Vehicle: 2022 Chrysler Pacifica Hybrid. Approximately 25,300 miles On Saturday 9/12/26 while driving at highway speeds the following message displayed "Stop Safely, Vehicle Will Shut Off Soon". When the message displayed the check engine light illuminated as well as the limp mode light. In limp mode the vehicle reduced speed rapidly and would not accelerate above 20 MPH. This happened at highway speeds on a multilane highway. We were able to get to the shoulder, but it was very dangerous changing lanes in limp mode. Turning the vehicle on and off several times cleared errors except the check engine light. We were able to get off the highway and get home without the message or limp mode triggering again. The check engine light stayed illuminated for 24 hours and went off when I refueled the vehicle the next day. For the next week I drove at local speeds with no issues. On Saturday 9/19/26 the vehicle would not start. This is a plugin hybrid and was fully charged. The electric motor would not start, and the check engine light illuminated again. I turned the vehicle off/on 3 times to get it to start. I did my errand and returned home. When I returned home, I turned the vehicle off then on again and the check engine light cleared. On Tuesday 9/21/26 I dropped the vehicle off for service, getting there without issue. The dealership stated the transmission will need to be rebuilt. Problem code is P0A43-00. This is the SECOND transmission failure since ownership. I've had the vehicle serviced only at the dealer where purchased. The dealership confirmed the FIRST incident with the same problem code was diagnosed on 1/30/24. The transmission was replaced at this time. The van had 12,134 miles. The 2024 incident also was triggered at highway speeds. If my memory is correct, I believe I reported the first incident to the nthsb at the time.
Vehicle suffered sudden loss of motive power due to Diagnostic Trouble Code P0A43 (Drive Motor A Position Sensor Circuit Intermittent). The failure is caused by thermal and chemical degradation of the internal transmission wiring harness, resulting in an internal electrical short submerged in transmission fluid. The vehicle is able to be powered on again and driving can be continued however certain speeds trigger the issue. The check engine light appears and remains for a short period. The vehicle was purchased through CarMax of Naperville, IL [XXX]. The vehicle began to experience the defined issue the last week of May 2026. The vehicle was brought to CarMax [XXX]. CarMax transferred the repair of the vehicle to Castle Chrysler of Naperville/Stellantis IL, and they conducted the initial repair. Stellantis attempted to mask this hardware failure via software recalibrations under NHTSA Recall 03A (23V-010) rather than replacing physical parts. The vehicle was returned and appeared to be repaired. [XXX] the same issue occurred while on a cross country road trip. The vehicle was able to be driven but under specific speed conditions as not to activate the transmission failure. [XXX] the vehicle was brought to an independent technician at Stephens Automotive in Palatine, IL and they were able to verify the issue and documented it. The vehicle was then brought to Castle Chrysler on [XXX], and the vehicle has been there since. Stellantis later acknowledged the true physical failure via Warranty Extension XE1 and TSB 21-014-26, requiring Electric Rebuild Kit #68729719AB. The vehicle has been continuously grounded at Castle Chrysler since [XXX] (>30 cumulative days) because Part #68729719AB is on indefinite backorder. Stellantis's failure to supply hardware repairs leaves the vehicle inoperable and creates a severe safety hazard on public roads. Stellantis Case #[XXX]. Engine

My vehicle is currently being recalled for the recall 52D, I have reached out twice in a thirty day span asking about a fix and been told both times there are no parts for this fix. The risk of this recall includes fire, while charging or while parked and off.
On July 19, 2026, I was driving my 2022 Chrysler Pacifica Hybrid on E-470 at approximately 75 mph. Without warning, the dashboard displayed "Stop Safely. Vehicle Will Shut Off Soon," and the amber check engine light illuminated. At the same time, the vehicle immediately lost all motor power, began slowing, and did not respond to accelerator input. There were six occupants in the vehicle, including four young children. I was traveling on a busy six-lane highway with little to no shoulder. I had to maneuver across traffic without motor power to reach the side of the road, placing all six occupants at immediate risk. After turning the vehicle off for about five minutes, I restarted it. Only the check engine light remained illuminated, and the vehicle appeared to have normal power. We continued driving at 55–65 mph to reach a safer location. About 10 minutes later, the same warning appeared and the vehicle again lost all motor power. This time we were on a steep, busy exit ramp and had to immediately pull off as the vehicle rapidly lost speed, again placing all six occupants at risk. After waiting about five minutes, we restarted the vehicle, drove it to a safe location, and had it towed to a dealership. The dealership was closed, so the cause of the failure has not yet been determined. This incident created an extremely dangerous situation, and we narrowly avoided a collision on the interstate. A sudden loss of propulsion at highway speeds without warning is a serious safety hazard that warrants investigation and consideration for a recall.

Recall to not charge my 2022 chrysler pacific plug in hybrid and there is no fix. Recall states not to charge or park the vehicle in or near any structures. That is unacceptable. They need to find a fix or buy my vehicle back.

Driver assistance

The contact owns a 2022 Chrysler Pacifica. The contact stated that while the vehicle was shifted into reverse(R), the back-over prevention camera intermittently failed to display an image of the rear of the vehicle. The back-over prevention camera failed to operate as needed and a bright green screen was displayed before the screen turned black. The vehicle was taken to the dealer, who was unable to determine if the radio software needed to be updated; however, the dealer was unsure if the update would fix the vehicle. The dealer informed the contact that the VIN was not included in NHTSA Campaign Number: 24V436000 (Back Over Prevention). The vehicle was not repaired. The manufacturer was not made aware of the failure. The failure mileage was 38,623.
Rearback up camera intermittently unavailable even after recall with software update. Sometimes is distorted and delayed, sometimes blue or black screen and says camera unavailable. Have to repeatedly reboot camera system.

Chrysler issued this safety recall 52D at the end of June and still no remedy is being reported as available from Chrysler. This is abysmal customer performance and and unacceptable turnaround time for what amounts to a serious safety risk. Chrysler has not and continues not to provide an estimated timelines for resolution. I am renting a vehicle in the meantime given the safety risk (both while driving and while parked), paying for the rental cost out of pocket and taking a risk that Chrysler will reimburse. In short, the manufacturer has failed or is unable to remedy this safety recall for this vehicle in a timely manner.

Interior & seats

The head rest of the second row broke and will not return to its upright position. It happened when we putting the seat in the stow position. Having the headrest sticking into your neck is very dangerous so we basically lost a seat in our vehicle. We can choose to remove the headrest, which is a danger for whiplash OR repair it. The repair is estimated over $300 which is ridiculous! This should be a covered repair under a recall as it seems to happen frequently.
The captains chairs in the second row of the vehicle both had their reclining mechanism fail. This causes the seats to move forward and back sliding when the car is in motion as they will not stay locked in place. They cannot be used for transportation due to it being unsafe for the passenger as the seat does not remain upright. The seat is available for inspection. The seats have not been inspected by the dealer/manufacturer as they are wanting to charge a large service fee and do not have appointments available in a timely fashion. There were no warnings, the seats each failed suddenly upon regular use.
Steering

**Steering rack failure** I bought this as a used vehicle from Calavan CDJR in [XXX] XXX with 89,219 miles [XXX] It now has approximately 117,000 miles on it. When I bought it, there was a suspension noise that appeared to be due to strut mounts. Due to this noise, dealer lowered the sale price. Since the time of purchase, I have replaced both struts, inner & outer tie rods, lower control arms, sway bar links and both cv axles. And it is still making the same noise, only it has gotten much worse. Based on the last mechanic's inspection, he states that the noise is from a bad steering rack. I reviewed this online and found that NHTSA has an open investigation for the steering rack and would like to see if there is anyway that you can help me with replacing it as it poses a severe safety hazard and is very expensive to replace. I was driving my Pacifica with my special needs children to thier therapy appointments. The power steering light came on all of a sudden going down the highway at 50-55mph. Steering became extremely difficult. I was barely able to take the the turn to pull into the parking lot. This is even more dangerous as their is a drop-off on the other side if i had not managed to turn aggressively enough. It scared both of my children and myself. The dealership wanted $206 diagnostic fee which we cannot afford. The light will go off and we regain steering once it has been off for a few minutes then light kicks on and we lose steering again.

I am filing a safety complaint regarding Chrysler Corporate's handling of recall campaign 06D (NHTSA 26V-189) for my 2022 Chrysler Pacifica.I received an official corporate notification via mail explicitly stating that parts were now available to remedy this active safety recall. However, when I immediately contacted my authorized Chrysler dealership to schedule the critical safety repair, I was turned away and informed that the parts are completely unavailable and backordered with no estimated release date.Sending out official notices claiming an open safety risk can be fixed when the necessary components are entirely missing from dealer inventory is a highly deceptive and unsafe practice. It causes vehicle owners to waste time coordinating service under false pretenses while forcing us to continue operating family vehicles with known, unaddressed safety hazards. Chrysler must be held accountable for coordinating its corporate communications with actual supply chain availability so owners are not driving compromised vehicles under the false impression that a fix is ready. It is alarming that the post card they just sent me had big letters "URGENT SAFETY RECALL" headline and the recall site says the parts are in and the dealership says they only gets parts to fix 2 a week and it will still be close to 6 months before the parts are available.

Brakes

1. Most emergency lights come on while the vehicle's brake engages automatically. The brake then pumps up and down on its own if any pressure is applied. Shortly after, the vehicle goes into "limp mode". The vehicle is currently at a Chrysler Dealership service department and available for inspection. 2. This has occurred 8 times since the vehicle was purchased in October 2024. All but one time, it has happened while driving. Once it happened while on a major highway. I had to quickly veer into a turn lane so our vehicle was not rearended by another car. Another time, I was driving my children to school and it happened about 10 seconds before crossing the worst intersection in our city. That could have resulted in a major crash based on the history of that intersection. A third time, I was traveling at about 40 miles an hour when warnings came on and the brake started to engage. Luckily, I had just gotten off a major highway and I was able to get to the side of the road. 3. The problem has been confirmed by a dealer, since it happened while I was in their service parking lot. However, they have been unable to recreate, find the source of, or fix the issue after 7 times of servicing it. 4. I am unsure whether the manufacturer has sent anyone outside of the dealer's service department to inspect the vehicle. We have been in contact with the manufacturer directly since December 26, 2025 in relation to this issue. Our extended warranty company may have had an inspector out on the first visit to the dealership for the issue, but no one since that I am aware of. 5. Emergency messages have included: Service Electronic Stability Control, Service Charging System, Service Hybrid Electronic Vehicle System, Auto Emergency Braking Unavailable Service Required. Multiple emergency lights come on including: Check Engine, Electronic Throttle Control, Electronic Stability Control Active, Service Forward Collision Warning. The first incident was January 22, 2025.
My 2022 Chrysler Pacifica Hybrid has developed a dangerous rear brake system malfunction. During a dealer inspection on 4/27/26, the certified technician confirmed that the rear brake pads are only contacting approximately 50% of the rotor surface, leaving the remaining half completely unused and heavily corroded. This is not normal corrosion or wear. It is a mechanical failure in which the pads are not engaging the rotor correctly. The technician also stated that the pads were sticking in the caliper slides, which prevented full engagement. This is a functional defect in the brake hardware, not environmental corrosion. Corrosion does not selectively prevent half of a brake pad from engaging the rotor unless the caliper mechanism is not operating properly. This defect results in: Reduced rear braking force Increased stopping distance Excessive load on the front brakes Potential loss of stability during emergency braking Possible degradation of the parking brake The vehicle was serviced on 9/23/25 (tire rotation + NYS inspection) and 12/10/24 (four new tires + NYS inspection). No rear brake issues were identified at either visit. If this were normal corrosion, it would have been visible during those inspections. The sudden onset of severe partial engagement by 4/27/26 indicates a latent mechanical defect, not gradual wear. Chrysler/Stellantis has refused to classify this as a defect and instead labels it “corrosion,” despite the technician’s findings of pad binding in the slides and improper brake engagement. The manufacturer’s internal system blocks any warranty or engineering review because it incorrectly categorizes the failure as routine maintenance. This is a premature and unsafe brake system failure at only 33,000 miles. The braking system is not operating as designed, and the defect could lead to reduced braking performance or loss of control. I am requesting NHTSA’s review of this issue as a potential safety defect affecting the rear brake system

The mirror on the passenger side of the vehicle "exploded".
09/17/23 Was driving 65mph, car was slowing Down in front of me and turned cruise control off. Immediately car started stalling. Was in middle lane with cars on both sides. Turned on flashers and no power in gas pedal. At about 30mph, was struck from behind by Jeep! Third car behind struck Jeep and Jeep struck me again. There were injuries and a death. My car was towed. 10/02/23; Now car is in body shop. Contacted Chrysler, they inspected car, but did not drive it. 10/12/23 Just received email stating that they found no problem. They stated that they are not responsible as they found no evidence of manufacturing defect or recall having contributed to this accident. Insurance companies and police are still investigating.

The contact owns a 2022 Chrysler Pacifica. The contact stated that while driving into California through a storm with the temperature at 39 degrees, in heavy rain and wind, the defrost button was pressed, and the windows immediately fogged up. The contact cleared the windshield with a cloth while attempting to navigate through traffic. The contact became aware that the defogger had caused the failure. The local dealer was contacted, who was unable to determine the cause of the failure. The vehicle was not diagnosed or repaired. The manufacturer was contacted, but the contact was unable to speak with a representative. The failure mileage was 30,000.
First of all, I would like to say that I received an "IMPORTANT SAFETY RECALL" notice citing that "certain 2022 Chrysler Pacifica vehicles failed to perform" However, my car's problem, though similar, is not quite the same as the problem mentioned by the Federal Motor Vehicle Safety Standard" - rear visibility. My car's DRIVERS' SIDE VIEW mirror is the one that poses the problem. When I put the vehicle in reverse, the right-side view mirror adjusts so that I can see the right backside of the vehicle. This, unfortunately, does not occur with the left sideview mirror. I can only see the ground. This an obvious determent to the safety of not only myself, but others as well. When I reported this irregularity to Hunter Dodge, I was told that was just the way the mechanism worked. I was told that there was nothing they could do. No one bothered to check it out. No one seemed concerned. This has been an issue since I purchased the vehicle. I don't know if anyone else has this same issue; but I feel that I am not the only one. This functionality necessary for the prevention of serious injury to pedestrians and/or property. Thank you, in advance for receiving this information.

Chest buckle can be slid down by toddler and toddler can wiggle arms out of harness and release the buckel.

Seat belts

I was in an accident. Where the van rolled 17 times. On or about the 12th roll. The retaining lock on the shoulder strap released. This allowed me to slide out of the seatbelt. This caused me more complicated injuries. The vehicle is currently at copart impound lot in Oklahoma City, OK. Will be available for inspection upon your request. The Oklahoma highway patrol report indicated. I was wearing my seatbelt but the retaining lock on shoulder strap had broken. The Oklahoma highway patrol report # OHP25-024333. There were no warning lights or messages to indicate that there was a problem.
The contact owns a 2022 Chrysler Pacifica. The contact stated that the rear passenger’s seats headrests failed to remain in an upright position. The contact stated that the headrests were not able to be raised into positions and locked to accommodate passenger's necks and back of the heads. The contact had not taken the vehicle to a local dealer or independent mechanic. The vehicle was not diagnosed or repaired. The manufacturer was informed of the failure. The failure mileage was approximately 81,000.

My car is a part of NHTSA Campaign No. 26V362 / Manufacturer Recall 52D and on July 30th, 2026 it suffered a fatal transmission error and stopped on the highway. I am unable to drive it and it officially needs its entire transmission rebuilt. My wife was driving back from work and the car shutdown in the middle of the highway and she had to risk her life to get it to an emergency zone. The turtle symbol came on the dash and the dealer confirmed the transmission needs to be rebuilt because of a manufacturer defect.
The contact owns a 2022 Chrysler Pacifica. The contact stated that while driving at an undisclosed speed, there was a loud sound coming from the engine with the message "Service Hybrid System" displayed. The sound was noticeable while accelerating and decelerating. The vehicle was taken to the dealer, but no DTC was retrieved; however, the contact was informed that the transmission control module needed to be reprogrammed. The TCM was reprogrammed; however, the issue persisted with a high-pitched whining sound. The manufacturer was notified of the issue. The approximate failure mileage was 48,000.

The van was going about 10 mph and the side wall just burst and went to flat right away and tire pressure warning came on. Yokohama had a recall for this same issue in 2022 but this DOT was not included. Nothing came in contact with tire or van when the issue occurred. This would have been a major issue if going full speed. Discount tire in Southaven MS stated the tire just gave out with no indication of anything hitting the tire. Yokohama refused to do anything as this is a BraunAbility handicap van which shows them as owner 1 to do the conversion through Chrysler. These Vans cannot be purchased through Chrysler directly. Yokohama did now even offer to report this issue internally as a complaint on the tire. I kept the tire for further inspection and investigation. I also have pictures and video of the tire prior to replacement. Tire Yokohama AVID S34 (S34RV) 236/65R17 104T. The P style is not indicated on the tire. I have 3 other tires remaining on the van with same tire/DOT (DOT 4UL8-PKX3321

We just purchased the used 2022 Pacifica today., it was 75 miles back home. I left to get Dairy Queen; I turned left onto side road into Norton commons and an alert came up Stop safely your car will slow down soon. Very scary with a family and newb and kids and wife. Especially because cost bought it. Perhaps I should return it!

THE VEHICLE STOPS AT WILL WHILE DRIVING. IT HAS STOPPED SEVERAL TIMES, IMADE IT TO THE NESSAN DEALER WHERE I PURCHASED IT THEY STARTED IT BUT ONCE I GOT ON THE INTERSTATE IT STOPPED ANDI I HAD TO HAVE IT TOWED. ON THE PANEL IT SAYS 'ELECTRONIC STABILITY CONTROL"
