Owner-reported complaints naming the driver-assistance system, filed with NHTSA
Data refreshed
Reports naming forward-collision avoidance, lane keeping, back-over prevention, cruise control and related electronic driver-assistance systems. Every record below is an owner-reported complaint about the 2019 Chrysler Pacifica submitted to NHTSA — not a confirmed defect, not a recall and not a manufacturer bulletin. 30 reports on file name it.
Complaints are reports submitted by owners and drivers to NHTSA. They are not verified and do not establish that a defect exists.

When trying making a sudden stop at a red light the or to stop the van it will not stop it keeps going
Rear back up camera intermittent with blank screen and occasional message on screen “camera not available” when in reverse. Automatic backup anti colision braking not functioning when camera not showing rear picture When signaling to change lanes, audio and mirror lighted warning signal not warning adjacent vehicle in danger zone NTSB web site list July 2026 recall for 16 vehicles in model years including mine. My VIN showed no open safety recalls and Chrysler dealer does not have recall authorization to address issue
For a few weeks prior our windshield wipers would start and were unable to be switched off. Then would just randomly stop. Then we got completely stopped on a major road during rush-hour with my whole family in the car. The car moved itself to park at a stop light. When we were stopped, everything in the car was going haywire. FCW unavail, ABS unavail, check engine light, wipers going, couldn't use turn signals or hazard lights. I turned the car on/off multiple times to get it to work. We were stuck in the middle of a busy road above a highway with cars exiting during rush hour. Cars moving fast around me with my whole family in the car. Could have caused major accident. Chrysler dealer could not help right away. Took to own mechanic. Recreated issue, replaced Aux Battery (which should NOT have all safety features tied to it), and re-flashed 3 modules that were ruined from this. Thought this was fixed but next time out, same things started happening, but now says to service airbags as well. I will not have my family in the car with an airbag malfunction. Now saying 2-4 thousand to fix, more modules fried and ONLY the beginning. Car has only been inspected by dealer to pull codes, then mechanic. Like I said the windshield wipers were going and couldn't be controlled but no other warning lights. This did not indicate a battery failure and EVERY safety feature to be turned off. This is not designed/wired correctly and I need Chrysler to take action on this situation. My whole family could have been seriously injured and any other Chrysler with this design needs to be fixed or taken off the road.
The vehicle intermittently displays a "Stop Safely – Vehicle Will Shut Off Soon" warning message accompanied by a turtle icon while driving, most often at highway speeds above approximately 50 mph. There is no consistent pattern or condition that triggers the warning. When the warning appears, I must immediately attempt to pull off the roadway because the vehicle indicates it will shut down. This creates a significant safety hazard, especially on interstate highways or roads without a safe shoulder. If the vehicle is turned off and restarted, the warning message disappears, but the check engine light remains illuminated. After the next ignition cycle, the check engine light also disappears, leaving no visible indication of the problem. The issue began occurring approximately once per year but has become progressively more frequent. It now occurs almost daily. During a recent 300-mile trip, the warning appeared four separate times. It also commonly occurs twice during a 60-mile drive. The increasing frequency has made the vehicle unreliable and unsafe to operate on highways. The vehicle has been taken to an authorized Chrysler dealership. However, the dealer stated they cannot diagnose or repair the problem unless the warning is actively displayed when the vehicle arrives. Because the warning clears after restarting the vehicle, the dealer has been unable or unwilling to identify the cause or perform a repair. I also contacted Chrysler customer service. I was informed that because there is no recall related to this issue, they could only refer me back to the dealership for diagnosis under any applicable warranty coverage. This defect poses a serious safety risk because the vehicle may lose propulsion while traveling at highway speeds, requiring an emergency maneuver to reach the shoulder while surrounded by fast-moving traffic. I am concerned that this could result in a rear-end collision or other serious crash if the warning occurs where there is no safe place
This has been ever since I got my van. Sometime when I drive my lights come on and it goes into limp mode. Most of the time. When I start my van my lights will turn on. I have to turn my car off . Then start again to get my van riding correctly. The other day I was making a U-turn and my engine stalled and I was almost hit by a truck.
The vehicle jerked, stopped and would not start again. After a while it started, people were honking to move. This happened about 3 years ago. The vehicle jerks often and afraid it will leave us on the street. About 2 years ago, the auxiliary battery ran down the main battery. The car would not start. Had to buy both batteries. The lights come on, engine, traction and auxiliary battery light. This happens often on a weekly basis.
During normal driving from office to home car suddenly started stuttering and I was apply break but speeded up and showed engine light on, I was about to hit front car due to additional speed. During that time I saw few quick messages on the dashboard, the one that I can remember was related with ACC. That's the dangerous situation. I looked d from OBD and found that code was U0412 and reported that code to Huntington Beach Chrysler service station. Please keep an eye out for some similarities between others reports.
Component failute: first the cruse control would stop working while driving. then the engine would rive way up while trying to accelerate and not gain much speed, gas mpg dropped. when breaking to stop i had to push the break pedal harder. when the cruse had stopped and i shut the engine off, it was hard to start, and would bump and shake as if it was hitting on 2 cylinders, cruse still would not work. after i made it home, the next time i started it, it was ok, then the last time i parked it the brake will not depress and it will not start. battery has 12 volts, jumped it anyway break still want depress. took battery cables off shorted them together , did not help. break pedal still will not depress.
My brake starts hissing, and starts pushing back against my leg. It happens much when I push brake padel then it hissing sound and brake padel pushes back, the vehicle starts crawling. I got messege blind spot monitor unavailable. Engine hood paint fall off, easy visible paint fall off or peeling paint. This one I brought used car from ALM Gwinnett
1. Brakes give way and cannot stop car until halfway into intersection. Brake is metal to metal. Hear clanking sound………. Also 2. When stepping on brake, car speeds up. I’ve had 2 accidents by rear ending car in front of me. This has happened 4-5 times this past year.
Shifter service keeps coming on. Won’t move out of park. Warning lights come on for service engine, fwc, have been brought to dealer 2 times already. They say multiple vans have been brought in for same issue and don’t know how to fix. They are saying they got it and next morning same issue. Photos attached. Plus a bill for $1500 and still not fixed.
Radio Uconnect resets every 3 to 5 minutes. When this happens screen goes black. If backing no warning on what is behind you. GPS is resetting with wrong location. Wireless calls stop. This has been a problem across all FCA brands and an issue since 2017. I had 2018 Pacifica with same issues. I believe there are two class action lawsuits over this problem. I had to pay $650 to replace radio. No help from Chrysler customer cares NO HELP.
The contact owns a 2019 Chrysler Pacifica. The contact stated that the back-over camera image remained on display while driving. There were no warning lights illuminated. The contact called the local dealer and was informed that the vehicle needed to be diagnosed. The vehicle was not diagnosed or repaired. The manufacturer was not contacted. The contact stated that the failure was related to NHTSA Campaign Number: 20V191000 (Back Over Prevention). The failure mileage was approximately 48,000.
The dashboard screen automatically shuts down and restart every 3 minutes. Stop/start light stays on.
The entire radio/ backup camera freezes and will not come back on for minutes at a time. The dealer is aware of the problem and replaced the entire system once already. They were not able to reproduce the issue however I have video and pictures of a few incidents. There were no warnings however it almost always happens when first starting the car and so the backup camera becomes unusable along with all temperature controls and radio.
The Uconnect 4 infotainment system is consistently rebooting every 2-3 minutes, leaving several safety measures for the vehicle from being used during those times. This leaves risk that the safety feature is not usable when needed for example rear warning for small child behind vehicle is not operable. As well the Navigation system is not working appropriately and has the vehicle always located at a street that I am no where near. This leaves risk that it will be a distraction to driver to locate their whereabouts in unfamiliar areas. Also not able to use hands free system for mobile device when the system is continuously rebooting as such and therefore would not be able to make call for emergency assistance when needed. FCA US LLC has been provided many complaints about this yet continues to ignore that this is a wide spread issue and is nationwide. Therefore they fail to extend the warranty related to the Uconnect 4 infotainment system and render thousands of people having to cover the cost of their faulty Uconnect product.
The vehicle has stalled as trying to continue after a stop at a light or stop sign. It will not travel except at 10 miles per hour or less while trying to cross the intersection. Other drivers are impatient and start honking. This has also happened while going down a busy street . It jerks when you start to go after stopping. Happened in July 2023 -- 2X August 2023 -- 2X September 2023 -- 2X
Vehicle has continually shifted roughly, but beginning last year it stalled while coming to a stop. We replaced the battery and removed the auxillary battery that controls the automatic start/stop and had no issues for almost a year. Approximately a week ago, the vehicle began to stall again when coming to a stop, and giving a message stating to "service shifter." It has happened multiple times since. There was a lawsuit for this model regarding the crankshaft position sensor, but we are only reimbursed if we have paid for repairs. Can you help?
The affected component is the backup camera and warning system. The malfunction happens in a loop around every 3 minutes. My safety and that of others is put at risk when the backup and rear collision warning fails to function and negates backward motion visibility to the driver by such failure. There are no warning lamps currently or any notification of any failure in the car. I have taken the car to the dealership and they have found no issues.
Steering wheel buttons began no working. Lights on steering wheel flicker when steering wheel turns. Randomly alerted that "adaptive cruise control is unavailable" both when the system is in use and when It is off. Forward collision warning system active breaking not working.
The vehicle is a 2019 Chrysler/Braun mobility van, my wife is a paraplegic that has no feeling from the chest down. The Uconnect system is the issue. The Blind Spot Alert does not work, the Power Sliding Doors do not work, the Windows will not operate, the Heat go to 76 degrees and will not turn off, the Info Center goes black, the Back Up Camera will not work, the Navigation system stops working, the Audio stops working, the Audio/Bluetooth will not work, the Panel Lights come on but do not work. We were on a trip when the Uconnect system malfunctioned and all the above happened. The temp outside was about 75 degrees and the heat came on and the windows would not work. My wife's foot got so severely burned it needed medical attention and is still dealing with this 3 months later. The vehicle became an oven. With the power sliding doors being inop my wife is either a prisoner in her own vehicle or she cannot enter the vehicle at all. With her limited mobility and the backup camera inop it makes it unsafe for to back the vehicle up safely. If we are not familiar with an area we are going to it makes it that much more difficult to find our destination without the Navigation system working.
While driving down the road, my vehicle stated that it was not in park out of no where, and i was driving 35 -40 miles per hour. No doors were open, it was me and my twin boys age 2 in car seats. It then decided to stop in the road. The vehicle shut off. I turned on my hazards and had to try to restart my vehicle. Almost getting in an accident. Got it restarted and pulled it over. I was terrified. This was the second time this has happened except the first time was a month prior and i was driving up to a red light, depressed the brake (start stop was deactivated prior by me) and once i stopped the car just shut off and didnt want to restart, car was placed in park. After about a minute i got it to restart, again i pulled over. The radio will turn off and restart every 3 minutes, with nothing connected. It is like this car has all of a sudden become possesed. I have replaced both the battery and aux battery.
The contact owns a 2019 Chrysler Pacifica. The contact stated while reversing at 1 MPH, the Back Over Prevention camera sensors failed to recognize a child behind the vehicle. The contact almost struck the child. There was no warning light illuminated. The vehicle was driven to the dealer but was not diagnosed or repaired. The manufacturer was informed of the failure. The approximate failure mileage was 52,000.
FCW is not working after new radio was put in took it to two dealers they both said that there are no error codes the fcw is turned on but no warnings or braking the rear system works fine I used three large boxes to test both front and rear
While traveling in the left hand lane I was hit on the right side pushing me into a median. I applied the brakes and they didn’t stop the vehicle. When the van came into contact with the curb. The airbags deployed. I smelt smoke and told my 4 young children in the back seat we needed to get out now. Two of my children were in 5 pt harness car seats in the third row. We were able to run to a safe area before it went up in flames less than a few mins later. There were no witnesses and no one around. My children and I could of been seriously injured or worse. No warning lights were active. No signs of failure until I tried pressing the brakes. The vehicle was inspected by the fire department that arrived on scene when it was fully engulfed in flames at that point.
Higher-selling and older vehicles accumulate more reports. Counts are not failure rates and are not directly comparable between vehicles that sold in very different numbers.
