NON-CRASH ENGINE COMPARTMENT FIRES
IN A LETTER DATED AUGUST 27, 2008, GENERAL MOTORS CORP. (GM) NOTIFIED NHTSA THAT IT WILL CONDUCT A SAFETY RECALL (NHTSA RECALL NUMBER 08V-441) TO ADDRESS A SAFETY RELATEDDEFECT THAT CAN RESULT IN ENGINE COMPARTMENT FIRE IN APPROXIMATELY 850,000 VEHICLES EQUIPPED WITH A HEATED WINDSHIELD WASHER FLUID OPTION.THE SUBJECT VEHICLES OF THIS INVESTIGATION COMPRISE OVER THREE-QUARTERS OF THE RECALL POPULATION. ACCORDING TO GM, A SHORT CIRCUIT ON THE PRINTED CIRCUIT BOARD FOR THE WINDSHIELD WASHER FLUID HEATER MODULE MAY OVERHEAT THE CONTROL-CIRCUIT GROUND WIRE. THIS MAY CAUSE OTHER ELECTRICAL FEATURES TO MALFUNCTION, CREATE AN ODOR OR CAUSE SMOKE AND IN RARE CASES IT MAY CAUSE A FIRE.AS A REMEDY, GM DEALERS WILL INSTALL A WIRE HARNESS WITH AN IN-LINE FUSE. IN ADDITION TO THE SUBJECT VEHICLES, SEVERAL OTHER GM MODELS WITH THE HEATED WINDSHIELD WASHER FLUID OPTION ARE INCLUDED IN RECALL 08V-441: MY 2006-2008 BUICK LUCERNE (MANUFACTURED AUGUST 2005 THROUGH NOVEMBER 2007); MY 2008 BUICK ENCLAVE (MANUFACTURED FEBRUARY 2007 THROUGH APRIL 2007); MY 2006-2008 CADILLAC DTS (MANUFACTURED MAY 2007 THROUGH NOVEMBER 2007); MY 2007-2008 SATURN OUTLOOK (MANUFACTURED AUGUST 2006 THROUGH APRIL 2007); AND MY 2007-2008 GMC ACADIA (MANUFACTURED AUGUST 2006 THROUGH APRIL 2007) VEHICLES.GM IS INSTRUCTING OWNERS TO TAKE THEIR VEHICLE TO A DEALER FOR SERVICE IF THEY EXPERIENCE SYMPTOMS OF WINDSHIELD WASHER HEATER MODULE SHORT CIRCUITS. GM'S RECALL RESOLVES THE SAFETY DEFECT CONCERNS PURSUED BY THIS INVESTIGATION.THIS ENGINEERING ANALYSIS IS CLOSED.
HUMMER H2 STEERING KNUCKLE FAILURES
ODI UPGRADED THIS INVESTIGATION AT THE CONCLUSION OF PE04-080.THE ALLEGED DEFECT INVOLVES THE FAILURE OF THE STEERING KNUCKLE AT THE LOWER BALL JOINT BOSS, RESULTING IN A SEPARATION OF THE LOWER BALL JOINT FROM THE KNUCKLE.A LOWER BALL JOINT SEPARATION COULD RESULT IN A LOSS OF STEERING AND/OR A DISABLED VEHICLE. IT APPEARS THAT THE FRACTURES OF H2 STEERING KNUCKLES CAN BE ATTRIBUTED TO WHEEL/TIRE COLLISION FORCES BEING TRANSFERRED DIRECTLY TO THE STEERING KNUCKLE.THE ORIGINAL KNUCKLE DESIGN APPEARS TO BE SUFFICIENT FOR THE H2 APPLICATION.EVIDENCE FROM INSPECTED VEHICLES, PHOTOGRAPHS, POLICE REPORTS AND EDR DATA INDICATES COLLISION IMPACT EVENTS PLACED STRESS ON THE STEERING KNUCKLE CAUSING IT TO FRACTURE.FURTHER USE OF AGENCY RESOURCES DOES NOT APPEAR TO BE WARRANTED.THE CLOSING OF THIS INVESTIGATION DOES NOT CONSTITUTE A FINDING BY NHTSA THAT NO SAFETY-RELATED DEFECT EXISTS WITH REGARD TO THE SUBJECT COMPONENT.THE AGENCY RESERVES THE RIGHT TO TAKE FURTHER ACTION ON THIS ISSUE IF WARRANTED.

The contact owns a 2006 Hummer H2. The contact stated while driving and activating the blower motor, the blower motor was intermittently inoperable. The contact stated that an abnormal electrical burning odor was coming from the vehicle. The contact stated that upon activation, the A/C and the heating systems were intermittently inoperable. The vehicle was taken to an independent mechanic, where it was diagnosed and determined that the blower motor wiring harness needed to be replaced. The vehicle was not repaired. The dealer and the manufacturer were not notified of the failure. The failure mileage was approximately 190,000.

THE OTHER DAY I TOOK A RIDE IN MY TRUCK AND UPON DRIVING APPROXIMATELY TWO BLOCKS FROM MY HOME THE STEERING CONTROL OF THE CAR TOTALLY FAILED. IMAGINE DRIVING YOUR CAR AND AT SOME POINT, YOU LOSE THE CAPABILITY OF CONTROLLING YOUR CARS DIRECTION WHILE HOLDING YOUR STEERING WHEEL. ONE CAN ONLY IMAGINE IF I WAS GOING 65 MILES AN HOUR AND DRIVING ON A HIGHWAY THEN LOSING THE ABILITY TO TURN YOUR WHEELS. I AM BLESSED THIS MALFUNCTION HAPPENED WHEN I WAS ONLY TRAVELING 10 MILES AN HOUR. AFTER CALLING A TOW TRUCK AND HAVING MY CAR TOWED TO A LOCAL REPAIR SHOP, IT WAS DETERMINED THE CONTROL ARM BOLT HAD BROKEN OFF THEREFORE LOSING TOTAL CONTROL OF THE CARS STEERING CAPABILITY. THIS IS UNHEARD OF AND SOMETHING LIKE THIS SHOULD NEVER HAPPEN AT ALL DURING NORMAL USE. WITH ALL OF THIS I CONTINUE TO THINK ABOUT HOW LUCKY I WAS TO SURVIVE A MECHANICAL PROBLEM OF THIS SORT AND HOW I COULD HAVE FATALLY INJURED SOMEONE ELSE WITH THIS DEFECTED CONTROL ARM OF THIS VEHICLE. SO, I DECIDED TO MAKE A CALL TO GM AND REPORT THIS ISSUE AND TO ADVISE THE COMPANY OF A PROBLEMATIC ISSUE WITH THE DESIGN OR ENGINEERING OF THE HUMMERS CONTROL ARM STEERING COMPONENT THE COMPANY REPRESENTATIVE ACKNOWLEDGED MY COMPLAINT AND RECORDED MY DESCRIPTION OF THE EVENTS AND THE ISSUES OF THE CARS STEERING SYSTEM. I REITERATED MY CONCERNS ABOUT THE ISSUES AND HOW NOT ADDRESSING THIS IN SOME SORT OF A RECALL MAY RESULT IN DEATHS. I AM ATTACHING PICTURES OF THE PART WHICH THE BOLT HAD BROKEN SO YOU MAY CONSIDER A RECALL OF THE VEHICLE'S CONTROL ARM. I GUESS PEOPLE NEED TO GET HURT BEFORE YOUR COMPANY ACTS AND TAKES THIS SERIOUSLY. VEHICLE HAD BEEN DRIVEN APPROXIMATELY 2 MILES FROM START TIME, WAS STATIONARY AND UNOCCUPIED FOR APPROXIMATELY 10-25 MINUTES. VEHICLE INTERIOR CAUGHT FIRE AT THE BASE OF THE TOP SITTING AREA OF THE REAR SEAT, FLAMES FROM FIRE MELTED THE OVER HEAD INTERIOR LIGHT AND IGNITED A FIRE ON THE HEAD LINER, WHICH CAUSED THE FIRE TO SPREAD UNDER NEATH THE HEADLINER AND ROOF OF THE VEHICLE, MELTING THE EXTERIOR PAINT. THERE WAS NO PRIOR INDICATION OF BURNING WHEN DRIVING OR AFTER PARKING THE VEHICLE PRIOR TO THE FIRE. FIRE DEPARTMENT ADVISED IT WAS BELIEVED TO BE RELATED TO REAR SEAT HEATERS MALFUNCTIONING CAUSING COMBUSTION THAT LEAD TO AN INTERIOR FIRE. DOORS WERE LOCKED PRIOR TO THE FIRE, AND STORE SURVEILLANCE DIDN'T SHOW ANY SIGNS OF EXTERIOR TAMPERING WITH THE VEHICLE.
