The interior door handle on my vehicle broke, and this creates a safety risk because occupants — including passengers, especially children in rear seats who may not know an alternate way out — may be unable to open the door and exit from inside. This is particularly dangerous in an emergency such as a collision, fire, or submersion, or in a situation where a passenger needs to exit quickly for personal safety, such as during an abduction or assault. This defect could also delay first responders during an extraction. The door broke again immediately after picking up the vehicle.

Unable to open my driver's door from the inside. In order to get out of the vehicle, I need to put the driver's window down and open the door using the outside door handle. I took it to the dealership on July 29, 2026 for a warranty repair. I was informed that this is a known issue and there are currently 1,600 door handle mechanisms on a national back order. Unknown when the part will come in. This could be a very serious safety issue if there is an accident and a driver is unable to open the door to get out, especially in the situation in which getting out of the car is a life or death situation, such as a vehicle fire.

I am reporting a severe safety hazard on my 2025 Toyota Tacoma caused by incorrect alignment specs and an uncorrected extreme toe setting. On a recent service visit, alignment inspection showed a Total Toe of +1.62 degrees (OEM target is approximately +0.10 degrees). The alignment machine database loaded incorrect tolerance specs (+1.37 to +1.87 degrees displayed as acceptable green range), leading technicians to leave the extreme toe-in completely unadjusted. In less than 2,000 miles of normal driving since a prior inspection, both front tires suffered extreme, rapid toe wear down to the internal cord layer. Operating a vehicle with +1.62 degrees total toe causes severe lateral tire drag and creates an imminent threat of a catastrophic tire blowout at highway speeds, presenting a significant vehicle safety defect.

My father and I both purchased a 2025 Toyota Tacoma SR5 2WD double cab last year brand new from different dealerships, both were in FL. My father was driving his new Tacoma with adaptive cruise control engaged, this was on an interstate, and the vehicle in front of him moved to the left lane next to him and started braking and this caused his vehicle to start braking extremely hard with no vehicles in front of him (he was driving about 55mph when this happened). He said this has happened to him before. I was in the vehicle with him when this happened. When I was driving from his home in GA a few days later, back to my home in FL, I was on the interstate ([XXX]) with the adaptive cruise control engaged and the exact same thing happened with my truck. A semi truck in front of me moved into the lane to the right and started braking. This caused my vehicle to start braking extremely hard with no vehicles in front of me. Had a vehicle been behind me, this could have caused me to be rear-ended going about 70mph. The sudden automatic braking with NO vehicles in front of you is aggressive enough to cause a rear-end collision and possibly cause the driver to make a startled, impulsive, corrective mistake to the sudden aggressive braking. Problem has not been confirmed or reproduced. Neither my father's or my vehicle has been inspected for this issue. Im betting its a software issue. My 2025 Toyota Tacoma engaged the emergency braking/pre-collision system system while my vehicle was going under 10 mph on a steep road in winter conditions. My vehicle was completely under control before the system engaged and there was no risk of a collision with any vehicles in front of me. The emergency braking system engaged and rendered my vehicle completely out of control. I tried desperately to pump the brakes in hopes I could regain control of the vehicle, but the brakes were fully locked. Had the brakes dis-engaged, I could have easily avoided the collision. The car not only did not decelerate, it accelerated down the hill while the brakes were locked for several seconds. My vehicle eventually slid into another vehicle that had been parked on the side of the road. From doing extensive research I believe a perfect storm of events caused this malfunction. Previously on this day, several inches of snow had fallen. The road had not been plowed and cars were parked on the side of the road. There was also a curve in the road near my vehicle's collision where the other vehicle was parked. My windshield was also still defrosting in the uppermost section near the rear view mirror. It is also possible the radar block on the front of the vehicle had snow or ice on it. It was also actively snowing. From what I have read, any of the above factors can cause a malfunction in the emergency braking pre-collision system. I had never been in an accident in over 20 years of driving and my vehicle did the least safe maneuver it could have in the given situation. There is no circumstance where one would purposefully slam the brakes on a snowy/icy down hill road if they know how to drive in the snow (even if you were going under 10mph as I was). At the most basic level of failure, I can't imagine this system should ever engage if going as slow as I was. Also, the system is designed to decelerate (was accelerating) and I was pumping brakes in effort to disengage

The contact owns a 2025 Toyota Tacoma. The contact stated that while driving at approximately 10 MPH, the rear driver's and passenger’s side rotors and brake pads suddenly detached. No warning lights were illuminated. Upon further inspection, the contact became aware of metal debris on the ground by the rear wheels. The vehicle was scheduled to be towed to a local dealer for further investigation. The vehicle was not diagnosed or repaired. In addition, the contact related the failure to NHTSA Campaign: 25V058000 (Service Brakes, Hydraulic); however, the VIN was not included. The manufacturer was not made aware of the failure. The failure mileage was approximately 18,000.

The driver's side window switch panel failed and caught on fire. It is available for inspection.It will be towed to a toyota dealership tomorrow. Nobody was in the vehicle when the fire started. The problem has not been confirmed or reproduced by a dealer. The vehicle has not been inspected, is going in for inspection tomorrow. There was no warning or indication prior to the fire starting.

My 2025 Toyota Tacoma TRD Pro (VIN [XXX] , purchased from Delaney Toyota, DuBois PA) exhibited a recurring Smart Key and door lock malfunction beginning in late October/early November 2025 and continuing through the date the vehicle was traded in February 2026 with the defect unresolved. Symptoms observed: Doors locking autonomously on exit, sometimes before occupants could exit the vehicle System falsely detecting the key fob as being inside the vehicle when it was in the owner's pocket outside, causing the truck to refuse to lock Touch-lock refusal at the door handle Random unprompted beeps while parked and locked with no corresponding lock movement Key card unable to lock the vehicle Dealership witness: During a January 2026 service visit, Delaney Toyota Service Advisor Ricky Carlson was personally locked inside the vehicle by this malfunction before he could exit. This was witnessed firsthand by a trained Toyota service employee and is part of the service history, though it was not captured in the official repair order. The contact stated that while driving in the rain, when passing another vehicle, the vehicle began shuddering and experienced a loss of power. The check engine light illuminated and multiple other warning indicators. The vehicle pulled over to the vehicle was stopped and eventually restarted. The vehicle was taken to a dealer were it was diagnosed with an air intake failure. Shortly after, the vehicle experienced the same issue. Again and was returned to a dealer. The dealer diagnosed with a turbo mud and water found inside the air intake, water is covering the filter and there was no additional repairs that could be completed so the vehicle. The manufacturer was not notified of the issue. The approximate failure mileage was 19,600.

The door handle mechanism which is made of plastic broke on the inside of the door. It was the piece the cable connects to which pulls to release the door latch for opening. Only way to get out of door is to roll window down and open from outside the vehicle. Truck has 7k miles currently and no heavy use. Under proper circumstances this could be very dangerous.

This complaint is about the rear passenger side window on the vehicle. It is available for inspection upon request. This was a huge safety risk for me today. I had the rear passenger side door open (with the window rolled all the way up) gathering my things from the back seat. I closed the door with a normal amount of force and the window completely shattered causing me to be covered in glass shards. Luckily, no one was sitting in the back seat and at least I didn't get glass in my eyes. However, this is quite concerning. I don't believe the glass was under any stress or pressure. The temperature was 78 degrees and cloudy outside today, which doesn't seem extreme to me. The window was in perfect condition as I just bought the truck 2 months ago. After the window broke, I went directly to the dealership I bought it from, where they inspected it, and taped a trash bag over it. They told me there was a recall on windows for the new Toyota Tundra models, however, not yet the Tacoma. I believe this is a potential danger to many people and it should investigated.

We rented a vehicle consisting of the identified Toyota Tacoma with an installed cab-over camper (Travel Lite 550U-XS, VIN: [XXX]). While driving this rental vehicle on a two-lane highway with our family, including two young children (ages [XXX] and [XXX]), a fire ignited in the vehicle's built-in propane/gas stove system. We were not alerted to the fire by any onboard safety equipment, though the camper was equipped with a smoke alarm. We were alerted only by a passing motorist. The vehicle's smoke alarm did not activate until several minutes after the fire had already been extinguished by us using the vehicle's fire extinguisher, and after the smoke inside the camper had already dissipated. The smoke alarm sustained visible smoke damage and its exterior housing was melted, which we believe indicates the alarm was exposed to significant heat/smoke without triggering, or triggered only very late in that exposure. We believe this represents a serious safety equipment failure: a smoke/fire detection system that did not provide any early warning during an active fire, in a small space. This delay in detection could have resulted in serious injury or death. Additionally, we received no safety briefing regarding the hazards of the propane system at the time of vehicle handover. The vehicle overview video provided to us contained no safety information regarding the gas system, and we later discovered it depicted a different vehicle unit than the one we were actually provided, raising questions about whether this vehicle's specific gas/stove configuration and safety equipment had been properly reviewed or tested prior to rental. We made a best guess at cause in speaking with the responder fire department, but we cannot know for sure. To be clear, there was no issue with the Toyota Tacoma itself. We are submitting this complaint to ensure NHTSA is aware of this potential safety defect in the smoke alarm system and/or propane stove. Vehicle Cannot Be Safely Driven – Structural / Suspension Defect Suspected – Dealer Claims “Repair Completed” Despite Unequal Wheelbase My vehicle is currently unsafe to operate, due to the following critical issues: ⸻ 1. Left–right wheelbase inconsistency The front and rear wheelbase measurements are not equal side-to-side. This is a strong indicator of structural deviation, such as: •Subframe misalignment •Control arm mounting point displacement •Deformed suspension components •Manufacturing or assembly error in chassis geometry ⸻ 2. Front caster angles remain abnormal •Left caster: 3.9° •Right caster: 4.9° •Cross-caster difference: 1.0° A 1° difference exceeds manufacturer tolerances and causes: •Vehicle drifting •Poor straight-line stability •Uneven steering return •Potential instability during emergency braking This is a safety-related steering geometry defect. ⸻ 3. Real-world driving confirms unsafe behavior The vehicle cannot maintain straight-line travel, the steering wheel is off-center, and the car exhibits instability at normal speeds. This makes the vehicle unsafe to drive. ⸻ 4. Despite these unresolved issues, the dealership insists the vehicle is “repaired” and demands that I pick it up. They refused to provide frame measurement reports or explanation for the unequal wheelbase. Returning a vehicle with unresolved steering and chassis geometry defects constitutes unsafe vehicle delivery. ⸻ Conclusion Based on unequal wheelbase, abnormal caster geometry, and inability to drive safely, this vehicle likely contains a structural or assembly defect. I formally request investigation, suspension geometry verification, and required action from the manufacturer, as this is a matter of public road safety.

On several occasions in the previous two months, I have been driving during heavy rain. On the first occasion, while high speed wipers were on, they suddenly stopped working. I slowed down, but was able to get them going again prior to pulling to the shoulder of the highway. I had to switch to the slow wiper speed to get them going. I switched back to the high speed wipers due to heavy rain, and kept stopping each time. It continued this cycle and stayed on after about 15 attempts. Today, on [XXX], I drove down [XXX] from Flint, MI towards Detroit at approximately [XXX]. The wipers again quit working and I had to switch the wiper speed to get it working again while at full highway speed during a deluge of rain and nearly had to stop in a construction zone with no shoulder and heavy traffic, nearly causing a collision until I was able to get the wipers to begin working again. On a previous occasion, I turned them on. When the rain stopped, I was unable to stop the wipers, but was eventually able to stop them by carefully moving the intermittent delay wheel to a position between on and off. It appeared that there may be a short in the wiper circuit. This was a definite safety issue in that my attention was taken away from driving to try to fix the wiper situation. In speaking with my service adviser at Randy Wise Toyota in Grand Blanc, MI, he went out to their lot and tried to duplicate the problem, which he confirmed happened on that same Toyota Tacoma model. I attempted to get this repaired under warranty. Their response was that there was nothing they could do since it appeared to be in the design of the wiper system. The contact owned a 2025 Toyota Tacoma. The contact stated that while driving approximately 75 MPH, a Semi Truck attempting to merge onto the highway crashed into the passenger side of the vehicle, causing the driver to lose control of the steering and cross three lanes of traffic before crashing into the concrete median. During the crash, no air bags in the vehicle deployed, the collision avoidance systems did not operate, and twelve error messages were activated, including a brake error, a Four Wheel Drive error, and an ABS error. The driver sustained a concussion from the head contacting the steering wheel and lacerations from shards of glass entering the vehicle during the crash. The front seat passenger sustained lacerations to the legs and arms, the rear driver side passenger sustained fractured ribs and lacerations to the arms leg, and face. The rear passenger side occupant sustained lacerations to the arms and legs. All occupants in the vehicle were transported to the hospital with injuries, along with the semi-truck driver. No further information was available. A Police report was taken, and the vehicle was towed away. The manufacturer and local dealer were notified of the incident. A representative from the manufacturer was sent to inspect the vehicle and deemed no failure had occurred. The vehicle was destroyed. The failure mileage was 4,000.
