Each component in detail
What the reports in each category say, in the owners' own words
Driver assistance
Reports naming forward-collision avoidance, lane keeping, back-over prevention, cruise control and related electronic driver-assistance systems.
From owner reports, verbatim
When disengaging Full Self Driving (FSD), a feedback notification pops up on the bottom left corner of the infotainment screen that fully blocks the media control panel and does not disappear until the driver provides feedback to Tesla for why FSD was disengaged. By forcing the driver to answer the feedback question in order to access the media controls, Tesla causes a distraction to the driver, who is forced to take their eyes off the road and select a feedback response to why they chose to disengage FSD, increasing the chance of an accident. This is an intentional but dangerous design decision.
When cruise control is engaged on a Tesla (all models to my knowledge) the Auto Wipers automatically come on and then randomly wipe. There is no option to disabled the Auto Wipers, as you can only disable by manually selecting control panel and then turn off each time you engage cruise control. This creates a significant risk to driver safety as I have had numerous instances of wipers randomly starting (without any rain) and smearing the windshield. This forces me to look away from the road and go through a manual disable every time I use cruise control. NOTE: this is a software default that cannot be removed
Electrical
Reports naming the electrical system — wiring, battery, charging, control modules, infotainment and electrical accessories.
From owner reports, verbatim
I am writing to report an accident involving my son who was driving Tesla 2024 Model-3 on July 18th 2026 approximately 10:15 AM EST. He was returning home from his volunteering with Emergency medical Service(EMS) when the vehicle's steering wheel suddenly froze. while attempting to maneuver the vehicle to safe location , it collided with a roadside barrier. fortunately , my son was not injured , but the vehicle sustained significant damage, including deployment of airbag and damage to front passenger side. this incident occurred at a speed of approximately 26 MPH
Rear passenger door electronic release did not work. Technician indicated rear door harness corroded making the door inoperable.
Steering
Reports naming the steering system — racks, columns, power-steering assist and steering linkage.
From owner reports, verbatim
On July 18, 2026, at approximately 10:00 AM EST, my son was manually driving a 2024 Tesla Model 3 at approximately 26 MPH when the steering suddenly became extremely stiff and difficult to turn. He attempted to steer away from a roadside barrier but was unable to turn the steering wheel sufficiently, resulting in a collision. There was no warning or indication observed on the dashboard before the steering suddenly became extremely stiff. Autopilot, cruise control, and Full Self-Driving were not activated; the vehicle was being driven manually. Fortunately, my son did not sustain any physical injuries. However, a sudden loss of normal steering control is a serious safety concern and could potentially cause severe injuries to the driver, passengers, pedestrians, or occupants of other vehicles. After the accident, the vehicle was towed to the insurance-designated facility and is currently being examined by the insurance adjuster. Within a few minutes of the accident, the vehicle also became completely locked, preventing us from retrieving personal belongings. I previously reported this incident to NHTSA under complaint number 11751698, but all required documentation was not available at that time. I am therefore submitting the complaint again with the additional information and documentation now available. I respectfully request that NHTSA investigate this incident and determine the root cause of the sudden steering stiffness and whether there is a safety-related defect or condition that could affect other vehicles.
The steering wheel turn signal buttons on my Tesla Model 3 intermittently becoming unresponsive or getting stuck signaling left or right. This prevents me from reliably signaling lane changes or turns. This is a safety risk because I may be unable to properly signal maneuvers as required under Maryland Vehicle Law §21-604, potentially confusing nearby drivers and increasing the risk of an accident. As this is a moving violation, I can be fined and or points on my driving record The issue has occurred multiple times during normal driving but has not yet been formally confirmed by a service center.
Exterior lighting
Reports naming headlights, tail lights, turn signals and other exterior lighting.
From owner reports, verbatim
The push type turn signals for the model 3 almost caused when accident when I had to enter and exit a roundabout. The orientation wasn’t clear and I was trying to signal to not exit instead - another car misunderstood my intent. this miscommunication lead to a close call. I’ve been driving for over 20 years - this is an unsafe implementation of something basic.
To whom it may concern, I am reporting an incident regarding my Tesla Model 3 (2024) that occurred on June 30th around midnight. My partner was using FSD (full self driving) and driving down i5 towards Vancouver, WA and I was attempting to turn on Bluetooth to listen to music when the exterior lights completely shut off. The section of the freeway we were on had no lights, so it was completely dark. The FSD made my partner take over the wheel and we careened off the road because there was zero visibility. Thankfully and luckily we only hit a traffic reflector. What followed has been a few concerning months of back and forth with Tesla. I reported the incident to them immediately and requested data from that night. The data that they eventually sent over was from the previous day and following days, and conveniently missing that crucial period of time. The service appointment I had with them produced a multiple thousand dollar bill to fix the damage caused by the traffic reflector which I did not pay. I have not heard back from them since then. I am reporting this incident because I don’t think lights should be able to shut off completely at night, whether it was a software malfunction or someone accidentally pressing a button on the screen. If this incident continues to happen in the future many passengers may not be as lucky as us to escape with only a scratch. Airbags
Reports naming the air-bag system — deployment behaviour, inflators, sensors and warning lights.

SUPPLEMENT TO PREVIOUS COMPLAINT (VIN: ...XXX): CRITICAL NEW EVIDENCE OF MULTI-POINT SAFETY FAILURE. On Dec 10, 2025, Tesla Service performed a full diagnostic and repair of the airbag safety system. In addition to the previously reported "damaged wires" in the passenger seat (OCS Module), the technician ALSO had to replace the Steering Column Control Module (SCCM) to resolve the active safety faults. Safety Implication: The vehicle was sold with simultaneous hardware failures in both the passenger occupant sensing system AND the driver's steering column airbag controls. This confirms widespread physical/electrical trauma to the vehicle's Supplemental Restraint System (SRS) that was concealed at the time of sale. The dealer released a vehicle with compromised safety hardware in both the driver and passenger zones. I have attached the official Tesla Invoice confirming these replacements. Suspension
Reports naming suspension components — springs, struts, shocks, control arms and related hardware.
From owner reports, verbatim
Creaking noise coming from front of car when going over speed bumps or driving through a dip at low speeds. Tesla SC says normal but many others have replaced lateral and compliance links and issues has been solved. Known issue and raises a safety concern over safety of suspension.
The tesla service center in Ridgepoint drive in Austin TX has repeated safety issues and almost cost my wife her life in the tire inspection and undercarriage check after a full service including full tire failures after 4 tire checks and in building service center.

The Michelin tires used for this particular model have a foam issue. This foam is put by Michelin to reduce road Noice and they are coming apart thus creating a rattle in the car making it a hazard while driving on the highway. Tesla replaced one of my tires on May 27, 2025 free of charge and told me it was a known issue. Today 2/25/26 I have the same issue and they're quoting me almost $400. I told them about the tire issue and why there's no recall and they told me to contact NHTSA. They just want to change them one at a time until warranty expires so they don't have to replace all defective tires. I guess they're waiting for somebody to have an accident and sue them to fix the problem.

When driving vehicle over the past month, vehicle turn signals on the steering wheel fail to respond to commands and feel almost as if the controls are locked out by a steering wheel software fault. Upon further inspection, checking service mode, found the SCCM service alert to correlate with the exact moment of turn signal unpressability directly. No debris or liquid has ever entered the area, and this issue has gotten so bad that it happens 3-4 times over a 45min drive I cannot activate the turn signals at all when the failure occurs. There is no alternative way to activate them. The steering wheel is the only way to activate the turn signals and the fault does not permit me to use either turn signal when attempting to change lanes on the highway. Tesla reports that it's a normal software bug and to "deal with it" because there's nothing they can do to fix it. This is directly indicative of a known defective part, and this is a safety-critical defect where a recall must be issued to resolve. Numerous units are impacted through research of same failures. I cant use my turn signals when attempting to change lanes, and Tesla does not want to fix/refuses to address this issue. Even through a phone call from Paul from the service center in Orchard Lake, MI, reported that the condition is a "known defect with INTERAL DOCUMENTS" that confirm it, but he cannot share the doc with me, and "that the idea the issue of getting pulled over for not being able to use your turn signal is an imaginative concern." This, is inaccurate, as the inability to actuate turn signals is a safety-critical failure. the service center replaced my steering when and SCCM module, however the issue persisted after the visit, and the service manager of the West Bloomfield, MI service center called me 12 hours before my appointment to deter me from dropping the vehicle off, and then told me this would be a customer pay diagnostic for a warrantable safety failure.

Tesla has told me the AP4 computer has failed in my car which has resulted in them having to replace it. This issue impacts: emergency braking, blind spot detection, lane keep/centering, assisting with parking, automatic high beams and wipers, cruise control/ adaptive cruise control, Vehicles built in GPS to be stuck at one location every time, has made my touchscreen not automatically brighten or dim, my auto dimming mirrors be too dim in most driving conditions, constantly fail to install system improvements and safety updates, and because my vehicle keeps trying to do this, it has caused increased battery drain as well. This has increased the safety risk of my vehicle tremendously... Tesla has not seemed to publicly acknowledge the issue yet either.

on Jan 4th 2026 my car popped a waring RCM2_a700 Front Right Safety Restraint System Issue - I took it in Jan 5th 2026 they said they could not replicate the warning and it was a GLITCH. On Jan 10th 2026 the warning popped up again
NHTSA Complaint Narrative (Copy & Paste) CRITICAL SAFETY DEFECT: SALE OF VEHICLE WITH DISABLED AIRBAGS Incident: 10/24/2025 | Dealer: Braman Cadillac (Miami, FL) | VIN: [XXX] Braman Cadillac sold me this 2024 Tesla Model 3 representing it as "clean/no accidents." In reality, they released a vehicle with a disabled airbag system resulting from a crash that occurred in their inventory. 1. DISABLED RESTRAINT SYSTEM (RCM): Tesla Service Mode diagnostics confirm the Restraint Control Module (RCM) is in "Post Replacement" state, displaying the prompt: "Post Replacement Tasks - Routine requires Service Mode Plus authentication." This proves the airbag computer was replaced following a crash/electrical trauma but NEVER calibrated. Consequently, the SRS is disabled and will not deploy. The vehicle explicitly warns: "The Safety Restraint System may not perform as designed." The dealer released a non-roadworthy vehicle. 2. EVIDENCE OF DEALER KNOWLEDGE: Internal logs show critical crash fault codes (VCFRONT_a059 & RCM2_a003) logged on Oct 15, 2025. Dealer records confirm the car was in their exclusive inventory on this date (listed since Sept 23). These faults were active 9 days before sale. The dealer ignored active safety warnings to facilitate the transaction. 3. CONCEALED DAMAGE: Inspection by Classic Collision confirmed hidden structural damage requiring $9,676.69 in repairs, consistent with the uncalibrated module. CONCLUSION: Braman Cadillac knowingly sold a vehicle with a lethal safety defect. I demand an investigation into their release of vehicles with uncalibrated safety modules. The rear motor became completely disabled while driving. The rear motor suddenly made noise and locked up when I was driving about 10 mph. I received a red alert on my screen saying to pull over now and another alert saying the car is shutting off. The car suddenly stopped on its own before I could do anything. I then got a yellow warning message saying that the rear motor is disabled. I was luckily driving slowly in a parking lot when this occurred but if it happened on a public road, things could have gone much worse. I contacted Tesla support when it occurred but I cancelled the support request because the alerts went away once I exited the car. The vehicle drives normally but not sure if it will happen suddenly again.

I was sitting in the back seat adjusting my seat belt. The person sitting in the front seat ahead of me was adjusting their seat position. When they did this, the steel seat mechanism came down on the big toe of my left foot. It crushed the toe so hard that I was not able to remove it from the lowering seat mechanism. This was extremely painful and required medical attention at urgent care and at my primary care physician. I consider this a serious safety defect and design error in this model and possibly other models of this car.
The contact owns a 2024 Tesla Model 3. The contact stated that the driver lowered the front driver's seat using the seat adjustment mechanism while seated in the rear driver's seat. The contact stated that the front driver's side seat crushed his toe. The big left toe was crushed. The contact had to have medical attention. The contact stated that the medical attention consisted of an x-ray and pain medication. The contact stated that the injury was extremely painful. The manufacturer was not made aware of the failure. The failure mileage was approximately 15,000.
