Recalls, owner-reported complaints, investigations and safety data

Owner complaintsComplaints are reports submitted to NHTSA by owners and drivers. NHTSA does not verify most reports before publishing them, and a complaint does not establish that a defect exists. Volume is influenced by how many of a vehicle were sold, how long it has been on the road, and how much attention an issue has received.
0
Safety recallsA safety recall is issued when a manufacturer or NHTSA determines that a vehicle has a safety-related defect or does not comply with a federal motor vehicle safety standard. Recall repairs are free. Whether a specific vehicle is covered depends on its VIN — a recall listed for a model year does not necessarily apply to every vehicle of that year.

InvestigationsNHTSA opens an investigation to examine whether a safety defect may exist. An open investigation is not a determination that a defect exists, and many investigations close without a recall. Investigations can, however, lead to one.
0
Crash or fire reportedComplaints whose submitter recorded that a crash or a fire occurred. These are the reporter's own answers on the NHTSA form, not verified findings.

May 11, 2021
915 units potentially affected
The defect: E-One Incorporated (E-One) is recalling certain 2017-2021 Cyclone 2, Cyclone N, Quest 2, Typhoon, and Typhoon N vehicles. The sealing washer may not seat correctly in the pilot bore holes, allowing the high pressure fuel rail assembly to leak.
The risk: A fuel leak in the presence of an ignition source increases the risk of a fire.
The remedy: Dealers will inspect and replace the rail as necessary, free of charge. Owner notification letters were mailed out on May 28, 2021. Owners may contact E-One customer service at 1-352-237-1122.

POWER TRAIN:AXLE ASSEMBLY; SUSPENSION:REAR:SPRINGS:LEAF SPRING ASSEMBLY:U-BOLT, LEAF SPRING TO AXLE

Sep 9, 2020
15 units potentially affected
The defect: E-One Incorporated (E-One) is recalling certain 2018-2020 Cyclone 2 and Typhoon ambulances. The lower shock mount bracket may cause the U-bolts to bind and break, possibly preventing the rear axle from remaining secured.
The risk: The unsecured rear axle may shift sideways, resulting in loss of control of the vehicle, thereby increasing the risk of crash.
The remedy: E-One has notified owners, and dealers replace any of the affected lower shock mount brackets, free of charge. The recall began September 21, 2020. Owners may contact E-One customer service at 1-888-237-1751.

ELECTRICAL SYSTEM:STARTER ASSEMBLY

Oct 14, 2019
1,603 units potentially affected
The defect: E-One Incorporated (E-One) is recalling certain 2013-2019 Typhoon and Cyclone 2, 2013-2014 Cyclone 3, 2014-2019 Quest 2 and 2017 CHSH emergency vehicles equipped with a Cummins ISL, L9, ISX12, X12, ISX15, or X15 engine. The engines do not have a diode in the starter solenoid circuit to improve the longevity of the lock-out relay, resulting in accumulated damage over time.
The risk: The damaged starter lock-out relay may prevent the vehicle from starting, hindering personnel from responding to emergencies, increasing the risk of an injury.
The remedy: E-One will notify owners, and dealers will install a diode in the starter solenoid circuit and install a new starter lock-out relay, free of charge. The recall began November 15, 2019. Owners may contact E-One customer service at 1-352-861-3612. Note: Vehicles previously repaired under recall 18V-019 also need this recall remedy.

ELECTRICAL SYSTEM:WIRING

During a routine truck delivery inspection a malfunction of the air flow restriction indicator light was detected. Our investigation determined that the malfunction was introduced in 2007 with the launch of the new ISSPRO gauge cluster. The new dash gauge cluster changed the requirement for air flow sensor signal from active high to active low. This malfunction went unnoticed until now due to our published air filter routine maintenance requirements and the air filter?s longevity. Revised units affected by year.

Manufacturers file copies of the bulletins they send to dealers with NHTSA. These often describe diagnostic or repair procedures for a known condition. They are not recalls: repairs described in a bulletin are usually only free if the vehicle is still under warranty or the manufacturer has extended coverage.
