The contact owns a 2022 Ram 3500. The contact stated that while driving at an undisclosed speed, the engine made abnormal knocking and ticking sounds. The knocking and ticking sounds became louder while accelerating. There was no warning light illuminated. The vehicle was taken to an independent mechanic, who determined that the piston had ingested the grid heater nut. The mechanic replaced the piston, cylinder head assembly, gaskets, rod, gasket intake, Hamilton flat tappet conversion kit, and XDP grid heater. The manufacturer was made aware of the failure. The failure mileage was 88,745.

### **NHTSA Complaint Statement of Facts** **RE: Recall Y43 (NHTSA Campaign 22V-406) – Failure of Recall Remedy** **Summary:** The manufacturer (Stellantis/FCA) is refusing to provide a complete and effective remedy for **Recall Y43**. My 2022 Ram 3500 was manufactured without the **Particulate Matter (PM) Sensor**, a defect officially acknowledged in Recall Y43. This factory omission has caused terminal blockage of the **Diesel Particulate Filter (DPF)**—a specific consequential failure documented in the manufacturer's own recall filing. **Technical Argument:** 1. **Causal Link:** The official Y43 recall notice states: *"The PM sensor... may not have been installed... This could increase vehicle emissions and **block the diesel particulate filter**."* My vehicle currently suffers from the exact "blocked DPF" condition described. 2. **Ineffective Remedy:** Stellantis has offered to install the missing sensor (the Y43 part) but refuses to replace the ruined DPF. Installing a sensor on a filter already terminally blocked due to the sensor's absence is not an "effective remedy" under the **National Traffic and Motor Vehicle Safety Act**. 3. **Warranty Admission:** Per **TSB 09-014-22 (X97)**, the manufacturer has issued a 1-year unlimited mile warranty extension for the DPF specifically for vehicles affected by Y43. Refusing to replace a filter that failed *before* the recall was performed is an attempt to circumvent this warranty obligation. **Safety Risk:** The vehicle is currently in "Limp Mode" with a "Filter Full - Power Reduced" message. This creates an unreasonable safety risk of **stalling or loss of power** while operating on public highways, a condition explicitly mentioned in the Y43 safety report as increasing the risk of an accident. I am seeking an order for the manufacturer to replace the DPF as part of the Y43 recall remedy at no cost to the consumer. I own this truck and have had issues with the exhaust regeneration process for about 6 months. I have taken it to the dealer 3 times and they have done diagnostics and replaced the diesel particulate filter, but the issue continues to happen. They have now told me that the system is not reparable and they don't have support from RAM manufacturer to troubleshoot and repair this issue. They have multiple customers with the same issue with the diesel exhaust system not operating properly, hampering the engine performance, damaging the engine and exhaust system, polluting from the exhaust discharge. This is a widely known issue with the 2022 Ram 3500 from other owners, yet RAM refuses to properly address the issue. They advertise their trucks with certain horsepower and output specs, which aren't accurate due to their faulty exhaust systems. Consumers are now completely removing the factoring exhaust systems because they damage the engines and cost thousands of dollars to attempt to repair, which many times never work. This needs to be addressed with RAM and recalls put in place to address the failures with their exhaust systems. My truck has 28,000 miles on it, 3 years old, and is not operable. The dealer has had 3 attempts to repair this and is now turning me away. Please help! Thank you

The contact owns a 2022 Ram 3500. The contact stated that while driving approximately 10-15 MPH, there was an abnormal rattling sound coming from the vehicle. The engine warning light was illuminated. The local dealer was contacted, and it was determined that the grid heater bolt had melted and fallen into the engine. The vehicle was repaired after two months due to the piston being on backorder. The manufacturer was contacted, but no additional assistance was provided. The approximate failure mileage was 35,000. The VIN was not available.

***1*** The emissions system on this truck (2022 Ram 3500 with 34,435 miles) has been problematic since I acquired it 5 months ago. It continually attempts to regenerate and rid itself of ash/soot buildup. If it does not successfully regenerate, it will go into a state of reduced power, also known as limp mode. The truck is available for inspection and will be at Lithia CDJR in Corpus Christi tomorrow, 07 Jan 2026. ***2*** Today specifically, the truck flashed a message EXHAUST FILTER NEARING FULL SAFELY DRIVE AT CONSISTENT SPEED TO CLEAR. I got on the highway to achieve this and avoid lights and stop/go traffic. It typically takes around 25 minutes of consistent driving for it to clear. But today, shortly after I reached highway speeds, the truck entered limp mode, displaying EXHAUST FILTER FULL REDUCED POWER SEE DEALER. This was a very alarming incident that created a bottleneck on the highway and presumably for me to nearly be rear ended as it happened at 5:35pm. I was able to safely make it off of the highway, however this incident has put the lives of all motorists on the road around me today in danger. ***3*** The truck has been inspected numerous times by the dealership. The Diesel Particulate Filter was replaced on 01 October 2025 (this was the first time the truck went into limp mode on me). The frequent regenerations continued after the DPF replacement and a sensor was replaced on 29 October 2025. Regens continue at approximately 1 per week. ***4*** This is a known issue amongst the community of RAM truck owners. It's unfortunate that many do not report it because the safety issue could easily set off a chain reaction, especially if this heavy duty truck were to be towing a heavy load as it capable of doing.

The check engine warning light was illuminated. The vehicle was taken to a local dealer, where it was diagnosed; however, the cause of the failure was not determined. In addition, the contact stated that the vehicle was taken to the same dealer approximately eight times, and a loaner vehicle was provided; however, after several unknown repairs, the failure reoccurred. The vehicle was previously repaired under warranty. The manufacturer was made aware of the failure. The failure mileage was approximately 2,000.

The # 4 cam bearing seized to the camshaft. Upon further investigation the #4 main bearing was damaged from metal chips that were not removed during the manufacture of the engine. This damage from the manufacturing defect left the engine block damaged beyond repair. So a short block was ordered and installed.

My pm sensors or particulate matter system malfunction and failed which lead to emissions issues and created my vehicle to drive in limp mode. Limp mode basically slowed downs the vehicle is slow as 5 mph as soon as this light displays at any time while driving on the highway. This put my safety in danger and the public as I transport what dot considers a type hazardous material. The sudden loss of power in the middle of the road has made vehicles around me extremely vulnerable to fatal collisions. I took my vehicle to the dealership where they confirmed this malfunction and problem with my vehicle but no recall has been issued as of yet no warning lamps appeared prior to the failure.

The contact owns a 2022 Ram 3500. The contact stated while driving 40-45 MPH and hauling a 14-ft loaded trailer, the engine made an abnormally loud knocking sound. The check engine warning light was illuminated. The contact was able to pull over to the side of the road. The vehicle failed to restart and was towed to the residence, where a mobile independent mechanic diagnosed that the grid heater nut had melted and entered cylinder #6. The vehicle was then towed to the dealer, where it was diagnosed that the grid heater nut failure had caused catastrophic failure of the pistons, turbo charger, and engine. The dealer determined that the engine assembly needed to be replaced. The vehicle was not repaired. The manufacturer was notified of the failure and informed the contact that the repair could not be covered under warranty due to the mileage on the vehicle. The failure mileage was approximately 112,700.

Emission system failure. Code P2459 Regeneration Frequency. Overheating of the DPF while stuck in regen cycle causes burning smell and diesel fumes in the cabin while driving. Suspected vehicle on fire. Date of incident 01/25/2025. Vehicle at dealership for repair 01/28/2025. Dealership repair unsuccessful following STAR approved process of elimination. Vehicle returned to dealer 02/18/2025 for same issue. Many customers with 2019-2022 diesel engines having same issue. No known fix according to dealership.

On 9 September, while driving, I got an emergency dashboard notice that stated Throttle System Malfunction contact dealer. It went out and then another immediately flashed that stated Exhaust System Malfunction contact dealer. The vehicle lost power within seconds after the second notice. I coasted to a stop on the side of the road. Once stopped, smoke quickly started coming out of the engine compartment and then flames. I contacted 911, an engine arrived but the truck was fully engaged and completely burnt.

The engine stopped while driving with no warning and provided no opportunity to exit the travel lane of the road. No warning lights were illuminated on the dashboard and all gauges appeared normal leading up to the event. This event is extremely frustrating, but I am thankful that it didn't happen on the interstate as there is no telling what the outcome may have been......... Leading up to the event, I had been driving on the interstate around 70mph for about 180 miles since the last restart. I exited the interstate and made it about 3/4 miles on a secondary road and the engine died without warning and the truck came to a quick stop in the lane of travel. No abnormal sounds were observed, just a sudden unwelcome deacceleration that did not provide enough time to exit the lane of travel. It was like stepping on the brakes. I attempted to restart the truck and the starter would not crank the engine and briefly pulled enough voltage to kill all the electronics. I attempted starting it again with a jump box attached and met the same result. The computers freaking out from the low voltage during the starting attempts wouldn't allow me to put the truck in neutral to move it out of the lane of travel. A police officer called for the tow truck on rotation since I was stuck in a live travel lane. The truck was towed to the Dealer on 8/19/2024 after the service department had closed and still would not turn over when removed from the tow truck. There were no signs of any fluids leaking from the truck after the event........ The next day the Service Advisor called and advised there were only 3 codes stored related to low voltage. Truck is waiting for a tech to become available to further investigate.

I was driving coming to a stop for a red light and it turns green. As I go to press on the gas my dashboard lights up with error codes stating a 4WD System error of some kind. As I go to press on the gas the truck loses power and acceleration and starts making a horrible clunking noise. I took it to the dealer because it seems like a transmission issue. This is a 2022 Ram 3500 with 9300 miles at time of failure. I would hate to see what happens if I was towing a large load to lose power all of a sudden risking my life and others around me. Based on what I have researched it seems like a K1 Snap Ring issue to the T. Waiting on the "official" diagnosis from the dealer.

The dealer and manufacture have not been able to fix emissions issue. It has cause severe fuel saturation in oil and has cause severe motor damage. No one cares and they brush it off as no big deal.

I have tried to get recall fixed they tell me dealershot is only allowed to receive 1part every week they tell me i cant drive my vehicle due to the fire hazard but will not provide me a vehicle i am going on driving vaction with my 2 young children how am i supposed to survive without a vehicle. They have parts asked them to expedite part they told me know. I make all my payments on ths vehicle uncertain why they cant provide me parts or a loaner vehicle thanks rich weber

hello, on the truck, a review was opened, they took the truck to the dealership, they didn’t even give us anything in return, since they were in california themselves from new york for another 2 weeks, they waited in line, they said to replace the transmission and something light to do, more than a month has passed, the costs will grow hotel trailer work we don’t know what to do, they don’t pick up the phone by the truck, they take 80 thousand each and the attitude is terrible

Transmission failure while towing a travel trailer on a busy street on a slight incline. While traveling about 45 mph felt a jolt and truck lost power and would not accelerate. Felt like truck was in neutral. Truck would not engage Drive and was completely disabled in the number one lane. Tried to re engage the transmission and had success at engaging a higher gear with slow speed and high RPMs causing the engine and transmission to overheat and possibly start a fire. Safely moved across lanes to shoulder and stopped. The street is 55mph. Cars were slamming on their brakes to avoid colliding into my truck or trailer. This happened on March 30, 2023 and has been at the dealership for repairs since and has not been even inspected yet yet. No indicator or warning lamps prior to failure.

DEF warning: see dealer. 125 miles until 5mph max speed. Happened 250 miles to nearest dealer during winter storm in SD and NE. Stranded at night, in storm, in freezing conditions, without available help. This is a potentially fatal situation due to failure of new vehicle despite preparation per vehicle manual. DEF tank 2/3 full, DPF gauge at 0. Eventually towed 135 miles to dealer to have DEF code reset. This is dangerous and very common.

The vehicle had been parked in the street for several hours. I started the vehicle in order to back it into my driveway for the night and then turned it off (total run time for the operation was most likely under a minute from the time the engine was started until it was shut down). The temperature was relatively cool and I did have to wait for the grid heater to cycle before the engine started. I did not notice any signs, warnings, safety indicators, or dash lights to alert me to any possible problems. Upon exiting the truck I detected a very faint and quickly disipating odor of hot electrical components, similar to a fourescent light ballast that has gone bad. I looked around but didn't see anything. I began retrieving some tools from the rear passenger side of the truck and smelled the same odor but stronger. Over the next 30-40 seconds I began to search for the source and realized that white smoke was now pouring out from under the hood of the truck. I was able to get it to start and pulled it into the street just prior to flames coming from under the hood near the drivers windshield. 911 was contacted as I tried to remove any belongings from the cab. The truck is now a total loss due to fire most likely. The fire and police responded to put the vehicle out but it was almost completely engulfed on their arrival. My insurance company is attempting to gain possession of the vehicle to begin their investigation. The Vehicle has approx 16,900 miles and was Serviced for safety recall Y76 for risk of engine fire on September 21st of 2022. The vehicle had 14,192 miles when it was inspected and passed according to the recall standards at the time. The vehicle has no other history of repairs or service items and has had no other mechanical or electrical problems to date. This was a very close call that could have resulted in the loss of my home, all my possessions, and risked harm to myself and my family if I had not stopped to get tools from the cab.

Diesel Particulate Filter (DPF) Sensor never installed, digital gauge has never read anything but 0%. This can cause issues with regeneration of the DPF that will result in Limp mode.

My truck had around 230 miles on it and I experienced a thumping noise from the engine compartment. The thumping disappears once the truck reaches operating temperature and the EGR valve opens. Dodge claims they have no idea what the cause is. Additionally, there is an electrical draw. I have to jump start my truck every morning. The claim it is the amplifier. I pulled the fuse overnight and still had to jump start the following morning. I’m getting the run around.
