This incidence occurred twice, one year apart, under the same driving and weather conditions and on exactly the same route. On XXX, after departing [XXX], we were driving north on [XXX] near [XXX], with the adaptive cruise control set to 70 mph. Suddenly the dashboard displayed the warning, “Some driver assist systems cannot operate: Clear area shown,” and the Honda emblem indicator illuminated. Following the vehicle’s instructions, we stopped and cleaned the Honda emblem by wiping and rinsing it, but the warning remained. After we continued driving north on [XXX] for approximately three to four hours, the warning cleared without further action. The weather was clear and sunny. An identical incident occurred one year earlier, on XXX, along the same section of northbound [XXX] and under the same driving and weather conditions. Accord was also set to 70 mph with adaptive cruise control engaged. That warning again cleared on its own after a few hours. In addition, while driving in the left lane on XXX with adaptive cruise control engaged, Accord braked aggressively while passing semi-trucks traveling in the right lane, even though there were no vehicles ahead. On August 23, 2026, the electronic power steering and braking systems on a 2025 Honda Accord Hybrid completely failed and locked up simultaneously while traveling at highway speeds. This caused an immediate and severe safety risk, resulting in a near-collision on a busy freeway with three occupants inside the vehicle. The vehicle is currently available for inspection upon request. Prior to and during the failure, widespread dashboard warning lights illuminated simultaneously, including the Collision Mitigation Braking System, Lane Keeping Assist System, Road Departure Mitigation, and Low Speed Braking Control. Additionally, 11 malfunction alerts were triggered via the HondaLink app. The vehicle was inspected by an authorized dealer from August 24 to September 11, 2026. As documented in the attached Honda dealer service report, technicians initially noted low voltage codes causing multiple system failures. However, instead of identifying and repairing the root cause of the voltage drop, the dealer merely cleared the codes, test-drove the vehicle, and closed the repair order stating they "could not duplicate concern." Furthermore, the attached text message conversation with the service advisor proves the service department explicitly admitted in writing that the software is up to date and that the failure could happen again if the vehicle encounters the same error. The vehicle has not been inspected by a manufacturer field engineer, police, or insurance representatives. American Honda Corporate refused requests to dispatch a field engineer and closed the case file. Note: Full video evidence of the dashboard warnings and loss of vehicle control during the incident exists, but cannot be attached here due to file format limitations. The manufacturer and dealer have returned this vehicle without identifying or repairing the underlying defect, leaving it highly susceptible to another catastrophic loss of steering and braking at highway speeds.

The contact's son owns a 2025 Honda Accord. The owner stated that while coming to a stop, the brake pedal was depressed, but the vehicle failed to stop. No warning light was illuminated. The owner fully lifted the foot from the brake pedal and forcefully depressed the brake pedal while attempting to stop the vehicle. The vehicle failed to stop, causing the contact's son to rear-end a semi-truck. The contact's son stated that the vehicle powered off after the crash. The contact's son also stated that the air bags did not deploy during the crash. The vehicle was taken to the dealer, but the failure was not diagnosed or repaired. The dealer informed the contact that the failure could be duplicated. The manufacturer was made aware of the failure by telephone, and the contact was corresponding with a case manager via email. A case was opened. The failure mileage was approximately 14,000.

Recurring steering/front-end clicking/tapping noise occurs when turning the steering wheel left or right, including while the vehicle is stationary and at low speed. I first noticed the steering clicking noise at approximately 1,500 miles but initially overlooked it. The issue was previously confirmed by an authorized Honda dealer. In September 2025, the technician documented that the noise was easily duplicated while driving or stationary, used diagnostic equipment to locate it, identified the clicking/tapping as coming from the EPS steering rack assembly, and replaced the EPS rack under warranty. A later service record stated that the steering clicking problem was fixed. The same/similar clicking noise later returned. In August 2026, the dealer again documented the complaint but stated that the noise was a “normal characteristic” of the model/year rather than identifying and permanently correcting the cause. I have spent many personal hours testing, documenting, and comparing the vehicle with other similar Honda Accords. I obtained multiple comparison videos showing comparable vehicles tested under similar stationary steering conditions that do not produce the repeated clicking noise present in my vehicle. I provided this evidence and the service history to American Honda Corporate. American Honda also published a March 20, 2026 Tech Line Summary, “Clicking Sound While Turning the Steering Wheel,” applicable to 2023 and later Accord models, describing a clicking/light ticking/binding condition involving the steering gearbox/rack area. Because this recurring condition involves the steering system, I am concerned about safe vehicle operation if the underlying cause is not properly identified and repaired. My family is uncomfortable riding in the vehicle. The condition remains unresolved, and the vehicle is available for inspection. - The steering system appears to be malfunctioning. The steering wheel becomes sticky or feels like it catches while turning, and there is a clunking and grinding noise when the steering wheel is turned. The vehicle and affected components are available for inspection upon request. - The steering issue creates a potential safety risk because the steering wheel may not turn smoothly or respond as expected. The sticking sensation and grinding/clunking noises could affect my ability to safely control the vehicle, especially while turning, parking, or avoiding a hazard. - Not yet. The problem has been experienced while operating the vehicle and needs to be inspected and diagnosed by a qualified service center. -Not yet.The vehicle has not yet been inspected regarding this specific issue. -There were no warning lamps or messages displayed, if applicable. Automatic braking has engaged at least 3 times in the last year when there was no car nearby or obstacle in the road. This has happened on city streets at speeds of approx 30-45mph. If this sudden braking happened at freeway speeds it's highly likely I would be rear ended. I have not taken it to dealer as I expect that they will say what they typically do ...nothing is wrong. Granted it is difficult to reproduce the phantom braking for them to observe the issue. The alarm sounded and the large red "Brake" light came on on the dash. 8.12.26 incident was in a lane adjacent to traffic cones but I was nowhere near hitting them.

Honda dealerships are unable to address this issue without further intervention and software updates from Honda corporate. Dealer has stated that this issue may interfere with reliable over-the-air software updates, including any potential safety-related updates delivered through this OTA system.

In May 2025, I received an error message upon vehicle start up which states “Software update is not available. Please see your dealer.” This error has continued to appear each time the vehicle is started. When I took it to my dealer in May 2025, they told me that there was nothing they could do and that Honda had to put out an over the air software update to fix the issue. I have asked about this during subsequent dealer visits and each time they say the same thing. The issue has not been fixed nearly 14 months now.

I was driving an interstate freeway with my cruise on set to 75mph while passing a truck, both the truck and I were in are respective lanes and neither of us was straddling the lane divider. Suddenly the orange brake light came on the dash and the car braked hard without any input from me. The braking was so hard it pushed me forward twice and caused my seat belt to lockup.

The local authorized Honda service center has failed to resolve this threat. Below is the mandatory timeline and documentation regarding this defect: [Date of Incident #1 – XXX]: While driving at approximately 35 MPH on a open road with no obstructions, the vehicle abruptly engaged heavy emergency braking without warning. This nearly caused myself to get into a severe rear-end collision. [In-person Service Visit #1 -XXX– Paramus, NJ]: On this date, I brought the car to Paramus Honda, and was advised that their service department “has never heard of any vehicle made by Honda, phantom breaking”. Upon my insistence that this happened and that experiencing such made me feel unsafe and nervous to drive the vehicle. The service department advised me to ensure that both the rear and front sensors on the vehicle were free and clear of any type of debris. I did so and have ensured to always be of the sensors moving forward. [Date of Incident #2 - XXX]: While driving at approximately 25 MPH on a clear road, with no obstructions, the vehicle abruptly engaged heavy emergency braking without warning. My two children, age [XXX] & [XXX] were in the vehicle at this time & again nearly caused a severe rear-end collision. Phone contact- Service Visit #2 - XXX. – Honda of Paramus, NJ]: On this date, I called Paramus Honda, and was advised again that their service department “has never heard of any vehicle made by Honda, phantom breaking.” * [Date of Incident #3: XXX]: While driving at approximately 35 MPH on a clear road with no obstructions, the vehicle abruptly engaged heavy emergency braking without warning. This CAUSED a severe rear-end collision with the three vehicles behind me to all crash into each other- with 7 of the 9 people that were in those vehicles, get hurt and require medical attention & ambulance transport. The contact owns a 2025 Honda Accord. The contact stated that while driving approximately 55 MPH on a highway, the steering wheel seized, resulting in a loss of steering control. The contact was able to pull the vehicle onto the shoulder and then drove off the highway to a gas station, where the vehicle was towed. The contact stated that the power steering warning light, the brake warning light, and several unknown warning lights were illuminated. The vehicle was towed to a dealer, where it was diagnosed that the steering rack had failed. The vehicle had not been repaired. The manufacturer was notified of the failure, but no response was provided. The failure mileage was approximately 10,500.

Dashboard shows that there is a problem with the adaptive cruise control system, lane keeping assist system, road departure mitigation system, and automatic high beams. Message occurs intermittently, usually lasting a few seconds, and goes away. Can happen while driving or while parked. Usually occurs a handful or times per day. Seems to happen more frequently when engine bay is warmed up. This is a safety issue because it suddenly and unexpectedly disables the advanced driver assistance systems. For example, while driving on the highway, cruise control and lane keeping assistance will turn off suddenly, forcing the driver to intervene with all of these systems at once. This also has other implications, like automatic emergency braking not functioning correctly to help prevent an accident. Vehicle has been brought to various Honda dealers a total of three times. A case has been opened with American Honda Motor Co, but no solution to the problem has been found.

I purchased this vehicle at 0 miles in July 2025 from Honda Accord in Pensacola Florida. I reported in July 2025 that there was a noise and I was unhappy with the vehicle and would like to exchange the vehicle within the 30 days months later, the noise got louder, and I took it in to see what the problem was I was told there was no issue that this is the loud start noise when you first crank the car up I then proceed to advise them that there’s clicking under my steering wheel. I brought the vehicle back in to Honda of Pensacola they checked it and said that this was a steering box issue that had been a recall on most of the Honda Civic. I brought it back in and a tech looked at it, diagnosed it and said that it was a clock spring. They fix the clock spring and said that should fix the problem. Unfortunately, the north continue and got louder. I brought the vehicle back in and they said that they had to fix the steering box and that it was fixed and would not have any more issues. A month later, May 2026 the noise reappeared and I took it back to Honda. They are now saying they know that it is a safety issue it is diagnosed that it is the gear steering box once again, but there is no resolution to fix it. My car have been Nyos wrong three times and the issue is still not fixed. From the beginning, I was misled and told that they would have in place for me about exchanging the vehicle never happened never try to reach out or anything at this time I’m paying for a vehicle they have had thousands and thousands of dollars worth of work and no resolution to fix the main problems I am asking for help because I am a single mother thinking I’m getting reliable transportation and unfortunately I’m getting the short end of a stick with what seems to be a lemon for a vehicle I’m very unhappy and asking for help.

1. Loss of left rear wheel while driving. The vehicle is with the insurance and may be available for inspection. 2. Loss of control on a tollway with speed limit 75 mph as the wheel came off while driving, involving the vehicle collapsing on the rear left side and air bags on the left side being deployed. It could have lead to a fatal incident involving many vehicles. 3. Not yet. 4. It was inspected by the police and has currently been released from the tow yard to the insurance. 5. the steering on the '25 Hybrid Sport L is a little loose? Even small movements make the car drift, and it's hard to keep it straight. the feels like the car won’t track straight. The Lane keep assist does not work like the 10th gen. It’s like a ping pong bouncing off the lines. Who’s experiencing this as well.
