Electrical

Vehicle in question had problems since day. One computer was affected was never fixed. Many problems ended up in agency for repairs a total of 9 1/2 months and I’ve only have a three year lease on the vehicle and have not been able to use it fully because of electrical and further issues. Litigation found with my attorneys and Ford attorneys, that the vehicle had multiple problems and was issued by a judge, repayment and penalties.
After stopping the vehicle to run an errand, the vehicle locked up. The steering wheel would not turn. The vehicle would not shift into any gear. The vehicle was inoperable and I found myself stranded 25 miles from home. The vehicle was towed to our local Ford dealership where it has been for the past two weeks. This failure could just as occurred on the road causing a dangerous situation.
Driver assistance

We took it to the dealership immediately who were unable to duplicate. They diagnosed it as a network overload and attempted to release the car back to us. Currently working with a Ford Advocate as a Network Overload will shut our brakes off again. It's just a matter of when. Which is why they can't duplicate it. They can't predict when the Ford Network that communicates to the MachE braking mechanism will overload again. In the event of a differential pinion shaft failure, the customer may experience a loss of motive power and/or unintended vehicle movement with Park requested if the parking brake is not applied. A loss of motive power or unintended vehicle movement increases the risk of a crash

Fuel system

I’ve had this situation happen at least 2 to 3 times but specifically I’ll talk about the most recent. I was reversing out of my parking spot approximately 20 feet or so to where I can turn around and circle around the parking lot and exit. When I switched over to drive the vehicle accelerated rapidly unprompted. I was able to stop it by using the break, but I can also see how this could be a significant safety issue.
On 2 occassions in the past 2 week, our Mustang Mach E has accelerated on its own. In the first incident, it was a minor acceleration. It caught me by surprise as I was pulling up to a curb to drop off a passenger. It was easily stopped with brake pressure. In the 2nd incident, the acceleration was greater and more pronounced while pulling into a parking spot, requiring the pressure of both feet to quickly stop it before it went up onto a sidewalk and into a store front. It's never done this before, but certainly could cause an accident and was unsettling. We have not taken it to the dealer because it is random and not replicate-able. There were no warning lamps or error messages at the time. The vehicle was functioning and acting normal otherwise.
Engine

The contact's husband owns a 2023 Ford Mustang Mach E. The contact stated that while her daughter was driving at an undisclosed speed, the vehicle that was in front of the vehicle stopped abruptly. The contact stated that the driver depressed the brake pedal, and the vehicle started decelerating. The contact stated that Automatic Emergency Braking system then engaged, and the vehicle stop decelerating and crashed into the rear end of the other vehicle. The contact stated that the vehicle sustained severe front end damage, but no air bags deployed. No injuries were reported. The other driver exited the vehicle, returned inside the vehicle and drove away from the scene. A police report was filed, but the police report number was not available. The vehicle was able to be driven away from the scene. The vehicle was taken to an autobody repair shop to be repaired. An unknown dealer was notified of the failure, but no assistance was provided. The vehicle was not diagnosed or repaired. The manufacturer was not notified of the incident. The failure mileage was approximately 12,000.
Was driving on hightway another vehicle was hit by a truck and sent ti my lane. Y hit it almost head on. My vehicle tire and front axle was ripted off. My side or car also damaged. Cas was total loss. No collision light and no air bags deployed

The contact owns a 2023 Ford Mustang Mach-E. The contact received notification of NHTSA Campaign Number: 25V404000 (ELECTRICAL SYSTEM, LATCHES/LOCKS/LINKAGES); however, the part to do the recall repair was not yet available. The local dealer was not contacted. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was not made aware of the issue. The contact had not experienced a failure.

Steering

Vehicle: 2023 Ford Mustang Mach-E (leased) The issue involves the cruise control system and steering wheel control layout (human-machine interface). Cruise control is unintentionally activated when attempting to use the turn signal due to the placement of the cruise control buttons. The vehicle is available for inspection upon request. On two separate occasions while using one-pedal driving, unintended cruise control activation caused sudden acceleration during low-speed turning maneuvers. The first incident occurred while signaling to turn into a parking spot. The second occurred while entering a left-turn pocket. In both cases, the unexpected acceleration created an immediate risk of collision with surrounding vehicles and required sudden panic braking to avoid an accident. The issue has occurred multiple times during normal driving but has not yet been formally reproduced or confirmed by a dealer or independent service center, and the vehicle has not yet been inspected by the manufacturer or other parties regarding this issue. No warning lamps, alerts, or messages appeared prior to the unintended acceleration. Cruise control engaged without driver intent and without advance warning.
Dear NHTSA Team, I am writing to urgently raise concern and seek answers regarding what appears to be a systemic safety issue affecting vehicles equipped with Advanced Driver Assistance Systems (ADAS) and self-driving technologies. I have personally experienced repeated and dangerous incidents of phantom braking, overcorrection, and loss of control while driving a 2019 Toyota Prius. The car suddenly brakes, swerves, or pulls sharply when it detects shadows, road inclinations, or mild curves — making it unsafe to drive, especially at highway speeds. These events occur without any driver error. To make things worse, this issue is not limited to one brand or vehicle. Over the past year, I have driven and rented multiple cars from different brands and models through rental companies, and many of them presented the exact same unsafe behavior — unpredictable braking, steering correction, and traction instability. This clearly indicates that the issue is industry-wide, not isolated to a single manufacturer. I have been a professional rideshare driver for over five years, completing more than 500,000 miles without a single accident or ticket. Since October 2023, these vehicle behaviors have made it nearly impossible for me to continue working safely. This situation has destroyed my livelihood — I am now three months behind on rent, my mental health has been severely impacted, and I am facing homelessness for the second time in two years. All of this has happened because of unsafe technology that drivers cannot fully disable, even when it is clearly malfunctioning. I respectfully ask NHTSA: Why are automakers allowed to release and maintain these unsafe systems on public roads? Why do drivers not have the right to safely disable malfunctioning ADAS or self-driving features, like any other electronic system that can be shut down in an emergency? When will NHTSA take meaningful action to stop these dangerous practices before more lives are affected?

Body & structure

Driver's door actuator/plunger jammed in the extended position and would not allow the door to be closed. Could not close the door to drive the car. Vehicle has not been inspected yet. Found a temporary solution by searching online, which said to push hard on the door to close it. This worked to get the door closed but likely will not prevent the problem from reoccurring.
While driving on the highway at speed, the front undercarriage cover of the Ford Mach-E came loose, started vibrating violently, then started dragging on the highway. Thankfully the driver was able to pull over safely. The incident could have caused life-threatening injury to the driver and other vehicles has the part come totally lose, come under a tire, or started hitting the battery system that sits just behind the car. This could have caused the driver to loose control, the part to hit another vehicle, or a fire in the case of the battery system and the violent impact and friction as a result of the vibrating loose part. We took the vehicle to the dealer and they confirmed that there was damage to the undercarriage cover, and also "impact" damage to what is under the undercarriage cover (albeit they assumed the impact damage to be due to user/environment, as you'll see from the pictures, it was likely due to the vibrating loose part itself.) No warnings or indicators came on before the problem. The vehicle is only 3 months old and used exclusively for city driving/commuting. The car has not been any vehicular accidents or even minor collisions.

Seat belts

Intermittent issue with the driver seat belt sensor which illuminates the restraints indicator warning light. Message sent in FordPass app says: "The restraints control module has detected a fault with the driver seatbelt buckle sensor circuit" Consultation with the dealership indicated that when the restraints warning light is illuminated the airbags become disabled. However, stopping the vehicle, turning it off, removing and reinserting the seatbelt into the buckle, and starting the vehicle clears the warning light. This is extremely intermittent and replication is difficult.
VIN # [XXX] have turned up is if I start my new 2023 mustang Mach-e at home without anyone in the rear seats. When I pick up children, it does not recognize. They are now in the rear seats i.e. seatbelt warnings. Our children picked up from daycare left in a hot car, so what I’m saying in a nutshell is once I leave the residence with only me in the vehicle, and I add additional passengers the seat monitors do not detect anyone being in the rear seat until the seatbelt is fastened, if the seatbelt is not fastened, it has no clue in the rear of the car which is annoying because it constantly asked me to check the backseat, but it only does this if someone has been in the car when it was started originally before it left its origin destination This is a huge issue and should be reported to the NTSB INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

Approximately 90 seconds after startup, every speaker emitted a deafening sound that can be described as a high-powered air compressor. The sound was debilitating, and in the moment it felt like the car would explode (solely from sound, no visible defects). The vehicle was in motion, and could have caused a wreck under different conditions. The issue continued even after turning off then restarting the car. The issue stopped once disconnecting/reconnecting the battery. There was never an error message or any other warning, other than the sound. The dealership was unable to recreate the issue, resulting in no repair action. The problem has not recurred as of 12 months later; however, I still get nervous when driving the vehicle.

During rapid deceleration from ~35mph to <5mph the contents of the frunk (front trunk) shifted and was able to strike the emergency release button within the frunk causing the hood to open while the vehicle was in motion. At the time the button was pressed various alarms showed on the screen to alert the driver to pull over which could not be done safely based on the current road conditions. The design of the emergency release button is deficient in preventing inadvertent opening of the frunk while the vehicle is in motion. The vehicle has been inspected by the dealer and they have indicated that all systems are working as intended.

I am locked out of the car despite having keys and phone in hand. When I was in the car it wouldn’t start. Screen was black. I got out of the car to try other doors. When I got back to driver door, the car was locked and I tried all normal modes to get in the car. It is NOT the 12 volt battery. I tried to use the manual jump to get the hood to pop open but it didn’t work. If I had a baby in the car and it was hot out, this could be a life-threatening disaster.
