Automatic Emergency Braking Errors
On May 26, 2023, the Office of Defects Investigation (ODI) opened a Preliminary Evaluation (PE23010) to assess reports of false positive activation of the Automatic Emergency Braking (AEB) system in model year (MY) 2017-2022 Daimler Trucks North America (DTNA) vehicles. The reports alleged that activation of the AEB system occurred while driving with no apparent obstruction in the vehicle's forward path, resulting in sudden vehicle deceleration. DTNA indicated that it received of a total of 315 reports that may relate to the alleged defect in MY 2017-2022 DTNA vehicles. DTNA stated that the vast majority of false positive activation of the subject systems occur in very limited durations and result in a five mile per hour deceleration. ODI considered four AEB systems offered by DTNA: Detroit Assurance 2.0, Detroit Assurance 4.0, Detroit Assurance 5.0, and Wabco OnGuard. ODI’s analysis shows that of the Detroit Assurance versions, all are failing at similar rates. The OnGuard AEB systems equipped on the same model and model year vehicles have experienced a significantly lower claim rate. To date, ODI has received a total of 32 consumer complaints of inadvertent activation of AEB in MY 2017-2022 DTNA vehicles. In total, ODI reviewed 347 unique (i.e., non-duplicative Vehicle Identification Numbers) reports that may relate to the alleged defect, which include 24 reports involving a crash, 13 reports involving an alleged injury and one report with a fatality. PE23010 has been upgraded to an Engineering Analysis to further assess the scope, frequency, and potential safety related consequences of the inadvertent AEB activations in MY 2017-2022 DTNA vehicles. Engine Electrical Wiring Harness Chafe
The Office of Defects Investigation (ODI) opened this Preliminary Evaluation to investigate reports of engine electrical wiring harness chafe on the subject vehicles.ODI analysis found thousands of unique harness part numbers, spanning the two generations of vehicle chassis assembled at multiple vehicle assembly plants in two different countries, with multiple sub-models, incorporating over thirty powertrain choices.Each wiring harness could contain several dozen individual wires for power, ground and communication purposes depending on the multitude of equipment options available for custom vehicle configuration. ODI identified three primary areas where a wiring harness bundle may chafe: the air compressor support bracket, the transmission case, or the coolant surge tank. In each case, the root cause was some combination of inadequate design clearance, installation variability and relative motion between the wiring harness and the point of contact, such as from vibration. The chafing could result in a range of consequences from none to various diagnostic fault codes, telltale illumination, driver warning center messages, erratic electrical component operation, difficult or no engine start condition, engine de-rate (power reduction), engine shut down, inoperative transmission gear selection, or an unexpected shift to neutral.These potential consequences vary depending on the specific damaged wire in the harness and the exact failure mode, such as intermittent short circuit to ground or open circuit. Generally, the chafing resulted in a progressive series of driver warnings with high levels of detectability over the course of several days and ignition cycles. Drivers usually heed warnings before an engine shutdown condition occurs and the driver may override an engine shutdown countdown timer using the override switch. If the driver allows the shutdown event to occur, the engine can almost always be immediately restarted. Incident rates for an engine shutdown with no warning or a transmission shift to neutral while driving are both very low. ODI received one complaint (NHTSA ID Number 11366557) from a fleet mechanic, who reported in a subsequent interview that he repaired at least six subject vehicles that experienced the alleged defect condition. However, there have been no new Vehicle Owner Questionnaire complaints received by ODI since opening this investigation, and there are no known crashes, fires or thermal events, or injuries or deaths related to the alleged defect. Accordingly, no safety-related defect trend has been identified. Based on available information, this investigation is closed.The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist. The Agency reserves the right to take additional action if warranted by future circumstances. Halogen Headlights May Cause Fires
In its process for evaluating Early Warning Reporting data, the Office of Defects Investigation (ODI) found twenty-four (24) field reports involving headlight fires on Freightliner Cascadia trucks, model years 2017 and newer produced by the vehicle manufacturer, Daimler Trucks North America (DTNA). ODI contacted DTNA and learned that it was aware of halogen bulb-related thermal events. DTNA stated a belief that the issue was due to incorrect bulb replacement during maintenance. However, some vehicles experienced fires within the original equipment halogen bulb's expected life range.In July 2019, DTNA released a service bulletin and a training video covering proper headlight bulb replacement. There have been 17 fires, with 3 total vehicle losses, since DTNA released this extra training information.On February 25, 2021, ODI opened a Preliminary Evaluation (PE21-006) and sent an Information Request (IR) letter to DTNA.After the PE opening, DTNA reevaluated the data and found additional thermal events at low mileages and decided to file a recall. On May 14, 2021, DTNA submitted recall 21V-357 for 72,508 vehicles covering 2017-2022 models (exceeding the population and model years of this investigation). The planned owner and dealer notification date is July 12, 2021.For the remedy, DTNA will open up the 2-inch access into a 4-inch access hole into the inner fender to provide better access for the maintenance replacement of the bulb, and add a label that provides a visible warning to aid in the correct installation during the change. Additionally, a new foam grommet will replace the flammable rubber boots for both the high and low beam bulbs, improving access to the interface point where the bulbs install into the socket of the reflector. ODI believes that the scope of DTNA recall 21V-357 is sufficient, and accordingly, PE21-006 is closed. The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist on other models or MY vehicles outside of the recall scope. The vehicle whose VIN and other identifying details are stated above was sold on May 27 by [XXX] , owner of Koza Transportation LLC, to my company, Skyturk Transportation LLC, which I established in the United States. At the time of sale, the vehicle’s mileage was represented as 395,204 miles and supported by video and photographic evidence. However, subsequent investigations revealed that the same vehicle had been listed and sold on April 29 through BidCars with a recorded mileage of 843,194 miles. This clearly shows the odometer was rolled back by the seller or individuals acting with his knowledge. The vehicle, with reduced mileage, was sold to me at a grossly inflated price. It was also later discovered to be a severely damaged/salvage vehicle, a material fact not disclosed at the time of sale. After I brought this to the seller, he initially stated he would repurchase the vehicle but later claimed he could not contact the person he bought it from and gave inconsistent statements. Information obtained thereafter confirmed these actions constitute a federal offense. Official reports have been filed with the District Attorney’s Office and the DMV through my attorney, and legal and criminal proceedings are ongoing. Due to the investigation, mileage irregularities, and the vehicle’s damaged status, it cannot be sold at this time. My authorized stay in the United States is nearing expiration. As a result of the seller’s actions, my company has been unable to operate and suffered serious financial losses, and I have personally incurred significant damages. I respectfully request necessary actions to protect my legal rights and address the damages suffered.I learned on September 18 that the odometer had been tampered with and that the vehicle I also respectfully request that necessary measures be taken to prevent the seller or any individuals acting similarly from engaging in such vehicle sales to others in the future, and that stricter penalties be applied. I have experienced this issue a few times, and so have several acquaintances who drive the same make and model of truck as I do. Whenever it rains or I go through a truck wash, the starter shifter blocked comes on the screen while driving, causing the RPMs to fluctuate and resulting in a complete loss of power to the vehicle. One dealership informed me that this is a common problem, attributed to moisture seeping into a harness located near the transmission. Tried connecting the truck to a computer so see fault codes nothing comes up because it's a glitch and clears out. This situation poses a serious risk, as it could potentially lead to a fatal accident. With no proof of the malfunction.

While driving on the highway on cruise at 70mph the right front brakes locked up. The day was clear. I was driving across a bridge. The truck pulled to the right and I fought to pull it back on to the road. My company has the incident on camera. I was pulling a loaded tanker. Had the pavement been wet or snowy I could have easily jack knifed. If a vehicle would have been next to me I would have struck them. The vehicle was inspected by our mechanic and was told the system was within specs. The truck has also hard braked and pulled to the right when vehicles in the lane next to it are turning.

A collision avoidance system (CAS), also known as a pre-crash system, forward collision warning system (FCW), or collision mitigation system, is an advanced driver-assistance system designed to prevent or reduce the severity of a collision. System applied brakes bringing our Unit to a complete stop traveling 55 MPH on an Interstate while making a lane change, Because of this our Unit was rear-ended by another CMV, On board cameras video on both V1 and V2 clearly show incident. A print out of the DDEC shows code for braking system control at exact time of incident. In our fleet we have the same 2020 Freightliner Cascadia with this system and in trying to maintain the safety of our drivers and equipment no to mention our Insurance cost and CSA scores, This is a major issue as you can see on the Web the many cases and stories of these same incidents happening. Semi was traveling on the highway and transporting a 48,000 pound steel coil. The steel coil was secured with 4 chains. The automatic emergency braking system engaged with nothing in front of the vehicle, causing it to suddenly come to a dead stop in traffic. This caused the steel coil to break free of its bounds, crash through the trailer and into the vehicle cab, trapping the driver.

Ascending Tehachapi Pass CA Hwy 58 Eastbound in the left lane grossing 45,000lbs and traveling at 55mph with cruise control ON. I was passing another slower moving truck(~35-40mph). Before I came along the other vehicle the highway curved to the left. The Collision Aviodance System(CAS) didn't alarm, however the cruise control cut power and Automatic Emergency Braking(AEB) hardbraked my truck reducing the speed from 55mph to 40-45mph. This was sudden and unexpected. Once I determined the issue, I applied throttle overriding the AEB/CAS. However my forward momentum was lost and accelerating uphill was slow. It was midday and clear skies. No one was following me fortunately, but sudden and unexpected braking could have had a vehicle tailing too closely(It was an uphill grade with a speed limit of 65 mph for POV) rear-end my trailer(48 foot stepdeck trailer). Please explain to me how CAS/AEB are safety systems for a CMV?!? This is the 4th report I have filed with NHTSA about the same Freightliner Cascadia tractor. According to the vehicle operating Manual that this occurrence of unexplained and unwarranted braking is a "feature" of this "safety system". Meaning that the CAS/AEB was functioning as designed(A flawed designed in my professional opinion). I have been contacted by the NHTSA about previous filed report about an ongoing investigation with the manufacturer. What action is this agency going to take? Will there be an investigation when the system performs as designed causing an avoidable accident, injury, or death? Will I be a defendant, witness, or victim? The computer system that logs and keeps track of hours is not working properly. The wires are I guess are messed up. Is that dangerous? My hubby works at the Walmart dc and he said a girl slipped and got a bad spinal injury from the flooring not being non-skid.
