Timing Belt Failure
On December 18, 2025, the Office of Defects Investigation (ODI) opened PE25020 to investigate instances of timing belt failure in model year (MY) 2015-2017 Ford Fiesta and MY 2015-2018 Ford Focus vehicles manufactured by Ford Motor Company (Ford), equipped with a 1.0L engine and manual transmission (subject vehicles). Additionally, MY 2018-2021 Ford EcoSport and MY 2016-2018 Focus vehicles with a 1.0L engine and automatic transmission were analyzed as peer vehicles because they utilize a wet timing belt comprised of similar material as the timing belt in the subject vehicles. Through the PE, ODI investigated a potential failure/degradation of the timing belt that may affect oil pump function resulting in a lack of engine lubrication and potential loss of motive power. The complaints commonly allege a low engine oil pressure warning lamp immediately preceding a complete loss or reduction of motive power while the vehicle is in motion. Some complaints allege that a sudden loss of motive power or engine seizure occurred without warning or prior detectable symptoms. ODI’s analysis supports that this failure can occur without sufficient and advanced warning to the driver that a loss of motive power/engine seizure is imminent. ODI found that the timing belt material may degrade and create debris that clogs the mesh oil pump pick-up screen, resulting in reduced oil pressure in the engine. As additional debris accumulates on the screen, this can progress to a loss of motive power while driving due to a lack of engine lubrication. The failure data provided in response to this investigation revealed an average failure mileage of approximately 70,000 miles, and that 98% of the failures occurred prior to the 150,000 mile timing belt replacement interval per the vehicles’ maintenance schedule. Based on evidence collected via consumer surveys and an engine teardown, failures have occurred despite evidence of proper and routine oil maintenance. The oil pump pick-up screen is not serviceable, so debris accumulation cannot be inspected or manually removed without removing the engine from the vehicle. On June 2, 2026, Ford informed ODI of a non-safety customer satisfaction program for global vehicles with the 1.0L Fox Classic Timing Belt. This program will notify customers globally of a revised maintenance interval for the timing belt, reducing the original 150,000 mile interval down to 100,000 miles/6 years. This customer satisfaction program will also offer reimbursement to eligible customers who previously paid to have their engine repaired or replaced due to a timing belt related issue. Based on NHTSA’s analysis of the data, failure rates, information provided by Ford, preliminary engine teardown analysis, and precedent recalls regarding loss of engine oil pressure with the presence of driver facing warnings, ODI believes there is an unreasonable risk to motor vehicle safety. Door Latch Failures
In December 2019, the Office of Defects Investigation (ODI) opened this investigation based on 235 Vehicle Owner Questionnaires (VOQs) alleging doors failed to latch when closed, and/or inadvertently opened while driving, on model year (MY) 2012-2014 Ford Fiesta, and 2013-2014 Fusion and Lincoln MKZ. Some VOQs reported the failure occurred after the latch had been remedied under a prior recall which involved fracture of the latch pawl spring tab due to thermally induced cyclic fatigue. Other VOQs alleged a similar failure in vehicles not previously recalled. A latch with a fractured pawl spring tab typically results in a ?door will not close? condition, however in certain somewhat rare circumstances, may result in the door coming open while driving. In response to ODI?s investigation, Ford has announced three field actions, as discussed below.On March 23, 2020, Ford submitted Safety Recall 20V-177 (Ford 20S15), a regional recall campaign to address door latch failures in vehicles not previously recalled under Safety Recalls 15V-246, 16V-643 and 17V-210. The recall affects vehicles currently or ever registered in certain high ambient temperature states (see 20V-177 for affected states). As a remedy, Ford will replace all four door latches free of charge.On August 24, 2020, Ford issued a non-safety regional field action (Ford 20R01) to address door latch failures on the same model/MY vehicles not covered under 20V-177 (i.e., not currently/ever registered in affected states). According to Ford, spring tab failure rates for vehicles covered by this action are about half that of the 20V-177 vehicles. Under this action, dealers will replace all door latches, free of charge, if either; a) the vehicle exhibits a related latch concern, or b) if the vehicle owner expresses a concern about the latches in their vehicles. Details can be found under NHTSA TSB No. 10190990, a copy of which is available in this investigative file available at NHTSA.gov.On June 8, 2020, Ford submitted Safety Recall 20V-331 (Ford 20S30) addressing vehicles previously remedied under NHTSA Recalls 15V-246, 16V-643 and 17V-210, where the repair may not have been properly completed, specifically not all of the door latches were replaced, or replaced correctly. This recall involves an inspection of the latches, with the option for a consumer self-inspection; owners could view the date codes on their latches (and confirm child safety locks are present on the rear doors) and then submit the information to Ford?s mobile app or website. A dealership inspection option was also available. Any vehicle found to have unreplaced defective latches requires the replacement of all door latches as directed by Ford.Regarding the 4,494 reports cited above, 1,485 involve scope-related failures (with 13% alleging door opening), and the remaining 3,009 reports involving remedy concerns (with 18% alleging door opening - note ODI was unable to verify the allegations of door opening). The three ODI injuries were all relatively minor. Ford?s reported injury incidences include one (unconfirmed) report of an ejection, three minor injuries with the others involving undisclosed injuries. Ford?s crashes involved two (unconfirmed) crashes alleging the incident door contacted another vehicle and three involving doors striking other property.The recalled vehicles include MY 2011-2015 Ford Fiesta, 2013-2016 Ford Fusion and Lincoln MKZ, 2015 Ford Mustang and Lincoln MKC, 2013-2015 Ford Escape and Ford C-MAX, 2012-2015 Ford Focus and 2014-2016 Ford Transit Connect.Ford's actions address ODI's concerns at this time. Car sputters everytime I put gas in it. Sometimes shuts off. It’s every time I put gas. I’m not filling up the gas tank because I notice if I do fill it up. It gives me more problems .. occurring problem

The contact stated that the vehicle was taken to the dealer for recall repairs; however the issue persisted. After the repairs, while mother was driving on the highway, the vehicle stalled. The clutch became inoperable. The vehicle was towed to the residence. The vehicle was not further diagnosed or repaired. The manufacturer was notified of the failure. The approximate failure mileage was 20,000.

I was driving on the highway, and my car started shaking. All of the sudden I couldn't push on the gas anymore, and out of the blue a notification popped up on my screen that said "transmission failure, service immediately" and the car fully stopped working. I was forced to coast to the side of the highway, where I ended up in an incredibly dangerous spot. I had to call the cops, have them come so I wouldn't get hit (it was fully night time), and had to wait for a tow truck to come tow me and my car off the side of the highway.

I was driving for couple hours then my oil light came on check engine light came on. Vehicle started to shake very badly. I had to pull over and see what happened. I started the vehicle up again and the oil light and check engine light are still on. I know that these vehicles have the wet belt system on the automatics. But they don't have one for the manual transmission. There should be a recall for these just like the automatics.

Transmission going out with car having only 71,000 miles on it .a know fact that Ford knew when I bought this car new .and going out when on freeways or just about anywhere could me in danger

Low pressure oil light came on and I stopped driving it 18 miles later drove about 25-30 mph hoping it wouldn’t seize on me before I got home. because I figure the oil pump is clogged with timing belt fragments and I can’t afford to replace the timing belt right now. Afraid to drive it because I have read it could seize up the engine. 1.0 eco boost manual transmission. Car ran great and I had no issues with it until the low pressure oil light came on. Been sitting since March. I have started it 4 times since then and it has been moved around the yard but never ran longer than a few minutes. Still starts just afraid with the low pressure oil light on that it could seize up if I drive it again.

ELECTRICAL SYSTEM; STRUCTURE:BODY
Ford and Lincoln vehicles equipped with wired keyless entry keypad systems and accessory wireless keyless entry keypad systems may or may not come with a wallet card containing the master code. Unlike the integrated wired keypad, the accessory wireless keypad master code cannot be retrieved from the vehicle using a diagnostic scan tool or from the label printed on the body control module (BCM). The Factory Keyless Entry Code application within the diagnostic scan tool will not provide an applicable master code for the accessory wireless keypad. If the wallet card for an accessory keypad is not available, the "Wireless RF Keypad Diagnosis Guide" can be referenced and provides direction on how to retrieve the master code. This guide is located under the Workshop Manual tab > Accessories > Installation > Wireless RF Keypad Diagnosis Guide > PPT C > Step C2.<br /><br /> Note: The only available keypad for some vehicles from the assembly plant is the accessory wireless keyless entry keypad. ELECTRICAL SYSTEM; POWER TRAIN
Some 2015-2021 Ford and Lincoln vehicles may exhibit an illuminated MIL with diagnostic trouble codes (DTCs) P0011, P0012, P0014, P0015, P0016, P0017, P0018, P0019, P0021, P0022, P0024 and/or P0025. Pinpoint test HK in the Powertrain Control and Emissions Diagnosis (PC/ED) or in Section 303-14 of the Workshop Manual (WSM) has been updated to address this concern.
Certain 2014-2015 Model Year Fiesta, 2014-2016 Model Year Fusion, MKZ and 2014-2016 Model Year Transit Connect Vehicles Not Covered in Safety Recall: 20S15 or 23S36 Door Latch Replacement and Inspection
EQUIPMENT
All accessory stick-on keyless entry keypads come with a wallet card containing the master code. If the accessory keypad was factory installed, this card will be placed in the glove box and should be handed to the vehicles owner during initial delivery. Unlike the integrated (wired) factory keypad, the accessory keypad master code cannot be retrieved from the vehicle using a diagnostic scan tool. If the wallet card for an accessory keypad is lost, follow these steps to recover the code. Dealer Key Code access is granted through the Dealer Program Enrollment System (DPES); dealership management can authorize users. The authorized user must go to FMCDealer.com > Parts and Service > Parts Product Line Information > Keys/Entry Devices > Aftermarket Keyless Entry Keypad Code Access. Follow the instructions on the screen to obtain the master code for the accessory keypad.
ENGINE AND ENGINE COOLING
For 2015-2024 Ford and Lincoln vehicles equipped with turbocharged engines and customer concern of an engine oil leak, refer to Workshop Manual (WSM), Section 303-00 for oil leak inspection and testing methods. Ford has evaluated turbochargers replaced for oil leak symptoms and although oil may collect on the external surface of a turbocharger, it is extremely rare for a turbocharger assembly to be the root cause of the oil leak. Careful inspection of the turbocharger oil feed and drain tubes should be performed along with the recommended WSM procedures as needed. Warranty claims for turbocharger replacement going forward will be assessed.
