Driver assistance

Two separate incidents occurred on July 10th and July 12th 2026 involving adaptive cruise control. While adaptive cruise control was engaged my car slammed on the brakes for no reason. There was nothing in the roadway to cause abrupt braking. The second time it occurred my car was close to being rear ended by a vehicle driving behind me. I took the car which is still under warranty to Tesla who confirmed there is a known firmware issue. No time frame was provided as to when the issue will be fixed.
Whilst driving on highways which feature multiple speed limits - eg. 70 cars, 65 Trucks, 60 Buses. If full self driving more or when in adaptive cruise control - the car systems fail to recognize the restriction linked to the lower speed limits. As a result, you can be driving on Full Self Drive at 75mpm, the car sees a 65 followed by a 60 speed limit (neither applicable to cars), but the car brakes hard dropping the speed from 75 to 62 almost instantly and, on more than one occasion, a closely following car has almost collided with me. Until such time as the Tesla system can recognize the 'sub text' on a speed limit - the FSD and Adaptive cruise should not be certified. I also drive a BMW i4 and this reads all signs correctly, only applying those applicable to a car to the vehicle.
Steering

The steering rack in my 2023 Tesla Model 3 is defective and I can show it for inspection. 1.5 years ago it failed and the torque sensor in the rack is showing that there is a constant demand (even when parked and my hands off the wheel) to turn the wheel to the right that you can see in the cars diagnostics menu. I took it in and showed the service advisor the raw data and took a drive with the tech who both acknowledged the data and the car wanting to drive off the road. They did an alignment on the car but the issue still persists even after I told them the night I drove it home in their survey. I live on very narrow country roads with little run off room and often times less than foot between cars as they pass. Several times I would get the 'corrective steering applied' warning as the car falsely thinking I was turning to drive off the road on the right and applied steering to pull the car left. A few of these times it was right as a oncoming car was approaching almost causing me to run into other cars multiple times because you have to get close to the edge of the road to make room for other cars to pass. I took it to a independent service center who confirmed that the steering rack torque sensor is at fault. I took it back again this week, showing them that the car is turning to the right 30-45% easier than to the left and they did another alignment with the same exact outcome. The car wants to drive right off the road.
While making a right turn from a near-stopped position, the steering wheel became stuck or extremely difficult to turn. The vehicle initially turned right normally, but when I attempted to steer back left/counterclockwise to complete the turn and straighten the vehicle, the steering wheel did not respond as expected. This caused the vehicle to continue in the wrong direction and resulted in a collision. I believe the electronic power steering assist system malfunctioned or failed. My concern is that the power steering assist experienced an overstress condition, causing a loss of power steering assist after the vehicle almost reached a stop and then accelerated again. This put my safety and the safety of others at risk because I was unable to steer the vehicle as intended during a low-speed turning maneuver.
Brakes

-I was driving down into an indoor basement level garage very slowly as there is a steep incline. It descends from street level to basement level within a distance of approximately 30 to 35 ft. At the end of the incline is a sharp curve to the right, followed by a sharp left turn to proceed straight. -While on the incline, the car was slowing to a stop. I gently tapped the accelerator to keep it moving. -The car suddenly lunged forward, rapidly accelerating. The acceleration was not consistent with the amount of accelerator input I applied. -I quickly steered right to avoid hitting the wall, then immediately steered left to avoid hitting parked cars and garage columns. -I braked as hard as possible, pumping it. The brake pedal was stiff. The car kept moving forward despite my efforts and did not stop until I crashed into a parked car at the end of the garage. -My son was in the front passenger seat. We both had seatbelts on. Other than whiplash, there were no major injuries. -There were no warnings, messages or any indications that a problem was present prior to the incident. -There was no one in the parked vehicle that was struck and no other people in the garage. -My cell phone automatically alerted emergency services. The police responded, and I reported the incident.
Regen braking sporadically not working when going down mountain or steep roads. Sometimes they work and sometimes they won’t. This is after extensive driving on warm days while battery is between 70-80%. Support could not help me and they don’t have any technicians available. I’ve sent them videos and time stamps and they are ignoring the issue and unable to help due to lack of knowledgable representatives This is a serious safety concern
Electrical

The vehicle was purchased pre-owned directly from Tesla. On the day of pickup and five days later, while operating on Full Self-Driving (FSD), the vehicle phantom-braked abruptly and then dangerously accelerated through a solid red light into cross-traffic intersections. The driver had to forcefully override the vehicle to prevent a crash. Tesla Service Center was notified but dismissed the issue, performing only a camera recalibration and charging a fee. Following the service appointment, the red-light running behavior stopped, but the vehicle continues to suffer from aggressive, sudden phantom braking multiple times on every single trip, creating a severe and continuous risk of a rear-end collision on public highways.
Full Self Driving software runs a red light in complete disregard of traffic laws.

Vehicle: Tesla Model 3 Issue: Power trunk failed to detect obstruction Date of incident: July 5, 2026 Location: Surprise, AZ During normal use of the vehicle, my child pressed the “close trunk” button while I was leaning into the trunk area. The powered trunk began closing and did not stop when it made contact with my head. Instead of reversing direction, the trunk continued applying downward force until I moved out of the way. This indicates a failure of the trunk’s obstruction detection system, which is supposed to prevent injury by stopping or reversing when an object or person is detected. This malfunction poses a significant safety risk, especially to children who may accidentally activate the trunk or to adults loading items. A powered trunk should not be able to continue closing on a person’s head. I request that NHTSA investigate whether this is a defect in the Tesla Model 3 trunk sensor system and whether similar incidents have been reported.

There's a problem with the airbags, and the steering wheel is hard to turn.
Suspension

A bolt securing the rear subframe assembly is loose and missing its nut. This condition has caused damage to the rear subframe assembly, which now requires replacement. The vehicle was inspected by a licensed dealer who confirmed on video that no impact damage, scraping, bending, or collision damage is present on any surrounding components. The only affected area is the bolt and nut connection point. The dealer's professional assessment is that this is consistent with a factory assembly defect — either the nut was never installed or was not properly torqued during manufacturing. No routine service or alignment would require access to this fastener. Tesla's service records for this VIN should confirm no prior work has been performed in this area, meaning the fastener has not been touched since the vehicle left the factory. Factory-installed suspension bolts of this type are typically secured with a locking mechanism or thread locker; a properly installed nut does not detach through normal vehicle operation. This constitutes a safety defect. A rear subframe fastener that was not properly secured at the factory could lead to suspension component failure, loss of vehicle control, and a serious accident. Tesla's service center has denied warranty coverage for this repair despite the vehicle being within its warranty period, citing that "a manufacturing defect would have presented itself earlier" and that because I am not the original owner, they "don't know what might have happened." Tesla quoted $2,812.49 to replace the damaged rear subframe assembly.
Compliance arm bolt securing it to the subframe is fractured. Tesla has dismissed my ongoing concerns of vibration and instability while driving. Stating they performed an in depth inspection of all suspension and subframe components and found nothing of concern during last service appointment. I did my own inspection and found the fractured bolt. Sent photo to service and they are trying to play it off as normal and safe to drive. Contacted several manufacturers and mechanics stating otherwise. Told this will end in catastrophic failure and loss of vehicle control but apparently Tesla wants me to drive it to the service department for next service
Visibility

The contact owns a 2023 Tesla Model 3. The contact stated that while driving at an undisclosed speed, a rock struck the front windshield, causing a crack. The contact stated that the front windshield had previously been replaced for the same failure. The vehicle was not diagnosed or repaired by an independent mechanic or dealer. The manufacturer was made aware of the failure and a case was opened. The failure mileage was approximately 23,000.
Windshield is defective which appeared crack without any solid object hitting incidence.

The contact owns a 2023 Tesla Model 3. The contact stated that upon starting the vehicle and shifting to drive(D), the front trunk opened inadvertently, blocking the visibility of the roadway. The contact stated that the failure had recurred several times upon shifting to drive(D), which required the contact to exit the vehicle and manually close the trunk. The vehicle was taken to a Tesla service center and to Tesla Service Sunnyvale(680 E El Camino Real, Sunnyvale, CA 94087), where unknown repairs were performed several times; however, the failure persisted. Additionally, the contact stated that while attempting to reverse out of the garage, the contact was unaware that the garage door was closed, and the vehicle hit the garage door. The contact did not recall whether the rearview camera activated; however, the contact stated that the rearview camera pedestrian warning alert failed to engage prior to the incident. The contact stated that the garage door and the rear end of the vehicle were damaged. The manufacturer was contacted through the Tesla mobile app and advised the contact to take the vehicle to a Tesla service center for service. The vehicle was not diagnosed or repaired. The failure mileage was approximately 10.

Purchased 2023 Tesla Model 3 Performance in January 2024. On [XXX] at approximately 2 pm, I got a flat tire drivers side front on a remote highway outside of Reno Nv. At that time, I found out that I didn't have a spare tire or jack supplied by Tesla. I called Tesla Roadside Service and was told before they would send out assistance, I had to agree to purchase a new tire at 540.00 per tire, the towing company couldn't be to my location till after 6pm. I declined Tesla service. The Tesla had less than 18000 miles on it. My son came and assisted me in removing the flat and we took it to Les Schwab Tire center where I purchased a new tire for 375.00 dollars. Les Schwab stated that the wear on the tire was caused by alignment. I contacted Tesla made a service appointment for alignment for August 26, 2024. I got another flat on the drivers rear on [XXX], which cost 283.00 from Les Schwab with the same disposition as caused by alignment. I took the car in the next morning and had the remaining two tires changed as it was no longer safe to drive 766.00 cost, and alignment was to blame mileage was documented by Les Schwab at 18792. When I went for the service at Tesla, I was told they do not warranty tire or alignment and when I told them that I would let small claims decide if that the tire wear was excessive and caused by improper alignment, they threw me out of the dealership and have refused to work on my car. The main reason for filing the complaint is none of this was disclosed when I purchased the car, every car I have ever bought has come with a spare tire, jack, and tools needed to change it. There are no other tire repair shops that can do Tesla alignments, Tesla offers no warranty on their tires and Les Schwab stated even their lowest cost tires come with a 20k mile warranty. Tesla is price gouging for tires 540.00 each and alignment 275.00. Any of the Tesla forums state that this is happening thru all models and tire sizes to most owners. Documenting a sudden unintended acceleration experience. Today while driving and trying to stop for a red light, I lightly applied pressure on my brake pedal to brake for the red light. Midway through the braking, my car lunged forward for about half a second. Went through the dash cam footage, and I noticed that the footage skipped half a second as well. Seems like something electrical that caused the car to lunge. This is a huge safety issue.
Vehicle was in cruise control, and cruise control did not deactivate causing the vehicle to propel into a building, causing airbag deployment. Safety sensors of car failed to recognize building, other vehicles, and obstructions as required.

Incident Summary: On [XXX], I was in an accident and dropped my Tesla Model 3 at Tesla’s North Phoenix Collision Center in Nov 2024. I received the car back in Dec/Jan. Immediately, I noticed dangerous swaying, bouncing, and veering when hitting dips—especially on the freeway. The car was unstable and unsafe to drive at high speeds. In Feb/Mar, Tanner (assistant manager at the collision center) drove the vehicle with me in the passenger seat. After hitting a dip on the freeway, he panicked, exited, and agreed something was wrong. I was later told they did a wheel alignment, but the issue remained. This was the second time they returned the vehicle in an unsafe condition. I nearly crashed due to how unstable it felt. In May 2025, I brought the car back again. Tanner suspected the steering column but said no diagnosis was done. He gave me a loaner vehicle, which had a flat tire. I had to drive my own unsafe car for nearly a month while waiting for another loaner and parts—no resolution during that time. On 6/13/2025, I contacted the Tesla Glendale Service Center. The advisor expressed frustration, saying this collision center often offloads unresolved issues. I also contacted Tesla Support (877-798-3752), who advised me to file this NHTSA complaint. As of June 2025, the issue is still unresolved. My car continues to sway and veer dangerously on the freeway, and Tesla North Phoenix Collision has failed to properly diagnose or fix it despite multiple returns. Rims cracked with normal usage. Current mileage under 17,000. This should not occur, and poses a safety risk with loss of tire pressure.

COMPONENT: Battery and Charging System SUMMARY: 2023 Tesla Model 3 Long Range lost 15% battery capacity (345→300 miles) in 2 months at 9,816 miles. Comparison vehicle shows this is a defect, not normal wear. DESCRIPTION: Vehicle: 2023 Tesla Model 3 Long Range, VIN: [insert VIN], 9,816 miles Defect: Between September 2025 and November 2025, displayed range at 100% charge dropped from 345 miles to 300 miles—a 15% loss in just 2 months. Evidence This Is A Defect: I own a second 2023 Model 3 (base model) with 66,680 miles that only dropped from 272→260 miles (4.4% loss). The Long Range with 7x FEWER miles has 3x MORE degradation, indicating a battery pack defect. Service Refusal: Tesla Service ran “range analysis” but refuses to test battery capacity or check for dead cells. Only provided scripted responses about EPA vs real-world driving, which doesn’t explain why my high-mileage vehicle has healthier batteries. Safety Concern: Rapid cell degradation may indicate failing cells, thermal management defect, or fire risk. Unpredictable range loss creates stranding risk. Request: Investigate 2023 Model 3 LR battery packs for systemic defects. Require Tesla to perform cell-level diagnostics and replace defective packs. Documentation available upon request.
I was turning onto a busy street and the car suddenly stopped and would not respond to the gas pedal. A message said "shift into drive". I had been driving for about a mile already and had not touched the shifter. I then tried to shift into drive and it would not change. I tried multiple times. I then opened the driver door, shut the door, pressed the break to put in my pin, and then it would shift into drive. This was very dangerous as it stopped with the car partially in the road. Luckily there were no cars around.

Turn signals shut off after three blinks with no lane change or steering wheel movement. Lane departure / assistance fails to function. Blind spot monitor / alert fails to function. Adaptive cruise control / braking fail to function. Problem first reported to Tesla on 7/6/2023. Telsa installed software update to resolve remotely. Problem recurred 8/6/2023, towed to Tesla 8/7, Tesla replaced side camera assembly. Problem recurred 8/132023, software update installed & resolved. Problem recurred 8/19/2023. Vehicle to be towed to Tesla for further diagnosis / repair.

Tires & wheels

The contact owns a 2023 Tesla Model 3 equipped with Pirelli Tires, Tire line: P Zero, Tire Size: 235/35/R20, DOT Number: (Not available). The contact stated while driving 65 MPH an hour after purchasing the vehicle brand new, the rear passenger's side tire experienced a blowout, and the rim was severely damaged. The vehicle was towed to a Tesla Service Center, where the rear passenger's side wheel and tire were replaced; however, the rear driver's side tire experienced a blowout a week later. The vehicle was towed to the Tesla Service Center. The manufacturer was notified of the failure and informed the contact that he would be charged for the repair. The tire was not replaced. The tire and vehicle failure mileage was approximately 80.
