Manufacturer Recall Number25TB06 NHTSA Recall Number25V322 In regards to the above mentioned recalls. The vehicle's reverse camera began flashing while driving forward. The R light on the gearshift was also flashing. While driving forward about 15 mph out of a parking lot, the vehicle suddenly went into reverse on it's own, locked up and would not move. I had to have the vehicle towed to the dealership. This happened around the week of June 8. The dealership says that they reset the electrical recall, we picked up the truck and it ran fine for about a week. The reverse camera and R light on the gearshift began flashing again on June 17. It did not shift itself into reverse, but the dealership requested that we bring the vehicle back. The dealership stated that Toyota has "grounded" the vehicle and does not have a fix for the recall to date and continuing to drive it is a safety hazzard. It has been sitting at the dealership since June 18 with no end in sight. The dealership and Toyota Corporate continues to tell me every few days that they do not have a fix for the issues/recalls.
Summary of Complaint I am submitting this complaint to report what I believe may be a safety-related defect involving the twin-turbocharger system on my 2022 Toyota Tundra (VIN: [XXX]). The driver’s side turbocharger failed at approximately 58,000 miles and had to be replaced. Approximately 6,000 miles later, at around 64,000 miles, the passenger-side turbocharger also failed. The vehicle has been properly maintained and serviced by authorized Toyota dealerships. A failed turbocharger causes a significant loss of engine power, which can create a hazardous condition when merging onto highways, overtaking other vehicles, or driving in traffic where acceleration is necessary to avoid a collision. Toyota requested that I return the vehicle to an authorized dealership for an additional diagnosis, which confirmed the failure of the second turbocharger. I paid $250 for this diagnosis at Toyota’s request. Despite the repeated failure of both turbochargers at relatively low mileage, Toyota declined to provide further assistance and informed me that its decision was final. Based on the failure of both turbochargers within only 64,000 miles, I believe this may represent a manufacturing or design defect rather than normal wear. I respectfully request that NHTSA review this issue and determine whether similar failures are occurring in other 2022 Toyota Tundra vehicles equipped with the same twin-turbo engine, and whether a safety-related defect investigation is warranted. Fuel system

Driving on the interstate and the truck surged very hard. The truck began to surge very hard and went into decreased speed mode. We were told by the dealer there is a TSB for the PCM. This decreased power caused the truck to not be able to travel over 25 MPH in a 70 MPH zone. This caused an unsafe situation.
After I put gas into the vehicle gas tank, the gas tank gauge indicated the tank was 75% full. The following day when I started the vehicle the gauge indicated the tank was 100% full and I did not add a drop of gas into the tank. Days later the tank appears to be stuck on 100% full. My fear is that I will not realize when the tank approaches empty at a very inopportune time.

I have not had an actual vehicle accident as of yet but the vehicle has a serious hesitation issue during acceleration from a quick stop or rolling situation whereas if you were making a left turn at a traffic light and were waiting for traffic while execrating to clear the next on coming vehicle there is sometimes a 3 second or more delay before the vehicle will accelerate which could cause an accident. Toyota has recognized this issue and came out with a technical service bulletin (T-SB-0032-26) with an update program for the TCU but has not included my vin or several others that have this same issue. I have been told by my dealer that they could do the update but since my vin is not included they would have to charge me. This is a major safety issue and needs to be addressed at no cost to the consumers. I have contacted Toyota brand engagement and they were no help at all acting like they had no idea what I was talking about. It happens every, everywhere. Major safety issue. Making complaints before accidents or fatalities happen because of this!
Driver assistance

In this vehicle, I regularly experience a "delay" in gas pedal response. I drive a non-hybrid V6 twin turbo variant engine 2022 Toyota Tundra, and for a long time I thought this was just related to turbo spooling delays. But Toyota's recent TSB-0032-26 publishing last month confirmed that it is actually a TCM logic issue requiring a re-flash. I spoke to the dealership and this is not covered outside of the standard 3-year bumper to bumper warranty, and they wouldn't cover it under the powertrain warranty which lasts longer, even though this is technically a power train issue. This concern has left me challenged with timing gas pedaling from a full stop, often with a noticeable 1-2 second lag before the truck responds. I have had a couple of close calls because this behavior inhibits safe judgement calls for acceleration. Especially from full stops this is most noticeable, but also when you are trying to accelerate suddenly, there's always a brief delay.
The contact owns a 2022 Toyota Tundra. The contact stated that there was moisture accumulated in the taillight housings. While shifted into reverse, the camera image was not properly displayed. There were six unknown warning lights illuminated. The contact received notification of NHTSA Campaign Number: 25V322000 (Exterior Lighting). The vehicle was taken to the local dealer, but the vehicle was not diagnosed or repaired. The dealer was waiting for authorization from the manufacturer. The manufacturer was notified of the issue and informed the contact that the part for the recall repair was not yet released. The failure mileage was approximately 65,000.

The regain from brake for the hybrid system jerking when go to full stop
The contact called on behalf of the owner of a 2022 Toyota Tundra. The owner received notifications of NHTSA Campaign Numbers: 22V501000 (ELECTRICAL SYSTEM, BACK OVER PREVENTION), 22V661000 (SERVICE BRAKES, HYDRAULIC), 23V142000 (STRUCTURE), 23V566000 (FUEL SYSTEM, GASOLINE), 24V125000 (POWER TRAIN), 24V381000 (ENGINE AND ENGINE COOLING), 25V657000 (BACK OVER PREVENTION), and 25V322000 (EXTERIOR LIGHTING). However, the part to do the exterior lighting recall repair was not yet available. The contact stated that the electronic parking brake failed to function as intended. The contact stated that there are no local dealers on the island in Alaska where the owner resides. The nearest Toyota dealer was 750 miles away and was in Washington State. The manufacturer was contacted and refused to cover the cost to transport the vehicle to Washington State. The failure mileage was approximately 60,000.

Water is leaking into the cab of my 2022 Toyota Tundra Platinum 4x4 through the high-mounted third brake light. The leak allows water to enter the headliner and interior, creating the potential for mold, damage to the interior, and electrical issues. After troubleshooting, the third brake light gasket appears to be the source of the leak. Toyota does not appear to offer the gasket as a replacement part, requiring owners to replace the entire third brake light assembly despite the failure being limited to the seal. Numerous owners have reported similar water intrusion issues on 2022+ Tundras, suggesting this may be a common defect rather than an isolated incident. I believe Toyota should investigate the failed gasket design and consider a recall or service campaign to replace the defective seal or assembly.

The contact owns a 2022 Toyota Tundra. The contact stated that while the vehicle was parked, the contact became aware that the rear bumper had detached. The contact stated that the vehicle was previously associated with a recall related to the failure. The vehicle was taken to the dealer, where it was determined that the rear bumper had failed and needed to be replaced. The contact was informed by the dealer that parts were not available and were on backorder. The vehicle was not repaired. The vehicle was taken to another deale,r where the same information was provided, and the contact was informed that the rear bumper needed to be replaced. The vehicle was repaired; however, the failure recurred. The vehicle was taken back to the dealer, where the contact was informed that the rear bumper had failed again and needed to be replaced. The vehicle was not repaired. The manufacturer was made aware of the failure and referred the contact to the NHTSA Hotline for assistance. The approximate failure mileage was 49,150.

Front seat driver side shield cover has cracked in multiple places and may affect performance of side air bag deployment.
Driver-side seat plastic side cover (seat shield) cracked/broke under normal use. Concern about durability and potential safety hazard if seat components fail,I was bring into local Toyota dealer ask for replacement but answer is vehicle is out of warranty (3year /36000mile)I need to pay for it ,which my vehicle is just past warranty about 5month and the mileage is only 25956,I do find out a lot Toyota owners have the same issue.

The vehicle was purchased brand new from an authorized franchised dealership, sold and contracted as being equipped with a dealer-installed 2.5-inch suspension lift. At approximately 20,000 miles, a severe front-end squeaking symptom developed. The vehicle was brought to a separate authorized Toyota dealership service center for inspection. An authorized Toyota Master Technician formally inspected the vehicle and documented that the selling dealership did not install an engineered suspension lift. Instead, they installed unauthorized, budget aftermarket spacer pucks and non-certified upper control arms. The Master Technician documented in writing that the vehicle "isn't even lifted properly" and a factory front strut has failed prematurely. SAFETY RISK: The unauthorized spacer modification has severely altered and compromised the factory steering and suspension geometry. Because the front suspension, upper control arms, ball joints, struts, and steering rack are forced to operate continuously at unsafe, extreme, out-of-spec angles, the components are subjected to premature, destructive stress. The Master Technician confirmed that this improper configuration creates an imminent highway life-safety hazard, presenting an extreme risk of the front CV axles binding or snapping at highway speeds, which would result in an immediate, catastrophic loss of steering control. The vehicle is available for inspection upon request. The safety defect has been explicitly reproduced and confirmed in writing by an authorized Toyota Master Technician. The selling dealership management has been formally notified of this safety diagnosis but has actively refused to remediate the unsafe components, leaving a structurally compromised and highly dangerous vehicle operating on public highways.
Front end alignment is incorrect from Toyota factory. This is reflected in the service campaign 24TD01 for this vehicle. Toyota refuses to fix the problem, stating the campaign has expired. The alignment causes premature uneven wear of the tires, which is a safety problem. The 24TD01 campaign document states the dealer should complete the service prior to sale. Marietta Toyota DID NOT complete this service on my vehicle before sale.
Visibility

Driving down the Highway at about 75-80 MPH and my Passenger side mirror camera flys off lost to the highway.
I have a 2022 Tundra with 18,700 miles on it that is part of the recall for a new engine! This is my fourth tundra, and the materials used in the 3rd Gen are sub-par to say the least! I have already had to replace the actuator and motor in my passenger window because it was stuck halfway open/closed and it took the dealership almost 2 weeks just to get the parts and put them in. My tailgate isn't aligned properly and the mold growth on all the rubber window trim is a nightmare as well as the roof wells that are also covered in mold up and down both sides. My rear passenger door doesn't close correctly and nobody at Toyota knows how to fix any of these issues! I am very concerned about a dealership putting a new engine in my truck when they can't even change the oil or rotate my tires correctly. I also have other serious issues like, I feel a giant thud like somebody rear ended me when I step on the gas from a red light or stop sign and sometimes when I step on the gas on the highway and it scares the heck out of me. It almost feels like the back of my truck fell off. When it's raining out and my hood is covered in water and then the rain stops and I turn my windshieldwipers off, the water that rolls down the hood is spit straight into my windshield from the radiator fan! I also took it to the dealership to fix the fuel line recall, and they said they don't have the parts available, it's been over 2 weeks now and the recall was issued last year.

This recall {25TA06} has been out since May 15 2025, it's been over a year now. I have called a few times and nobody know anything when this will be completed. Rumors on line say they took care of the ones at the dealership but not the ones that customers purchased. All I need is about when this will be taken care of. Thank you

Tires & wheels

Premature tire wear. Outside of tire on front wearing faster than inside. Had a realignment and also put on new tires. Same issue is happening
Tire size 265/60R20. All tires have cracked around the bead of the rim. This was discovered at 24,663 miles during regularly scheduled maintenance at Copeland Toyota in Brockton, MA on March 20, 2025. Defective manufacturing suspected. Upon further investigation of this tire model on the Internet, I discovered dozens of reports of the same sidewall failure by Toyota Tundra owners with very low mileage. I contacted Bridgestone Support on March 20, and per their instruction, took the vehicle to a Bridgestone authorized dealer (Firestone Complete Auto Care, Warren Ave. Brockton, MA) on March 21. The inspection determined that the tires have failed. The technician indicated that Bridgestone has very limited inventory of this tire model, indicative that the tire is being discontinued. I contacted Bridgestone Support on March 21, indicating that there is likely a manufacturing defect, and was advised to report this to the NHTSA, because the Bridgestone employee stated that they have no internal capability to report a potential manufacturing defect. I suspect there is a likelihood that these tires have been discontinued because of defects, but Bridgestone has not alerted customers of such a safety hazard. My Bridgestone Support reference number is [XXX]. Please investigate this as I believe there is a safety hazard due manufacturer defect. Airbags

What component failed and is it available for inspection? The front driver-side roof seam on my 2022 Toyota Tundra developed corrosion under the paint and eventually opened enough for rainwater to enter the cabin. Water drips directly into the A-pillar area onto the side curtain airbag wiring, squib connector, and padded harness. It also reaches the dashboard and electrical buttons near the steering wheel during rain. The roof seam, A-pillar wiring, interior rust staining, and all affected components remain available for inspection. How was safety put at risk? The A-pillar contains the SRS curtain airbag system, its wiring, and connectors that should never be exposed to moisture. Continuous water intrusion over many months can cause corrosion, increased resistance, short circuits, or damage to the airbag inflator/squib circuit, which may lead to unintended deployment or failure to deploy in a crash. Water reaching the dashboard controls and electrical switches could cause intermittent electrical faults or malfunction of driver controls. These issues present a significant safety risk because the vehicle’s airbag and electrical systems may not function as designed. Has the problem been reproduced or confirmed? Yes. The Toyota dealer has repeatedly observed the active leak, interior rust streaks, and bubbling/corrosion along the roof seam. The water intrusion occurs every time it rains. Has the vehicle been inspected? Yes. Toyota has inspected the truck multiple times, including by a Field Technical Specialist. However, Toyota has been unable to provide complete documentation or photos from the FTS inspection, and the underlying safety issue remains unresolved. No inspections have been done by police or insurance. Warning lamps or messages? No warning lamps appeared. The first symptom was water leaking into the cabin around March 2024, followed by rust streaking inside the A-pillar and exterior bubbling. The issue continues during rainfall.
The contact owns a 2022 Toyota Tundra. The contact stated that while driving approximately 40 mph the vehicle crashed into a tree causing severe damage to the front end. During the crash, the air bags did not deploy and the collision avoidance system did not activate to prevent the crash from occurring. No injuries were reported. A police report was filed at the scene and the vehicle was towed away. The cause of the failure was not determined. The manufacturer was notified of the failure. The local dealer was not contacted. In addition, while driving at highway speeds the Tonneau cover nearly came off the vehicle. The cause of the failure was not determined. The failure mileage was 5000.

The outer edges of front tires wearing excessively. They replaced 2 around 20k milage and there are many reports of the same issue from factory alignment and camber. The tire appears to be splitting prematurely.
Apparently widespread issue with uneven tire wear. I know multiple owners with the same issue despite rotating tires even 5k miles (myself included. They did apparently correct the issues with the trd pro but have failed to solve it for the rest of the line up. Much more wear on the outer aspects of the tires.
