MAZDA MX6 BRAKES ENGAGED WHILE ON A BRIDGE CAUSING THE CAR TO SKID INTO THE CENTER MEDIAN WALL. UPON THE IMPACT MY MOUTH HIT THE STEERING WHEEL AND BUSTED MY LIPS OPEN, THE AIRBAG THEN DEPLOYED CAUSING MY HEAD TO BE SLAMMED AGAINST THE HEADREST WHILE A BURST OF HOARD GAGGING SMOKE/DUST, PARTICLES, WERE EMITTED. IN A GASP OF TERROR AND FEAR THAT I MAY DIE FROM SUFFOCATION FROM THE CAR FILLED SMOKE/DUST FROM THE AIRBAG I KICKED THE DRIVER DOOR OPENED AND ROLLED OUT ONTO THE GROUND GASPING FOR AIR AND LAID THERE UNTIL AN AMBULANCE ARRIVED. THE VEHICLE ENGINE ETC. WAS NOT ON FIRE OR SMOKING FROM THIS WRECK. WHILE MY FACIAL INJURIES HEALED, THE DAMAGE TO MY LEFT JAW RESULTED IN A VERY COSTLY SURGERY SEVERAL YEARS LATER OF WHICH DID NOT REPAIR THE JAW AND NOW LIVE WITH DAILY DEBILITATING FACIAL/CRANIAL PAIN. THERE IS A SPECIFIC RANCID SMELL FROM THIS AIRBAG THAT WILL NEVER LEAVE MY MIND AS WELL AS THE TEXTURE OF THE HARD CANVAS LIKE MATERIAL THAT RIPPED THE SKIN OFF MY FACE. THE DEPLOYMENT OF THIS AIRBAG AFTER THE INITIAL IMPACT IS A VIVID IMAGE TO THIS DAY IN 2014. THIS MANUFACTURER SHOULD BE ASHAMED TO HAVE NOT CORRECTED THIS AS MY WRECK WAS IN 1995. *TR

STOPPED AT A CONVIENCE STORE, LEFT THE CAR RUNNING BUT TURNED OFF THE HEADLIGHTS. RETURNED TO THE VEHICLE, OPENED THE DOOR, SAT DOWN IN THE DRIVERS SET AND REACHED OVER TO TURN ON THE HEADLIGHTS. BOTH AIRBAGS DEPLOYED. THE DRIVER SIDE AIR BAG SMACKED ME IN THE FACE, WHILE THE PASSENGER AIR BAG DEPLOYED AND PUSHED THE PLASTIC FACE PIECE INTO THE WINDSHIELD, BRAKING THE WINDSHIELD AND FORCING THE REAR VIEW MIRROR OFF THE WINDSHIELD. BOTH SIDES OF THE HEADLINER ARE PULLED LOOSE.

ON MY 1995 MAZDA MX-6, WITH 67,000 MILES, THE TRANSMISSION HAS STARTED BEHAVING ERRATICALLY. WHEN YOU PUT IT IN DRIVE, IT DOESN'T ENGAGE IMMEDIATELY, THEN EVENTUALLY ENGAGES SUDDENLY WITH A LURCH. I HAVE READ ON-LINE THAT THIS PARTICULAR TRANSMISSION IS KNOWN TO BE TROUBLESOME. BTW, DATE IS 2003, THE FORM ISN'T UPDATED YET.

ELECTRICAL SYSTEM: INSTRUMENT CLUSTER/PANEL
In accordance with Federal regulations, follow the procedure listed below when replacing a speedometer. NOTE: Tampering with a motor vehicle odometer is strictly prohibited by Federal, and State laws. It is the dealer's responsibility to comply with Federal, State, and local laws concerning odometer and related documentation requirements when providing a repair or replacement. Mazda vehicles are manufactured with tamper-proof speedometers. When a speedometer is replaced, the new speedometer will read zero (0) miles. This procedure is extremely important to accurately represent actual vehicle mileage. Prior to speedometer removal, complete the information required on the "Speedometer Replacement Label". Labels can only be ordered through Mazda eStore. In an effort to achieve a higher level of Customer Experience and retain the value of Mazda's vehicles, MNAO Warranty Operations is allowing all Mazda dealers to utilize a third party, SPECMO Enterprises, Inc., for Instrument Panel Cluster (IPC) odometer setting service. With the use of SPECMO's services, it is possible that the odometer will be set between 10 to 31 miles over the odometer mileage at the time of replacement for both warranty and customer pay repairs. Please make sure that the customer is aware that there may be a slight variance to the odometer, in which case the customer may have to accept the slightly higher mileage. Since odometer setting service is not part of the warranty repair, it is not warrantable nor required, but optional. However, Mazda Warranty policy provides after-warranty assistance (AWA) or DSA that can be utilized to cover this service expense.

Manufacturers file copies of the bulletins they send to dealers with NHTSA. These often describe diagnostic or repair procedures for a known condition. They are not recalls: repairs described in a bulletin are usually only free if the vehicle is still under warranty or the manufacturer has extended coverage.
