIGATION. THE VEHICLE NOW IS A HAZARD TO DRIVE. I CURRENTLY CAN'T AFFORD THE $1500 DOLLARS TO REPLACE THE COMPUTER. COULD IT BE RELATED TO THE KIND OF TIRES PUT ON OR DUST OR METAL IN THE SENSORS? FAILURE CODES ARE C0210 (RIGHT REAR WHEEL SPEED SENSOR SIGNAL MALFUNCTION, C1223 (ABS CONTROL SYSTEM MALFUNCTION) , AND C1310 (ACTIVE BOOSTER SOLENOID). THE SEQUOIA'S VSC LIGHT COMES ON OFTEN AND THE VEHICLE WILL NOT ACCELERATE EVEN WHEN THE PEDAL IS PRESSED ALL THE WAY TO THE FLOOR. IT HAS ALMOST CAUSED ABOUT 2 BAD ACCIDENTS. WE CURRENTLY DRIVE IT BUT DON'T RELY ON IT FOR PASSING. IF THE SYSTEM FAILS WE JUST HAVE TO DRIVE SLOW AND STAY IN OUR LANE. I DO KNOW THAT TOYOTA KNOWS THAT THIS IS A HUGE PROBLEM. THEY ALSO USE A DIFFERENT SKID CONTROL COMPUTER IN NEWER MODELS. WE NEED A RECALL. I HAVE SEEN MANY REFERENCES TO THIS SAME EXACT PROBLEM. FOR THE SAFETY OF OUR DRIVERS PLEASE ISSUE A RECALL. THE MANUFACTURER OBVIOUSLY DIDN'T TEST THE SYSTEM BEFORE IT WAS ALLOWED ON THE MARKET. *TR

VSC TRACK SENSOR AND ENGINE CHECK LIGHTS CAME ON WHILE DRIVING AT 40 MPH IN THE CITY. I HAVE NOTICED IN THE PAST TWO MONTHS THAT WHEN I SLOW TO MAKE A TURN THE BREAKS LOCK UP AND GRIND, BUT NOT ALWAYS. THE SAFETY BREAK IS ALSO ON. I HAVE READ THAT WHEN THE VSC LIGHTS DO COME ON THAT YOU CANNOT GO OVER 50 MILES AN HOUR OR WHEN ON THE HIGHWAY THE CAR WILL SLOW DOWN TO 50 AND CANNOT GO ANY FASTER. I'M AFRAID OF THIS HAPPENING WHEN TRAVELING ON THE HIGHWAY.

TL* THE CONTACT OWNS A 2004 TOYOTA SEQUOIA. IN 2011, THE CONTACT STATED THAT THE LIFTGATE LATCH FRACTURED. THE LATCH WAS REPLACED BY CULVER CITY TOYOTA (9077 WASHINGTON BLVD, CULVER CITY, CA 90232); HOWEVER, THE LATCH FRACTURED AGAIN AND THE TAIL LIGHT NEEDED TO BE REPLACED. IN ORDER FOR THE DEALER TO REPAIR THE VEHICLE, THEY HAD TO OPEN THE LIFTGATE, BUT WERE UNABLE TO DO SO. THE VEHICLE WAS NOT REPAIRED. THE MANUFACTURER WAS NOTIFIED OF THE FAILURE.
