The defect: K2 MOTOR IS RECALLING 1,921 AFTERMARKET HEADLAMPS OF VARIOUS MODELS SOLD FOR USE ON THE ABOVE LISTED VEHICLES. THESE HEADLAMPS ARE MISSING THE AMBER SIDE REFLEX REFLECTOR WHICH FAILS TO CONFORM WITH THE REQUIREMENTS OF FEDERAL MOTOR VEHICLE SAFETY STANDARD NO. 108, "LAMPS, REFLECTIVE DEVICES, AND ASSOCIATED EQUIPMENT."
The risk: WITHOUT THE AMBER SIDE REFLEX REFLECTORS, THE LIGHTING VISIBILITY MAY BE AFFECTED, POSSIBLY RESULTING IN A VEHICLE CRASH.
The remedy: K2 MOTOR WILL NOTIFY OWNERS AND OFFER A FULL REFUND. THE RECALL BEGAN ON DECEMEBER 17, 2008. OWNERS MAY CONTACT K2 MOTOR AT 1-909-839-2992.

Crash reported

The contact called on behalf of a 1994 Chevrolet GMT-400. The contact stated while driving uphill at night, she suddenly noticed that there was a vehicle at a complete stop in front of her. The contact stated that she swerved to avoid the vehicle; however, the driver's side front end of the vehicle hit the rear passenger's side bumper of the other occupied vehicle. The contact stated that the tail lights and the brake lights of the other vehicle were not engaged as they had failed. The contact stated there were no injuries sustained; however, the occupants of the vehicle were transported to the hospital with the assistance of emergency units. A police report was filed. The contact's vehicle was totaled. The other vehicle was driven to the residence. There was no further information provided. The failure mileage was unknown.

TL*THE CONTACT OWNS A 1994 CHEVROLET PICKUP. THE CONTACT STATED THAT WHILE ATTEMPTING TO ACTIVATE THE WINDSHIELD WIPERS, THEY FAILED TO WORK. INTERMITTENTLY, AFTER SEVERAL MINUTES, THE WIPERS MAY FUNCTION PROPERLY; HOWEVER, IT IS DIFFICULT TO DETERMINE BECAUSE THEY ARE NOT USED OFTEN. THE LOCAL DEALER AND MANUFACTURER BOTH STATED THAT THE VIN WAS NOT INCLUDED IN THE RECALL. THE RECALL NUMBER WAS UNKNOWN. THE FAILURE MILEAGE WAS 70,000 AND CURRENT MILEAGE WAS 74,000.UPDATE 09/10/08 *CW

COMPLAINT RECEIVED VIA E-MAIL. I HAVE A 1994 CHEVROLET PICKUP, THE WINDSHIELD WIPERS HAVE STOPPED SEVERAL TIMES WHILE I WAS DRIVING IN THE RAIN. I HEARD THROUGH A FRIEND THAT THERE WAS A RECALL FOR MY TYPE OF TRUCK. THIS PROMPTED ME TO COME TO THIS SITE AND LOOK IT UP. I FOUND A RECALL WITH CAMPAIGN ID NUMBER 03V159000. I THEN CALLED A LOCAL DEALER AND AFTER GIVING MY VIN #, I WAS TOLD THAT THERE WERE NO OPEN CAMPAIGNS ON THIS VEHICLE. I THEN CALLED THE 800 NUMBER FOR CHEVROLET THAT WAS ON THE REPORT AND WAS ALSO TOLD BY THEM THAT THERE WERE NO OPEN CAMPAIGNS ON MY VEHICLE. I UNDERSTAND THAT THERE WAS AN ANALYSIS DONE THAT PROBABLY PRODUCED A LIST ON VINS THAT WERE AFFECTED BY THIS RECALL. HOWEVER, MY VEHICLE WAS EXHIBITING THE SAME SYMPTOMS AS THOSE LISTED IN THE RECALL. THIS WAS A VERY DANGEROUS SITUATION. AT THE CURRENT TIME I DO NOT HAVE THE RESOURCES TO HAVE THE REPAIRS MADE. I BELIEVE THAT IF THIS PROBLEM RESULTED IN A RECALL OF THESE VEHICLES TO REPAIR THE CIRCUIT BOARD, AND MY VEHICLE WAS SHOWING THE SAME SYMPTOMS, THE COMPANY SHOULD AT LEAST EVALUATE THE PROBLEM TO SEE IF THIS WAS THE CAUSE OF MY WINDSHIELD WIPERS MALFUNCTIONING. This Preliminary Information communication provides information to the technician on how to unlock radios that are Delco TheftLock. Technician will use the radio to get the scrambled code. Technician will need to contact the radio unlock center to retrieve the unlock code from the scrambled code.  The PI update to version D is to advise the unlock procedure is temporarily unavailable.  E version update is to add administrative details.
