Moving about 55 miles per hour on right lane of I-95 the Jeep in front of me braked abruptly. I braked hard but as my Sienna was quickly slowing down to stop, the brake pad felt spongy when it was slower and as I pressed even harder, it seemed to have stopped decelerating at that slower speed and that was when my Sienna slammed into the back of the Jeep. Happened today on July 15, 2026 at around 4:24 EST. I will also file this same report to Toyota. My wife and I are at the hospital getting checked, but my Sienna looked undrivable.

When approaching a stop sign or light and while the brake is already engaged the car will accelerate lunging the car forward from 3-10ft while driver maintains the same pressure on the break pedal.

I have never been in an accident in my 31 years of driving. However, I am worried that this van is going to cause me to be hit. Several times I have pulled out to make a left or right turn and my gas pedal does NOTHING. I can push it clear to the floor and NOTHING!!! Cars end up slamming on their brakes and honking at me but there's nothing I can do. This is EXTREMELY scary and a HUGE HUGE HUGE manufacturing flaw. There needs to be a recall for this to be fixed before people are injured.

The contact owns a 2024 Toyota Sienna. The contact stated that while approaching a stop light and pressing the brake pedal the vehicle crashed into the rear of a second vehicle. During the crash the collision avoidance system did not activate and no air bags had deployed. No injuries were reported. A police report was taken at the scene.. The failure mileage was 25,000.

What component or system failed or malfunctioned, and is it available for inspection upon request? -The engine malfunctioned. It is available for inspection. How was your safety or the safety of others put at risk? -The engine went out at 77k miles in the middle of driving with no warning. Could have caused a serious accident. We have always done proper oil changes. The rods separated from the pistons and punched a hole in the engine block. Has the problem been reproduced or confirmed by a dealer or independent service center? The dealer diagnosed it and our mechanic said this has happened with Toyotas too recently Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? -no Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? -no warnings or messages before the failure of the engine.

I notice the driver seat had a clip that popped off unto my carpet, when inspected it came off the seat rail. While moving my seat back using the electronic seat lever a white clip with two metal bearings was sticking out, I notice the ball bearings were not in place and had came off the white clip. Yes, it is available for inspection. This pose a safety concern as the seat rails will not function correctly when moving seat or using the memory seat buttons for each driver. I have not brought the vehicle to a dealer to inspect as there is a estimate fee involved. I am currently out of my 2 year toyota care warranty. I am not seeing any warning lamps or messages besides the seat making a clicking noise when I move the seat back and forth.

When a lever is pulled, the second row seat can move freely back and forth on a track until the back of the seat is returned to the upright position. I was driving at highway speeds and had to brake suddenly. The seat was at the back of the track with the lever pulled, and it slammed into the back of the driver seat, hurting my back. It stuck there putting pressure on the seat (which could be felt) until I stopped and had to use force to make it slide back.

The power liftgate on my 2024 Toyota Sienna consistently fails to complete a full close-and-latch cycle. When activated, the liftgate begins its closing sequence normally but then aborts the cycle during the final stages of latching. The system emits an audible beeping signal and the liftgate reverses direction, reopening approximately one-quarter of the way. This failure occurs repeatedly when the vehicle is stationary with no external obstruction present. Multiple repeated attempts to close the liftgate using standard operation (key fob, interior switch, and exterior switch) result in the same failure pattern. The system does not complete a secure latch. Eventually, the liftgate can only be fully closed manually with physical force, bypassing the powered closing function entirely. This condition occurs intermittently at times but has become frequent and repeatable enough to prevent reliable use of the power liftgate system. No obstructions are present during operation. The local Toyota dealership service department observed this and indicated a lift gate adjustment is required, noting it is a known issue Toyota Customer Care has indicated they have no documented record of a known defect relating to this condition. However, multiple independent owner reports and dealer-level discussions indicate similar behavior may be occurring in other vehicles of the same model line, suggesting a potential systemic issue that may not yet be formally recognized. This malfunction raises potential safety concerns, including but not limited to: failure of the rear liftgate to fully secure and latch, increased risk of the liftgate remaining partially open or unlatched without clear driver awareness, and reliance on manual force to override an electronic safety system designed for automatic secure closure. I request that this complaint be documented for investigation and review to determine whether a defect trend exists in the power liftgate system of the Toyota Sienna platform.

Dear Sir/Madam, I am writing to formally raise a complaint regarding a serious and ongoing issue with my 2024 Toyota Sienna XLE AWD, purchased in April 2024 in Illinois. From the very first day, the trunk door has not functioned properly — often failing to close, randomly opening, or requiring manual force to shut. Despite repeated service visits across multiple states (IL, SC, NC) and investing considerable time, fuel, and lost income as a contractor, this issue persisted for over 18 months, creating safety concerns and inconvenience for me and my family. Key Issues: Trunk door opens on its own – even while reversing, creating serious safety risks. Malfunctioning power close mechanism, requiring manual closure frequently. Unreliable performance: door would close, then immediately re-open. Visited Toyota service centers at least 6 times across 3 states. Each visit took 2–3 hours, costing me work hours as a contractor. Customer service experience has been poor, especially with manager Patte, who: Failed to schedule proper service appointments. Directed me without coordination. Refused to escalate my request or connect me to her manager. Dismissed my request for compensation by saying, “It’s your issue.” Please help me on this issue. Thanks in advance.

The driver's side mirror vibrates when the vehicle is at speed. I've been to two dealers who say that it's normal, and everything is intact and installed correctly. When driving, the mirror vibrates a lot, which is a safety issue when trying to use the side mirror during changes, etc.

Wobbling vibration on exterior mirror, driver side. It looks like incoming vehicles shake sideways. This happens on any bumpy road and it is easier to observe at highway speeds. Purchased on [XXX]. Vehicle is available for inspection. The distorted image resulting from mirror wobbling affects driver's awareness of surroundings, visibility gets worse at nights/rain due to blurred lights. This can lead to an accident. My regular dealerships in Milford, CT have confirmed the issue but have not been able to provide a fix. They say the vibration is normal. I do not believe so as the passenger side mirror never has issue like that. I have first noticed this issue a couple of weeks when driving on highway. Posting my case online has gotten at least 4 people reporting the same issue with the Sienna since 2021 in multiple trims. I am the original owner of a 2024 Toyota Sienna Hybrid (VIN: [XXX]). The vehicle has required two significant engine repairs within the first 57,000 miles of ownership. Both repairs were performed at authorized Toyota dealerships under warranty. INCIDENT 1 — ENGINE REPLACEMENT (10,000 MILES): At approximately 10,000 miles, the engine developed severe knocking. The dealership determined the engine was defective and performed a complete engine replacement under warranty. INCIDENT 2 — OIL LEAK (57,000 MILES): At approximately 57,000 miles, the engine developed an oil leak between the oil pan and the timing belt cover. The dealership repaired the leak under warranty. The dealership informed me that this repair carries only a one-year warranty on the work performed. The fact that the engine required complete replacement at only 10,000 miles indicates a manufacturing defect. The second engine issue at 57,000 miles suggests a pattern of powertrain reliability problems. I contacted the Toyota Brand Engagement Center (Case #XXX) and spoke with both an advocate and a supervisor. Toyota declined to offer any extended powertrain coverage and would only agree to review future issues on a "case-by-case basis." I am concerned about the safety and long-term reliability of this vehicle's powertrain given the recurring nature of these engine failures. A complete engine failure at 10,000 miles on a brand-new vehicle is not normal wear and tear — it is a defect. I am reporting this so that NHTSA is aware of potential systemic engine issues with the 2024 Toyota Sienna Hybrid. The contact owns a 2024 Toyota Sienna. The contact stated that upon starting the vehicle while parked in the garage, the low tire message was displayed. The contact stated that the tire was reinflated. The vehicle was taken to the local dealer and the contact was informed that the issue might be weather-related. There was no repair performed. The manufacturer was contacted, but no assistance was provided. The failure mileage was approximately 17,826.

Incident Description (Firestone Destination LE3 – P235/65R17) I purchased four new Firestone Destination LE3 tires (size P235/65R17, DOT IW2 8ILE31) and within the first week three of them developed noticeable bulges in the sidewalls. I returned to the tire service center, and they replaced the three tires due to the bulging. The following week, new bulges appeared again in the sidewalls on multiple replacement tires, for a total of six affected tires. Sidewall bulging creates a risk of sudden tire failure and loss of vehicle control, and I do not feel the tires are safe to drive on. One service center stated this was “normal from driving on roads,” but I disagree due to the number of tires affected in such a short time. The condition has been repeated and confirmed by multiple service centers. The tires remain available for inspection upon request. There were no warning lamps or messages before the bulges appeared. The issue began immediately after installation of the first set, and the problem persisted with the replacement set. In my assessment, the repeated sidewall bulging indicates a potential structural or manufacturing defect.

Keeps bottoming out suspension is specification is stated at 5.6 inches and I measured 3.3/4 unloaded never get a response on my complaints from my last repair and resolution on what they admitted that suspension was too low and wouldn’t put it in writing only saying that vantage mobility saying that it’s height requirements are as stated by vantage but no mention of ground clearance that I wanted them to verify that it’s specifications are stated at 6.5 inches and i measured 3 3/4 empty then they tell me not to go by specs and I’m a retired from wright Patterson Air Force base in the engineering department with logistics command and I’m like really guys you have to to have a specifications for a engineered system and they can’t even call back after they admit you have to go by specs! And then the authorized repair specialist tells me i had a bad brace on the front tire that was welded wrong and didn’t even mention it on the invoice or reported it to vantage mobility to call me about anything I had concerns about ?

There are no messages/warnings. The vehicle will surge at very low speeds such as when I am coming to a stop, trying to avoid a person in a parking lot etc. It was SO scary this evening as I accelerated into a very very busy intersection...I pressed on my brakes harder and it was instant - but about 1 second (which is a LOT in this scenario) later it then slowed instead of accelerated. I have owned the vehicle almost a year. I will be bringing in for inspection this week and let the dealer know. I have put on about 11K in miles and would say this has happened a dozen times thus far with this evening being the scariest one yet. I figured it was just something normal for a hybrid but as I researched online - many people have this issue. TOO SCARY for me and dont want to hurt anyone or myself.

Van purchased new October 2024. On two separate occasions, one in late 2024 and next today, [XXX], when pulling into street from a stop, there is no throttle response. You push the accelerator at varying pressure and no response for 2-3 seconds. This is a severe safety issue as it could cause an accident with oncoming traffic. No error lights illuminated on dash. My child [XXX] ) engaged the automatic locking retractor (ALR) and as it wound back in it wrapped around his neck. By the time we got to him it was tight enough that it was difficult to move or rotate or slide the seatbelt over his head, and we ultimately cut the seatbelt to free him. In addition to the actual safety incident, I was very surprised to learn that this was even possible. I had never heard of this safety issue, and had absolutely no game plan or awareness about this issue. I saw the SBIR program addressing innovation in ALR systems which is exciting! However I would also strongly encourage some sort of awareness campaign or warning messaging to raise awareness and ensure parents keep belt cutters in their cars. Rear Tail Lights Failed... Brake and Signals work. This was repaired through the dealership. 4 connectors and housing replaced invoice shows part 909890-10942. I am writing to express my concern regarding repeated tail light failures on my black Toyota Sienna that appear to be caused by wiring connection issues. This defect creates a serious safety hazard for my family, other motorists, and pedestrians, particularly in rural areas where there is little to no street lighting. I live in the country, where many roads are unlit and visibility at night is already limited. Because my vehicle is black, it blends into the darkness when the tail lights fail. Drivers approaching from behind may not realize my vehicle is on the road until it is too late to stop safely, significantly increasing the risk of rear-end collisions. This issue is especially concerning because I frequently transport children in my vehicle. Their safety depends on other drivers being able to clearly see my vehicle at night. A tail light failure caused by a wiring defect could place them at unnecessary risk of serious injury or death if another vehicle is unable to see us in time. The defect also puts other drivers at risk. Motorists traveling at highway speeds on dark rural roads rely on tail lights to judge the presence, distance, and speed of vehicles ahead. Without functioning tail lights, they may have little or no warning that another vehicle is in their lane, particularly around curves, hills, or during poor weather conditions. This appears to be more than a routine bulb replacement issue. If the root cause is an unreliable wiring connection, the lights may work intermittently or fail unexpectedly, making it difficult for owners to recognize the problem before driving. An intermittent electrical failure is especially dangerous because drivers may believe their lights are operating normally when they are not. Started a case with Toyota 260715001670

We had a complete tire blowout of the rear driver tire in the desert, the temperature was 108° ambient. Toyota did not mention to us nor did the dealership that the car does not have a spare tire. We were 50 miles from any service Including cell service, I had to walk a mile to find cell service meanwhile two of our kids experience heat exhaustion. It took four hours to get the Vehicle towed. In that time our children experience heat exhaustion and had to be taken to the hospital. This is a huge oversight part to not make the customer aware that the vehicle especially family vehicle does not have safety equipment like a spare tire. In the desert this is a life and death issue that was not addressed by the dealership nor Toyota. All that is given is an inflation kit. We were not told by the dealership that a card did not come with What turns out to be essential equipment in the middle of a desert.
