What component or system failed? The inner brake pad material on the front wheel of the door side and off door side separated. The material did not come unbonded from the pad plate, the material itself separated. I have the pads and pictures of the failure. How was your safety put at risk? The material could have become jammed in the brakes. This could have caused the single brake to lock up (possible loss of control), cause a brake to overheat to failure, or catch the tire on fire. The material could have totally come loose. This could have caused a brake line to be severed causing loss of brakes or, due to loss of pad material, a reduction in braking capability. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes, the technician working on my vehicle stated he has seen this brake pad material failure on Kodiak pads multiple times. Has the vehicle or component been inspected by the manufacturer? Not yet. Were there any warnings? No

The contact owns a 2022 Alliance Paradigm RV. The contact stated that while the RV was parked, the contact and his wife became aware that the awning fabric had separated partially at the heat weld of the fabric. The awning was then closed and almost instantly, the fabric fell to the ground, and the arms were bent. The RV was not diagnosed or repaired. The contact believed that the failure was associated with NHTSA Campaign Number: 22V512000 (Equipment). The manufacturer was notified of the failure.

The contact owns a 2022 Alliance RV Paradigm. The contact stated while driving at various speeds, the front passenger’s side awning slid out unintendedly. The manufacturer was notified of the failure and the contact was referred to a dealer. The dealer inspected the RV and informed the contact that the failure was related to structural damage. The manufacturer contested the dealer's findings and referred the contact to another dealer. The second dealer also confirmed that the failure was related to structural damage caused by the wall studs fracturing from the RV steel subframe. The RV remained in the possession of the dealer unrepaired. The failure mileage was approximately 15,000.

We got the Morryde Independant Suspension (7K) installed in March, 2022. They used the Alliance factory disk brakes that were on the unit (Alliance 340RL 2022 (2021 build date)) when they did the install. When I was at Morryde on 5/17/24 getting some basic maintenance done (alignment, tire rotation, balancing, bearings, shock replaced, etc.) Morryde found all four of my Kodiak disk brake caliper brackets had broken "tabs". One also had a busted disk brake caliper mounting hole which caused the brake caliper to be loose and rub on the rim. Dexter covered this under warranty. There were no symptoms of this before the wheels were removed. The caliper could have jammed against the wheel causing loss of control or the caliper could have been ripped off causing loss of brakes. While I was at Morryde, two other owners were there (different make, Grand Design) who had experienced major failures on the highway (blown tire and damaged rim, loss of brakes) due to the brakes mount failing and the caliper coming loose. I have photos of the broken brake mounts.

After finalizing the purchase of a 2022 Alliance RV Paradigm 295MK trailer at Giant RV dealership in Colton California we started our drive home. After driving 2 hours and 80 miles, I was notified by a fellow driver on 101 freeway in Woodland Hills California that I should pull over something was wrong. When I was able to pull off the freeway safely and made a turn I saw a trail of water behind my trailer. Once I pulled over I saw that my freshwater holding tank had fallen and I was dragging it.
