The vehicle’s radiator developed a coolant leak that was discovered and confirmed by the dealership during a routine oil change, tire rotation and state inspection. There were no warning lights, warning messages prior to the dealership discovering the leak. The radiator required replacement. The dealership also told me that they had the replacement radiator on hand because this type of radiator failure had been occurring fairly often with this type of vehicle. The radiator failure presented a potential safety concern because a coolant leak can worsen without warning and result in a sudden loss of coolant, engine overheating, or vehicle breakdown while driving. Because there were no warning lamps or other symptoms before the leak was discovered, we may not have had any indication that the cooling system was failing. The problem was confirmed by the dealership during inspection, and the dealership determined that the radiator was leaking and needed to be replaced. The vehicle/component was inspected by the dealership. No manufacturer, police, or insurance inspection has occurred, to my knowledge. There were no warning lamps, messages, overheating, or other noticeable symptoms prior to the dealership finding the leak. The vehicle is only three years old, with 39000 miles on it at the time of the failure. I am reporting this because a radiator failure at such a young vehicle age, particularly one that develops without any warning to the driver, could potentially create a safety hazard if the coolant loss becomes severe while the vehicle is being operated. I’ve seen many complaints in online forums that this is an ongoing issue with these radiators used by Chevy. It seems the replacement radiators are having the same issues. The vehicle was towed on a flatbed to the local dealer. The vehicle was not diagnosed or repaired. The dealer provided an estimate for the transmission replacement. The contact owns a 2023 Chevrolet Blazer. The contact stated that while driving at various speeds, the vehicle hesitated while depressing the accelerator pedal. There was no warning light illuminated. The vehicle was taken to the dealer on several occasions to be repaired; however, the dealer was unable to duplicate the failure or determine the cause of the failure. The contact stated that the dealer performed a transmission fluid flush; however, the failure persisted. The manufacturer was notified of the failure but offered no assistance. The vehicle was not repaired. The failure mileage was approximately 25,000.

Driver assistance

I am reporting a recurring safety issue with the Forward Collision Alert/Automatic Emergency Braking system on my 2023 Chevrolet Blazer 2LT. The vehicle frequently activates the Forward Collision Alert when there is no vehicle, pedestrian, or other obstruction in front of me. The red collision warning will illuminate and the system will beep even when the roadway is clear. More concerning, on three separate occasions the vehicle has unexpectedly applied the brakes extremely hard while I was driving in traffic despite there being no obstacle or imminent collision. The most recent incident occurred on August 11, 2026. I was making a turn through a busy intersection when the Automatic Emergency Braking system suddenly activated and applied the brakes extremely hard. There was no vehicle, pedestrian, or obstruction in my path that would have required emergency braking. The sudden braking occurred while I was in the intersection and created a serious risk of being struck by surrounding traffic. The braking force was severe enough that my seat belt restrained me very hard and caused an injury to my chest and flew me forward into the steering wheel while also risking the vehicle behind me to almost slam into me. This was not an isolated occurrence. This is the third time the vehicle has unexpectedly applied the brakes hard while I was driving in traffic. In addition to these three braking incidents, I have experienced frequent false Forward Collision Alerts where the red warning and audible alert activate when nothing is in front of the vehicle. I am concerned that the Forward Collision Alert/Automatic Emergency Braking system is malfunctioning and could cause a collision by unexpectedly stopping the vehicle in moving traffic. I am requesting that this recurring safety issue be documented and investigated.

Upon driving off, the vehicle then jerked and jolted. The vehicle returned to normal functionality, but the failure became a regular occurrence. No warning lights were illuminated. The vehicle was taken to the dealer, who informed the contact that the vehicle functioned as intended during the time in the Service Center. The vehicle was not repaired. Brakes

Rear brake automatic parking brake motor locked rear wheels. Dealership diagnosed and repaired at my cost. Service parking brake light illuminated and rear brakes locked not allowing vehicle to be moved. After a few restarts brakes released.

The contact stated that while his father was driving at approximately 40 MPH straight forward, the brake pedal was depressed, and it traveled to the floorboard. Then, the brake pedal was lifted, and the brake pedal traveled a second time to the floorboard. The vehicle did not slow down as intended, and the vehicle crashed into the rear of another vehicle. The air bag did not deploy, and no warning lights illuminated. The contact's father sustained a facial bruise. Medical attention was not needed. A police report was filed. The vehicle was towed to a lot where it was pending an inspection from the insurance company. The manufacturer was not notified of the failure. The failure mileage was 34,000.

Odometer Fraud. The contact purchased a 2023 Chevrolet Blazer. It was discovered that there was a mileage discrepancy. The vehicle was a dealer sale. At the time of the purchase the vehicle mileage was 57,692. It was discovered using the Kelley Blue Book/Experian that in 2022 vehicle was listed for sale with mileage of 57,705, and in 2023 same vehicle was listed with 23 miles.
"I am the owner of a 2023 Chevrolet Blazer RS. The digital odometer and speedometer screen intermittently crashes and goes completely black for 15 to 20 seconds while the vehicle is in motion on public roads. This creates an immediate safety hazard as I am left with zero knowledge of my current speed. General Motors has acknowledged this specific software stability bug under Technical Service Bulletin 22-NA-023 but refuses to fix the issue due to the vehicle being 9 days past its 3-year calendar warranty, despite having only 32,400 miles." I contacted your GM agent @ 800-222-1020 and was told its out of warranty sorry, that is horrible service espeacially for a safety issue and a 55,000 vechicle that only has 32,400 miles

Steering

The contact owns a 2023 Chevrolet Blazer. The contact stated that while driving, a code appeared on the dashboard P0128. When the vehicle's engine was switched off, the engine cooling fan continued to run. Warning light stating the steering would be disable.; however power steering was not lost. The vehicle was taken to a dealer where it was diagnosed with a thermostat failure. The manufacturer was notified of the failure. The approximate failure mileage was 48,000
Re: Vehicle Safety Defect Complaint – 2023 Chevrolet Blazer To Whom It May Concern, I am submitting a formal safety defect complaint regarding my 2023 Chevrolet Blazer. This vehicle has exhibited multiple system failures that present a serious risk to driver safety and public roadway safety. The vehicle was purchased new on July 20, 2023. Beginning on April 21, 2025, at approximately 27,610 miles, the vehicle developed a progressive defect initially presenting as oil-related issues. Despite multiple repair attempts, the condition worsened and required a full engine replacement in January 2026. Following the engine replacement, the vehicle continued to experience critical safety failures, including engine stalling, reduced engine power warnings, and electronic stability control (ESC) alerts. On April 26, 2026, the vehicle experienced a driver assistance system malfunction in which it attempted to steer off the roadway. I was forced to manually disable the system to maintain control of the vehicle. On April 27, 2026, the vehicle exhibited a fuel system failure, including fuel leakage and fuel odor, along with multiple simultaneous warning indicators: • Reduced Engine Power • Service ESC • Steering Assist Reduced / Drive with Care • Power assist driver system disabled This combination of failures presents both a fire hazard and a loss-of-control risk. Additionally, I was informed by the dealership that a high-pressure fuel line, a one-time-use component, was not replaced during the engine installation. This raises concerns regarding improper repair and continued unsafe operation of the vehicle. The vehicle has undergone multiple repair attempts and a major powertrain replacement, yet continues to present serious safety concerns. Due to these ongoing issues, I do not feel safe operating this vehicle. I respectfully request that NHTSA review this matter for potential safety defects that may impact other vehicles of the same make and model.

Exterior lighting

My bright lights do not go off when oncoming vehicles or vehicles in front of me are close and when they do dim they go on and off. People flash their brights at me causing me not to be able to sleep and this happens even when they turned off of auto. Another item of concern is that the car would lock and unlock and the interior lights would go on and off. This was fixed but it was a line under the front that was not soldered together at the manufacturer is what the Chevy dealership stated.

Engine light on caused by emission system failure. Sensor unable to be changed until recall. Remote start unavailable while engine light is on. Had issue. Looked at four times at dealership. No correction taken.

The contact stated while driving at an undisclosed speed, the Lane Keep Assist feature steered the vehicle to the left while the vehicle was driving inside the lane. The Lane Keep Assist warning light was illuminated. Additionally, the vehicle also experienced a loss of power steering assist. The contact stated that the failure occurred after the vehicle drove over a pothole. The contact also stated that the dealer had previously replaced the front-end bracket after another vehicle struck the front end while the vehicle was parked and unoccupied in a parking lot. The vehicle was taken to a second dealer, Escude Chevrolet of Easley (5010 Old Easley Bridge Rd, Easley, SC 29642), but the cause of the failure could not be determined. The contact was informed that the tires were the incorrect size for the vehicle. The vehicle was not repaired. The manufacturer was not made aware of the failure.
