Owner-reported complaints naming the fuel system system, filed with NHTSA
Data refreshed
Reports naming fuel storage or delivery — tanks, pumps, lines, injectors and fuel leaks. Every record below is an owner-reported complaint about the 2021 Ford F-150 Hybrid submitted to NHTSA — not a confirmed defect, not a recall and not a manufacturer bulletin. 18 reports on file name it.
Complaints are reports submitted by owners and drivers to NHTSA. They are not verified and do not establish that a defect exists.

the issues associated with this recall still dont seem to be remedied. I believe a solution should have been provided from the manufacturer by now
The contact owns a 2021 Ford F-150. The contact stated that since purchasing the vehicle, there was a misfire in the engine, the vehicle hesitated and accelerated slowly while driving in gasoline mode and depressing the accelerator pedal. The failure mostly occurred while accelerating from a complete stop. The vehicle was taken to the dealer on several occasions, where the failure was duplicated. The contact received notification of NHTSA Campaign Number: 25V455000 (Fuel System, Gasoline), and the dealer associated the failure with the recall; however, the part to do the recall repair was not yet available. The dealer notified the manufacturer of the failure. The failure mileage was approximately 80,000.
The contact owns a 2021 Ford F-150. The contact stated that the vehicle lost electrical power, and the contact initially believed that the failure was due to a dead battery. The battery was replaced and the vehicle was hesitating while accelerating. The contact received notification of NHTSA Campaign Number: 25V455000 (Fuel System, Gasoline); however, the part to do the recall repair was not yet available. The local dealer was contacted and stated that the contact had to pay for a diagnostic test to verify if the failure was associated with the recall. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was made aware of the issue.
Experiencing total stop of acceleration in two occasions: 1) when putting the car in reverse and pressing the accelerator and 2) when making a left or right turn after a yield or stop. And when I say loss of acceleration, I don’t mean hesitation, reduced power, or sluggish acceleration. I mean absolutely ZERO acceleration. I’ll press the accelerator and nothing happens. The engine stays basically at idle, and the truck will not accelerate for roughly 5-10 seconds, regardless of how much accelerator I’m giving it. Then suddenly power comes back and the truck drives normally again. This happens with normal operating temps while driving most of the day. It got to a point where it happened once every day. Very scary stuff when you're pulling out into traffic - major safety concerns with oncoming traffic.
I have replaced spark plugs and coil packs due to code saying severe misfiring. However, the root cause of this is the continued open recall of the fuel pump. This has effected the way I can drive my vehicle on daily activities, such as going to work, doing family things, and other things while driving this truck.
On XXX, my 2021 Ford F-150 PowerBoost, VIN [XXX], stalled while I was turning from [XXX] onto [XXX]. My wife and daughter were in the vehicle. The engine lost all power in the intersection. No collision occurred. This is the exact failure described in open safety recall Ford 25S75 / NHTSA 25V455 (low-pressure fuel pump). I had already reported the unrepaired recall to NHTSA on XXX. I took the truck to LaFontaine Ford in [XXX] the same day. As of XXX, Ford still has no released remedy or parts, the dealer had not completed an inspection, and Ford Customer Care refused a courtesy vehicle. I am submitting a separate 49 C.F.R. Part 557 petition asking NHTSA to hold a hearing on Ford’s failure to remedy this recall. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
My 2021 Ford F-150 PowerBoost, VIN [XXX], is included in Ford recall 25S75 / NHTSA recall 25V455. The vehicle has stalled more than five times and is experiencing the same dangerous condition described in the recall. The Ford dealer told me I can bring the truck in, and they will diagnose the problem and repair it if they can. However, I must pay all costs upfront and then request reimbursement after Ford releases the official recall remedy. I should not be required to pay out of pocket for the diagnosis and repair of a known safety defect covered by an active recall. There is also no clear timeline for when a remedy will become available or when I would be reimbursed. I cannot reasonably be expected to pay now and potentially wait years to recover my money. This vehicle repeatedly stalls and creates a serious crash risk. I am requesting that NHTSA investigate Ford’s delay and require Ford to provide an immediate, no-cost diagnosis and repair or provide safe alternative transportation until the recall remedy is available. I am reporting a safety-related transmission problem I experienced with my 2021 Ford F-150 equipped with the 10R80 automatic transmission. While driving normally, the transmission suddenly shifts hard/lags on gears 3-5 and sometime higher gears 7-9 at higher speeds. I’ve also noticed a buzzing/metallic rattling coming near the engine from inside. There is also times i can tell i have no power when trying to speed up and the other day i almost thought i was going to get T-Boned. This occurred while on my daily traffic commute to work, creating a potentially dangerous situation because the truck did not respond as expected when I needed predictable acceleration or propulsion. I believe this incident may be related to the known 10R80 CDF clutch-cylinder/sleeve problem. Ford technical information identifies axial movement of the CDF sleeve that can cause hydraulic-circuit leaks and harsh or delayed engagement and shifting. This condition can result in slipping, RPM flares, delayed engagement, unexpected gear changes, harsh downshifts, lurching, or loss of propulsion. This is a safety concern, not merely a transmission quality or reliability issue. Unexpected loss of propulsion or unpredictable shifting while merging, accelerating into traffic, crossing an intersection, turning, or traveling at highway speed can create a collision hazard. Because I personally experienced this unexpected transmission event, I request that NHTSA investigate the 10R80 CDF clutch-cylinder/sleeve condition in Ford F-150s. Please review Ford warranty claims, dealer repair records, transmission replacements, consumer complaints, and field-failure data to determine whether this is a safety defect requiring a recall. I believe owners should not have to wait for a potentially dangerous transmission failure before this known condition is addressed.
On July 15, 2026, I took the vehicle to Lafontaine Ford for maintenance and to address open recalls. I was informed that the open recall for the low-pressure fuel pump (campaign 25S75) did not have a solution available. Due to the severity of the recall, I contacted the Ford Customer Care Center. A representative stated that a solution would be available on July 17, 2026, and that they would contact Lafontaine Ford to prioritize the vehicle for repair on July 18, 2026. On July 17, 2026, I followed up with Lafontaine Ford and confirmed they had received no contact or notation from Ford regarding prioritization. Lafontaine Ford also stated they had no information on when parts would become available and that nothing could be done at that time. I then contacted the Ford Customer Care Center again because no action had been taken, even though the recall has been open since approximately July 8, 2025. A Ford Customer Care Center representative confirmed that no remedy was available for my model year and that they did not know when one would be available. Due to the seriousness of the recall, I requested a courtesy vehicle. The Ford representative contacted Lafontaine Ford regarding this request and then disconnected the call. The Lafontaine Ford representative informed me they could not provide a courtesy vehicle without written confirmation from Ford due to the dealership having no reasonable timeframe of when parts/remedy would be available. When I called the Ford Customer Care Center back and explained what Lafontaine Ford had told me, I was treated indifferently and was disconnected twice. I eventually spoke with a supervisor who refused to escalate the concern or provide any assistance with the documentation Lafontaine Ford said they needed in order to provide a courtesy vehicle. The low-pressure fuel pump is the component identified in the recall and can fail, resulting in loss of engine power or stall while driving. This creates a significant safety risk, part
The contact owns a 2021 Ford F-150. The contact received notification of NHTSA Campaign Number: 25V455000 ( FUEL SYSTEM, GASOLINE) however, the part to do the recall repair was unavailable. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was made aware of the issue. The contact had not experienced a failure.
Several times, the vehicle has failed to accelerate or respond to accellerator pedal input for several seconds. It seems to happen when switching from electric to hybrid modes, and when shifting between lower gears simultaneously. Once the switch to hybrid occurs the engine will rev high with some acceleration even if the accelerator pedal is no longer being pressed. This acceleration has happened when I had my foot over the brake pedal in anticipation of pulling over to stop since the truck would not accelerate. There are no warning lights or messages that appear on the vehicle. It is dangerous because it seems to happen when trying to accelerate from a lower speed. If it happens when trying to change lanes in stop and go traffic or while pulling into traffic other cars would have to brake hard to avoid hitting our vehicle that has experienced stalled acceleration. It is also dangerous because once the transition happens there is brief uncommanded acceleration.
2021 ford f150 PowerBoost hybrid. There is a LOT of internet chatter and forum chatter about KNOWN issues with the following, and it recently almost led to a major and likely serious injury accident. Specific Parameters: Low speed (~5 mph) slow roll Under internal combustion power, not battery In 2nd gear only When gas pedal is pressed vehicle does nothing. Does not respond. Two choices are to hammer the gas, or hit the breaks. Does two things, slams forward eventually after 3-4 second delay if gas pedal depressed OR shifts down or into electric mode (internal combustion off) and then will once again go from first to third in normal acceleration progression and pace They must have an issue with the standard 1-3 skip programming with ICE power and 2nd gear being a hang-up I almost got T boned by a semi truck due to this with family in car. This could have been deadly. It happens as an annoyance at least once per day but this one was scary. Online, and based on what I can find - Ford is aware of this issue as they have the 1-3-5 programming in the transmission but when it finds itself (due to real world driving conditions) in 2nd gear under Internal combustion, it does not know what to do. I brought it to Ford last year in Anchorage and they are attempting to blame it on normal hybrid system operation. That is NOT the case. It would be something that could be duplicated but with effort. A basic, simple test drive is NOT going to find this. They are aware of this being an issue. If you have any situation where you are rolling through, merging into traffic, or other scenario where you end up in 2 under ICE you are in TROUBLE. Dangerous, scary, and totally a manufacturer issue that is known and needs to be taken VERY seriously.
Vehicle has completely unresponsive gas pedal for 3-5 seconds. Gas pedal is completely dead. This occurs randomly but mostly in a stop and start accelerating situation. Once the 3-5 seconds passes gas pedal responds normally. No codes generated during incident.
Despite having been told my truck was fixed at a dealer shop, my truck still displays an absence of forward propulsion when applying the gas pedal and trying to accelerate from slower speeds. This has happened when trying to make a left or right turn when there is oncoming traffic. Sometimes this happens while trying to accelerate on the on-ramp of a highway to get up to normal piste speed limits with traffic very fast behind me. The dealer I took it to said they fixed the issues. Luckily I can apply the Manual mode with a button, but this is distracting when trying to look for the button, and the issue comes back in Automatic mode.
After a right turn in a residential street, I pressed the accelerator to progress on the roadway. The truck did not accelerate, so I looked at the tachometer and observed it above 0 and below 1,000 rpm. The engine was running, but unresponsive despite pumping the accelerator. While rolling at 5 mph I shifted the transmission to neutral and continued pumping the accelerator. There was no response for the next 200'. Finally, the engine became responsive and ran normally. It runs normally now. I cannot reproduce this issue. I called the Ford dealer service on 8 November 24 at Noon and they said "watch for the issue". I expressed that in traffic, a suddenly undependable engine is a serious concern. The accelerator/engine was unresponsive for a continued period of time. The gas engine was in control. There was no check light/warning light. The Ford dealer inspected the telemetry of the vehicle remotely and no anamolies were observed. The dealer declined to inspect the vehicle since the issue is not reproducible.
For months have been having two issues with my 2021 F150 Powerboost with the CO2 sensors as well as an issue with the Exhaust Heat Exchanger Bypass Valve A. Which after my bumper to bumper warranty expired now needs replaced even though the same issues have been throwing a CEL for months
Complete loss of the throttle while in an intersection. Was able to shut down vehicle, then wait about 30 seconds, then turn it back on clears issue. This has happened at least three times
